Abstract

Ketidakpatuhan dalam mengonsumsi obat antihipertensi merupakan salah satu faktor yang memengaruhi pengendalian tekanan darah pada pasien hipertensi. Penelitian ini bertujuan mengetahui perbedaan kepatuhan minum obat antihipertensi sebelum dan sesudah penerapan inovasi Sobat Tensi (WhatsApp Blast Reminder) di wilayah kerja Puskesmas Sapeken, Kabupaten Sumenep. Penelitian menggunakan metode Research and Development (R&D) untuk mengembangkan media edukasi kesehatan berupa pengingat minum obat melalui WhatsApp. Populasi penelitian adalah seluruh pasien hipertensi di Puskesmas Sapeken sebanyak 80 orang, dengan sampel 67 responden yang dipilih menggunakan teknik purposive sampling. Kepatuhan minum obat diukur menggunakan kuesioner Morisky Medication Adherence Scale (MMAS). Data dianalisis menggunakan uji statistik sesuai distribusi data dengan tingkat signifikansi 0,05. Hasil penelitian menunjukkan bahwa sebelum intervensi, sebagian besar responden memiliki tingkat kepatuhan sedang, yaitu 54 orang (80,6%). Setelah diberikan inovasi Sobat Tensi, sebagian besar responden memiliki kepatuhan tinggi, yaitu 36 orang (53,7%). Hasil uji Paired T-Test menunjukkan nilai p = 0,01 (p < 0,05), sehingga terdapat perbedaan signifikan kepatuhan minum obat sebelum dan sesudah pemberian inovasi Sobat Tensi. Inovasi WhatsApp Blast Reminder berpengaruh terhadap peningkatan kepatuhan minum obat antihipertensi pada pasien hipertensi di wilayah kerja Puskesmas Sapeken. Inovasi ini dapat menjadi salah satu alternatif untuk meningkatkan kepatuhan pasien dalam menjalani pengobatan antihipertensi.

Abstract

RSUD Grati Pasuruan is a district-level referral healthcare facility with a very high service intensity, resulting in an increasing volume of non-medical waste each year. However, its waste management system has not yet fully implemented the principles of 3R (Reduce, Reuse, Recycle) and the circular economy approach. This study aims to determine the effectiveness of strategies for managing domestic waste, wastewater, and plastic bags at RSUD Grati Pasuruan. The research used a descriptive method through observation of data before and after the implementation of waste management strategy interventions. The observation results indicate that domestic waste production decreased because the hospital processes waste into compost and fish feed, reducing the amount of waste disposed of as residue. The use of plastic bags also declined due to the adoption of reusable plastic bags. In addition, water usage became more efficient as plant watering activities were replaced by the use of treated wastewater from the wastewater treatment plant (IPAL). These three interventions collectively contributed to cost efficiency. Overall, the findings show that the strategies implemented by RSUD Grati Pasuruan are effective in supporting the implementation of 3R principles (Reduce, Reuse, Recycle) and the circular economy, as well as in promoting the development of a green hospital environment.

Abstract

Mental disorders are chronic illnesses that require a long process for recovery. One factor influencing the success of treatment in patients with schizophrenia is adherence to medication. It was found that 60% had a low level of medication adherence, which has the potential to hinder their recovery and increase the risk of relapse. Mental health community cadres act as an extension of healthcare workers in implementing mental health efforts. The purpose of this study was to examine medication adherence among people with mental disorders after a cadre empowerment intervention. This study used a quasi-experimental design with a pretest–posttest one-group approach. The sampling technique used was total sampling. The sample consisted of all People with Mental Disorders (ODGJ), totaling 18 individuals. The independent variable was cadre empowerment, while the dependent variable was medication adherence among people with mental disorders. The research instrument was a questionnaire adapted from the Morisky Medication Adherence Scale (MMAS-8). Data were analyzed descriptively using frequency distribution tables and diagrams. The results of this study show that medication adherence among people with mental disorders (ODGJ) increased following a community health worker empowerment intervention. These findings indicate that community health worker empowerment is an effective approach to supporting medication adherence and treatment success among people with mental disorders. Community health worker empowerment has the potential to serve as a community-based mental health care model in efforts to improve medication adherence among people with mental disorders at community health centers (Puskesmas).

Abstract

Every healthcare facility is required to ensure patient safety in order to improve the quality of care. The 2025 IKP report for East Halmahera shows that only 5 community health centers (31.25%) reported zero IKP incidents, while 11 community health centers (68.75%) did not submit a report. IKP reports tend to increase during Community Health Center accreditation surveys and decrease again after accreditation (IKP application reports). Low incident reporting rates are common, although the levels vary. Development of an Educational Innovation to Implement the “Lapor Pak” Culture (Report Patient Safety Incidents Immediately to the Community Health Center) as a Model for Strengthening the Implementation of Patient Safety Goals at Community Health Centers. The objective of this study is to analyze the educational program for implementing the “Lapor Pak” culture as an effort to improve patient safety incident reporting and foster a patient safety culture. The research design was quasi-experimental with a single-group pretest–posttest approach. The sample consisted of 57 staff members at the Subaim Community Health Center, with simple random sampling used as the sampling technique. The independent variable was education on the implementation of  culture “Lapor Pak” (Report Patient Safety Incidents Immediately to the Community Health Center), while the dependent variables were the rate of Patient Safety Incident (PSI) reporting and patient safety culture. The research instrument was based on the Hospital Survey on Patient Safety Culture Version 2.0 User Guide published by the AHRQ (Agency for Healthcare Research and Quality). Data analysis was performed using a paired t-test. The results of this study show that there was no difference in the number of patient safety incident reports between the periods before and after the implementation of education on patient safety culture (Lapor Pak); p-value = 0.322 (p > 0.05), with the average score increasing from 0.00 to 0.0175, however, this increase was not statistically significant, and patient safety culture improved significantly before and after the implementation of education on patient safety culture (Lapor Pak). p-value = 0.044 (p < 0.05).
