Abstract

Osteoarthritis and hypertension are diseases often attack elderly. Incidence of hypertension based on blood pressure carried out in the population aged> 18 years in Indonesia is around 34.1%. Lampung Province is ranked 16th out of 33 provinces with the incidence of hypertension around 22%. The incidence of Osteoarthritis in women in Indonesia is 12.7%. This study analysis is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record. Assessment is based on a holistic diagnosis from the beginning, process, and end of study in a qualitative and quantitative manner.The 67-year-old Mrs. S, who has been diagnosed with Osteoarthritis and hypertension, has worries about her deteriorating health condition. Limitations of the patient's knowledge of the illness and bad lifestyle is a factor that causes problems that occur in patients and families.Holistic and comprehensive management of the patient's problems has been carried out by providing counseling to improve patient and family knowledge and providing a diet according to the patient's calorie needs.

Abstract

Chronic energy deficiency (CED) is a state of chronic energy and protein intake shortages that persists (chronic) resulting in health problems for the mother and fetus. Implementation of evidence based medicine based family doctor services by identifying risk factors, clinical problems, and managing patients holistically with a patient centered approach, family approached, and community oriented. Primary data obtained through history taking, physical examination and home visits, psychosocial and environmental data. Assessment is based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. The patient, a 25-year-old woman pregnant with her fourth child, 24 weeks gestasional age with chronic energy deficiency, has concerns about growth, development and the fetus and herself. Complaints feel weak, no appetite at the beginning of the trimester, and difficulty to gain weight. The patient also has a history of miscarriage twice. The risk factor obtained is the lack of adequate nutritional balance in patients. In the process of behavior change, Mrs. T has reached the trial stage. The process of behavior change for a balanced nutritional diet, addition of food portions, routine vitamin intake and Supplementary feeding (SF)-mild physical exercise is seen after the patient is given an intervention.

Abstract

Scabies is a skin disease caused by Sarcoptei scabei mites hominis variety. According to WHO, an estimated 300 million people are currently infected with scabies globally, with a prevalence ranging from 0.2% to 71%. Scabies attacks community because it is transmitted easily through skin contact. There are various factors that can lead to the failure of management of patients with scabies. Therefore, proper holistic management using a family medicine approach is needed. This study is a case report with primary data obtained from anamnesis, physical examination, and home visit. Secondary data were obtained from patient medical records. Patient A, a 14 year old student complained itching between his fingers, especially at night since 3 months. He already looking for treatment three times but his symptoms have not decreased. Then an initial holistic diagnosis is carried out. Pharmacological and non-pharmacological management is carried out with the principles of patient-centered, family focused, and community-oriented. The intervention was done using posters and pamphlets, including general knowledge about scabies, how to maintain personal and environmental hygiene, how to use medicine properly, and how to prevent transmission in the community. After the pharmacological and non-pharmacological management, evaluation is carried out. There is an increase in knowledge and changes in behavior for both patients and his families. It can be concluded, holistic management with a family medicine approach is needed for the management of patients with scabies in order to support the success of therapy and prevent transmission to the community.

Abstract

Asthma is a chronic inflammatory process of the respiratory tract involving many cells and their elements. This chronic inflammatory process causes the respiratory tract to become hypereponsive, facilitating the occurrence of bronchoconstriction, edema, and gland hypersecretion, resulting in restriction of air flow in the respiratory tract with periodic clinical manifestations of wheezing, shortness of breath, heavy chest, coughing particularly Night or early morning. Applying a holistic and comprehensive approach to the family doctor in detecting internal and external risk factors that solve problems by Evident Based Medicine from family-approached and patient-centered. This study is a case report. The primary data obtained through anamnesis, physical examination and a home visit. Secondary data were obtained from medical records of patients. Mrs. S, 64 years old, complained of shortness since one day ago, followed coughing and wheezing. Obtained internal factors such as genetic factors, lack of knowledge of patients about disease, curative treatment, and never exercising. External factors include lack of hygiene in items at home, cold allergies and lack of support and family knowledge about the patient's illness. Three home visits were made to intervene on patients and families about illnes, prevention, management at home, and the importance of avoiding risk factors. In the evaluation, we found patients began to make improvements but not all of them. This shows that by increasing knowledge, correct treatment and several intervention visits can change the behavior of patients.

Abstract

Dyspepsia fungsional is health problems that are often encountered in daily practice and are diseases with a large number of cases in Indonesia. Family doctor services play an important role as a treatment approach for patients because it applies as a help of healing, health promotions, and preventing diseases. The family doctor approach includes patient centred, family approach strategies, and community oriented. Important role in reducing the incidence and mortality of dyspepsia, as well as increasing the cure rate. Patients must know about their disease and have the awareness and motivation to complete treatment and lifestyle modifications so that treatment goals can be achieved. The study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record at the Puskesmas. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. A patient with complaints of heartbrun since 1 days before going to the Puskesma. Patient complains of abdominal discomfort such as feeling of bloating and pain in the form of stinging in the middle of abdomen and burning sensation in the pit of stomach. Another complaint that is feltĀ  is feeling nauseous, vomiting and no appetite. In this case, diagnosis and treatment have been carried out according to the latest theory and journals. After the intervention, there was a decrease in clinical symptoms and an increase in the knowledge of patients and their families. patient Patients diagnosed with dyspepsia fungsional was in accordance with the theory from several guidelines and journals, there was a change in knowledge of the patient and his family after an intervention based on Evidence-Based Medicine which was patient-centred and family approach.

Abstract

Scabies is a skin disease caused by infestation and sensitization of the parasite Sarcoptes scabiei var. hominist. According to the World Health Organization (WHO), scabies often occurs in tropical and hot countries and in areas with high population density. It is also estimated that there are 200 million people worldwide suffer from scabies at one time and an estimated 10% of children in poor areas. Risk factors for scabies include socio-economic and low knowledge, personal hygiene and poor environment. Scabies is often overlooked because it is not life threatening. Appropriate case management holistically using a family medicine approach is needed to achieve therapeutic success and break the chain of disease transmission to the community. Patient An. AK aged 18 months came with complaints of itching since 2 months ago and there were small nodules the size of a pin filled with clear fluid in the itchy area which were then scratched and broke. Complaints have not been treated, and the longer the itching gets worse. The patient is worried that the complaint getting worse, not disappear and not heal so that it interferes with the patient's activities. Clinically the patient was diagnosed with scabies (ICD-10 B.86; ICPC-2:S72). Internal risks in the form of lack of personal and environmental hygiene, lack of knowledge, inappropriate treatment behavior. External risks include friends and family members who have the same complaint, lack of family hygiene, lack of family knowledge. Functional grade 1, which is able to carry out activities as before the illness without difficulty. Furthermore, holistic management is carried out, namely intervention using poster media. In the evaluation, the results obtained in the form of a better understanding of the disease and changes in behavior that have an impact on the success of therapy.

Abstract

Pneumonia is a disease that attacks the lower respiratory tract by pathogens. The most common causes in toddlers and children are viruses and bacteria. This article is a form of implementing family doctor services for patients by identifying risk factors, clinical problems, management and prevention efforts for patients based on a patient-centered and family problem solving framework. This study is a case report using primary data obtained through alloanamnesis, physical examination, home visits to complete family and psychosocial data, as well as environmental conditions. The assessment is based on a holistic diagnosis from the beginning, process and end of the study both quantitatively and qualitatively. A 7 month old toddler patient with a diagnosis of pneumonia. In this case, the internal factor that played a role in the patient was the patient's age, which was 7 months. External factors, namely the lack of family knowledge about pneumonia, its complications and the environment that has trigger factors. These patients were then given pharmacological and non-pharmacological interventions according to evidence-based medicine. The evaluation results showed an improvement in the patient's clinical condition and increased family knowledge about pneumonia. The diagnosis and management of this patient have been carried out according to the guidelines. Changes in behavior and knowledge of patients and their families are seen after interventions that are patient centered and family approach.

Abstract

Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis, which can attack the lungs and other organs. As many as 10.6 million people worldwide suffer from tuberculosis (TB) with an increase in the incidence rate of 3.6% between 2020 and 2021. Type 2 diabetes mellitus (DM) is one of the most important risk factors for worsening TB with a risk of active TB 2 -4 times higher in individuals with type 2 DM than non-DM. The study conducted in the form of a case report. Data were obtained through anamnesis, physical examination, laboratory examination, and medical records at PUSKESMAS. The assessment carried out based on the initial holistic diagnosis, process, and the final of the study both quantitatively and qualitatively. Based on the study, case a 63-year-old female patient with complaints of dry cough and phlegm for 2 months accompanied by an itchy throat, chills, decreased appetite and body weight. The patient has type 2 DM since 6 years. Family doctor services carried out to identify risk factors, clinical problems, as well as evidence-based medicine with patient management through a family approach, patient-centred, and community-oriented. Interventions done both medically and non-medically in the form of patient and family education about pulmonary tuberculosis and type 2 DM. Evaluation after the intervention obtained a decrease in fasting blood sugar, an increase in knowledge about the patient's disease, and an improvement in eating patterns.
