Abstract

Background: Non alcoholic fatty liver disease (NAFLD) has been associated with cytokines and inflammatory mediators. Adiponectin has insulin sensitizing effects and has correlation with severity of NAFLD disease. However, the study about the relationship between adiponectin level and NAFLD is lacking. The objective of the study was to determine the association between adiponectin level and NAFLD in obese adolescents through the role of insulin resistance. Methods: This was a cross-sectional study, that was done in August 2007. The inclusion criteria were obese adolescents aged 11-14 years, and normal weight adolescent as control group. Adiponectin was assessed by using ELISA, insulin resistance was obtained by Homeostasis Model Assessment Insulin Resistance (HOMA-IR). NAFLD was confirmed by abdominal ultrasound, which represented by fatty liver imaging. The comparison of adiponectin level and HOMA-IR among 3 groups were analyzed by Kruskal Wallis test, meanwhile the correlation between adiponectin level and some variables were analyzed by spearman correlation. Result: There were 73 subjects, consisted of 37 obese and 36 non obese. Among obese subjects, 54.1% got NAFLD. All of our obese subjects were insulin resistance, the HOMA-IR level of obese non NAFLD was 6.1 and obese with NAFLD was 6.8. The adiponectin levels in normal children was (5.1mg / ml), obese non NAFLD (4.1mg / ml) and obese with NAFLD (4.0 mg / ml) (p <0.001). There were no association between adiponectin level and other variables. Conclusion: There were significant differences of adiponectin levels and insulin resistance measured by HOMA-IR between normal and obese subjects, but no significant differences between the obese groups with or without NAFLD. Keywords: NAFLD, adiponectin, HOMA-IR, obesity, adolescent.

The One Student, One Achievement program implemented at Al-Rifa'ie Modern Middle School aims to train students' honesty and see students' non-academic achievements. The purpose of this study is to explore in detail the implementation of One Student, One Achievement at Al-Rifa'ie Modern Middle School. This study uses a qualitative approach. Data collection was carried out by conducting observations followed by documentation. Observations were carried out by directly observing the One Student, One Achievement process, while documentation was carried out by looking at student summary documents. The results of the study indicate that the One Student, One Achievement program is effective to be implemented at the junior high school level. With the One Student, One Achievement program, it is hoped that students will be accustomed to having good character.

Abstract

This research discusses the role of the government in tackling early marriage and strengthening the 2045 demographic bonus. The method used in this research is a literature study using library data collection methods, reading and taking notes, and managing research data objectively, systematically, analytically and critically, such as journals. , articles, internet sites, and others relevant to this research. The research results show that a person's ability to become high-quality human resources can be determined by their level of education. Countries that have a lot of human resources but are incompetent will add to the burden. This will then stop economic growth and development. The large number of cases of early marriage in Indonesia can hinder efforts to improve the quality of human resources and pose a threat to the 2045 demographic bonus, because many children drop out of school and children who marry under the age of 18 will be vulnerable to access to basic educational needs. has the potential to perpetuate intergenerational poverty. Therefore, the government must prepare several strategies to tackle early marriage by preventing child marriage which can be carried out in collaboration with the government and community organizations. Among these prevention strategies are STARNAS PPA and strategies from BKKBN.

Abstract

Introduction: Patient safety remains a critical global health agenda, as emphasized by the WHO and IOM. Fostering a robust patient safety culture within hospitals is paramount for minimizing adverse events and improving the quality of care. Understanding the psychosocial factors that drive this culture is essential for effective organizational interventions. This study aims to explore the direct and indirect effects of transformational leadership and team cooperation on patient safety culture, with a particular focus on assessing the mediating function of motivation within these relationships among healthcare professionals. Methods: An analytical cross-sectional research design was applied in this study. Study utilized a survey to collect data from 198 Healthcare Providers (HCPs) at Sulianti Saroso Infectious Disease (SSIDH). The primary instrument was the Hospital Survey on Patient Safety Culture (HSOPS) Version 2.0. Data analysis involved Structural Equation Modelling (SEM) based on Partial Least Squares (PLS-SEM) using SmartPLS 4.0 to test the proposed causal relationships. Results: Descriptive analysis showed that the variables of Transformational Leadership, Team Cooperation, Motivation, and Patient Safety Culture were all in the moderate category. However, hypothesis testing revealed significant findings. Both transformational leadership and team cooperation demonstrated a direct and significant positive influence on patient safety culture. Crucially, motivation was found to significantly mediate the relationship between transformational leadership and safety culture, as well as the relationship between team cooperation and safety culture. Conclusion: This study confirms that transformational leadership and teamwork are crucial factors that effectively enhance patient safety culture. The central finding of this study is the vital role of motivation as a psychological mechanism that translates managerial initiatives and team collaboration into proactive safety behaviors. Therefore, the practical recommendations from this research focus on strategies that directly target motivation, such as strengthening programs for recognition and responsibility, accompanied by improved interprofessional communication and more specific leadership training programs.
