Abstract

Ginekomasti adalah pembesaran payudara laki-laki yang mengacu pada proliferasi komponen duktal. Ditemukan kasus ginekomasti yang jarang terjadi. Seorang pria berusia 44 tahun datang dengan keluhan utama pembesaran kedua payudara sejak tiga tahun terakhir. Pada pemeriksaan fisik, ditemukan ginekomasti kelas II (tanner IV) dengan galaktorhea. Pemeriksaan genitalia menunjukkan panjang penis 10cm dan volume testis 6mL dengan konsistensi lunak. Nilai testosteron 22,65ng/dL, LH <0,07mIU/mL, dan FSH <0,3mIU/mL. Pemeriksaan ultrasonografi (USG) payudara menunjukkan ginekomasti bilateral dengan ektasis duktal bilateral. Ultrasonografi (USG) abdomen menunjukkan tidak ada signifikansi yang terkait ginekomasti. Ultrasonografi (USG) skrotum menunjukkan testis yang normal di kedua sisi. Magnetic Resonansi Imagine (MRI) kepala (dengan kontras) ditemukan mikroadenoma hipofisis. Pasien didiagnosis sebagai ginekomasti dengan mikroadenoma hipofisis. Pasien direncanakan untuk mendapatkan terapi penggantian testosteron.

Abstract

Dalam mengidentifikasi jenazah yang tidak terdeteksi, peran post mortem interval (PMI) sangat penting untuk memperkirakan waktu kematian dengan menentukan umur larva lalat yang terdapat pada jenazah. Umur larva lalat sangat dipengaruhi oleh suhu lingkungan, makanan, kelembapan, intensitas cahaya, dan kontaminan. Kontaminan yang dapat mempengaruhi pertumbuhan larva lalat adalah narkotika. Keberadaan bahan narkotika mampu mengacaukan estimasi waktu kematian hingga 29 jam. Penelitian ini merupakan penelitian eksperimental menggunakan media dua bangkai tikus tanpa paparan morfin dan dengan paparan morfin dosis letal. Kedua media tumbuh ini dimasukkan kandang dengan lalat Sarcophaga Sp. Dilakukan pengamatan setiap 24 jam mulai dari larva lalat stadium satu sampai lalat dewasa. Dari setiap media tumbuh diambil lima larva secara acak setiap hari, direndam dalam air panas sampai mati. Selama penelitian dilakukan penentuan stadium larva berdasarkan panjangnya dan jumlah slit pada posterior spirakelnya. Hasil menunjukkan pada media tumbuh yang dipapar morfin dosis letal menunjukkan hasil pertumbuhan larva baik panjang maupun berat lebih tinggi secara signifikan dibandingkan pertumbuhan larva pada media tumbuh yang tidak dipapar morfin dosis letal dengan durasi pencapaian stadium lebih cepat. Perbedaan panjang larva dan durasi pencapaian stadium mulai tampak setelah larva stadium satu. Kata Kunci: Entomologi forensik, morfin dosis letal, Sarcophaga Sp.

Abstract

This study aimed to evaluate the effects of ciplukan (Physalis angulata L.) leaf ethanol extract on L-NG-nitroarginine methyl ester (L-NAME)-induced hypertensive rats. We randomly divided twenty-five Wistar rats into five groups. The sham group was given a PBS injection. The hypertensive group was injected with L-NAME on days 1 to 28. Three groups of hypertensive rats were given the extract on days 4 to 28. Blood pressure was measured using the tail-cuff method on days 0, 4, 10, and 27. The endothelial progenitor cells (EPCs) in the blood were measured by flow cytometry as a percentage of circulating angiogenic cells (CACs, CD34+/CD309+/CD45+) and endothelial colony-forming cells (ECFCs, CD34+/CD309+/CD45-). Serum NO and MDA levels, as well as serum SOD activity, were measured colorimetrically. Serum TNF-α levels were measured by the ELISA method. The ciplukan leaf extract reduced systolic and diastolic blood pressure, reduced the percentage of EPCs in the blood, increased serum NO levels, reduced MDA levels, increased serum SOD activity, and reduced serum TNF-α levels in L-NAME-induced hypertensive rats. It is concluded that ciplukan ethanol leaf extract exerts protective effects on L-NAME-induced hypertensive rats. These study results can strengthen the scientific basis of using ciplukan leaf ethanol extract to treat hypertension.

Abstract

Based on epidemiological data, the incidence of ED and BPH increases with the same risk factors, such as age, metabolic disease, and others. Aim: This study aimed to determine the prevalence of ED and its relationship to age and the presence of hypertension (HT) and type-2 diabetes mellitus (T2DM) comorbid in BPH patients. This is a cross-sectional study conducted at RSUD Dr. Saiful Anwar and Hermina Hospital Tangku-banprahu Malang utilizes two questionnaires, including the translated version of  The Interna-tional Index of Erectile Function (IIEF-5) and Erectile Hardness Score (EHS) in Indonesian. There were 44 research subjects with only 21 respondents to the IIEF-5 question-naire, while the EHS questionnaire was given to all subjects. The prevalence of ED was found to reach 100.0% at the age of 70-79 years through the IIEF-5 questionnaire and aged ³ 80 years through the EHS questionnaire. The prevalence of ED in BPH patients with HT was found to be higher than non-HT patients through both questionnaires. The prevalence of ED in BPH patients with T2DM was found to be higher through the IIEF-5 questionnaire and slight-ly lower through the EHS questionnaire than non-T2DM patients. Age had a significant rela-tionship with ED through the EHS questionnaire (p = 0.006), while no significant relationship was found through the IIEF-5 questionnaire (p = 1.000). HT and T2DM did not have a signif-icant relationship with ED (IIEF-5: p = 0.229 and 0.526; EHS: p = 0.518 and 1.000). The prevalence of ED in BPH patients increases with age and the presence of comorbid HT and T2DM. Even so, there was no significant relationship between HT and T2DM with ED in BPH patients. A significant relationship between age and ED was only found in the EHS questionnaire but not in the IIEF-5 questionnaire.

Abstract

Late Onset Hypogonadism (LOH), commonly known as andropause, poses significant hormonal challenges for elderly men, impacting their overall quality of life. With testosterone levels naturally declining from the age of 30 and accelerating beyond 40, this hormonal imbalance can manifest in symptoms that degrade the well-being of men. This community service initiative aimed to improve understanding and early detection of LOH among the Indonesian male population. A web-based application, "Skrining Andropause" was developed, allowing individuals to self-assess their LOH risk through the validated Androgen Deficiency in the Aging Male (ADAM) questionnaire. The application included educational videos to enhance public awareness of andropause. Pre- and post-tests were conducted as success indicators, involving 40 male participants aged 40 and above in Malang Regency. Statistical analyses were performed to assess the impact on knowledge related to andropause across different educational backgrounds and age groups. Significant improvement in public understanding of andropause was observed (p=0.000). While educational backgrounds were not significantly associated with initial knowledge (p=0.100), nor with the increase in pre-to-post-test scores (p=0.100). When associating age groups with initial knowledge (p=0.308) and the increase in pre-to-post-test scores (p=0.308), results were not statistically significant. The observed substantial improvement in andropause understanding highlights the efficacy of our educational approach. Respondents, irrespective of educational background, shared a baseline understanding, with those with basic education showing the most significant improvement. Notably, the youngest age group displayed the highest baseline awareness. However, the oldest age group exhibited the least responsiveness to our educational media.

Abstract

Demam berdarah dengue (DBD) merupakan masalah kesehatan di seluruh dunia yang disebabkan oleh virus dengue dan ditularkan melalui nyamuk Aedes sp. Pencegahan dari penyakit ini salah satunya adalah menggunakan larvasida nabati. Jeruk memiliki kandungan senyawa limonoid yang berpotensi menjadi larvasida Aedes sp dengan efek toksik dan antifeedant yang diberikan. Ulasan scoping ini bertujuan untuk mengidentifikasi senyawa limonoid pada ekstrak berbagai spesies jeruk yang berpotensi sebagai larvasida Aedes sp. Pencarian artikel yang diulas dilakukan secara sistematis pada basis data Pubmed, Science Direct, Google Scholar, ProQuest, dan Semantic Scholar. Selanjutnya dilakukan penilaian kualitas studi menggunakan JBI dan didapatkan 4 artikel yang memenuhi kriteria. Hasil studi ini menunjukkan bahwa pada 24 kultivar dari 11 spesies jeruk memberikan potensi larvasida yang beragam. Hal tersebut dipengaruhi oleh beberapa faktor. Jeruk dengan kadar limonoid yang tinggi menyebabkan potensi larvasida lebih kuat daripada jeruk dengan kadar limonoid yang rendah. Dapat disimpulkan bahwa senyawa limonoid pada berbagai spesies jeruk memiliki potensi larvasida terhadap Aedes sp.
