Abstract

Instruments which can be used to identify factors affecting the medication adherence of elderly with diabetes mellitus (DM) have not been discovered in Indonesia. The objective of this study was to develop a questionnaire to identify factors affecting elderly patients’ medication adherence. The development of the questionnaire was conducted based on published references attained through the exploration of research evidence with several keywords and databases. Content validity was conducted using experts’ assessment (two experts). Face validity was conducted in 13 subjects including seven (7) non-elderly subjects and six (6) elderly subjects. Construct validity test was conducted based on calculated R against table R while reliability test was conducted with Cronbach’s Alpha. There were 30 elderly subjects included in the construct validity. Present study produced a valid and reliable questionnaire comprising 35 questions in 14 subcategories from 4 factor domains with the calculated R = 0.357-0.788 and Cronbach’s Alpha = 0.934. The questionnaire can be used for further research throughout Indonesia. Hence, a complete overview of factors affecting elderly DM patients’ medicine consumption adherence can be obtained. The identification of factors related to the usage of DM patients’ medicine is expected to serve as a base for the government, both of state and local government, to produce policies related to health interventions to be implemented.

Abstract

Headache is a symptom which people often self-medicate and community pharmacies are often places where self-medicating patients with headache obtain the medicines. Therefore, community pharmacists played an important role in providing appropriate advice for these patients. A cross-sectional study design was used. A questionnaire consisting of (1) pharmacy and pharmacists characteristics, and (2) two vignette cases of headache (tension headache and migraine due to an oral contraceptive) was developed. Data was collected using a structured interview with community pharmacists in an East Javan district. Data was analysed using content analysis. Appropriate recommendation was determined based on the literature and expert panel opinion. The total population was 81 community pharmacists. Of these, 47 agreed to participate. Of 47 participating pharmacists, 36 (77%) and 23 (49%) pharmacists provided appropriate advice in the case of tension headache and migraine due to the use of an oral contraceptive respectively. Appropriate recommendation provided by pharmacists varied according to the cases. Optimization of pharmacists’ recommendation in a case of headache requiring medical referral is needed.

Abstract

Appropriate recommendations given by pharmacists play a critical role in determining the efficacy and safety of self-medication practices in pharmacies, including in the case of diarrhea with alarm symptoms. This observational study with a cross-sectional design aims to determine the profile and appropriateness of recommendations given by pharmacists in Surabaya when they faced self-medication requests for diarrhea with alarm symptoms. Data were collected using a questionnaire consisting of two parts, i.e., participants’ characteristics and one case of adult diarrhea with alarm symptoms. The case used was newly developed and the content had been validated. The content validation and the determination of appropriate recommendations given by pharmacists involved experts in clinical pharmacy, community pharmacy, and public health. The final data were analyzed and presented descriptively. A total of 160 pharmacists were involved in this study, the majority of whom were managing pharmacists (82.50%), female (85.50%), and have been working as pharmacists for 1 to 7 years (48.75%). Most pharmacists (56.87%) gave more than one type of recommendation, the majority of whom recommended dispensing of pharmaceutical products accompanied by a referral to physicians (21.88%). Only 42 pharmacists (26.25%) gave appropriate recommendations including referral to physicians (18.13%) or referral accompanied by pharmaceutical product (6.88%) and non-pharmacology recommendations (1.25%). Need assessment is required to identify interventions preferred by pharmacists to optimize their role in promoting responsible self-medication practices.

Abstract

Assessing the rational use of medicines or prescription quality is essential in any health system. However, data on the quality of prescribing in patients with hypertension is currently lacking. Hence, this review aimed to identify prescribing quality indicators in patients with hypertension and understand the results. A narrative review was conducted to include literature searching in three databases, including PubMed (Medline), Science Direct, and CINAHL, using keywords and Boolean Logic as follows: “(hypertension OR antihypertensive drugs OR antihypertensive medication OR antihypertensive agent) AND (quality prescription OR inappropriate prescribing OR potentially inappropriate medication OR rational prescribing)”. As a result, a total of 1889 articles were obtained. The articles were then selected based on inclusion and exclusion criteria in which 13 articles were included in this review. Based on the extracted data, explicit as well as implicit frameworks have been used to assess the quality of prescribing in patients with hypertension. Explicit approaches compared patient prescription versus guidelines (n=7 articles) or lists of potentially inappropriate use of medicines (n=3 articles). The implicit framework assessed patient prescription using two tools, including the prescription Quality Index (PQI) – 22 indicators (n=2 articles), and the prescription quality checklist – four indicators (n=1 article). The results of the quality assessment on the prescriptions showed that prescriptions categorized as good quality were between 40-80%. While varied levels of inappropriate selection of antihypertensive therapy were reported (0-65%). In addition, more than 30% of patients with hypertension experienced Potentially Inappropriate Medication (PIM); thus, prescribing among patients with hypertension needs to be optimized.

Abstract

A cross-sectional study was performed to describe self-medication practices of customers at kiosks and minimarkets in District X, Indonesia, during COVID-19 pandemic in 2021. A sample of 98 customers from 67 kiosks, and 100 customers from 52 minimarkets were interviewed using a validated questionnaire. Descriptive statistics were used to summarise the findings. Most drugs purchased at kiosks and minimarkets were general sales drugs, i.e., analgesic-antipyretic (64.3% versus 46.0%); however, prescription drug sales were reported (3.1% vs 3.0%, respectively). Kiosk and minimarket customers purchased medications mainly based on the recommendations of non-health workers (69.4% vs. 34.0%, respectively), or electronic media (36.7% vs. 43.0%, respectively). The reasons to choose kiosks or minimarkets included accessibility, personal preferences, and service quality. Most kiosk/minimarket customers had knowledge on the drug brand names and indications, but only some knew the composition (8.16% vs. 29.0%, respectively) or side effects (9.18% vs. 35.0%, respectively). These findings indicated limited quality assurance on self-medication practices at non-pharmacy outlets.

Abstract

Adherence with medication is one of the important factors in controlling blood glucose levels in patients with diabetes mellitus (DM), which in turn also determines the risk of diabetic complications. The profile and factors that influence adherence behaviour in the use of medication in elderly diabetic patients in Indonesia, especially in East Surabaya, are not yet known. The purpose of this study was to identify profiles and factors that influence the behavior of drug use adherence in elderly DM patients. A cross-sectional study was conducted in 32 Karang Werda or support groups for elderly in East Surabaya. Identification of the respondent’s level of adherence was carried out using the Adherence to Refill and Medication Scale (ARMS) questionnaire while the contributing factors towards adherence behaviour were identified using a published questionnaire. Both questionnaires have been validated in elderly people living in Surabaya. Result of adherence profile analysis was expressed in terms of percentage while determinant factors analysis was identified by using the highest mean among 14 sub-domains included in the questionnaire. In total, 281 elderly diabetic patients were recruited in this study. It was identified that 67.26% of the respondents were classified as non-adherence patients. The dominant factors that contribute to the behavior of medication adherence in elderly diabetic patients were social support, product-related factors, health workers, and emotional factors. Appropriate interventions to optimize the use of medications in elderly diabetic patients need to be identified and implemented according to the identified contributing factors.

Abstract

Diare akibat penggunaan antibiotik (antibiotic-associated diarrhea; AAD) merupakan salah satu gangguan klinis yang seringkali terjadi pada anak dan perlu mendapat intervensi dari dokter untuk mengatasi masalah tersebut. Dengan mempertimbangkan bahwa swamedikasi seringkali menjadi pilihan masyarakat ketika menghadapi kasus diare, apoteker di komunitas memiliki peran penting dalam mengarahkan masyarakat ke dokter untuk mengatasi masalah terkait AAD. Tujuan penelitian ini adalah untuk mengetahui jenis dan ketepatan rekomendasi apoteker dalam menanggapi permintaan swamedikasi terkait kasus AAD pada anak. Penelitian ini merupakan penelitian potong lintang yang dilakukan di wilayah Timur kota Surabaya. Sebuah kuesioner yang berisi pertanyaan terkait karakteristik peserta dan sebuah kasus digunakan pada proses pengambilan data. Validasi isi dari kasus serta penentuan kunci jawaban dilakukan melalui diskusi yang melibatkan pakar farmasi klinis, farmasi praktis, dan kesehatan masyarakat. Total terdapat 84 apoteker terlibat dalam penelitian ini; response rate 38,71%. Pemberian rekomendasi produk obat baik dengan maupun tanpa rujukan ke dokter atau saran non-farmakologi diberikan oleh 75 (89,29%) partisipan dan jenis obat yang paling sering direkomendasikan adalah probiotik, kaolin-pektin, domperidon, attapulgit. Sebanyak 26 apoteker (30,95%) memberikan rekomendasi yang tepat, yaitu: rujuk dokter segera dengan atau tanpa disertai rekomendasi lain. Hasil penelitian ini mengindikasikan perlunya intervensi untuk mengoptimalkan pemberian rekomendasi apoteker komunitas pada kasus AAD anak.

Abstract

Salah satu tujuan dalam kurikulum pendidikan farmasi adalah mempersiapkan mahasiswa agar mampu memberikan rekomendasi terkait gangguan kesehatan ringan di komunitas. Sampai saat ini, informasi terkait kemampuan mahasiswa dalam memberikan rekomendasi di Indonesia belum ditemukan dalam literatur terpublikasi. Penelitian ini bertujuan untuk mengidentifikasi jenis dan ketepatan rekomendasi mahasiswa prodi apoteker di sebuah institusi pendidikan farmasi saat menghadapi permintaan swamedikasi pada kasus nyeri pinggang (low-back pain; LBP). Pengambilan data pada penelitian potong lintang ini dilakukan dengan menggunakan kuesioner yang terdiri dari karakteristik partisipan dan sebuah kasus LBPyang diadaptasi dari pustaka terpublikasi. Ketepatan rekomendasi ditetapkan melalui diskusi pakar dan bukti penelitian digunakan sebagai dasar dalam mendefinisikan rekomendasi yang tepat. Data karakteristik partisipan dan ketepatan rekomendasi dianalisis secara deskriptif. Total terdapat 86 partisipan terlibat dalam penelitian ini (response rate 82,69%). Sebagian besar partisipan (91,86%) memberikan rekomendasi obat, dan 61 dari antaranya merekomendasikan obat golongan anti-inflamasi non-steroid (AINS; baik oral maupun topikal). Sebanyak 70,93% partisipan memberikan rekomendasi tepat, yaitu: obat analgesik golongan AINS dan topikal counter-irritantsdengan/tanpa rekomendasi lainnya. Penelitian ini menunjukkan partisipan dalam penelitian ini mampu memberikan rekomendasi yang tepat dalam menanggapi kasus LBP. Namun demikian, penelitian lanjutan untuk mengeksplorasi penyebab ketidaktepatan rekomendasi pada sebagian mahasiswa diperlukan sebagai upaya perbaikan aktivitas pembelajaran dan kurikulum pendidikan

Abstract

ABSTRACT Drug logistics management is a series of activities related to planning, storing, distributing, and disposing of drugs that are managed optimally to ensure the right amount and type of drugs and medical supplies. This study analyzes drug logistics management at the District/City Health Service Pharmacy Installation. Ten articles were retrieved through Google Scholar to conduct a literature review using drug logistics, pharmaceutical installation, and health service. The search results show that the implementation of logistics management is directly related to several problems, including human resources, facilities and infrastructure, planning, storage, distribution, recording, and reporting of drugs. All health services, especially pharmacy, must meet the requirements of drug logistics management. From the literature review results, if implemented properly, drug logistics management at the district/city health service pharmacy installation can increase the efficiency and productivity of services in health facilities Keywords: Disease Drug Logistics, Pharmacy Installation, Health Service ABSTRAK Manajemen logistik obat adalah rangkaian kegiatan yang berkaitan dengan perencanaan, penyimpanan, pendistribusian, dan penghapusan obat yang dikelola secara optimal untuk memastikan jumlah dan jenis obat dan perbekalan kesehatan yang tepat. Studi ini menganalisis pengelolaan manajemen logistik obat di Instalasi Farmasi Dinas Kesehatan Kabupaten/Kota. Untuk melakukan peninjauan literatur, sepuluh artikel diambil melalui Google Scholar dengan  kata kunci logistik obat, instalasi farmasi, dan dinas kesehatan. Hasil penelusuran menunjukkan bahwa penerapan manajemen logistik terkait langsung dengan beberapa masalah, termasuk sumber daya manusia, sarana dan prasarana, perencanaan, penyimpanan, pendistribusian, pencatatan, dan pelaporan obat. Semua layanan kesehatan, terutama kefarmasian, harus memenuhi persyaratan manajemen logistik obat. Dari hasil literatur review dapat diambil kesimpulan yaitu manajemen logistik obat di instalasi farmasi dinas kesehatan kabupaten/kota dapat meningkatkan efisiensi dan produktivitas pelayanan di fasilitas kesehatan jika diterapkan dengan baik. Kata Kunci: Logistik Obat, Instalasi Farmasi, Dinas Kesehatan

Abstract

Introduction: Healthcare for patients in pasung is often neglected. Limited data is available on the roles of Primary Health Centres (PHCs) as the main providers of primary care in Indonesia. This research aims to explore health services provided for patients in pasung at PHCs and to identify the roles of pharmacists. Methods: This descriptive qualitative research used in-depth interviews with a range of stakeholders [i.e., patients in pasung (n=14) or the representative family caregivers (n=12), mental health nurses (n=5), pharmacists (n=5)] at 5 PHCs in Trenggalek, Indonesia from May to July 2023. Inductive thematic analysis was used to address the research questions. Ethical approval was obtained from the Research Ethics Committee of Universitas Surabaya (number 130/KE/V/2023), and participants provided written informed consent. Results: Patients in pasung have generally had adequate access to PHCs with no charge for their treatments. Health services provided for patients in pasung at PHCs were categorised into two main themes: outpatient services (i.e., medication supply, and home-care), and referral to inpatient services. Pharmacists’ roles have largely related to drug supply, while clinical pharmacy services have been very limited (e.g., providing drug information when patients obtained their medications at the pharmacy units). Conclusion: In conclusion, findings in this study have provided a baseline for future development of mental healthcare for patients in pasung. Pharmacists have the potential to take up more clinical roles to ensure quality use of medicines among patients in pasung. Supporting policy and resources should be allocated for PHCs to expand pharmacist’s services.
