Abstract

Imidafenacin is a new antimuscarinic agent in Indonesia that was recently approved in 2019 to treat overactive bladder (OAB) symptoms, including urinary incontinence, urinary frequency, and urinary urgency. As a new OAB medication in the Indonesian commercial market, a comprehensive review on the efficacy, safety, and cost of imidafenacin are essentially needed to optimize the responsible use of this drug, particularly in the era of universal coverage implementation in Indonesia. This review aimed to describe the profile of imidafenacin and to compare the efficacy, safety, and cost of imidafenacin with the other antimuscarinic drugs in Indonesia, and the guideline recommendations towards when to use imidafenacin for OAB treatment. Only results from systematic reviews and meta-analyses were used in the efficacy and safety comparison. Our review found that the efficacy of imidafenacin was relatively comparable to the other antimuscarinic drugs in Indonesia, including solifenacin, tolterodine, dan propiverine. Also, a better safety profile was found in the imidafenacin group, in particular, imidafenacin caused less dry mouth and constipation. This could be attributable to the higher affinity of imidafenacin to the M3 receptor resulting in less antimuscarinic effect at the other receptors. From the cost perspective, the cost of medication by using imidafenacin was relatively lower compared to the other antimuscarinic drugs in Indonesia. It, therefore, could be suggested to use imidafenacin as a potential agent for OAB.

Abstract

Education is a critical component that cannot be separated from an individual’s life. One of many important factors that could support, encourage, and give zeal in the learning process is motivation. The COVID-19 pandemic nowadays indirectly triggers a rapid and drastic change in the learning process, one of them being lecture that must be held online. This repeated cross-sectional research aimed to give a description regarding the fluctuation of students’ academic motivation level before, in the beginning, and until one-year after online learning. The data used in this study are primary and secondary data which are collected gradually and analyzed using Summary Independent Sample T-Test. The result of this research showed no significant differences in academic motivation before COVID-19 and during the early days of Learning From Home (µbefore = 4.777; µbeginning = 4.808; sig = 0.432), nevertheless, a significant decrease is observable one year after the online learning (µnow = 4.345; sigbefore-now = <0.001; sigbeginning-now = <0.001). The decrease in students’ academic motivation is certainly a crucial problem that is prioritized to be resolved. Hence, lecturers need to adapt with this change and create learning innovation in order to trigger students’ learning motivation.

Abstract

Providing appropriate yet timely treatment to stop bleeding is the key pathway in the management of acute variceal haemorhage (AVH). Several agents can be used to manage bleeding, and one of the new agent recently approved in Indonesia in 2019 is Terlipressin. Information toward the efficacy dan safety of Terlipressin is essentially needed to help optimising effort to use this agent responsibly, specifically in this early phase of entering Indonesian market. This narrative review mainly includes the basic profile, the current updated evidence of the efficacy and safety, the position in the therapy, and the cost of Terlipressin compared with other vasoactive agents. Only systematic review and meta-analysis articles were used to describe the efficacy and safety of Terlipressin considering it were ranked at the top level of evidence-based ladder. Some unique pharmacokinetics and pharmacodynamics properties of Terlipressin, which are not found in other vasoactive, can provide advantages when it is administered to patients in the clinical practice. However, it does not necessarily mean that Terlipression is more superior compared to other vasoactive agents. Findings of our review on three systematic review and meta-analysis found that the efficacy and safety of Terlipressin were not significantly different compared to other vasoactive agents. Therefore, Terlipressin is not the only recommended agent to be used as the first-line vasoactive agent in the management of AVH. Owing to it is relatively new introduced in Indonesia, the cost of using Terlipressin is relatively higher compared to other first-line vasoactive agent. It therefore could be suggested to use Terlipressin only for patients who can attain benefit at most such as patients with contraindications or history of side effect from other vasoactive agents

Abstract

Salah satu tujuan dalam kurikulum pendidikan farmasi adalah mempersiapkan mahasiswa agar mampu memberikan rekomendasi terkait gangguan kesehatan ringan di komunitas. Sampai saat ini, informasi terkait kemampuan mahasiswa dalam memberikan rekomendasi di Indonesia belum ditemukan dalam literatur terpublikasi. Penelitian ini bertujuan untuk mengidentifikasi jenis dan ketepatan rekomendasi mahasiswa prodi apoteker di sebuah institusi pendidikan farmasi saat menghadapi permintaan swamedikasi pada kasus nyeri pinggang (low-back pain; LBP). Pengambilan data pada penelitian potong lintang ini dilakukan dengan menggunakan kuesioner yang terdiri dari karakteristik partisipan dan sebuah kasus LBPyang diadaptasi dari pustaka terpublikasi. Ketepatan rekomendasi ditetapkan melalui diskusi pakar dan bukti penelitian digunakan sebagai dasar dalam mendefinisikan rekomendasi yang tepat. Data karakteristik partisipan dan ketepatan rekomendasi dianalisis secara deskriptif. Total terdapat 86 partisipan terlibat dalam penelitian ini (response rate 82,69%). Sebagian besar partisipan (91,86%) memberikan rekomendasi obat, dan 61 dari antaranya merekomendasikan obat golongan anti-inflamasi non-steroid (AINS; baik oral maupun topikal). Sebanyak 70,93% partisipan memberikan rekomendasi tepat, yaitu: obat analgesik golongan AINS dan topikal counter-irritantsdengan/tanpa rekomendasi lainnya. Penelitian ini menunjukkan partisipan dalam penelitian ini mampu memberikan rekomendasi yang tepat dalam menanggapi kasus LBP. Namun demikian, penelitian lanjutan untuk mengeksplorasi penyebab ketidaktepatan rekomendasi pada sebagian mahasiswa diperlukan sebagai upaya perbaikan aktivitas pembelajaran dan kurikulum pendidikan

Abstract

Diare akibat penggunaan antibiotik (antibiotic-associated diarrhea; AAD) merupakan salah satu gangguan klinis yang seringkali terjadi pada anak dan perlu mendapat intervensi dari dokter untuk mengatasi masalah tersebut. Dengan mempertimbangkan bahwa swamedikasi seringkali menjadi pilihan masyarakat ketika menghadapi kasus diare, apoteker di komunitas memiliki peran penting dalam mengarahkan masyarakat ke dokter untuk mengatasi masalah terkait AAD. Tujuan penelitian ini adalah untuk mengetahui jenis dan ketepatan rekomendasi apoteker dalam menanggapi permintaan swamedikasi terkait kasus AAD pada anak. Penelitian ini merupakan penelitian potong lintang yang dilakukan di wilayah Timur kota Surabaya. Sebuah kuesioner yang berisi pertanyaan terkait karakteristik peserta dan sebuah kasus digunakan pada proses pengambilan data. Validasi isi dari kasus serta penentuan kunci jawaban dilakukan melalui diskusi yang melibatkan pakar farmasi klinis, farmasi praktis, dan kesehatan masyarakat. Total terdapat 84 apoteker terlibat dalam penelitian ini; response rate 38,71%. Pemberian rekomendasi produk obat baik dengan maupun tanpa rujukan ke dokter atau saran non-farmakologi diberikan oleh 75 (89,29%) partisipan dan jenis obat yang paling sering direkomendasikan adalah probiotik, kaolin-pektin, domperidon, attapulgit. Sebanyak 26 apoteker (30,95%) memberikan rekomendasi yang tepat, yaitu: rujuk dokter segera dengan atau tanpa disertai rekomendasi lain. Hasil penelitian ini mengindikasikan perlunya intervensi untuk mengoptimalkan pemberian rekomendasi apoteker komunitas pada kasus AAD anak.

Abstract

Frailty and drug related problems (DRPs) are common among elderly who lives in the nursing home. There is currently limited information available regarding the profile of frailty and DRPs among elderly living in the nursing homes in Indonesia. This project aims to describe the profile of frailty and DRPs among nursing home residents in Surabaya. This cross-sectional study was conducted in three nursing homes in Surabaya. Integrated Systematic Care for older People (ISCOPE) questionnaire was used to identify the frailty profile among participants. Beers Criteria, screening tool of older person’s prescriptions (STOPP), geriatric dosage handbook and drug interactions analysis and management were used as tools to identify and analyse drug related problems in this study. Several DRPs were identified in this study including inappropriate drug choices, drugs interaction, and inappropriate dose of drugs. In total, 68 elderly were recruited in this study and 41.18% of them were classified as frail older people. There was no drugs interaction with “clinical significance” being identified, however, inappropriate dose of drugs and potentially inappropriate drug choices were found in 58.82% and 19.12% of nursing home residents, respectively. Findings regarding the profile of frailty and DRPs among elderly may indicate the needs to implement an appropriate intervention strategic in order to optimize the use of medications among nursing homes-dwelling elderly people.

Abstract

Appropiate recommendation provided by pharmacists is considered as a crucial factors to prevent morbidity and mortality among children with acute diarrhea in the community. This study aimed to determine the type and the appropriateness of recommendations provided by the community pharmacists in the eastern part of Surabaya to children presenting with acute diarrhea. This was cross-sectional study conducted by using a questionnaire consisting of questions about participants’ characteristics and a case of acute diarrhea in children without complications and other “alarm symptoms” requiring medical referral. The appropriate recommendation for the case was to give a combination of oral rehydration solution (ORS) and zinc with or without other recommendations. Data were analyzed descriptively using SPSS version 22. A total of 84 pharmacists provided consent to be participants in this study. The majority of participants (73,81%) were pharmacists manager and more than 50% of them completed pharmacist professional degree between 2010 and 2019. Type of pharmacists’ recommendations were further classified as: medical referral, provision of medicine, laboratory testing, and non-pharmacology treatment. The most provided recommendations were provision of medicine (97,62%) with or without other recommendations. Medical referral were recommended by 22 pharmacists (26,19%). Of the total participants, 13,09% provided appropriate recommendations. Findings of this study indicate the necessity to optimise the role of community pharmacists in managing acute diarrhea in children. Further study to identify the needs of community pharmacists, either conducted with qualitative or quantitative approach, is required as the key step before implementing further intervention.

Abstract

Headache is a symptom which people often self-medicate and community pharmacies are often places where self-medicating patients with headache obtain the medicines. Therefore, community pharmacists played an important role in providing appropriate advice for these patients. A cross-sectional study design was used. A questionnaire consisting of (1) pharmacy and pharmacists characteristics, and (2) two vignette cases of headache (tension headache and migraine due to an oral contraceptive) was developed. Data was collected using a structured interview with community pharmacists in an East Javan district. Data was analysed using content analysis. Appropriate recommendation was determined based on the literature and expert panel opinion. The total population was 81 community pharmacists. Of these, 47 agreed to participate. Of 47 participating pharmacists, 36 (77%) and 23 (49%) pharmacists provided appropriate advice in the case of tension headache and migraine due to the use of an oral contraceptive respectively. Appropriate recommendation provided by pharmacists varied according to the cases. Optimization of pharmacists’ recommendation in a case of headache requiring medical referral is needed.

Abstract

Appropriate recommendations given by pharmacists play a critical role in determining the efficacy and safety of self-medication practices in pharmacies, including in the case of diarrhea with alarm symptoms. This observational study with a cross-sectional design aims to determine the profile and appropriateness of recommendations given by pharmacists in Surabaya when they faced self-medication requests for diarrhea with alarm symptoms. Data were collected using a questionnaire consisting of two parts, i.e., participants’ characteristics and one case of adult diarrhea with alarm symptoms. The case used was newly developed and the content had been validated. The content validation and the determination of appropriate recommendations given by pharmacists involved experts in clinical pharmacy, community pharmacy, and public health. The final data were analyzed and presented descriptively. A total of 160 pharmacists were involved in this study, the majority of whom were managing pharmacists (82.50%), female (85.50%), and have been working as pharmacists for 1 to 7 years (48.75%). Most pharmacists (56.87%) gave more than one type of recommendation, the majority of whom recommended dispensing of pharmaceutical products accompanied by a referral to physicians (21.88%). Only 42 pharmacists (26.25%) gave appropriate recommendations including referral to physicians (18.13%) or referral accompanied by pharmaceutical product (6.88%) and non-pharmacology recommendations (1.25%). Need assessment is required to identify interventions preferred by pharmacists to optimize their role in promoting responsible self-medication practices.
