Abstract

Background: Teeth discoloration is one of the adverse effects of orthodontic treatment which causes aesthetic issues that effect on a person’s appearance. The management of teeth discoloration can be done with bleaching because it is easier to perform, faster, more effective, and more conservative than other treatments such as crowns and veneers. There are two types of bleaching techniques supervised by dentists: home bleaching and in-office bleaching. In-office bleaching is one of the most conservative and non-invasive treatment options to restore the aesthetics of discolored teeth. Purpose: This case report observes management of tooth discoloration after orthodontic treatment with in office bleaching using hydrogen peroxide 40%. Case 1: A 24-year-old woman came to Universitas Hasanuddin Dental and Oral Hospital with complaints of yellowish upper and lower teeth after removal of her fixed orthodontic appliance. Case 2: A 24 years old woman came to Universitas Hasanuddin Dental and Oral Hospital with complaints of upper and lower front teeth appearing yellowish in color, noticed from 2 years ago and had undergone orthodontic treatment for four years. Case Management: From clinical examination, both patients’ teeth have no history of caries, sensitive tooth and gingival recession. The Teeth were sensitive to vitality test. The discoloration was treated with external bleaching using 40% hydrogen peroxide. Conclusion: External Bleaching using 40% hydrogen peroxide have results in significant discoloration without changing the shape and arrangement of the patient’s teeth

Abstract

The permanent first molar is a very important tooth in the oral cavity because it plays a major role in chewing food, the key to occlu-sion, maintaining vertical facial dimensions, and anchors in orthodontic treatment. The purpose of this study was to determine the prevalence of caries in permanent first molar teeth. Qualitative research with descriptive observation method in cross sectional data obtained from medical records at RSGMP Hasanuddin University from January to October 2023. Data of 906 permanent mo-lar caries were obtained from 1,397 data with a prevalence of 64.85% in the Dental Conservation Polyclinic as much as 39.74%, female gender as much as 63.35%, in the age range of 15-34 years as much as 45.03% with a diagnosis of irreversible pulpitis as much as 47.02% and root canal treatment therapy as much as 72.19%. It is concluded that the prevalence of permanent first molar caries is most prevalent in the female gender with an age range of 15-34 years at the Dental Conservation Polyclinic with a diagnosis of irreversible pulpitis and root canal treatment therapy.

Abstract

Diskolorasi gigi merupakan masalah estetik yang dapat disebabkan oleh faktor intrinsik, faktor ekstrinsik, atau kombinasi keduanya. Salah satu perawatan pada kasus diskolorisasi gigi adalah bleaching eksternal dengan keunikan perawatan pendekatan minimal invasif untuk mendapatkan hasil estetik yang baik pada diskolorasi gigi ekstrinsik. Tujuan laporan kasus ini adalah untuk menyampaikan penatalaksanaan diskolorasi gigi ekstrinsik pada pasien dengan perawatan minimal invasif. Laki-laki berusia 22 tahun datang dengan keluhan gigi depan rahang atas dan rahang bawah bawah tampak berwarna kuning dan pasien ingin memutihkan giginya. Pada gigi pasiendilakukan perawatan minimal invasif dengan prosedur bleaching eksternal teknik in-office bleaching menggunakan bahan hidrogen peroksida (H2O2) konsentrasi 40%. Penatalaksanaan pada gigi yang mengalami diskolorasi gigi ekstrinsik adalah perawatan bleaching eksternal dengan teknik in-office bleaching karena merupakan perawatan minimal invasif yang dapat memberikan hasil perubahan warna dengan prosedur yang lebih cepat.

Abstract

Root resorption is a physiological and pathological process that leads to progressive tooth damage, particularly the loss of den-tine, root cementum, and ultimately tooth loss. Generally, the process of root resorption is classified into internal and external cate-gories, with several subclassifications. Treatment for internal and external root resorption may include preventive measures such as adequate endodontic treatment, as well as therapeutic interventions, for example, the use of intra-canal materials and antibio-tics, surgery, or extraction. This study compiles existing and up-to-date knowledge on root resorption classification to facilitate sim-ple diagnosis and explains root resorption treatment based on its subclassifications in the field of dental conservation. It is con-cluded that the accuracy and early diagnosis of root resorption classification are crucial for determining appropriate treatment.

Abstract

ABSTRAKPendahuluan: Obliterasi saluran akar sering terjadi akibat respon dari trauma gigi. Trauma gigi dapat berasal dari akibat kecelakaan atau benturan terhadap gigi dan cedera akibat perawatan gigi seperti pada penggunaan ortodontik. Perawatan ortodontik bila tidak dilakukan secara tepat dapat menjadi salah satu pemicu kerusakan pada jaringan pulpa dan pendukung gigi. Peristiwa tersebut dapat mendorong terjadinya perubahan dimensi internal kavitas pulpa yang berdampak pada diskolorasi mahkota. Tujuan dari laporan kasus ini melaporkan keberhasilan penatalaksanaan diskolorasi gigi disertai obliterasi saluran pulpa akibat perawatan ortodontik yaitu perawatan endodontik konvensional dan internal bleaching. Laporan kasus: Seorang perempuan usia 24 tahun dirujuk ke RSGM Pendidikan dengan keluhan perubahan warna gigi insisivus sentral kiri rahang atas akibat cedera traumatik perawatan ortodontik Pemeriksaan radiografi menunjukkan adanya kalsifikasi saluran pulpa dan radiolusen difus di periapikal. Pemeriksaan vitalitas gigi (-), perkusi (+), dan palpasi (-). Rencana perawatan saluran akar yaitu perawatan endodontik konvensional dan internal bleaching dilanjutkan bleaching internal. Prognosis dari kasus ini baik. Dental trauma may arise as a result of accidents or bumps [impacts] to the teeth, as well as injuries from dental treatments  including some orthodontic appliances. Orthodontic treatment, if not done properly, can be one of the triggers for biological events that affect the pulp tissue and supporting teeth. Such events have the potential to promote changes in the internal dimensions of the pulp cavity and may result in coronal discoloration. The aim of this case report was to describe the management of tooth discoloration with pulp canal obliteration through using the method of internal bleaching. Case report: A 24-year-old female was referred to the dental hospital due to the discoloration of the clinical crown of her maxillary left central incisor. The radiographic examination revealed the calcified canal and diffused periapical radiolucency. Tooth vitality (-), palpation (-), and percussion (+). The root canal treatment was determined followed by internal bleaching. The prognosis of these cases is good. Abstract

ABSTRAKPendahuluan: Perubahan warna gigi pada sebagian besar masyarakat masih sering dikaitkan dengan gangguan estetik dan dianggap tidak sehat. Bleaching eksternal adalah salah satu upaya non-invasif untuk memperbaiki penampilan gigi yang mengalami perubahan warna, seperti kasus staining. Perubahan warna gigi akibat stain ekstrinsik bisa diakibatkan karena konsumsi minuman kaya tanin (seperti teh) yang berlebihan. Keuntungan perawatan ini adalah prosedur lebih cepat, faktor risiko dapat diminimalkan karena dilakukan di bawah kontrol dokter gigi. Efek samping peningkatan sensitivitas gigi dapat dikurangi oleh karena penggunaan desensitizer seperti kalium nitrat dan fluoride. Tujuan laporan kasus ini adalah untuk mengkaji keberhasilan bleaching eksternal pada perawatan diskolorasi ringan pada gigi anterior dan premolar. Laporan kasus: Perempuan berusia 22 tahun datang ke RSGM mengeluhkan gigi berwarna kekuningan yang berhubungan dengan kebiasaan mengkonsumsi kopi sedikitnya 5 kali dalam seminggu. Tidak ada riwayat hipersensitif dentin. Dilakukan prosedur bleaching eksternal menggunakan bahan hidrogen peroksida 40%. Hasil perawatan menunjukkan gigi anterior dan premolar mengalami peningkatan warna setelah aplikasi bahan bleaching sebanyak 2 siklus dalam 1 kali kunjungan. Simpulan: Prosedur bleaching eksternal menggunakan hidrogen peroksida 40% efektif menghilangkan diskolorasi ringan pada gigi anterior dan premolar.Kata kuncibleaching gigi, hidrogen peroksida, diskolorasi gigiSuccessful external bleaching in the treatment mild discoloration of staining anterior and premolar teeth: a case reportABSTRACTIntroduction: In the majority of societies, tooth discoloration is still often associated with aesthetic disorders and considered unhealthy. External bleaching is one of the non-invasive treatments to correct the visual appearance of the discolored teeth, such as those affected by  staining. Teeth discoloration could be caused by extrinsic stains resulting from the excessive consumption of tannin-rich drinks, such as tea. The advantages of this treatment are that it is faster, and can minimize the potential risk factors because it is under the (dentists’ supervision) control of a dentist. The side effects of the increased dental sensitivity can be reduced by using desensitizers, such as potassium nitrate and fluoride. The aim of this case report is to review the successful external bleaching in the treatment of discoloration of stained anterior and premolar teeth. Case report: A 22-year-old woman visited the dental hospital complaining of yellowish teeth associated with the habit of drinking coffee at least five times a week. There was no history of dentin hypersensitivity. The external bleaching was performed using 40% hydrogen peroxide. The treatment outcomes showed that the anterior and premolar teeth had improved color after two-bleaching cycles in one visit. Conclusion: The external bleaching procedure using 40% hydrogen peroxide effectively removes mild discoloration on the anterior and premolar teeth.Key wordstooth bleaching, hydrogen peroxide, tooth discoloration

Abstract

ABSTRAKPendahuluan: Perawatan saluran akar (PSA) satu kali kunjungan harus memenuhi prinsip triad endodontic. Keuntungan PSA satu kali kunjungan adalah mengurangi resiko kontaminasi mikroorganisme, waktu lebih singkat tanpa mengurangi kualitas hasil perawatan. Tujuan laporan kasus ini adalah membahas PSA satu kali kunjungan pada gigi molar pertama kanan rahang bawah dengan restorasi akhir inlay porselen. Laporan Kasus: Seorang perempuan berusia 20 tahun datang ke RSGM dengan keluhan gigi geraham rahang bawah berlubang dan sering sakit. Tes vitalitas positif, palpasi negatif, perkusi negatif. Pemeriksaan radiografi terlihat karies sampai ruang pulpa, ligamen periodontal dalam batas normal. Dilakukan perawatan saluran akar satu kali kunjungan.  Prosedur awal adalah isolasi menggunakan rubber dam. Pembukaan akses dilakukan menggunakan endo access diamond bur. Panjang saluran kerja ditentukan menggunakan apex locator dan dikonfirmasi dengan radiografi. Cleaning dan shaping dilakukan dengan rotary file Protaper Next. Saluran akar diirgasi dengan larutan NaoCl 5,25%, EDTA 17%, dan Klorheksidin 2% yang diselingi dengan aquades. Ketiga saluran akar di obturasi dengan gutta percha dan sealer berbahan resin dengan metode single cone. Restorasi akhir dengan inlay porcelain. Simpulan: Perawatan saluran akar satu kali kunjungan pada gigi molar pertama kanan rahang bawah memperlihatkan keberhasilan yang memuaskan secara klinis dan radiografis pada kontrol 4 minggu setelah perawatan.Kata kunci: perawatan saluran akar satu kali kunjungan, molar pertama kanan rahang bawahManagement of one visit endodontic treatment of the right lower first molar: Case reportABSTRACTIntroduction: One visit endodontic treatment should comply with triad endodontic principles. The advantages of the treatment are the minimum microorganism contamination risk and time-saving without compromising the treatment outcome quality. The aim of this case is to discuss a one visit endodontic treatment of the right lower first molar with final restoration inlay porcelain. Case report : A 20-year-old female came to RSGM with a chief complaint of a toothache and pain in her right lower first molar. Objective examination showed vitality positive, palpation negative, percussion negative. Radiographic examination showed caries into the pulp chamber and the periodontal ligament was within normal limits. Performed one visit endodontic treatment.The initial procedure was isolating by rubber dam. Access opening using endo access diamond bur. Determining working length using apex locator and confirmed by radiography. Cleaning and shaping was done with rotary file Protape Next. Naocl 5,25, 17% EDTA, and Chlorhexidine 2% being used for irrigation. Obturation with gutta-percha and resin-based sealer by single cone method. Final restoration with inlay porcelain. Conclusion: One-time root canal treatment of the right lower first molar performed showed clinical and radiological success at the period control 4 weeks after treatment.Key words: One visit endodontic treatment, right lower first molar.

Abstract

ABSTRACTIntroduction: The incidence of anterior dental trauma in young adults is high. The impact of this has been previously reported that pulp necrosis occurs in 50% of young fixed anterior teeth so that the process of apex formation is disrupted and in some cases can cause a blunderbuss shape at the tooth root apex. Root canal treatment of blunderbuss teeth accompanied by dens invaginatus poses a challenge for a dentist. Aims of this case report to explain successful root canal treatment of a blunderbuss canal in the maxillary lateral incisor with dens invaginatus with apical plug application using MTA followed by esthetic rehabilitation. Case report: A 25-year-old female presented to RSKGM with a complaint of pain and swelling of the gingiva. History of trauma at the age of the patient was 10 years old without treatment. Clinical examination revealed a complete crown with dens invaginatus. Radiographic examination showed a periapical lesion and an open apex blunderbuss shape. Treatment performed was treatment with an apical plug using MTA and restoration using a peg crown. Shape root canal with a blunderbuss is an open apex root canal with the root apex diameter greater than the coronal diameter, hence requiring adequate material to form a cap over the root apex (apical plug). It has been reported that MTA is an apical plug material that has good sealing ability, biocompatibility, bacteriostatic activity and the ability to regenerate periradicular tissues. Perawatan saluran akar gigi blunderbuss yang disertai dengan dens invaginatus menimbulkan tantangan tersendiri bagi seorang dokter gigi. Tujuan laporan kasus ini melaporkan keberhasilan perawatan saluran akar gigi blunderbuss pada gigi insisivus lateral rahang atas dengan dens invaginatus dengan aplikasi apical plug menggunakan MTA yang dilanjutkan dengan rehabilitasi estetik. Laporan kasus: Seorang perempuan, 25 tahun datang ke RSKGM dengan keluhan nyeri dan pembengkakan pada gingiva. Riwayat trauma pada saat usia pasien 10 tahun tanpa dilakukan perawatan. Pada pemeriksaan klinis terlihat mahkota lengkap dengan dens invaginatus. Pemeriksaan radiografi menunjukkan lesi periapikal dan apeks terbuka berbentuk blunderbuss. Perawatan yang dilakukan adalah perawatan dengan apical plug menggunakan MTA dan restorasi menggunakan mahkota pasak. Bentuk saluran akar dengan blunderbuss adalah saluran akar apeks terbuka dengan diameter apeks akar lebih besar dari diameter koronal, oleh karena itu membutuhkan bahan yang adekuat untuk membentuk menutup ujung apeks akar (apical plug). Telah dilaporkan bahwa MTA adalah bahan apikal plug yang memiliki kemampuan sealing yang baik, biokompatibel, aktivitas bakteriostatik dan kemampuan meregenerasi jaringan periradikular. Abstract

ABSTRAK Pendahuluan: Keberhasilan endodontik diperoleh dengan persiapan akses yang baik, mendapatkan setiap saluran akar sesuai anatomi, pembentukan saluran akar, desinfeksi dan pengisian sistem saluran akar. Lokalisasi saluran akar diperlukan untuk desinfeksi dan obturasi saluran yang tepat. Perawatan endodontik, saluran akar yang tidak dirawat dapat berfungsi untuk menampung reservoir mikroorganisme sehingga menjadi penyebab umum dari infeksi intra radikular persisten dan periodontitis apikal pasca perawatan. Tujuan dari laporan kasus ini yaitu untuk menjelaskan penatalaksanaan perawatan ulang saluran akar molar dua kiri rahang bawah akibat adanya missed canals. Laporan kasus: Perempuan usia 25 tahun datang ke RSGMP dengan keluhan gigi belakang kiri bawah tambalan lepas sejak 1 tahun yang lalu, merasa tidak nyaman saat mengunyah atau menggigit sejak 1 bulan yang lalu. Pemeriksaan objektif tidak terdapat restorasi koronal, perkusi positif, palpasi negatif, mobilitas normal. Gambaran radiolografi menunjukkan radiolusensi periapikal, tampak saluran akar mesial yang tidak dirawat dan saluran distal yang kurang hermetis. Diagnosis berdasarkan pemeriksaan subjektif, objektif dan radiografi gigi 37 yaitu  previously treated teeth. Prevalensi missed canals paling tinggi pada molar pertama rahang atas dan pada rahang bawah yaitu molar kedua. Gigi molar kedua rahang bawah missed canals paling sering ditemukan di akar mesial umumnya disertai lesi periapikal. Laporan kasus ini, ditemukan missed canals dengan radiolusensi periapikal serta tidak adanya restorasi koronal. Metode perawatan yaitu dengan perawatan ulang saluran akar non bedah. Localization of the root canal is necessary for proper disinfection and obturation of the canal. In endodontic treatment, untreated root canals can serve to harbor a reservoir of microorganisms, thereby causing a common cause of persistent intraradicular infection and post-treatment apical periodontitis. The aim of this case report was to describe the root canal management of a missed mandibular second molar after retreatment. Case report: A 25-year-old woman came to RSGMP with complaints of a loose filling in her lower left back tooth since 1 year ago and feeling uncomfortable when chewing or biting since 1 month ago. The aim of the examination was to determine that there were no coronal recovery, positive percussion, negative palpation, or normal mobility. Radiographic images showed periapical radiolucency, untreated mesial root canals, and fewer hermetic distal canals. The diagnosis was based on a subjective, objective, and radiographic examination of 37 teeth, namely those that have been treated previously. The prevalence of missed root canals is highest in the maxillary first molars and in the mandibular second molars. Missing canals of mandibular second molar teeth are most often found in the mesial roots, generally accompanied by periapical lesions. In this case report, a missed tract was found with periapical radiolucency and no coronal recovery. The treatment method was non-surgical root canal re-treatment. Conclusion: Root canal retreatment due to missed canals gives optimal results if supported by operator skills, equipment assistance, and good cooperation with the patient. Keywords perawatan saluran akar, missed canal, periapical lesion
