Abstract

Latar belakang: Stroke non hemoragik adalah suatu penyakit yang diawali dengan terjadinya serangkaian perubahan dalam otak karena terhambatnya atau berhentinya suplai darah ke otak karena adanya sumbatan. Pasien stroke non hemoragik pada umunya akan mengalami gangguan sensoris dan motoris yang mengakibatkan gangguan keseimbangan termasuk kelemahan otot, serta hilangnya koordinasi, hilangnya kemampuan keseimbangan tubuh dan postur (hemiparesis). Tujuan: Membandingkan kedua masalah keperawatan Stroke Non Hemoragik kedua pasien dalam pemenuhan kebutuhan aktivitas dan latihan. Metode: Jenis penelitian deskriptif analitik dengan pendekatan studi kasus untuk mengeksplorasi masalah asuhan keperawatan pasien stroke non hemoragik dalam pemenuhan kebutuhan aktivitas dan latihan. Subjek dalam studi kasus ini dua pasien dengan inisial Ny.N yang berusia 62 tahun dan pasien kedua dengan inisial Ny.M yang berusia 46 tahun, Asuhan keperawatan dilakukan diruang penyakit dalam disalah satu rumah sakit Swasta kota Palembang. Sedangkan proses pengambilan asuhan keperawatan dari tanggal 24 s/d 26 Mei 2019. Analisa data yang digunakan dalam studi kasus ini adalah analisis deskriptif yang digunakan untuk menganalisis data dengan cara mendeskripsikan data yang terkumpul untuk membuat suatu kesimpulan yang disajikan secara tekstular/narasi. Hasil: Pengkajian mendapatkan data bahwa Ny.N tidak bisa menggerakan seluruh badannya, pasien hanya berbaring. Sedangkan Ny.M tidak bisa menggerakan tangan dan kaki kirinya. Intervensi keperawatan berfokus utama diagnose hambatan mobilitas fisik memiliki tujuan meningkatan kekuatan otot, pasien berpartisipasi dalam aktivitas fisik. Implementasi keperawatan Terdapat perbedaan hasil implementasi keperawatan pada Ny. Tidak terdapat peningkata kekuatan otot dikarena kondisi kesadaran samnolen sedangkan Ny.M peningkatan kekuatan otot pada ekstremitas atas menjadi skala 6 dan ekstremitas bawah skala 4. Saran: diharapakan ada jadwal rehabilitasi terpadu yang terinteritas antara perawat, fisioterapi dan keluarga untuk mencegah tidak terjadi kontraktur ataupun atrofi otot.

Abstract

Gastritis termasuk proses inflamasi atau gangguan kesehatan yang disebabkan oleh faktor iritasi dan infeksi pada mukosa dan submukosa lambung. Penyakit Gastritis biasa dikenal dengan penyakit maag. Tujuan penelitian ini Memperoleh pengalaman dan pengetahuan secara nyata serta dapat mendokumentasikan dalam Penerapan Teknik Relaksasi Nafas Dalam terhadap Intensitas Nyeri pada Asuhan Keperawatan Gastritis. Metode penelitian ini adalah Jenis Karya tulis ilmiah ini menggunakan metode deskritif dengan pendekatan studi kasus untuk mengeksplorasi masalah Asuhan Keperawatan pada dua pasien dengan Gastritis. Asuhan keperawatan dilakukan pada dua pasien yang dilakukan pada tanggal 16-18 Juni 2021. Dari hasil asuhan keperawatan pada Tn”H” dan Ny”S” dengan diagnosa pertama yaitu Nyeri Akut berhubungan dengan agen pencendera fisiologis (inflamasi pada mukosa lambung). Hasil evaluasi pada pasien pertama Tn “H” didapatkan pasien tidak merasakan nyeri lagi, sebelum dilakukan Teknik relaksasi nafas dalam skala nyeri 5 (sedang), masalah nyeri teratasi, dan intervensi di hentikan. Untuk evaluasi yang didapat pasien kedua Ny “S” didapatkan skala 2 (Ringan), sebelum dilakukan Teknik relaksasi nafas dalam skala nyeri 6 (sedang), masalah nyeri teratasi Sebagian, dan intervensi dilanjutkan oleh perawat ruangan. Berdasarkan kesimpulan, atatan perkembangan kepada kedua pasien penerapan teknik relaksasi nafas dalam untuk menurunkan intensitas nyeri. Hasil evaluasi keperawatan pada Tn “H” masalah teratasi sedangkan pada Ny “S” masalah teratasi sebagian.

Abstract

Background: Therapeutic communication is a shared experience between nurse-clients that aims to resolve client problems. Therapeutic communication includes interpersonal communication with a starting point for mutual understanding between nurses and patients. The fundamental problem of this communication is mutual need between nurses and patients, so that it can be categorized into personal communication between nurses and patients, where nurses help and Clients receive assistance. Objective: this study aims to determine the relationship between the application of therapeutic communication by nurses and patient satisfaction in the Emergency Room (IGD) Institute of Muhammadiyah Hospital Palembang. Method: this study used a cross sectional design. The research sample consisted of 32 respondents (16 nurse respondents and 16 patient respondents) were taken using incidental sampling techniques for patient respondents while those perwat respondents used a total sampling technique. Results: the study showed no relationship between the application of communication (p-value 0.213), orientation phase (p-value 0.213), work phase (p-value 0.213), termination phase (p-value 1.00). Suggestion: for hospitals to improve the application of more optimal therapeutic communication to patients, especially in the emergency room.

Abstract

Background: The Situation, Background, Assessment And Recommendation (SBAR) method was very effective in increasing the effectiveness of Handover process between shifts or between staff in the same or different clinical areas. The SBAR method was used when nurses carried out the handovers, moved the treatment rooms and reported the patient's condition to the doctor. One of the factors that influenced the implementation of the effectiveness of SBAR communication was the role of room head supervision as a form of learning guidance, sharing experiences, providing motivation and counseling. Objectives: To determine the effect of head nurse’s supervision on the implementation of effective communication (sbar) during the handover process. Methods: This study used a qualitative approach with the Pre-Experimental Experiment method, while the research design was One Grup Design pre-intervention - post-intervention. This research was conducted at a private hospital in Palembang. The data normality test was carried out by using the Kolmogorov-Smirnov statistical test, while the bivariate analysis used a non-parametric statistical test, namely the Wilxocom test. Results: The results of the study on the implementation of SBAR effective communication showed the results before intervention as much as 100.0% in the poor category, while after the intervention treatment as many as 60.0% in the perfect category. Based on the Wilcoxon signed rank statistical test, the Z value was -3.411 and the asymp sig value. (2-tailed) 0.001 is smaller than the alpha level 5% (0.05) so that Ho was rejected; there is a significant average difference before and after the supervision of the head of the room on the implementation of SBAR effective communication. Suggestion: It is expected to improve the implementation of SBAR effective communication at the time of Handover which is more optimal, especially in the room by resocializing SPO, optimizing the supervision of the head of the room and continuing to attend training, especially on Effective SBAR communication.

Abstract

The quality of clinical education and the generation of nursing resources in Indonesia are influenced by the success of achieving clinical nursing learning by students in clinical practice. Because clinical learning is an important stage in improving clinical competence for students to practice skills, increase knowledge and develop a professional spirit of students into an atmosphere of clinical trials to evaluate real cases. The purpose of this study was to analyze the factors that influence learning outcomes in clinical learning among nursing students in Indonesia. This study used the Literature review method by taking electronic database data sources, namely Google Schloar, Science Direct, PubMed and Garuda with articles published in 2017 to 2022 with the keywords Clinical Learning, Learning Outcome, Clinical Competence, Nursing Student. After selection and identification according to the inclusion criteria, 15 articles were reviewed. Based on the results of the analysis of 15 research articles reviewed. There are several factors that influence the learning outcomes of clinical competency learning for clinical nursing practice students in Indonesia, namely the application of clinical learning methods, the role of academic preceptors and clinical preceptors and factors from students (preceptees). Suggestion: it is hoped that students, academic preceptors and clinical preceptors can improve interpersonal relationships and communication optimally and determine effective learning methods to improve the Learning Outcome Of Nursing Clinical Practice.

Abstract

Standardized nursing documentation is essential for ensuring consistency, accountability, and quality in patient care. In Indonesia, the 3S framework—comprising the Standar Diagnosa Keperawatan Indonesia (SDKI), Standar Luaran Keperawatan Indonesia (SLKI), and Standar Intervensi Keperawatan Indonesia (SIKI) serves as a national standard. However, its application in inpatient settings remains inconsistent due to the lack of integrated, user-friendly tools. This study aimed to develop and validate a nursing care documentation instrument based on the 3S framework to improve documentation accuracy, support clinical decision-making, and promote standardization in inpatient wards. A Research and Development (R&D) approach was employed, using a modified Borg and Gall model across five stages: preliminary study, planning, product development, expert validation, and limited field testing. Data were gathered through literature reviews, chart audits, clinical interviews, and focus group discussions with nurses. The instrument was designed to address the 12 most frequently encountered nursing diagnoses in general medical-surgical wards, which were systematically mapped to appropriate SLKI outcomes and SIKI interventions. The final tool features structured diagnosis formats (PES, PR, PS), outcome targets with timelines, and categorized interventions (observation, treatment, education, collaboration), implemented in a shift-based checklist format. Validation by five expert nurses yielded strong content validity (I-CVI range: 0.833–1.000), and field testing with 15 clinical nurses demonstrated acceptable internal consistency (KR-20: 0.790–0.845). Additionally, 87% of nurses reported improvements in documentation quality and workflow efficiency. In conclusion, the 3S-based nursing care instrument is valid, reliable, and practical for clinical use, and its adoption is recommended to enhance nursing documentation standards and the quality of care in hospital settings.

Abstract

Background : Appendectomy is a surgical procedure to remove the appendix which is done as soon as possible to reduce the risk of perforation. Nursing problems that arise after appendectomy is acute pain. Objectives: Gaining real experience and knowledge and being able to document the application of deep breathing relaxation techniques is an appropriate non-pharmacological action in reducing pain scale. Method: this type of scientific paper uses a descriptive method with a descriptive case study approach with a case study approach to explore the problem of nursing care carried out on two patients carried out on June 15 - 20 2021. Results: from the results of nursing care to Mrs "S" and Mr "M" with a nursing diagnosis in the first patient, namely acute pain related to physical injury agents (eg abscess, amputation, burns, cuts, heavy lifting, surgical procedures, trauma, excessive physical exercise), impaired sense of comfort related to symptoms of the disease ( pain), impaired physical mobility related to decreased muscle strength (pain), and the second patient's diagnosis was acute pain related to physical injuring agents (eg abscess, amputation, burns, cuts, heavy lifting, surgical procedures, trauma, excessive physical exercise), discomfort related to symptoms of disease (pain), anxiety related to situational crises, lack of exposure to information, impaired physical mobility associated with decreased muscle strength (pain). The interventions in both patients were: identification of location, characteristics, duration, frequency, quality and intensity of pain, identification of pain scale, identification of non-verbal pain responses. The evaluation of the first patient had a pain intensity scale of 6 (moderate), and the second patient a pain scale of 7 (severe). The results of the study in the Surgery Room of the Palembang Bari Regional General Hospital after the deep breathing relaxation technique was carried out in the first patient on a pain scale of 6 (moderate) to 3 (mild), and the second patient pain scale of 7 (severe) to 4 (moderate). Conclusion: Progress notes to both patients on the application of deep breathing relaxation techniques to reduce pain intensity. The results of the nursing evaluation of the problem were partially resolved.

Abstract

Accurate and comprehensive nursing documentation is essential for ensuring continuity of care, legal accountability, and quality outcomes. However, many nurses face challenges in implementing standardized documentation practices. The 3S-based framework comprising SDKI (Standar Diagnosis Keperawatan Indonesia), SLKI (Standar Luaran Keperawatan Indonesia), and SIKI (Standar Intervensi Keperawatan Indonesia) has been developed to enhance the quality and consistency of nursing records. This study aimed to evaluate the effectiveness of 3S-based nursing documentation training on nurses’ knowledge, performance, and documentation skills. A quasi-experimental design with a pretest-posttest approach was conducted at two hospitals: RSUD Sungaililin in December 2024 and RSUD Bayung Lencir in April 2025. A total of 80 nurses participated, with 40 respondents from each hospital. The intervention consisted of structured training sessions on the 3S documentation model. Data were collected using validated questionnaires and observation checklists, and were analyzed using paired and independent t-tests to assess changes before and after the intervention. The findings showed a significant improvement in nurses’ knowledge, performance, and documentation skills following the training (p < 0.05). Both hospitals demonstrated consistent positive outcomes, indicating the effectiveness of the 3S-based training regardless of setting. The 3S-based documentation training significantly enhances nurses’ competencies in recording patient care. These results support the integration of structured documentation models into nursing education and hospital in-service training programs to improve care quality and documentation practices.
