Abstract

Pending claims are one of the crucial problems often faced by hospitals in the National Health Insurance (JKN) financing system. This condition not only causes disruptions to hospital cash flow but also reflects weaknesses in the claims management system administratively, technically, and systemically. This study aims to identify and map various factors causing pending BPJS Kesehatan claims in Indonesian hospitals through a scoping review approach. The review process was carried out with reference to the Arksey and O'Malley framework and the PRISMA-ScR guidelines, using literature sources from Google Scholar, Garuda, PubMed, and Neliti databases. Article selection was conducted systematically for publications from 2022–2024, in Indonesian, available in full-text form, and indexed at least SINTA 3. Of the 268 articles identified, 10 met the inclusion criteria and were further analyzed. The results of the synthesis show that the causative factors of pending claims can be grouped into five main themes: inaccuracy in coding diagnosis and procedures, incompleteness of claim documents, limitations of information systems, low competence of human resources, as well as inconsistencies in the implementation of SOPs and weak coordination between units. This study concludes that pending claims are multifactorial and interrelated, requiring comprehensive intervention in the form of strengthening human resource capacity, optimizing the digital claims system, and improving hospital claims policies and governance systemically.

Abstract

ABSTRACT This literature review suggests that clean water is a significant factor in nutrition-related health problems. A study reports that only 20% of Indonesians have access to clean water, mainly in urban areas, indicating the influence of various socioeconomic factors. This literature review aims to analyze the theoretical effect of clean water consumption on stunting incidence in children from low-income families. Articles from three databases: Sage Journals, Science Direct, and Pubmed are used in this literature review. The search focused on keywords ‘children under five’, ‘low-income family’, ‘clean water consumption’, ‘stunting’, and ‘nutritional status’, using Boolean operators ‘AND’ and ‘OR’. Eight articles were reviewed and showed that clean water consumption impacts the incidence of stunting in children, particularly those from low socioeconomic backgrounds. Key findings include: 1) Poor nutritional outcomes in children associated with inadequate water intake practices; 2) The association between low income, education levels, and child stunting; and 3) The significance of appropriate sanitation facilities to support children's nutritional status. The literature review results indicate that stunting is prevalent in children lacking access to good sanitation and living in undereducated families with low-income levels. It concludes that consumption of clean water affects stunting incidence in low-income children. The literature review recommends providing nutrition education for parents in low-income households and improving sanitation facilities in areas where stunting is prevalent. Keywords: Stunting, Nutritional Status, Clean Water Consumption, Low Income Family ABSTRAK Studi literature review ini menjelaskan bahwa air bersih adalah faktor signifikan dalam masalah kesehatan terkait gizi. Sebuah studi menyatakan akses air bersih di Indonesia hanya mencapai 20% dan didominasi oleh daerah perkotaan, yang mengindikasikan adanya perbedaan tingkat sosial ekonomi. Literatur ini bertujuan untuk menganalisis pengaruh konsumsi air bersih terhadap kejadian stunting pada anak yang berasal dari keluarga berpendapatan rendah. Metode kajian literatur dilakukan menggunakan tiga database: Sage Journals, Science Direct, dan Pubmed. Kata kunci untuk pencarian artikel adalah ‘children under five’, ‘low income family’,’clean water consumption’,’stunting’, dan ‘nutritional status’ dengan ‘AND’ dan ‘OR’ sebagai Boolean operator. Hasil dari kajian literatur terhadap delapan artikel menemukan bahwa konsumsi air bersih memiliki pengaruh terhadap kejadian stunting pada anak, khususnya anak yang berasal dari keluarga dengan status sosial ekonomi rendah. Hasil kajian artikel menemukan: 1) Rendahnya status gizi pada anak dengan praktik konsumsi air bersih yang kurang; 2) Kaitan pendapatan rendah, tingkat Pendidikan, dan kondisi stunting pada anak; dan 3) Pentingnya fasilitas sanitasi yang memadai untuk menunjang status gizi anak. Hasil kajian literatur mengindikasikan bahwa stunting lebih prevalen pada anak tanpa akses sanitasi yang baik dan berasal dari keluarga berpendidikan dan berpendapatan rendah. Kesimpulan dari kajian literatur adalah konsumsi air bersih berpengaruh terhadap kejadian stunting pada anak yang berasal dari keluarga berpendapatan rendah. Rekomendasi yang diberikan berupa edukasi gizi pada orang tua di keluarga berpendapatan rendah dan perbaikan fasilitas sanitasi di daerah dengan prevalensi stunting yang tinggi. Kata Kunci: Stunting, Status Gizi, Konsumsi Air Bersih, Anak, Keluarga Berpendapatan Rendah

Abstract

Pendahuluan : Gagal ginjal kronik (GGK) stadium akhir memerlukan terapi hemodialisis sebagai pengganti fungsi ginjal yang hilang.Meskipun terapi ini efektif dalam memperpanjang hidup, pasien sering menghadapi penurunan kualitas hidup yang signifikan, baik secara fisik, psikologis, sosial, maupun spiritual. Pendekatan holistik dalam pelayanan kesehatan di rumah sakit berfokus pada pemenuhan kebutuhan pasien secara menyeluruh, mencakup aspek fisik, emosional, sosial, dan spiritual. Metode : pencarian literatur dilakukan dengan sistematis di basis data PubMed, Springerlink, dan Scopus dengan kriteria inklusi : artikel berbahasa inggris, akses terbuka dan relevan. Sebanyak 10 artikel memenuhi syarat untuk dianalisis dan diekstraksi. Hasil : dari 10 artikel yang disertakan, studi menunjukkan bahwa intervensi seperti terapi suportif, dukungan keluarga, dan perawatan spiritual dapat meningkatkan kualitas hidup pasien hemodialisis secara signifikan. Terapi suportif yang melibatkan sistem edukatif, intervensi psikologis, dan ulasan hidup telah terbukti efektif dalam meningkatkan kualitas hidup pasien yang menjalani hemodialisis. Kesimpulan : Penting untuk mengetahui faktor predictor yang mempengaruhi kualitas hidup pasien hemodialisa agar dapat dilakukan tindakan preventif untuk mencegah perburukan kualitas hidup pasien.

Abstract

Diabetes mellitus is a catastrophic disease that burdens the financing of the National Health Insurance or Jaminan Kesehatan Nasional Program, and sufferers continue to increase. Specific participant segments still widely use access for participants with diabetes mellitus to utilise health services at advanced-level referral health facilities. Various factors also influence access to health services at advanced-level referral health facilities. This research aims to analyse utilisation based on membership type and examine the factors that affect the utilisation of advanced outpatient services by JKN participants with DM. This research uses secondary data from the 2022 DM contextual BPJS Health sample data. The data was analysed univariately, bivariately, and multivariately with the Zero Inflated Negative Binomial regression model. The results of statistical tests showed that in outpatient care, age, type of participant, gender, treatment class, and province of participants were significantly different in terms of health service utilisation. The influence of the type of participation in the JKN program significantly affects the utilisation of services in outpatient care; the utilisation of non-recipients of contribution assistance participants is 1.16 times greater than that of the recipient of contribution assistance participants in outpatient care.

Abstract

Fraud in the implementation of the National Health Insurance program poses a significant challenge to hospital governance in Indonesia, resulting in financial losses and eroding public trust in healthcare services. We conducted a scoping review to identify the root causes of fraud and summarize prevention strategies applied in hospitals participating in the scheme. The review analyzed literature published between 2022 and 2024, selected through database searches and manual screening, with a focus on fraud-related issues and prevention efforts within healthcare facilities. Findings reveal that multiple factors, including discrepancies between case-based payment tariffs and the actual cost of services, weak internal control systems, limited understanding among staff regarding the accountability of public funds, inadequate reporting mechanisms, and poor organizational ethics, drive fraud. Identified prevention strategies include the establishment of anti-fraud teams, implementation of internal audits, utilization of hospital information systems, staff training initiatives, and intersectoral collaboration. In conclusion, fraud prevention in national health insurance requires a comprehensive approach encompassing institutional policies, human resource capacity building, and the reinforcement of transparent and accountable governance across hospital management systems.

Abstract

Indonesia is facing a significant increase in its elderly population, requiring effective and sustainable integrated geriatric services, one of which is through the implementation of Integrated Care for Older People (ICOPE) to assess intrinsic capacity decline combined with the Ina-FRAIL questionnaire to measure frailty in the elderly. This study aims to assess the condition of frailty in older adults through the ICOPE approach in integrated geriatric services at Humana Prima Hospital in Bandung using a quantitative design with an experimental approach. The assessment was conducted using the ICOPE and Ina-FRAIL instruments to monitor intrinsic capacity and the level of frailty in older adults. The results showed a prevalence of frailty impairment (50%), mobility limitations (56%), decreased appetite (46%), visual impairment (56%), hearing impairment (23%), and loss of interest or symptoms of depression (73%), with significant differences between the control and intervention groups before and after the implementation of ICOPE, as well as a meaningful relationship between frailty levels and geriatric syndromes. In conclusion, the combination of ICOPE and Ina-FRAIL is effective in comprehensively detecting geriatric syndromes and frailty in the elderly, so it is recommended that its application be expanded to primary health care facilities through training of health workers and integration of clinical follow-up to improve the quality of life of the elderly.
