Abstract

Background: Anxiety can cause insomnia or sleep disturbances. Sleep disorders are characterized by abnormal sleep patterns that interfere with daily activities. This study aims to describe the relationship between the incidence rate and the severity of insomnia in medical students in the preclinical phase of the Faculty of Medicine, Public Health, and Nursing, Universitas Gadjah Mada. Methods: This research is an observational study with a cross-sectional design. This research was conducted online and carried out from October 2020 to June 2021. Data collection was carried out for 2 weeks using Jotform as a data retrieval medium. The questionnaire used is generalized anxiety disorder-7 (GAD-7) to measure the level of anxiety and insomnia severity index to evaluate the severity of insomnia. Spearman's Rank analysis test was conducted to determine the correlation between anxiety levels and the severity of insomnia. Results: A total of 130 respondents participated in this study. The majority of respondents are women (71.53%) and are more than 20 years old. Based on Spearman's rank analysis, there was a relationship between anxiety level and insomnia severity (ρ = 0.394), and it was statistically significant with a p-value = 0.000 (p<0.05). Conclusion: There is a relationship between the level of anxiety and the severity of insomnia in medical students in the preclinical phase of FK-KMK Universitas Gadjah Mada.

Abstract

Background: Anxiety is a condition in which a person experiences concern about an unpleasant event that will occur in the future. Medical students tend to experience anxiety during their studies. This study was attributable to the differences in anxiety in second and third-year medical students at the Faculty of Medicine Nursing and Health Science, Universitas Gadjah Mada. Methods: This research is an observational study with a cross-sectional design. The research will be conducted online using a self-reported questionnaire. Respondents' level of anxiety was measured by the general anxiety disorder-7 (GAD-7) questionnaire. Analysis of the data used is using the Mann-Whitney test method. Results: Of the total 109 respondents, the majority had a mild level of anxiety, with a total of 45 people. The majority of respondents are third-year students (59%), female (76%), and over 20 years old (69%). After analyzing using the Mann-Whitney Test between the GAD-7 score and the year of study, the p-value was 0.622. Conclusion: There is no difference in the level of anxiety in the second and third-year undergraduate medical students at the Faculty of Medicine, Nursing and Health Science, Universitas Gadjah Mada.

Abstract

Abstract. Medical students are vulnerable populations at risk of mental health problems, particularly anxiety. Specifically during COVID-19, prevalence of anxiety among medical students increased rapidly and several factors affected its causes. This study aimed to find out the frequency of anxiety among medical students by using the TMAS questionnaire and factors that significantly contribute to anxiety levels. This cross-sectional study used simple random sampling of undergraduate medical students. The correlation between TMAS scores and several variables was analyzed with Spearman and Kruskall-Wallis correlation test and also multiple linear regressions. Among 275 respondents, the average age is 18.81 (SD 1.18), dominated by females (62.2%), in their first year (84.4%), non-scholarship (96.7%), lived in boarding houses (57.8%), and academic problem as the dominant cause of anxiety (46.5%). Later, 40.7% were anxious, then age and the year of study were found to be negatively correlated to their anxiety. A survey regarding the preferred form of anxiety therapy by medical students showed that 27.4% of respondents chose relaxation. These results are expected to help the development of future anxiety therapy based on the specific needs of medical students.Keywords: medical students, anxiety, demographic factors, TMAS, mental health

Abstract

Delirium tremens (DT) is a serious complication of alcohol use disorder (AUD) with a poor prognosis. Early identification of DT is essential for appropriate clinical management. Neutrophil-lymphocyte ratio (NLR) has been proposed as a potential inflammatory biomarker for DT. This study aims to evaluate the effectiveness of NLR as a marker of DT in AUD patients through meta-analysis. A literature search was conducted in the PubMed, Scopus, and Web of Science databases to identify observational studies evaluating NLR in AUD patients with and without DT. Studies that met the inclusion criteria had their data extracted and analyzed using a random effects model. Subgroup analyzes were performed based on gender, age, and AUD severity. Twenty studies (n=2850 patients) were included in the meta-analysis. Results showed that NLR was significantly higher in AUD patients with DT compared with AUD patients without DT (standardized mean difference [SMD] = 1.45; 95% confidence interval [CI] = 1.12-1.78; p < 0.001). Subgroup analyzes showed that these effects were consistent across subgroups. NLR is an effective inflammatory marker for identifying AUD patients with DT. The use of NLR can help in early diagnosis and better management of DT, thereby improving the prognosis of AUD patients.

Abstract

Introductions: Alcohol effects are not limited to one's physical state but also mental processes, including psychosis such as hallucinations or paranoia. Psychotic manifestations can also occur in general medical conditions or other neurological disorders associated with alcohol dependence. Cases: A 29-year-old man presented with a major depressive episode with psychotic symptoms. Since junior high school, the patient admitted to having a history of consuming crystal methamphetamine, heroin, and alcoholic drinks and often saw and heard the voices of 2 men. The patient was treated with Chlorpromazine for anxiety and Olanzapine for maintenance. The patient was allowed to go home with Olanzapine and scheduled for follow-up one week later. Discussions: Previous history indicates a possibility of alcohol-induced psychosis. However, specific diagnosis and management for such a condition is not possible due to no alcohol blood level tests available. Conclusions: The risk of psychotic disorders exists in all alcohol users, especially those with a history of psychosis and use of other addictive substances. A detailed history and appropriate investigations are helpful in monitoring the etiology and course of patients with alcohol-related psychosis.

Abstract

A quick and self-administrative screening tool to detect dependence on various substances is vital in the early detection of multiple substance dependence. This study aimed to validate the severity of the dependence scale Indonesian version (SDS-Ina) in a sample of clinically diagnosed substance-related disorder subjects. The study was an observational study and rapid assessment of suspected patients with drug and alcohol dependence in an outpatient psychiatric unit in Yogyakarta, Indonesia. The translated severity of dependence scale-Indonesia (SDS-Ina) questionnaire was given out to 65 subjects through an online form. The content validity of the translated SDS was assessed with Aiken’s V, and concurrent validity was analyzed in comparison to the Diagnostic of Statistical and Manual Mental Disorder IV (DSM-IV) diagnosis criteria with Pearson correlation coefficient and factor validity through principal component analysis (PCA). Reliability was assessed using Cronbach’s Alpha and test-retest reliability. Aiken’s V index concluded that our content validity was valid (all>0.75). A Pearson correlation coefficient of=0.93 (P<0.001), PCA of all items ≥0.60 further validates the SDS-Ina. Internal reliability of Cronbach’s Alpha 0.907 and test-retest reliability (intraclass correlation coefficient (ICC): all≥0.75). In conclusion, the translated SDS is a valid and reliable tool to measure the psychological dependence of Indonesian subjects involved in this study.

Abstract

Background: Schizoaffective disorder is a chronic mental health condition characterized by the presence of both psychotic and mood symptoms. Managing schizoaffective disorder during pregnancy presents unique challenges due to the potential risks of pharmacotherapy to the developing fetus. This case report describes the successful management of a pregnant woman with schizoaffective disorder (manic type) using electroconvulsive therapy (ECT). Case presentation: A 31-year-old woman, G2P1A0 with a gestational age of 26 weeks, was admitted to the psychiatric ward with symptoms of mania and psychosis, including disorganized behavior, threats of violence, suspicious delusions, auditory hallucinations, and visual illusions. Her symptoms had worsened after discontinuation of her antipsychotic medication due to pregnancy. She was diagnosed with schizoaffective disorder, manic type. Due to the potential risks of antipsychotic medication to the fetus, ECT was initiated. The patient responded well to ECT, with a significant reduction in her manic and psychotic symptoms. She was able to continue her pregnancy safely, and delivered a healthy baby at term. Conclusion: This case report highlights the effectiveness and safety of ECT in managing schizoaffective disorder during pregnancy. ECT can be considered a valuable treatment option for pregnant women with severe mental illness when the risks of pharmacotherapy outweigh the benefits.

Abstract

Schizophrenia is a severe and chronic mental disorder characterized by a neurodegenerative process. Currently, there has been no cure for schizophrenia. However, a newer significant potential for modeling and curing schizophrenia has been recognized in human-induced pluripotent stem cells (HiPSCs), which are obtained from adult somatic cell reprogramming. This study aimed to assess the potential of HiPSCs in schizophrenia by reviewing relevant scientific journals published in reputable databases such as Springer, Elsevier, Science Direct, Nature, and Pubmed, in the last 10 years. Research findings indicate that stem cells can help determine the effectiveness of antipsychotics and repair damaged neuronal cells in certain areas of the brain by transplanting programmed stem cells. The proliferative ability of neuronal cells in schizophrenia tends to decline, affecting their physiological neurogenesis. HiPSCs have the potential to return the neuronal function in schizophrenia, offering new hope in psychiatry.

Abstract

The anxiety level in medical students is higher compared to other majors, thereby necessitating effective intervention. Gadjah Mada Anxiety Intervention for Medical Students (GAMAAIMS) was developed to overcome anxiety using cognitive educational therapy. Therefore, this study aimed to determine the effectiveness of the GAMA-AIMS application by examining the differences in anxiety levels of health students before and after use. A quasi-experimental with pretests and post-tests was carried out on 86 students of Faculty of Medicine, Public Health and Nursing, Universitas Gadjah Mada. Anxiety levels were measured using the Taylor Manifest Anxiety Scale (TMAS) questionnaire and statistical analysis was carried out through a paired T-test. Before the intervention, the prevalence of mild, moderate, and severe anxiety among participants was the same, at 33.3%. However, after the intervention, only 3.03% of participants experienced moderate anxiety, followed by severe and mild, with percentages of 12.12% and 84.84%, respectively. The results of the paired Ttest showed a fairly strong relationship between anxiety levels before and after the intervention (Cohen’s d coefficient = 1.04) which was statistically significant with a p-value of .000001 (p < .05). Therefore, the GAMA-AIMS mobile application was effective in reducing the anxiety levels of medical students.

Abstract

Background Breast cancer is the most frequently diagnosed cancer worldwide, including in Indonesia, with the highest cancer survivors in Central Java. Mastectomy is one of the procedures in breast cancer treatment which result in major physical and mental alteration, possibly causing Body Image Distress (BID), risking them for depression and suicide. Expressive Writing Therapy (EWT) is an intervention that has been proven to reduce BID in breast cancer patients. Objective: This study aimed to analyze the impact of providing EWT on reducing post-mastectomy breast cancer patients Methods This is an experimental study using a two-group pretest-posttest with CG design conducted on 46 respondents at the oncology surgery clinic at a General Hospital in Central Java. Sample obtained using purposive sampling technique. BID scores were measured using the Body Image Scale (BIS). Data were analyzed in univariate, bivariate, and multivariate analysis using SPSS. Results Data analysis using Mann-Whitney Test showed a significant difference in BID scores reduction between intervention group (IG) and control group (CG) (p=0.00, significance p<0.05). Multivariate analysis using Linear Regression Test showed a significant influence of EWT on reducing BID scores (p=0.00). Conclusions Expressive Writing Therapy (EWT) significantly reduces BID scores in post-mastectomy breast cancer patients in a General Hospital in Central Java, Indonesia.
