Abstract

Electronic cigarettes that are currently popular are used as nicotine replacement therapy. There are several substances contained in electronic cigarette liquid that have carcinogenic effects. These substances can also increase free radicals which in turn can cause oxidative stress. Nicotine, propylene glycol, and glycerin can cause disturbances in male reproductive function. Infertility can occur due to lifestyle factors, one of which is smoking. Nicotine found in cigarettes can reduce sperm quality, including concentration, mobility, viability, and sperm morphology. The purpose of this study was to determine the effect of electronic cigarettes on sperm quality. The research method uses a literature review system. The twelve studies that are in accordance with the topic of discussion are experimental studies on mice and rats, two studies on human, and one study on both human and rats. Five journals showed a significant decrease in sperm count and testosterone serum level, three other journals showed a significant decrease in sperm morphology and viability, six journals showed a decrease in sperm motility, seven journals showed a significant impairment of testis histological structure, four journals showed a decrease of steroidogenesis enzymes, and one journal showed a significant increase of DNA fragmentation due to the use of electronic cigarettes. Electronic cigarettes had an effect on sperm quality.

Abstract

Infertility is a failure to get pregnant after one year of sexual intercourse without using contraception. The causes of infertility, especially in men, are very complex, including the aging process, hormonal disorders, lifestyle, environment, metabolic disease, and genetic problems. One of the most difficult causes in male infertility  is genetic problems. In this case, a 35 year old man was reported with primary infertility and often experienced premature ejaculation and even anejaculation. Previously, the patient had repeatedly consulted about his condition at other fertility service centers. The conclusion obtained was that the patient had azoospermia with bilateral varicoceles and had been given hormone therapy but had not yet found a final diagnosis so the patient was still confused about what had happened to him. At the first visit the patient underwent a sperm analysis and the results were azoospermia. Then, based on the results of the history and physical examination, which indicated hypogonadism, a Y chromosome microdeletion examination was carried out, and a deletion was found in the AZFc region, which is a marker of infertility that causes spermatogenic failure. The examination was continued with karyotyping, the result was 47.XXY, consistent with the condition of Klinefelter syndrome. Conclusion: Carry out a karyotyping examination if you find signs and symptoms that suggest primary hypogonadism in male infertility. This can be done to streamline the diagnostic approach time in patients with primary hypogonadism, especially those with infertility.

Abstract

Background: Every married couple hopes to have children, but not all can fulfil that desire. A married couple finds themselves unable to achieve pregnancy after 12 months of regular unprotected sexual intercourse, which can be called infertility. Infertility can also occur in men. In dealing with infertility, intrauterine insemination (IUI) is a cost-effective and non-invasive Assisted Reproductive Technology (ART) procedure. The condition of male infertility can be detected through semen analysis. One of the parameters in semen analysis is the total motile sperm count (TMSC). Objective: This research seeks to establish the relationship between post-preparation TMSC and the success rate of the IUI procedure. Methods: This research adopts a descriptive-analytical design employing a cross-sectional methodology. Non-probability sampling was utilized as the sampling technique, and the data analyzed were secondary data from the medical records of male patients aged 25 to 45 years old diagnosed with infertility. The subjects had undergone IUI at Rumah Sakit Kasih Ibu Denpasar from January 1 to December 31, 2022. Results: From the 35 obtained samples, 12 individuals (34.3%) had a post-preparation TMSC of ≥10 í— 106, while 23 individuals (65.7%) had a post-preparation TMSC of <10 í— 106. The success rate of the Intrauterine Insemination (IUI) procedure was 17.1%, with 4 couples (33.3%) having a post-preparation TMSC of ≥10 í— 106 and 2 couples (8.7%) with a post-preparation TMSC of <10 í— 106. The Fisher's Exact test carried out has obtained a result (p=0.151), while the value of the odds ratio (OR) obtained (OR=5.250) Conclusion: Based on statistical analysis, it can be concluded that there is no significant association between post-preparation TMSC in infertile men and the success rate of IUI procedures (p > 0.05). The odds ratio (OR) obtained is 5.250 (95% CI 0.799-34.496).

Abstract

Hypogonadism is a clinical syndrome associated with aging, characterized by low levels of testosterone in a man's body. Hypogonadism can lead to various diseases, one of which is coronary heart disease. Coronary heart disease is a cardiovascular disease caused by the buildup of plaque in the coronary arteries. This research aims to examine the relationship between hypogonadism and coronary heart disease in men at RSUP Prof. Ngoerah. The research was conducted analytically with a case-control design. Hypogonadism was measured using the ADAM score questionnaire, which consists of 10 questions. The research sample comprised men who were coronary heart disease patients and men who were non-coronary heart disease patients aged 35–45 years in the Gedung Pelayanan Jantung Terpadu RSUP Prof. Ngoerah. The sample was determined using a consecutive sampling technique, with a total of 50 participants. The research results found that 28 participants (56%) tested positive for hypogonadism, and 22 participants (44%) tested negative for hypogonadism. A p-value of 0.004 indicated a significant relationship (p ≤ 0.05) between hypogonadism and coronary heart disease. The calculated Odds Ratio (OR 95% CI) between hypogonadism and coronary heart disease was 5.6 (1.648 – 19.232). Among the research participants, 19 (76%) were both positive for hypogonadism and coronary heart disease. Meanwhile, 16 participants (64%) were negative for both hypogonadism and coronary heart disease. The lower the testosterone levels in men, the higher the risk of developing coronary heart disease. Coronary heart disease patients with hypogonadism were 5.6 times more likely to have the condition compared to those without hypogonadism. There is a significant correlation between andropause and coronary heart disease in men at RSUP Prof. Ngoerah.

Abstract

Background : A decrease in testosterone level can cause a set of symptoms forming a clinical syndrome called hypogonadism. One of the factors that is known to disrupt testosterone levels is smoking habits. Currently, the use of e-cigarettes as an alternative product to conventional cigarettes is increasing rapidly in many countries, including Indonesia. The amounts of harmful substances in e-cigarette aerosols are similar to the smoke produced by conventional cigarettes, but the health hazards of e-cigarettes are still not well known. Objective : This study was conducted to determine the relationship between e-cigarettes and hypogonadism in adult men in the city of Denpasar. Method: This study uses a descriptive-analytical method in the form of a cross-sectional study. The research sample consisted of 49 e-cigarette users collected using consecutive sampling techniques. The data were then analyzed using SPSS® version 26. Results : Based on the research obtained from the ADAM score from a total of 49 samples, the prevalence of hypogonadism is higher in the group of people who smoked e-cigarettes, about 35,3%. Overall, this study found a fairly high prevalence of hypogonadism (26.5%) in Denpasar. The chi-square test shows a p-value of 0,032 and a prevalence ratio (PR) of 5.294. Conclusions : Active use of e-cigarettes showed a significant association with the incidence of hypogonadism experienced by adult men in Denpasar. Based on the PR calculation, the risk of hypogonadism in active e-cigarette users was 5.294 times higher than that of adult men who were not active e-cigarette users.

Abstract

Background : Coronary heart disease (CHD) has become one of the major economic burdens globally, as CHD has been the leading cause of mortality worldwide for the past 25 to 30 years. One of the factors that plays a role as a predictor of coronary heart disease is erectile dysfunction. Objective : To investigate the correlation between erectile dysfunction and coronary heart disease in male patients at RSUP Prof. Ngoerah. Methods : This research is an observational analytic study with a case-control design. Erectile dysfunction was measured using the IIEF-5 questionnaire, which consists of 5 items. Other risk factors were measured using a patient data questionnaire. The study sample consisted of 50 participants, divided into 25 cases and 25 controls. The case samples were male patients aged 25–60 with a history of coronary heart disease who were visiting or receiving treatment at the Cardiology Outpatient Clinic, Integrated Cardiac Services Building, RSUP Prof. Ngoerah, Denpasar. The control samples had the same criteria as the case samples but had no history of coronary heart disease. Results : A significant relationship was found between erectile dysfunction and coronary heart disease (p=0.049), with an OR of 9.33 (95% CI 1.05–82.8). Conclusion : There is a significant relationship between erectile dysfunction and coronary heart disease. Patients with erectile dysfunction have a 9.33 times higher likelihood of having coronary heart disease.
