Abstract

Angka kematian Ibu meningkat selama masa pandemic COVID-19 pada tahun 2020 sebagai dampak kebijakan pembatasan kegiatan sosial. Akses masyarakat terhadap layanan kesehatan maternal menjadi terbatas sehingga penanganan komplikasi ibu hamil, bersalin, dan nifas menjadi terlambat. Suami memegang peranan penting dalam mencegah terjadinya keterlambatan tersebut melalui implementasi program Suami Siaga. Tujuan penelitian adalah untuk mengidentifikasi faktor predisposisi yang berhubungan dengan implementasi suami siaga. Penelitian ini merupakan penelitian analitik observasional dengan desain studi cross-sectional di wilayah Pusat Kesehatan Masyarakat Panti, kabupaten Jember selama Agustus sampai dengan November 2020. Sampel penelitian sejumlah 170 orang yang dipilih secara acak menggunakan cluster random sampling. Instrumen penelitian menggunakan kuesioner terstruktur untuk mengukur implementasi suami siaga kemudian data yang diperoleh dianalisis menggunakan uji chi-square. Hasil penelitian menunjukkan bahwa sebagian besar suami termasuk dalam kategori suami tanggap dan siaga (85,88%). Faktor predisposisi yang berhubungan dengan implementasi suami siaga antara lain usia suami (p-value = 0,008), pengetahuan suami tentang persiapan persalinan dan pencegahan komplikasi (p-value = 0,002), dan jarak tempat tinggal ke fasilitas kesehatan (p-value = 0,000). Oleh karena itu, tenaga kesehatan di fasilitas pelayanan kesehatan perlu meningkatkan kegiatan edukasi dan sosialisasi terkait program Suami Siaga pada ibu hamil dan suami selama antenatal care sebagai pencegahan kematian ibu selama pandemic COVID-19.

Abstract

Background: Globally, approximately 940 million male adolescents and 193 million female adolescents were classified as smokers in 2019. The global prevalence of smoking has continued to decline over the years, but the number of smokers remains high due to population growth. Consistently, reports from the Indonesia Demographic Health Survey (IDHS) and Indonesia Fundamental Health Research have shown an increase in the number of adolescent smokers in Indonesia. Objective: This study aimed to investigate the relationships between parental marital status, age of male adolescents, socioeconomic family status, and educational level of male adolescents with tobacco smoking behavior. Method: The research design employed was a secondary data analysis using the IDHS 2017 data with n = 5,863 male adolescent respondents. Results: Most respondents did not smoke, had married parents, were between 15-17 years old, came from low to very low socioeconomic backgrounds, and had an educational level of junior high school or lower. All variables (parental marital status, age, socioeconomic status, and educational level) were found to be associated with smoking behavior among male adolescents aged 15-19 years. Conclusion: Adolescents with divorced parents were at a significantly higher risk, approximately 1.3 times greater, for smoking tobacco compared to those from non-divorced families. Intrinsic factors (age and educational level) emerged as dominant factors in the tobacco smoking behavior of male adolescents.

Abstract

Kecamatan Panti kabupaten Jember merupakan salah satu kecamatan dengan angka kematian ibu (AKI) tertinggi selama tiga tahun berturut-turut sejak tahun 2018 -2021. Desa Suci merupakan salah salah wilayah di kecamatan Panti dengan karakteristik masyarakat heterogen dan berada di wilayah dataran tinggi Gunung Pasang dan termasuk dalam desa penyumbang angka kematian ibu(AKI) di wilayah kecamatan Panti. Adanya ketidakpatuhan ibu hamil dalam melakukan pemeriksaan antenatal, kurangnya dukungan suami dan keluarga selama kehamilan, angka komplikasi kehamilan yang tinggi sebesar 30% dari seluruh ibu hamil di desa Suci dan hambatan dalam melaksanakan rujukan berdampak terhadap kematian ibu yang berulang di desa Suci. Program birth preparedness and complication readiness tidak dilaksanakan secara optimal selama tahun 2019-2021, sehingga menghambat deteksi dini risiko komplikasi pada ibu hamil. Oleh karena itu, tujuan dari kegiatan pengabdian kepada masyarakat ini untuk meningkatkan pengetahuan dan pemahaman ibu hamil, suami, keluarga, stakeholder   di desa Suci melalui Collaborative Prenatal Class. Solusi permasalahan mitra melalui Collaborative Prenatal Class melibatkan peran ibu hamil terutama ibu hamil, suami, keluarga, tenaga  kesehatan, kader, dan stakeholder. Collaborative Prenatal Class dilaksanakan melalui kegiatan prenatal education class, collaborative prenatal class, dan forum suami dan desa siaga. Terdapat peningkatan pengetahuan peserta terkait BPCR dengan peningkatan hasil pre-test dan post-test 42,00% dan adequacy of performance kegiatan sebesar 92,01%.

Abstract

Pregnant women are one of the vulnerable groups affected by the COVID-19 pandemic through limited access and reach to health services, resulting in the adaptation process during pregnancy and increased pregnancy stressors. Self-efficacy is one of the important factors in the ability of pregnant women to cope when experiencing stress and depression during the COVID-19 pandemic. This study aimed to examine the factors that influence the self-efficacy of pregnant women during the COVID-19 pandemic. This is a quantitative study with a cross-sectional design in the working area of ​​the Panti Community Health Center, Jember Regency, from August to October 2021. The research sample was 170 randomly selected using cluster random sampling. The research instrument used the General Self-Efficacy Scale (GSES) to measure self-efficacy and a structured questionnaire to measure the characteristics of the respondents. The results showed that the factors that influenced the self-efficacy of pregnant women during the COVID-19 pandemic were education (p-value = 0.007; OR = 3.852), income (p-value = 0.014; OR = 4.325), and occupation (p-value = 0.014; OR = 4.325), and occupation (p-value = 4.325). value = 0.006; OR = 5.464). And the most influential predictor of pregnant women's self-efficacy during the COVID-19 pandemic was occupation (p-value = 0.002; OR = 6.224). Working mothers tend to have higher adaptability through changing conditions at work as an effort to develop themselves, demands, initiatives, and targets at work to make the right decisions and other stressors at work, so they tend to have higher self-efficacy, be able to solve problems and solve problems challenges well, especially during the COVID-19 pandemic. Conclusion: The predictors that influence pregnant women's self-efficacy during the COVID-19 pandemic are the mother's education, the mother's occupation, and family income. Strengthening cooperation with stakeholders in the region to increase the capability and empowerment of mothers through training to develop the competence of mothers according to mothers’ education and local resources so that they can work in the formal and informal sectors so that mothers' self-efficacy increases.

Abstract

Norma gender diyakini dapat memengaruhi individu dalam berperilaku. Adanya ketidakseimbangan peran antara laki-laki dan perempuan dalam pandangan norma gender dapat menyebabkan inisiasi aktivitas seksual yang berisiko. Data SDKI 2017 menunjukkan bahwa kelompok usia 20-24 tahun memiliki persentase perilaku pacaran berisiko yang paling tinggi diantara kelompok usia lainnya. Hasil studi pendahuluan pada 112 mahasiswa jenjang sarjana di Universitas Jember menunjukkan bahwa terdapat indikasi sikap permisif terhadap aktivitas seksual. Penelitian ini bertujuan untuk menganalisis hubungan norma gender dengan perilaku seksual pada mahasiswa di Universitas Jember. Penelitian ini adalah penelitian kuantitatif menggunakan desain cross-sectional dengan sampel sebanyak 154 mahasiswa jenjang sarjana di Universitas Jember. Data dikumpulkan dengan wawancara terstruktur menggunakan kuesioner pada bulan Maret hingga Juni 2024. Data dianalisis menggunakan uji chi-square dan Spearman’s rank correlation dengan tingkat kepercayaan sebesar 95%. Penelitian ini menunjukkan bahwa sebagian besar mahasiswa memiliki perspektif norma gender tradisional (76%) dan termasuk dalam kategori perilaku seksual yang berisiko berat (44%). Adapun hasil analisis hubungan menunjukkan bahwa tidak terdapat hubungan antara norma gender dengan perilaku seksual (p-value>0,05). Oleh karena itu, penting bagi Universitas Jember untuk memberikan edukasi yang komprehensif mengenai kesehatan reproduksi dan hak-hak reproduksi bagi mahasiswanya.

Abstract

Background: Early marriage is still one of the problems affecting the health of society in Indonesia. The application for marriage dispensation in Jember Regency increases yearly; in 2020, Jember Regency was ranked second nationally. The purpose of this study was to determine the description of knowledge, attitudes and support of parents who have children who marry early in Silo District, Jember Regency. Method: This study is a descriptive study with a quantitative approach in Silo District, Jember Regency from July 2024 - August 2024. The population of this study was 77 parents who had children who married early in Silo District, Jember Regency. The sample was obtained from 75 because two people did not meet the inclusion criteria. The data were then analyzed univariately with a descriptive method before being presented in text and table form. Result: The results showed that 47 respondents (62.7%) had poor knowledge about the impact of early marriage, while only 28 respondents (37.3%) had good knowledge. Parents' attitudes towards early marriage were mainly negative, with 41 respondents (54.7%) having negative attitudes, and 34 respondents (45.3%) having positive attitudes. Conclusion: Most parents support children who marry early, with 52 respondents reporting high support, while 23 respondents reported low support.

Abstract

Latar belakang: program perencanaan persalinan dan pencegahan komplikasi upaya preventif untuk mencegah kematian ibu melalui pencegahan tiga keterlambatan. Studi terkait praktik perencanaan persalinan dan pencegahan komplikasi oleh ibu dan keluarga masih terbatas, terutama di kabupaten Jember sebagai kabupaten penyumbang Angka Kematian Ibu tertinggi di provinsi Jawa Timur selama tiga tahun sejak tahun 2019 sampai dengan tahun 2021. Tujuan penelitian ini untuk menganalisis hubungan antara karakteristik sosiodemografi dengan praktik P4K. Metode penelitian menggunakan analitik dengan pendekatan cross-sectional dilakukan di Puskesmas Sumberbaru, Jember pada bulan April sampai dengan Juni 2023. Ibu postpartum sejumlah 78 dipilih menggunakan teknik simple random sampling. Instrumen kuesioner diadaptasi dari kuesioner P4K oleh JHPIEGO dan observasi menggunakan buku KIA. Data dianalisis menggunakan chi-square test. Hasil penelitian menunjukkan mayoritas ibu postpartum dalam kategori usia bukan risiko tinggi, berpendidikan rendah, memiliki pendapatan keluarga kurang, tidak bekerja, memiliki asuransi kesehatan, dan bukan termasuk keluarga penerima bantuan sosial. Sejumlah 69,2% ibu postpartum termasuk dalam kategori “persiapan kurang” dalam praktik P4K. Hasil analisis menunjukkan faktor pendapatan keluarga (p-value = 0,042 dengan nilai OR= 0,30; CI95%= 0.10 to 0.86) berhubungan dengan praktik P4K yang dilaksanakan oleh ibu dan keluarga. Kesimpulan menunjukkan pendapatan keluarga berhubungan dengan praktik perencanaan persalinan dan pencegahan komplikasi yang dilakukan oleh ibu dan keluarga. Oleh karena itu perlu adanya kerjasama lintas sektor di wilayah terkait fasilitasi praktik P4K melalui penggunaan aplikasi di tingkat lokal sehingga dapat mengurangi beban finansial masyarakat dalam mengakses fasilitas pelayanan kesehatan.   Kata kunci: pendapatan keluarga, perencanaan persalinan dan pencegahan komplikasi, postpartum   The birth preparedness and complication readiness program is a preventive effort to prevent maternal death by preventing three delays. Studies related to birth planning practices and prevention of complications by mothers and families are still limited, especially in Jember district the which contributed to the highest maternal mortality rate in East Java province for three years from 2019 to 2021. Objective: This study analyzes the relationship between sociodemographic characteristics and P4K practices. Method: Analytical research with a cross-sectional approach was conducted at the Sumberbaru Community Health Center, Jember, from April to June 2023. 78 postpartum mothers were selected using a simple random sampling technique. The instrument was adapted from the P4K questionnaire by JHPIEGO and observations using the KIA book. Data were analyzed using the chi-square test. Results: Most postpartum mothers were in the non-high-risk age category, had low education, had less family income, did not work, had health insurance, and were not included in families receiving social assistance. 69.2% of postpartum mothers fall into the "insufficient preparation" category in P4K practices. The results of the analysis show that the family income factor (p-value = 0.042 with OR = 0.30; CI 95% = 0.10 to 0.86) is related to P4K practices carried out by mothers and families. Conclusion: Family income is related to birth planning practices and preventing complications from mothers and families. Therefore, there is a need for cross-sector collaboration in the region regarding the facilitation of P4K practices through the use of applications at the local level so that it can reduce the financial burden on the community in accessing health service facilities.   Keywords: birth preparedness and complication readiness, family income,  postpartum

Abstract

The Birth Preparedness and Complication Readiness Program (P4K) is an effort to overcome maternal mortality by reducing three delays (3T), late decision-making, late reaching health facilities, and late getting treatment. The purpose of this study was to analyze P4K filling in maternal and child health (MCH) books with the causes of maternal death based on three delays (3T). The method used in this study is quantitative and descriptive using data on 58 cases of maternal death found at the Jember District Health Office. The results of this study were that there were 15 (25.8%) cases of maternal death with P4K in the MCH handbook which were incompletely filled in and identified as experiencing three delays and 12 (20.6%) cases of maternal death with P4K in the MCH handbook which were not filled out. This research can be used as material for evaluating the Birth Preparedness and Complication Readiness Program (P4K).

Abstract

Pendahuluan: Stunting menjadi permasalahan gizi prioritas yang perlu dilakukan tindakan pencegahan atau penanggulangan secara optimal karena berdampak terhadap peningkatan risiko terjadinya morbiditas dan mortalitas anak. Tujuan: Menganalisis efikasi diri ibu, dukungan sosial ibu, dan PMBA sebagai faktor risiko kejadian stunting pada anak usia 6-23 bulan. Metode: Penelitian ini menggunakan jenis penelitian kuantitatif dengan rancangan studi case control. Sampel penelitian secara keseluruhan adalah 72 orang yang terdiri dari 36 baduta stunting pada sampel kasus dan 36 baduta tidak stunting pada sampel kontrol. Data diperoleh melalui pengukuran menggunakan infantometer dan wawancara dengan responden menggunakan kuesioner yang sudah valid dan reliabel. Data dianalisis menggunakan uji Chi-Square dengan confidence interval 95%. Hasil: Sebagian besar ibu baduta stunting memiliki efikasi diri yang rendah (52,8%), dukungan sosial yang rendah (55,6%) dan melakukan PMBA yang kurang (86,1%). Penelitian ini menunjukkan bahwa terdapat hubungan antara efikasi diri ibu (p-value < 0,001; OR = 8,941), dukungan sosial (p-value < 0,001; OR = 7,750), dan PMBA (p-value < 0,001; OR 10,989) dengan kejadian stunting. Kesimpulan: Terdapat hubungan antara efikasi diri ibu, dukungan sosial ibu dan PMBA dengan kejadian stunting pada anak usia 6-23 bulan. Abstract

Background: Menstrual disorders vary among adolescents between 23% and 76% which can negatively impact adolescents’ health, academic success, and extracurricular activities. Knowledge and menstrual hygiene practices are important factors that influence seeking health services. This study aimed to assess students' knowledge and menstrual hygiene practices at the Faculty of Public Health, University of Jember, and to analyze the correlation between the level of knowledge and menstrual hygiene practices and the menstrual cycle of Public Health students. Method: This cross-sectional study was performed on female students at the Faculty of Public Health, University of Jember, classes of 2021, 2022, and 2023. This study was conducted from June to August 2024. A total of 150 public health students were recruited by simple random sampling. The public health students were interviewed using the questionnaire, and the data were analyzed using univariate and bivariate analyses using the chi-square test. Result: The study found that more than half (65,3%) of female students had poor knowledge about menstrual hygiene, while only 54,7% demonstrated good menstrual hygiene practices. The analysis showed no correlation between the knowledge levels and menstrual hygiene practices with the menstrual cycle among public health students at the University of Jember. Conclusion: These findings underscore the low level of knowledge and menstrual hygiene practices among the respondents, emphasizing the urgent need for action and the urgency of enhancing awareness about adopting good menstrual hygiene practices, organizing meetings with the female students, and the importance of community support in addressing this issue
