Abstract

Background and purpose: An important component of healthcare access is transportation. Lack of transportation has been cited as an important barrier to healthcare access in remote islands region. Here we conduct a feasibility study on the implementation of water ambulance service to improve healthcare access in the region of Nusa Penida and its surrounding islands in Bali Province, Indonesia. Methods:  We employed a qualitative approach with interviews and focused group discussions with stakeholders. Data collection was conducted to assess the feasibility of operating water ambulance services, based on a framework that included technical, market, organizational, and financial feasibilities. Inductive coding was conducted and data was analyzed to evaluate the feasibility based on local capacity against pre-determined framework of assessment based in literature. Results: Qualitative interviews revealed perceived urgency and need for water ambulance service among stakeholders. They also perceive the need of water ambulance service was not mutually exclusive to other alternatives, such as expanding healthcare capacity on the islands. There was also readiness and adequate capacity from stakeholders which include a dedicated health emergency services under the health office that coordinate ambulance service in the region, readiness to create dedicated medical team and boat crew to operate water ambulance services, and readiness to cover the cost of initial procurement followed by operational costs which may include public-private partnerships. Conclusion: There was a strong perceived need for water ambulance service in Nusa Penida Islands. At the same time, local stakeholders revealed adequate basic capacity to feasibly operate water ambulance services according to benchmarks identified in literature review.

Abstract

Penyakit HIV, silifilis dan hepatitis B merupakan infeksi menular seksual yang dapat ditularkan dari ibu ke janin. Kementerian Kesehatan mengadakan program triple elimiasi sesuai dengan rekomendasi WHO untuk mencegah penularannya. Penelitian ini merupakan penelitian analitik dengan rancangan cross sectional, menggunakan kuesioner tentang pengetahuan dan persepsi ibu hamil terhadap pemeriksaan triple eliminasi, pada Puskesmas II Denpasar selatan dan UPTD Puskesmas II Denpasar Timur. Analisis dilakukan secara univariat dan bivariate. Dari 85 ibu hamil yang diwawancarai, mayoritas pengetahuan baik (97,6%), persepsi negatif (55,3%). Masing-masing sebanyak 29,4%, 32,9% dan 1,8% ibu hamil mengaku belum melaksanakan tes HIV, sifilis dan Hepatitis B mayoritas pada ibu hamil yang berada di trimester kedua. Hasil penelitian juga menunjukkan bahwa variavel jumlah anak hidup dan pendidikan terakhir berhubungan dengan pengetahuan ibu secara signifikan. Diperlukan penguatan sistem antenatal care terpadu (10T) serta kerjasama dengan praktik bidan mandiri dan praktik dokter kandungan untuk pemeriksaan triple eliminasi sedini mungkin. Abstract

People living with HIV (PLHIV) and key populations, including people who use drugs, sex workers, LGBTQ+ communities, and imprisoned individuals, represent groups with heightened vulnerabilities that are significantly magnified during public health emergencies such as the COVID-19 pandemic. These populations are disproportionately exposed to both the direct health impacts of the virus and systemic barriers that limit access to essential health services.1 One of the most critical aspects is access to antiretroviral therapy (ART), which is vital to suppress viral load and maintain immune system function in PLHIV.2 Equally important are other health services such as harm reduction programs, sexual and reproductive health care, and mental health support, which together ensure the physical, emotional, and psychological well-being of these groups.

Abstract

Toxoplasmosis is a zoonotic disease caused by infection with the parasite called Toxoplasma gondii (T. gondii). The health and social impacts of the infectionare enormous, including miscarriage, hydrocephalus, blindness, and mental retardation. The occurrence of toxoplasmosis in maternal women cannot be se parated from cats around their houses. This study aimed to determine the seroprevalence of the parasite in cats found in the human carriers residences and identify the risk factors of toxoplasmosis in maternal women in Badung District, Bali Province, Indonesia. A total of 80 cat serum samples were obtained from two residential groups, 40 from the housing where the maternal women were infected and another 40 from where there were no identified sufferers of the disease. All the samples were examined using the enzyme-linked immunosorbent assay (ELISA) method to detect the presence of antibodies T. gondiiin feralcat serum. The results showed that 47.5% of the examined subjects had the said antibodies. As much as 65% came from housing with cases of toxoplasmosisin maternal mothers, and 30% came from residences with none. The presence of feral cats is a major risk factor for the transmission of T. gondiito humans.
