Abstract

Background: Hospitals are healthcare institutions with complex systems and structures, involving various stakeholders ranging from managers, owners, payers, professional teams, to patients and families. In facing the challenges of service quality and financing efficiency, case management has become an important strategic intervention to ensure quality, integrated, and cost-effective care. Research Objective: To determine the implementation of case management functions in efforts to improve quality control and cost control of inpatient services at RAA Soewondo Pati Regional General Hospital. Research Method: This study employed a phenomenological qualitative method. Data collection was conducted through in-depth interviews with informants from various groups, namely PPA, DPJP, hospital management, health workers, case managers, and families/patients. Data analysis was conducted thematically using an interpretative approach. Research results: The implementation of the case manager function was not yet optimal, characterised by weak interprofessional coordination, overlapping roles, a lack of understanding of the role of the case manager, and the absence of a performance indicator-based evaluation system. Patients and families complained about ineffective service processes, including poor communication, long treatment durations, and a lack of education regarding follow-up care.

Abstract

Background: Hypertension is a widely recognized cardiovascular disease where patients' blood pressure rises above normal levels. Hypertension is also known as a "silent killer" because its symptoms only become apparent when complications occur. There are several management approaches for hypertension patients, both pharmacological and non-pharmacological, such as those implemented by researchers who provided warm water soaking and hand massage. Objective: To determine the effect of warm water foot soaking therapy and hand massage on blood pressure and anxiety in hypertensive patients and to compare the effectiveness between the two interventions. Methods: The research design used in this study was quasi-experimental with a pre-post design with control group approach. The sampling technique used was purposive sampling with a sample size of 48 respondents divided into 2 groups. Results: Differences in blood pressure before and after hand massage in the working area of Kombos Primary Health Center, Manado City, showed a p-value of 0.000, and there was a significant difference in blood pressure before and after warm water foot soaking therapy with a p-value of 0.000. Additionally, there was a difference in effectiveness between warm water foot soaking therapy and hand massage therapy with a p-value of 0.001. Conclusion: Foot soaking therapy using warm water and hand massage have an effect in reducing blood pressure in patients with hypertension.

Abstract

Pendahuluan: Penggunaan PCS diharapkan mampu memetakan kebutuhan perawat yang sesuai dengan kompleksitas kondisi pasien, sehingga distribusi perawat menjadi lebih efektif dan efisien. Tujuan: menghitung kebutuhan perawat profesional berdasarkan Sistem Klasifikasi Pasien. Metode: desain penelitian kualitatif dengan pendekatan riset tindakan. partisipan Perawat di RSUD Teluk Bintuni. Data dikumpulkan melalui observasi, wawancara, dan dokumentasi yang berkaitan dengan pengelolaan tenaga keperawatan di rumah sakit. Proses analisis data menggunakan pendekatan kuantitatif dan kualitatif dengan tematik. Hasil: Penggunaan PCS perlu untuk pembagian perawat profesional di agar meningkatkan pelayanan secara maksimal. Kekurangan sumber daya manusia khususnya perawat menjadi penyebab sistem dinas tiga shift belum dapat diterapkan. Selain itu, tawaran alternatif sistem dinas perawat menjadi tiga shift dengan memenuhi kebutuhan jam kerja 40 jam dalam seminggu diterima oleh para perawat dengan adaptasi dan evaluasi yang cukup untuk memastikan keberhasilan implementasi. Kesimpulan: mempertimbangkan penerapan PCS sebagai metode utama dalam pengelolaan tenaga keperawatan guna meningkatkan kualitas pelayanan kesehatan.
