Abstract

Background: Patients with end-stage renal disease (ESRD) undergoing hemodialysis are at risk of developing active TB 6 to 25 times higher than the general population. The importance of screening tuberculosis infection using diagnostic tests such as the Tuberculin Skin Test (TST) and QuantiFeron TB-Gold (QFT-G) and proper treatment in hemodialysis patients can prevent the increase of LTBI to active TB. Purpose: To determine the prevalence of Latent Tuberculosis Infection (LTBI) cases in hemodialysis patients using the Tuberculin Skin Test (TST) and Quantiferon TB-Gold (QFT-G). Methods: A Systematic Review study to determine the prevalence of LTBI in hemodialysis patients using the Prefered Reporting Items for Systematic Review and Meta-analysis (PRISMA) protocol. Results: Derived from 67 related articles and obtained 4 articles using the Randomized Control Trial (RCT) which met the criteria involving 516 ESRD patients undergoing hemodialysis. The study showed that the prevalence of LTBI using the diagnostic Tuberculin Skin Test (TST) (cut off ≥ 10 mm) and QuantiFERON-Tuberculosis Gold (QFT-G) showed TST + / QFT + 68 patients (18.47%), TST + / QFT- 22 patients (5.97 %), TST- / QFT + 77 patients (20.92%). Conclusion: This study shows a high prevalence of LTBI in hemodialysis patients. Screening and treatment of LTBI should be performed in hemodialysis patients to prevent the progression of LTBI to active TB. Keyword: Latent tuberculosis infection, hemodialysis, TST, QFT-G, ESRD.

Abstract

Introduction: Tuberculosis is an infectious disease caused by Mycobacterium tuberculosis. The leading cause of death worldwide is responsible for 1.5 million deaths each year. The cause of tuberculosis infection is the tubercle bacillus discovered by Robert Koch in 1882. The organism causing it is Mycobacterium tuberculosis. The spread of new cases is divided in several regions including Southeast Asia and Africa, which are the regions that have the biggest contribution to the prevalence of tuberculosis. So it takes pillars and components of TB control that cover all lines. Method: This study aims to discuss the potential of herbal plants spread in Indonesia as herbal medicines that have the potential to treat tuberculosis infection. This study uses a literature study method which prepares the same as other research, but the sources and methods of data collection are by taking data from the library, reading, taking notes, and processing research materials. Result: One of the components that are currently being developed in TB infection intervention methods is the development of herbal medicines for tuberculosis control. The herbal medicines used come from garlic (Allium sativum), Centellaasiatica, beluntas leaves, Hibiscus rosasinensis L., Boechmeria virgate (Forst) Guill, tobacco leaves, Morindacitrifolia L., Javanese wood bark, Spondiaspinnata (Lf)Kurz. Conclusion: The herbal medicinal ingredients contain major chemicals such as alicin, ajoene, asiaticoside, asiatic acid, madecassic acid, and madecassoside, alkaloids, flavonoids, tannins, saponins, phenolic, and triterpenoids which have the ability to have a bactericidal effect to kill the Mycobacterium tuberculosis bacteria.

Result: The bivariate relationship between the level of disability in the elderly according to sex, area, age, social status and in different cities was analyzedwith P value <0.05 was considered statistically significant. The disability rate in adults is 7.0%. Disability rates were significantly higher in women than men, significantly higher in rural areas than urban areas, and higher in northern China than southern China. The urban disability rate ranges from 5.7% to 1.2%. The difference is statistically significant, with Beijing having the highest disability rate and Shanghai the lowest. Defects increase with age. Conclusion: This study shows that the rate of disability among elderly people in northern China is significantly higher than in southern China

Abstract

LTBI infection is a dormant condition of the tuberculosis organism. It is often defined as a condition without symptom with the tuberculosis organisms in the lung tissue, without clinical signs, or radiological signs. LTBI can become active TB in immunosuppressed hosts caused by comorbid disease or medical therapy such as patients with end-stage renal disease. Screening TB infection using examination tests such as the TST, QFT-G, and complete treatment in dialysis patients can avoid the progression of LTBI into active-TB. This research identified the incidence of LTBI in dialysis patients using the two-step TST and QFT-G. A Systematic Review study to identification the incidence of LTBI in dialysis patients using the Prefered Reporting Items for Systematic Review and Meta-analysis protocol. Derived from 67 related articles and obtained 4 articles using the Randomized Control Trial (RCT). Totally 516 dialysis patients and the study showed that the incidence of LTBI using two-step TST (cut off ≥ 10 mm) and QFT-G (≥ 35 IU/ml) showed that TST /QFT-G positive 68 patients (18.47%), only positive TST 22 patients (5.97 %), only positive QFT-G 77 patients (20.92%). This study showed that a high incidence of LTBI in dialysis patients. Early detection and complete treatment of LTBI should be performed in dialysis patients to avoid the reactivation of LTBI into active TB.
