Abstract

Blood transfusions must be performed based on appropriate indications, selecting the correct volume and type of component, and administered at the right time. The risk of transfusion reactions is inherent in nearly every transfusion process and can manifest clinical symptoms in patients. Hospitals must conduct oversight of the blood transfusion process, known as hemovigilance, to prevent transfusion reactions and enhance the safety of blood services. This study aims to determine the number of transfusion reaction cases in blood donor recipients at RSUD Sayang, Cianjur Regency, during the period from 2020 to 2022. This qualitative research utilizes a total sampling technique. The population comprises all blood donor recipients at RSUD Sayang, Cianjur Regency, from 2020 to 2022. The sample includes medical records data of transfusion reaction cases in blood donor recipients at RSUD Sayang, Cianjur Regency, from 2020 to 2022. The results indicate 35 cases of transfusion reactions. The majority of transfusion reaction cases in blood donor recipients were acute category I (mild) reactions, totaling 21 cases (60%). Patients with a history of previous transfusions predominantly experienced category I (mild) and category II (moderate) reactions, each with 12 cases (57.1%). Most acute reaction cases were category I (mild), with 20 cases (57.1%), occurring during transfusions with Packed Red Cells (PRC). The majority of acute transfusion reactions occurred after the administration of the first blood bag, with 17 cases (48.6%), and these reactions were primarily category I (mild).

Abstract

Pulmonary tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. Patients diagnosed with pulmonary tuberculosis undergo Anti-Tuberculosis Drug (OAT) therapy for six months. In the first two months, known as the intensive phase, patients receive a combination of OATs such as isoniazid, rifampicin, ethambutol, and pyrazinamide. TNF-α is a pro-inflammatory cytokine that plays a crucial role in granuloma formation to contain the spread of Mycobacterium tuberculosis. This study aims to determine the difference in TNF-α levels in pulmonary tuberculosis patients before and after one month of OAT therapy. The study used an observational analytic design with a prospective cohort approach without comparison. A total of 17 newly diagnosed pulmonary tuberculosis patients at Bangetayu, Kedungmundu, and Tlogosari Wetan Health Centers, Semarang City, were included. TNF-α levels in serum samples were measured using the ELISA method. The mean TNF-α level before OAT therapy was 110.09±100,48 pg/mL, while after one month of therapy, it decreased to 90,05±60,12 pg/mL. The statistical analysis using the Mann-Whitney test yielded a p-value of 0.228 (p 0.05), indicating that there was no statistically significant difference in TNF-α levels before and after one month of anti-tuberculosis drug (OAT) therapy. However, the mean TNF-α levels showed a decreasing trend, which, although not statistically significant, may suggest an initial immunological response to the treatment. The researcher recommends further studies with a larger sample size and longer observation period—such as until the end of the intensive phase or the completion of therapy—in order to obtain more representative and statistically significant results regarding TNF-α dynamics during tuberculosis treatment.

Abstract

Lead (Pb) exposure poses significant health risks due to its non-degradable nature and profound toxicity, causes oxidative stress and organ damage, particularly targeting the kidneys. Urea nitrogen levels rise as a result of decreased renal filtration rate and urea excretion. This study investigates the potential of papaya juice, rich in flavonoids, vitamins C, E, and beta-carotene, as a protective agent against Pb-induced nephrotoxicity. A Completely Randomized Design experiment was conducted, involving six treatment groups of Wistar rats. Papaya juice was administered at doses of 3.6, 7.2, and 14.4 g/200g body weight (BW) to Groups P1, P2, and P3, respectively, prior to exposure to 50 mg/kg BW of lead acetate. Group PC received vitamin E (400 IU/kg BW) and lead acetate, while Group NC was exposed to lead acetate alone. Group NT served as the control. Following a 28-day treatment period, the P3 group exhibited the most significant improvements, with a notable increase in body weight (22.50 grams) and a substantial decrease in urea nitrogen levels (18.24 mg/dl). These findings underscore the efficacy of papaya juice treatment in mitigating Pb-induced nephrotoxicity, suggesting a potential therapeutic regimen for alleviating lead toxicity in exposed populations. Eventually, the optimal dosing for papaya juice treatment, either at 14.4 g per 200 g body weight for rats or consumption of one medium-sized papaya for humans, yields compelling benefits. This regimen demonstrates significant efficacy in increasing body weight and lowering serum urea levels in rats subjected to Pb acetate exposure.

Abstract

Metabolic syndrome is caused by a poor lifestyle, excessive food consumption, lack of physical activity. Someone with metabolic syndrome will be more susceptible to Covid-19. This community service activity aims to provide knowledge about metabolic syndrome as well as measurement of Body Mass Index (BMI), measurement of blood pressure and checking blood glucose levels. The method of carrying out activities is in the form of outreach related to metabolic syndrome and screening for metabolic syndrome through BMI measurements, measuring blood pressure using a digital tensimeter and checking blood glucose levels using the Point of Care method. The activity was carried out in Bangetayu Kulon RT 07 RW 01 Semarang City on May 13-15 2022. The target audience is adults aged 20-65 years with a total of 56 people. From the results of the examination, it was found that 87.5% of respondents had hypertension, 12.5% of respondents had diabetes mellitus and 73.2% of respondents were obese. Community service activities provide an overview of public health about metabolic disorders so that it is hoped that after this activity the community routinely conducts health checks to prevent metabolic syndrome.

Abstract

Diabetes Melitus Tipe 2 merupakan faktor risiko penyebab nefrolitiasis. Pasien mengalami hiperglikemia kronis akibat kelalaian kontrol kadar glukosa yang buruk yang akan memicu Urolithiasis. Gangguan amoniagenesis di ginjal mengakibatkan derajat keasaman urin menurun dan memicu pembentukan kristal urin sehingga prevalensi kristal urin lebih banyak pada pasien Diabetes daripada nonDiabetes. Pemeriksaan kristal urin bermanfaat untuk skrining urolithiasis, maka gangguan fungsi ginjal akut atau kronis yang terkait dengan presipitasi kristal di bagian intrarenal pada pasien Diabetes Melitus Tipe 2 dapat dicegah. Tujuan penelitian untuk mengetahui karakteristik dan prevalensi kristal urin pada pasien Prolanis DM tipe 2 di Puskesmas Bangetayu Kota Semarang. Jenis Penelitian adalah observasional analitik dengan desain studi kasus kontrol. Jumlah sampel penelitian sebanyak 40 orang diambil dengan metode total sampling 1:1 berdasarkan kriteria inklusi dan eksklusi. Hasil penelitian menunjukkan adanya kristal asam urat kelompok kasus sebanyak 10 orang (25%) dan kontrol 17 orang (42,5%), kristal amorf kelompok kasus sebanyak 4 orang (10%) dan kontrol 24 orang (60%) dan kristal oksalat kelompok kasus sebanyak 11 orang (27,5%) dan kontrol 13 orang (32,5%). Prevalensi kristal urin pada pasien prolanis Diabetes Melitus Tipe 2 di Puskesmas Bangetayu lebih sedikit menunjukkan adanya kristal urine dibandingkan dengan kelompok kontrol Non-Diabetes Melitus Tipe 2.
