Abstract

Background: Burnout is a condition of extreme fatigue and exhaustion that is widely experienced by workers. Nurses who work on the front line for 24 hours are at high risk of burnout, which can jeopardise their wellbeing as well as patient safety and quality of care. This study aims to identify the dominant factors, including individual characteristics, work environment, locus of control, and work experience, that influence burnout among hospital nurses. Methods: The study used a cross-sectional study was conducted to evaluate the level of burnout. The sample was 431 nurses who was selected from the hospital using stratified random sampling to ensure the representation of different work units or departments. Date analysis was performed using Structured Equation Modelling (SEM) to examine the complex relationships between the variables. Results: The results of the bivariate analysis indicate that there is a relationship between respondent characteristics and nurse burnout incidence. A significant negative relationship was found between the work environment and burnout occurrence, as well as a relationship between years of work experience and burnout incidence. Meanwhile, the results of the multivariate analysis showed that characteristics, work environment, locus of control, and work experience influence burnout by 10.9%. Conclusion: This study shows that the variables of characteristics, work environment, locus of control, and work experience account for 10.9% of the burnout. This study recommends improving the work environment, strengthening locus of control, and providing mentoring for nurses with low experience to prevent burnout.

Abstract

BSTRACT This literature review explores how Continuing Professional Development (CPD) and the nursing career ladder contribute to improving nurses’ competence, motivation, and quality of care. Thirty national and international studies published from 2019 to 2025 were examined, covering aspects such as CPD implementation, influencing factors, organizational support, nurses’ perceptions, and the outcomes of structured career progression systems. The analysis indicates that CPD plays a key role in strengthening clinical skills, professional conduct, and overall performance through continuous learning. Additionally, career ladder programs offer clearer pathways for professional advancement, enhance job satisfaction, and lower turnover intention. Despite these advantages, challenges persist, including time constraints, limited institutional backing, inconsistent policies, financial limitations, and the underuse of digital systems for tracking competencies. Overall, the review emphasizes the importance of consistent CPD guidelines, stronger institutional engagement, and the integration of digital innovations like e-logbooks to support long-term professional growth. Further research is needed to evaluate the long-term effects of CPD and career ladder integration on patient safety and nursing care quality. ABSTRAK Literature review ini meninjau bagaimana Continuing Professional Development (CPD) dan jenjang karir perawat berkontribusi terhadap peningkatan kompetensi, motivasi, dan mutu pelayanan keperawatan. Literature review ini mereview sebanyak 30 penelitian nasional dan internasional yang diterbitkan antara tahun 2019–2025 dianalisis, mencakup implementasi CPD, faktor-faktor yang memengaruhi, dukungan organisasi, persepsi perawat, serta dampak dari sistem jenjang karir yang terstruktur. Hasil analisis menunjukkan bahwa CPD berperan penting dalam memperkuat keterampilan klinis, perilaku profesional, dan performa kerja melalui proses pembelajaran yang berkelanjutan. Di sisi lain, jenjang karir memberikan arah perkembangan profesional yang lebih jelas, meningkatkan kepuasan kerja, dan menurunkan keinginan untuk berpindah tempat kerja. Meski demikian, masih terdapat beberapa tantangan, seperti keterbatasan waktu, minimnya dukungan institusi, kebijakan yang tidak seragam, kendala biaya, serta kurang optimalnya penggunaan teknologi digital untuk pencatatan kompetensi. Secara keseluruhan, review ini menekankan perlunya pedoman CPD yang lebih terstandardisasi, keterlibatan institusi yang lebih kuat, dan pemanfaatan inovasi digital seperti e-logbook guna mendukung pengembangan profesional jangka panjang. Penelitian lanjutan diperlukan untuk menilai dampak jangka panjang integrasi CPD dan jenjang karir terhadap keselamatan pasien dan kualitas pelayanan keperawatan.

Abstract

Clinical learning is a process of interaction between clinical preceptors and students as learners on the practice field. The clinical guidance process is often limited to supervising nursing students during clinical practice without any targets that need to be achieved, even students repeat the same activities every day. This study aims to determine the application of a target-based clinical learning model in improving the quality of preceptors. The study used a quasi-experimental design with a one group time series design on 40 nurses from several hospitals and educational institutions who had not received clinical preceptor training. The sampling technique used quota sampling technique according to the proportion of the number of students who were practicing in their respective institutions, the sample was given an intervention in the form of training using a target-based clinical learning model consisting of theory and practice. The practice was conducted in the laboratory and in the hospital from September to December. Evaluation using a checklist instrument for assessing preceptor competence and data analyzed using the Wilcoxon test using SPSS 25.0.  The result of the study, there was a significant increase in the ability of nurses to provide nursing clinic preceptors between before and after training with a significance value of 0.020, while in the first supervisory assessment significant=0.030, second supervision significant=0.040 and third supervision significant=0.050. The data shows how closely the target-based learning model affects the quality of clinical supervisors' abilities. This is important to be applied to all prospective clinical supervisors before conducting clinical guidance.
