Abstract

Ginekomasti adalah pembesaran payudara laki-laki yang mengacu pada proliferasi komponen duktal. Ditemukan kasus ginekomasti yang jarang terjadi. Seorang pria berusia 44 tahun datang dengan keluhan utama pembesaran kedua payudara sejak tiga tahun terakhir. Pada pemeriksaan fisik, ditemukan ginekomasti kelas II (tanner IV) dengan galaktorhea. Pemeriksaan genitalia menunjukkan panjang penis 10cm dan volume testis 6mL dengan konsistensi lunak. Nilai testosteron 22,65ng/dL, LH <0,07mIU/mL, dan FSH <0,3mIU/mL. Pemeriksaan ultrasonografi (USG) payudara menunjukkan ginekomasti bilateral dengan ektasis duktal bilateral. Ultrasonografi (USG) abdomen menunjukkan tidak ada signifikansi yang terkait ginekomasti. Ultrasonografi (USG) skrotum menunjukkan testis yang normal di kedua sisi. Magnetic Resonansi Imagine (MRI) kepala (dengan kontras) ditemukan mikroadenoma hipofisis. Pasien didiagnosis sebagai ginekomasti dengan mikroadenoma hipofisis. Pasien direncanakan untuk mendapatkan terapi penggantian testosteron.

Abstract

Sindrom Klinfelter merupakan kelainan kromosom seks yang banyak ditemukan pada laki-laki. Sindrom Klinefelter umumnya muncul pada masa pubertas dan dewasa dengan ciri khasnya: infertilitas, testis kecil, ginekomastia dan mikropenis. Anak-anak dengan Sindrom Klinefelter dapat memiliki manifestasi klinis mikropenis, hipospadia, kriptorkismus atau keterlambatan perkembangan. Namun, hipospadia merupakan manifestasi klinis yang jarang ditemukan pada sindrom ini. Pada laporan kasus ini, kami melaporkan  satu kasus seorang anak laki-laki yang memiliki hipospadia dan disertai kelainan kromosom 47, XXY. Laporan kasus ini menggambarkan Sindrom Klinefelter pada anak-anak dengan manifestasi klinis hipospadia, skrotum bifidum, kriptorkismus dan DSD di mana hal ini merupakan manifestasi klinis yang jarang ditemui pada kelainan ini.

Abstract

Background: Twenty two percent of men aged over 40 years suffer from erectile dysfunction moderate to severe degree. Twelve to twenty eight percent of men with erectile dysfunction report an impact on the relationship with the partner, body image, lowered self-esteem that might be a psychosocial stressor that cause symptoms of depression. Treatment of erectile dysfunction holistically is expected to improve a better life.Objective: This study analyzed the correlation between erectile dysfunction and severity of depressive symptoms through the role of self-esteem and psychosocial stressors on patients with erectile dysfunction in Andrology Outpatient Clinic of Dr. Soetomo General Hospital.Methods: This study using correlation analytic with cross sectional design. Research subject were patients who have erectile dysfunction in Andrology Outpatient Clinic of Dr. Soetomo General Hospital, collected using consecutive sampling. The research instruments used were The International Index of Erectile Function-5, Holmes-Rahe, Self-Esteem Rosenberg and Beck Depression InventoryResults: Thirty eight research subjects met the inclusion criteria. Statistical analysis revealed erectile dysfunction correlated with self-esteem (p=0.016 r=0,388), no correlation between self-esteem and the severity of depressive symptoms (p=0.116 r=0,259), psychosocial stressors correlated with the severity of depressive symptoms (p=0.001 r=0,001), erectile dysfunction correlated with the severity of depressive symptoms (p=0,000 r=-0,559)Conclusion: This study found correlation between erectile dysfunction and severity of depressive symptoms both directly, but in this case does not involve the role of self-esteem and psychosocial stressors

Abstract

Background: Primary Testicular Failure (PTF) in men with unilateral cryptorchidism is a rare case, which might be the first time reported. Case: A 34-year-old man came with infertility and azoospermia. Signs of secondary sex found. FSH levels: 60.68 mIU / ml, LH levels: 15.96 mIU / ml, T levels: 336.14 ng / dl, E2 levels: 27.81 ng / dl. Ultrasound showed the left testis in the left inguinal +/- 2,4x1,1x3,6 cm in size, with decrease vascularization; +/- 4.1 cm from the base of the penis. The right testis size +/- 2,8x1,1x2,2 cm in the right scrotum accompanied by spermatocele. The patient was referred to the Urology department for orchidopexy of the left testis in the inguinal. Discussion: Primary testicular failure, in this case, may occur due to idiopathic but does not rule out the mosaic type of Klinefelter syndrome. The patient has unilateral cryptorchidism for 20 years, there will be a risk of testicular cancer. Management of cryptorchidism must be performed orchidopexy the first year after birth. After orchidopexy, monitoring is needed every year until at least 5 years. Conclusion: PTF occurs when the parenchymal tissue contained in the testes is no longer able to produce sperm or testosterone. PTF diagnosis is only possible through pathology and testicular cytology, but the combination of FSH and Inhibin B examination remains the best recommendation as a biomarker for patients with PTF.

Abstract

Background: Phosphodiesterase type-5 (PDE5) inhibitors was the first choice for the oral treatment of erectile dysfunction because of its ability to prevent endothelial dysfunction and restored erectile function properly. However, the inflammatory parameter such as hs-CRP can be used to assess the performance of PDE5 inhibitors as well as clinical questionnaires, IIEF-5, and EHS. So that, this study aims to determine the effect of PDE5 inhibitors administration such as sildenafil citrate 50 mg per day can decrease the value of hs-CRP, as well as increased the IIEF-5 and EHS at Dr. Soetomo General Hospital.Methods: A true clinical experimental studies used pretest and posttest control group design were carried out among 20 patients during 4 weeks. In the next 2 weeks, the crossover was conducted among respondents for the washout period whereas divided into 2 groups, control (K1 and K2) and treatment (P1 and P2). Data were analyzed using Statistical Package for the Social Sciences (SPSS) version 16.0 software for Windows.Results: The average age of group K1 and P2 was 51 ± 12.07 years old whereas 48.60 ± 15.40 years old in group P1 and K2. The research was conducted in Andrology Clinic of Doctor Soetomo Hospital in Surabaya during June to November 2017. The research data were not normally distributed, then analyzed by two-tail T-group tests and Wilcoxon test at significance level α=0.05. The results showed that PDE5 inhibitors did not decrease the hs-CRP significantly (p>0.05), but increased EHS (p<0.05) and increased IIEF-5 (p<0.05) significantly.Conclusion: This study concluded that the administration of sildenafil citrate per day by oral increased the EHS and IIEF-5, but not able to reduced hs-CRP in patients with erectile dysfunction significantly.

Abstract

Background: Hypogonadotropic hypogonadism (HH) is congenital or acquired disease affects hypothalamus or pituitary, so the secretion of GnRH, FSH, LH, and testosterone are inadequate. Clinical manifestations both physical and psychological impair patient’s quality of life, so authors try to report this case.Case description: An 18 years old man complained about a small penis. Small penis cause patient had introvert personality and problems in social relations. On anthropometric status, body weight was below 3rd percentile, height was below 3rd percentile, and body mass index (BMI) was thin. The genital examination was 2nd-grade tanner, penis with 3 cm length and 3 cm circumference, minimal little pubic hair, and testicular volume <2 cc. Ultrasound examination showed no presence of testes in the right or left scrotum fossa, inguinal, and lumbar region. Chromosome analysis showed 46 XY, and hormones analysis found FSH, LH, and testosterone below the normal limit. The patient was given 2000 unit HCG injection and after that FSH, LH, and testosterone hormone increase compared before HCG injections.Conclusion: HH can impair physical and psychological patient’s quality of life through delays in sexual maturation and bone maturation. Strategies to recognize symptoms in early childhood and puberty induction on time can significantly improve the sexual and reproductive function of HH patients.

Abstract

Background: Erectile dysfunction (ED) affects 20-50% of men globally and considerably diminishes the patients’ and their partners’ quality of life. This condition has multifactorial causes, including depression and metabolic syndrome, resulting in a complicated interaction of physical and psychological components. Case: A man in his mid-twenties presented to the Andrology outpatient clinic at Dr. Soetomo General Hospital with a sudden onset of ED, reporting a decrease in his Erection Hardness Score (EHS) from 3-4 to 1 following penetration attempts. He had a history of childhood attention deficit hyperactivity disorder (ADHD) and recent unemployment, resulting in profound depression. Prior inconsistent treatments encompassed sertraline, fluvoxamine, and clobazam. Physical and laboratory examinations revealed metabolic syndrome. The patient was diagnosed with severe ED secondary to mixed etiology, class II obesity, major depressive episode, and metabolic syndrome. He received education on the diagnosis, possible underlying causes, and management options. Discussion: The case presented illustrates the intricate interplay between ED and comorbidities such as depression and metabolic syndrome. Depression exacerbates ED through hormonal dysregulation and reduced parasympathetic activity, while metabolic syndrome contributes to ED through endothelial dysfunction and nitric oxide (NO) depletion. Effective ED management requires a multidisciplinary approach, combining lifestyle modifications, psychological support, and pharmacological treatment. Tailored strategies addressing both physical and psychological aspects are essential for improving outcomes. Conclusion: Comprehensive and integrative care in managing ED, particularly when associated with complex comorbidities, is needed to enhance the patient’s erectile function and quality of life.
