Abstract

Latar Belakang: Serumen merupakan kombinasi dari sekresi kelenjar minyak dan kelenjar keringat yang melindungi liang telinga dari air, trauma, benda asing, serta infeksi. Kanalis akustikus eksterna memiliki mekanisme alami dalam mengeluarkan serumen dari liang telinga melalui gerakan rahang. Membersihkan telinga sendiri dengan memasukkan benda ke dalam liang telinga justru mengganggu mekanisme alami tersebut dan dapat menimbulkan gangguan telinga. Tujuan: Penelitian ini dilakukan untuk mengetahui tingkat pengetahuan dalam membersihkan telinga pada mahasiswa PSSKPD FK Unud angkatan 2019 dan 2020. Metode: Penelitian ini merupakan penelitian potong lintang deskriptif. Stratified random sampling digunakan sebagai teknik pengambilan sampel. Subjek penelitian berjumlah 102 mahasiswa yang terdiri dari masing-masing 51 mahasiswa angkatan 2019 dan 2020. Kuesioner didistribusikan secara daring untuk menilai tingkat pengetahuan dan perilaku membersihkan telinga. Data dianalisis dengan SPSS versi 20. Hasil: Hanya 16 mahasiswa (15,7%) yang memiliki tingkat pengetahuan baik dan masing-masing 43 mahasiswa (42,2%) memiliki tingkat pengetahuan cukup dan kurang. Mayoritas mahasiswa angkatan 2019 memiliki tingkat pengetahuan cukup (23,5%) sedangkan mahasiswa angkatan 2020 memiliki tingkat pengetahuan kurang (23,5%). Cotton buds merupakan alat pembersih telinga yang paling banyak digunakan (79,4%). Frekuensi pembersihan terbanyak yaitu seminggu sekali (23,5%).

Abstract

Background: The mean platelet volume (MPV) is known to serve as a platelet activation assessment system for cancer prognosis evaluation. However, its role in nasopharyngeal carcinoma (NPC) has not been studied. Purpose: To determine the correlation between MPV changes and stage progression, in undifferentiated-type NPC. Method: A retrospective cross-sectional study was conducted in Ear, Nose, Throat-Head and Neck Surgery (ENT-HNS) clinic of Sanglah General Hospital, Denpasar, from May to June 2020. Patients with undifferentiated-type NPC meeting inclusion and exclusion criteria were included. The mean MPV values were determined by calculating the difference in MPV from complete blood count tests after six chemotherapy sessions, compared to before chemotherapy. Changes in NPC staging were assessed using TNM classification pre- and post-chemotherapy. Logistic regression analysis was performed to examine the correlation between MPV changes and clinical stage changes, considering the confounding factors. Result: A total of 30 subjects were participated in this study, with mean age of 50.70±12.55 years. A significant correlation was observed between MPV changes and changes in clinical stages T (p=0.021, r=0.419), N (p=0.025, r=0.408), and M (p=0.048, r=0.364). Linear regression analysis showed a significant correlation between MPV changes and clinical stage T (p<0.001, r=0.268), clinical stage N (p=0.042, r=0.039), and clinical stage M (p=0.003, r=0.059) after adjusting for age and sex as confounding factors. Conclusion: There was a significant correlation between changes in MPV and clinical stage progression (T, N, M), in patients with undifferentiated-type nasopharyngeal carcinoma

Abstract

Squamous cell carcinoma of external auditory canal (SCC EAC) is a rare malignancy with  the incidence of  <0.2% of all malignancies of the head and neck region. This disease often mimics the features of other diseases as in chronic suppurative otitis media of the dangerous type with cholesteatoma. SCC EAC the most common malignancy pathology, reaching 90% of malignancies in the temporal bone and external auditory canal. This case report describes a patient with SCC in EAC with infiltration to intracranial. A 42 years old man with a history of discharge and blood from his left ear. Based on the history, clinical presentation and Computed Tomography (CT) Scan of the mastoid, found there was a mass in the left ear with intracranial infiltration. The results of histopathological examination obtained SCC EAC stage IV (T4N2M0) + infiltration of the cerebellum region S + non-communicating hydrocephalus. The patient already done a surgery. This case represents a rare presentation of SCC EAC with intracranial involvement in a relatively young patient (42 years), which is uncommon as most cases occur in patients aged 60-69 years. The aggressive nature with cerebellar infiltration and non-communicating hydrocephalus demonstrates the importance of early detection and multidisciplinary management. This case contributes to the limited literature on advanced SCC EAC with CNS involvement and emphasizes the need for comprehensive staging and aggressive surgical intervention.
