Abstract

Work accidents are a risk that can occur in various industrial sectors, including the oil and gas sector which has a high potential for danger. This article discusses the case of a pertalite tank fire at PT X, which became one of the significant incidents in the oil refinery industry in Indonesia. This study aims to analyze the causes of the incident, the impact caused, and preventive efforts that can be made to minimize similar risks in the future. Data was obtained through document reviews, incident investigation reports, and literature related to industrial fires. The results of the analysis showed that this fire was caused by a combination of technical factors, such as the failure of the tank safety system, and human factors, including a lack of compliance with safety procedures. The impacts caused include significant material losses, operational disruptions, and environmental impacts and the health of the surrounding community. To prevent similar incidents, the recommendations included increased periodic inspections, improvement of standard operating procedures (SOPs), occupational safety training for employees, and the application of more advanced fire detection technology. This study is expected to be an evaluation material for similar industries to improve risk management and occupational safety systems

Abstract

Water is a compound composed of hydrogen (H?) and oxygen (O?), with turbidity caused by suspended particles such as clay and sewage. This study aims to reduce the turbidity of the Bengawan Solo River using the jar test method with pre-sedimentation and no pre-sedimentation treatment. The results showed that pre-sedimentation decreased the mean turbidity from 55.08 NTU to 30.16 NTU. The primary coagulant Al?(SO?)? and the auxiliary coagulant Ca(OH)? affect the turbidity and pH of the water. The pH value was close to neutral (6.07) achieved at a concentration of 60 ppm with pre-sedimentation. This method is effective in reducing turbidity and has the potential to be applied in large-scale water treatment.

Abstract

The presence of harmful gases such as H?S and mercaptans in the natural gas purification process requires safe handling to avoid the risk of equipment damage and danger to the environment and occupational safety. This study aims to analyze occupational safety risks in the V-0301 Combination Column at the Caustic Treater Unit (CTU) of CPP Gundih, PT Pertamina EP Cepu Asset 4 Zone 11 using the Hazard and Operability Study (HAZOP) method. Observed processes include pre-wash, extraction, and water wash, with NaOH as the primary solvent and demineralized water to remove impurities from natural gas. The analysis is carried out by identifying potential hazards and process deviations based on operating parameters such as pressure, temperature, and flow. The results point to several key risk scenarios, such as the potential for corrosion due to excessive NaOH concentrations and failure of pressure control systems. Risk mitigation recommendations include improved control systems, periodic inspections, and safety training for operators. The study contributed to the improvement of occupational safety and operational reliability of CTU units in natural gas processing facilities

Abstract

The oil and gas industry is prone to workplace accidents such as explosions and fires. PT TOX experienced a fire in an oil tank that had an impact on the environment and surrounding living things. This research uses a qualitative approach with fishbone analysis and domino's factory method to evaluate the cause of the fire. Data was collected through scientific journals and related news based on online databases. The results of the analysis using the fishbone diagram analysis method showed that the causes of the fire included leaks that spread gas vapors, inadequate operator response, absence of early detection (such as gas detectors and automatic alarms), damage to the tank structure, and volatile and flammable chemicals. To support further identification, researchers used the domino's factory analysis method which revealed factors such as poorly prepared management, tank corrosion, and unsafe actions such as moving products without safe procedures. The fire caused huge losses, including the destruction of four tanks by fire. Prevention of workplace accidents, especially fires, in the future can be done with regular training for operators, the use of fire protection equipment, periodic inspections, and revision of emergency response SOPs

Abstract

Oil and Gas Occupational Safety and Health (OSH) has strict requirements in its application to support the safety of workers, including standardisation of equipment, personnel, general oil and gas installation policies and work procedures so that oil and gas agencies or companies can operate reliably and safely. A small mistake or negligence is a failure to comply with safety requirements, which can be fatal and cause severe disasters to the company and the surrounding community. The main factor of work accidents that often occur is human or worker negligence or carelessness due to lack of knowledge. Dumai City became a 'pearl' on the East Coast of Sumatra and earned the nickname 'Oil City’ which produces various raw materials such as petroleum, natural gas, palm oil, and coconut oil; knowledge of OSH behaviour should be instilled from childhood so that the Workshop and Certification of Oil and Gas OSH Operators to SMA / K Students as the First Step in Implementing OHS Culture in the Dumai City School Environment to equip people with knowledge and expertise in the field of Oil and Gas OSH. Prevent work accidents and support and improve the K3 culture. The workshop was held from 1 to 5 March 2024, while the Oil and Gas OSH Certification exam for operator level was in collaboration with LSP PPSDM Migas for the STTK exam on 27-28 August 2024. Fifty participants attended the certification, and 31 participants were declared competent.

Abstract

The heating stage of oil and gas refining commences with a heat exchanger. Using the pressure drop method and dirt factor in conjunction with the utmost standard permissible limit is one method of evaluating the thermal efficiency of a heat exchanger. This is performed to ascertain the value of the pressure drop, or pressure drop, which is directed at the fluid passage in the shell and pipeline. Moreover, a dust factor value is necessary to account for the potential accumulation of impurities and scale on the walls outside the heat exchanger conduit. The dust factor value is the maximum value that the parameter permits to ascertain the feasibility of the heat exchanger. Heat Exchanger 011-E-112 in the Fuel Oil Complex Unit The results indicate that heat exchanger 11E-112 can still be utilized despite the necessity for design enhancements. The design data for the shell is 1.7 psi, and the tube is approximately 18.92 psi. The Dirt Factor values for the shell and tube are 0.017 BTU/hour ft2°F and 0.034 BTU/hour ft2°F, respectively. The tube pressure drop value is 130.32 psi, while the shell pressure drop value is 2.53 psi.
