Abstract

Bedah kraniotomi merupakan pembedahan pembukaan kranium untuk meningkatan akses pada struktur intrakranial yang berisiko terhadap kerusakan jaringan dan komplikasi lainnya. Manajemen post operasi dalam perawatan dapat dilakukan dengan melakukan proses keperawatan. Masalah keperawatan merupakan dasar untuk diagnosis keperawatan pada rencana perawatan perawat. Tujuan penelitian ini adalah untuk menganalisis gambaran masalah keperawatan pasien post kraniotomi di RSD dr. Soebandi Kabupaten Jember. Jenis penelitian ini adalah deskriptif kuantitatif pada variabel masalah keperawatan menggunakan pendekatan studi retrospektif dengan metode simple random sampling. Sampel penelitian ini adalah 94 rekam medis pasien post kraniotomi yang dihitung menggunakan rumus slovin dari jumlah populasi pada Januari 2016-Desember 2017. Hasil penelitian menunjukkan bahwa masalah keperawatan utama pada pasien post kraniotomi adalah nyeri akut (58,51%), ketidakefektifan bersihan jalan napas (25,53%), dan ketidakefektifan pola napas (15,96%). Diagnosis keperawatan yang berdasarkan indikator dengan mencerminkan PES/PE sebanyak 39 (41,49%) dan yang tidak mencerminkan PES/PE sebanyak 55 (58,51%). Perawat memiliki peranan penting dalam manajemen post operasi khususnya pada asuhan keperawatan. Masalah keperawatan perlu dirumuskan dengan memperhatikan data-data yang mendukung munculnya masalah keperawatan pada pasien guna meningkatkan kualitas asuhan keperawatan

Abstract

Background: Dying is a normal human phenomenon that requires a holistic care approach. Nurses’ attitudes towards the care for dying patients need to be explored, understood, and analyzed to improve the quality of care in palliative setting, including in nursing students.Purpose: This study explored the nursing students’ attitude and its relationship with the students’ demographic profile in caring for the dying patients in Indonesia.Methods: This study used a quantitative descriptive cross-sectional research design.  The samples were 192 nursing students from Universitas Jember, Indonesia, who were recruited by using simple random sampling. Data were collected using the Frommelt Attitudes towards the Care of the Dying Care Form B Indonesian version (FATCOD-BI). The reliability test of FATCOD-BI showed a Cronbach’s alpha of 0.68, and the result of validity test using correlation coefficient showed the range of -0.278 to 0.544. Data were analyzed using frequency distribution, and mean differences test using t-test and One-Way Analysis of Variance (ANOVA).Results: Results showed that the mean of nursing students’ attitudes in caring for dying patients was 93.83±5.96 (range 30-120). Gender and training experiences had no relationship with students’ attitudes in caring for dying patients (p=0.22 and p=0.943). There was a relationship between the experiences and student academic level and the students’ attitudes in caring for dying patients (p=0.023 and p=0.036). The students’ experiences and academic level become a primary factor in the attitudes toward caring for dying patients.Conclusion: Findings revealed that student nurses’ attitudes in caring for dying patients was in low category and there was no significant relationship between gender and training experience and students’ attitudes towards caring for the dying patients. In contrast, experiences in caring for dying patients and academic level were associated with students’ attitudes. Further studies in the development of curriculum on dying patient care emphasizing on socio-demographic status are recommended.

Abstract

Background: Volunteering is a common concept used in various disciplines including in the health care context. Nursing plays a role in volunteering concept development. However, the use of volunteering concept is still interchangeable. The clear definition concept about volunteering will promote the improvement in healthcare system quality.Purpose: This study aimed to clarify the volunteering concept in the health care context using Rodger’s evolutionary concept analysis.Methods: This study used a literature review for collecting the exemplars. Researchers used articles published in 2010-2020 in ScienceDirect, PubMed, Web of Science, and Google Scholar. Articles were searched using OR and AND. The keywords and MeSH used were “Volunteering OR Volunteerism OR volunteers AND Health OR Care OR Nursing” in the title, abstract, and keyword articles. Inclusion criteria were full articles in English. Rodger’s evolutionary concept analysis was implemented in this study with six steps.Results: This study used 39 eligible articles analyzed by providing codes of each article and then classified them in attributes, antecedents, and consequences of concept. Five attributes of volunteering, namely, giving help freely, as long-term planned, as giving a benefit to another individual, group, or organization, as working at a formal organization, and as a long-life activity were found. Antecedents of volunteering include motivation, social demographic, and philanthropic behavior. The consequences of volunteering were consequences for volunteers, patients, and the health care system.Conclusion: Volunteering is a complex concept and many surrogate concepts have similar definitions. The attributes are still debatable and need to be explored. Also, the consequences of volunteering are still rarely discussed in a particular health care system context. Further studies need to analyze the measurement of volunteering development based on the nursing theory framework and explore the consequences of volunteering in the health care system related to the nursing care context.

Abstract

Pendidikan atau pelatihan mitigasi bencana dapat dilakukan sejak dini. Pelatihan mitigasi bencana pada sekelompok anak sekolah dasar merupakan solusi konkrit untuk meningkatkan kewaspadaan terhadap bencana banjir di Desa Sidomulyo. Permasalahan mitra yang dihadapi adalah Kurikulum atau mata pelajaran yang spesifik membahas mitigasi bencana atau kesiapsiagaan bencana di pondok pesantren Subulus Salam masih belum diberlakukan. Metode pelaksanaan pada pengabdian ini adalah: tahap wawancara dengan pengelola pondok dan guru/ustadz; Tahap Penyusunan modul; dan Tahap pelatihan. Hasil pengabdian didapatkan bahwa pengurus ponpes mendukung penuh terhadap program ini dan hasil pelatihan menggambarkan terdapat peningkatan kesiapan santri ponpes Subulus Salam dari 16.3% ke 100%.  Kedepannya, perlu adanya pelatihan

Abstract

Spiritualitas adalah faktor protektif dalam proses berduka pada keluarga pasien kronis. Bereavement life review adalah salah satu intervensi dalam penguatan spiritual keluarga pasien penyakit kanker. Stroke dan kanker adalah penyakit kronis. Tujuan penelitian ini adalah mengetahui pengaruh bereavement life review pada kesejahteraan spiritual keluarga pasien stroke. Desain penelitian ini adalah quasi-eksperimental dengan pretest posttest control group. Sampel yang digunakan adalah salah satu keluarga pasien stroke yang merawat pasien di rumah sakit. Sehingga didapatkan sampel sebanyak 28 responden dengan 14 kelompok kontrol dan 14 kelompok intervensi. Pengambilan sampel dilakukan dengan menggunakan consecutive sampling. Kelompok intervensi mendapatkan bereavement life review dengan dua sesi yang dilakukan oleh spesialis keperawatan jiwa. Kesejahteraan spiritual diukur menggunakan instrumen SWBS (spiritual well-being scale). Analisis data menggunakan dependent t-test, Mann Whitney dan Wilcoxon. Uji homogenitas memerlihatkan tidak satupun karekteristik responden antara kelompok intervensi dan kontrol berbeda secara signifikan (p > 0,05). Hasil penelitian menunjukkan adanya perbedaan skor rerata postest kesejahteraan spiritual pada kelompok kontrol dengan kelompok intervensi (98,71 ± 3,65 dan 106,5 ± 1,83; p = 0,000). Terdapat perbedaan skor rerata kesejahteraan spiritual pada pretest dengan posttest pada kelompok intervensi (99,07 ± 2,95 dan 106,5 ± 1,83; p = 0,001). Proses bereavement life review merupakan proses peningkatan spiritual melalui proses rekontekstualisasi, memaafkan terhadap diri, dan refleksi yang membentuk penguatan koping sehingga muncul pemaknaan terhadap diri sendiri. Dapat disimpulkan bereavement life review berpengaruh positif terhadap peningkatan kesejahteraan spiritual keluarga pasien stroke. Bereavement life review dapat digunakan sebagai intervensi perawatan pasien stroke dan keluarga. Saran untuk penelitian selanjutnya adalah melihat pengaruh bereavement life review untuk penyakit kronis yang lain, seperti diabetes melitus atau kanker. Selain itu, indikator psikologis pasien dan keluarga sebagai output intervensi perlu dikaji lebih mendalam.Kata kunci: Bereavement life review, keluarga pasien stroke, keperawatan spiritual, kesejahateraan paliatif.Influence of Bereavement Life Review on Spiritual Well-Being of Stroke Family CaregiverAbstractSpirituality is a protective factor of grieving process in patient and family with chronic illness. Bereavement life review is one of the interventions which is enhancing the spiritual well-being in cancer diseases. Cancer and Stroke are chronic diseases. The purpose of this study was to determine the effect of bereavement life review of the spiritual well-being of stroke family. Quasi-experimental with pretest posttest control group used in study. Sample in this study are stroke family who caring the stroke patient in hospital which is 28 respondents. The intervention group was given bereavement life review with two sessions which given by expert in psychiatric nursing. Spiritual well-being was measured by SWBS (spiritual well-being scale). Data analysis were using a dependent t-test, Mann Whitney and Wilcoxon. Homogenity of respondent characteristics showed that it have not correlation between control and intervention group (p > 0,05). The study showed the difference in the mean posttest scores of spiritual well-being of the control group with the intervention group (98.71 ± 3.65 and 106.5 ± 1.83, p = 0.000). There were differences in the mean scores pretest to posttest spiritual well-being in the intervention group (99.07 ± 2.95 and 106.5 ± 1.83, p = 0.001). Bereavement life review is a process of enhancing spirituality through recontextualization, forgiveness, and reflection proccess that strengthening coping process. Bereavement life review has positive effect on the spiritual well-being of the stroke family which can be considered as an intervention in the treatment of stroke patients and families. Further study know the effect of bereavement life review in other chronic diseases patient, like hypertension or diabetes mellitus. Moreover, other psychological outcome for this intervention needs to be explored.Keywords: Bereavement life review, palliative care, spiritual well-being, stroke family.

Abstract

The emergency room is the patients’ first stop and referral in hospital. Patient service in the emergency room requires immediate services, which are effective and efficient. The success of the nurse’s response time in handling patients can prevent damage to body organs, prevent disability and reduce mortality. This study aims to undrstand the correlation between workload and the response time of the emergency room nurse at Type C hospitals in Jember Regency. This study uses a correlational research design with a cross-sectional method. The sampling technique used total sampling technique. The result of this study indicated that the workload of the nurses in the emergency room was classified as moderate and had a response time according to the standard. The data analysis used Kendall tau-c statistical test and the p-value = 0,180. In conclusion, there was no correlation between workload and the response time of the emergency room nurse at Type C hospitals in Jember Regency. It happens because there are factors that influence the nurse’s motivation, chosen examination and treatment strategies when serving the patients. Their high motivation and hardwork make their performance better. In addition, the selection of inspection strategies and appropriate handling can make the services of the patients effective and efficient.

Abstract

Schizophenia is a mental illness shows psycological disorder, violent behavior, and inability self care. This has created a burden for family caregiver, financially, social, and psychological. The purpose of this study to identified the relationship between self compassion and stress of schizophrenis family caregiver in Puskesmas (PHC) Mumbulsari Jember. The study used correlation study with the cross sectional approach. The sampling was counted 35 family caregiver using total sampling. The instrument used self compassion scale (α=0,89) and stres in family (α=0,977). The result showed that the average self compassion of family caregiver was 71.31(26-104). While the average stress of family caregiver was 18,74 (0-28). Bivariate analysis using pearson test showed that there was no correlation between self compassion and stress of schizophenia family caregiver in Puskesmas (PHC) Mumbulsari Jember. There was no correlation between stress of schizophenia family caregiver and gender, age, education level, profession, relationship with schizophenia patient, and long tending of family caregiver. This has showed that family caregiver covered stress and showed a positive self acceptance. Actually, family caregiver showed that noncompliance of patient recovery, many of them was not accompanying schizophenia patient to public health, and neglecting schizophenia patient. Counseling and health promotion should be enhanced psychological health of family caregiver in treating schizophrenia patient in community through public health care serving. Abstract

Elderly is a condition where someone experience a lot of changes, for instance psychological change.Loneliness is a elderly psychological disorder elderly that can be overcome with spirituality. This studyaimed to analyze the correlation between spirituality and loneliness in elderly in the Pelayanan SosialTresna Werdha (PSTW) Jember. This research applied an observasional analytic design with crosssectional approach. 91 respondents are gained by employing simple random sampling using SpiritualWell-Being Scale (SWBS)(r=0,895) and UCLA Loneliness Scale (r=0,94). Data were analyzed byusing Spearmen correlation test with significance level of 0.05. The result of the spirituality variablereserach shows 71,00 median and ± 45-86, meanwhile on loneliness variable 38,00 median and ± 20-69. There was a significant negative correlation between spirituality ang loneliness (p value: 0.000 ; r:-0,718). Spirituality become one of coping strategy to overcome elderly’s loneliness. Spiritualitybecome power to elderly in facing problem and increasing life quality so can decrease their loneliness.The study shows that the importance of increasing the spirituality to decrease loneliness level onelderly who live at Panti Werdha.Keywords: elderly, spirituality, loneliness

Abstract

Cataract is a sight obstruction which causes many cases of blindness in Indonesia. One of the treatment managements of cataract is performed trough sugery. Surgery process may cause anxiety to a patient with sign hypertension. The research aimed to analiyzing the effect of the murottal Al-Qur'an therapy on blood pressure of pre operative cataract patients with hypertension. Independent variable was the murottal Al-Qur'an therapy and dependent variable was blood pressure of pre operative cataract patients with hypertension. The research method was quasi experimental with randomized control group pretest and posttest design. The sampling technique used quota sampling to 30 respondent fulfilled then it used simple random sampling divided into two groups, treatment and control group with 15 respondents for each. Data were analyzed by using Wilcoxon test and Mann Whitney test with 95% of CI. The research results showed that p value of Wilcoxon test in treatment group was 0,000 (systole) and 0,002 (diastole), while control group resulth p value of Wilcoxon test was 0,007 (systole) and 0,317 (diastole). The research results showed that p value of Mann Whitney test in systolic blood pressure category and diastolic blood pressure category was 0,000 for each. Summed up any impact the murottal Al Qur’an to pressure of patient pre cataract surgery by hipertension. Abstract

Background: Death is a psychological and physical event that affects patients caring for, especially in Emergency Departement (ED) and the Intensive Care Unit (ICU). Patients and families need the end of life care from a health professional, including nurses. Nurses need to have knowledge, skill, attitude, and interpersonal competencies to provide end of life care. Objective: This study aimed to explored differences of attitudes towards dying care between ED and ICU ward nurses in a rural hospital.Method: Variable in this study was the nurse's attitude. This study used a quantitative comparative cross-sectional research design. The samples were 24 nurses from the emergency ward and 16 from the intensive care unit who were recruited by total sampling. Data were collected to use Frommelt Attitudes Towards the Care of the Dying Care Form B Indonesian version (FATCOD-BI).Result: The results revealed that nurses' attitudes toward caring for dying patients in the ED were 101.42±6.646 (30-150) on average, and in the intensive care unit was 106.44±7.633 (30-150) on average. Nurses in the ICU had a more positive attitude than the ED (p = 0.034). There are differences between the proportion of respondents based on gender, level of education, length of time working in the emergency ward, and ICU. The result showed gender, level of education, and range of time working could influence nurses' attitudes towards caring for dying patients in the emergency ward and ICU.Conclusion: There is a difference between nurses' attitudes towards EOLC in the ED and ICU room at rural hospitals. Moreover, the ICU nurses have a more positive attitude than ED nurses.
