Abstract

Cardiac arrest causes global ischemia followed by reperfusion injury, resulting in diffuse myocardial damage and cardiac troponin release. This condition is commonly associated with type 2 myocardial injury caused by an imbalance between myocardial oxygen supply and demand rather than acute coronary occlusion. This article summarizes evidence on the cellular and molecular mechanisms of type 2 myocardial injury and troponin release after cardiac arrest. A narrative literature review was conducted using articles published between 2015 and 2025 from PubMed and Google Scholar. The findings indicate that ischemia causes ATP depletion, ionic imbalance, intracellular calcium overload, and mitochondrial dysfunction. Reperfusion further aggravates myocardial injury through reactive oxygen species generation, inflammasome activation, pro-inflammatory cytokine release, apoptosis, and pyroptosis. These processes impair cardiomyocyte membrane integrity and cause structural damage, leading to troponin release. Troponin elevation may reflect reversible membrane permeability changes or irreversible cardiomyocyte injury. Therefore, high-sensitivity cardiac troponin levels should be interpreted according to pathophysiological mechanisms and clinical context to distinguish type 2 myocardial injury from myocardial infarction caused by acute coronary occlusion.

Abstract

Inadequate care after circumcision care may increase the risk of complications and infection in children. This community service activity aimed to provide education to parents regarding wound care and medication administration following circumcision in Cipare District. The activity involved 40 children aged 3–8 years who had undergone circumcision, accompanied by their parents or caregivers. Some participants had medical conditions, including phimosis, balanitis, and a history of urinary tract infections. Individual education was provided to parents or caregivers after the children had completed the circumcision procedure through direct explanations, wound care demonstrations, medication administration simulations, and distribution of written guidelines. The educational materials covered wound care and hygiene, dressing changes, use of antiseptics, administration of medications according to clinical indications, and recognition of signs of infection and complications requiring medical attention. The medications provided included ibuprofen as an analgesic and amoxicillin as an antibiotic according to clinical indications. This activity provided parents with practical guidance on post-circumcision care and appropriate medication use at home. Individual education as part of community-based circumcision services may support family involvement in post-procedural care and serve as a promotive and preventive health effort to support wound healing and prevent complications in children.
