Abstract

Background Clear cell renal cell carcinoma (ccRCC) is the most common histological subtype in renal cell tumors (RCT). This is a very aggressive cancer, often diagnosed at advanced stage leading to increase morbidity and mortality rates. Nicotinamide adenine dinucleotide phosphate (NADPH) oxidase 4 (NOX-4) expression plays role in the regulation of several cancer types. The expression of NOX-4 is associated with a poorer degree of differentiation and a shorter survival in RCT. Therefore, NOX-4 expression is predictor of prognosis and potential therapeutic target. Research on the correlation of NOX-4 expression in RCTs is still very limited, while Correlation of NOX-4 expression in ccRCC does not exist yet.   Objective To assess the relationship between NOX-4 expression and ISUP grading in ccRCC.   Method In this study, 30 slide samples and ccRCC paraffin blocks were diagnosed histopathologically at Department of Anatomic Pathology of H. Adam Malik General Hospital from January 2012 to April 2022. All clinicopathological data were taken from medical records/pathology archives. The expression of NOX-4 immunohistochemical was assessed in nucleus and cytoplasm of tumor cells. The assessment was carried out semiquantitatively based on intensity and percentage of expressed tumor cells. The relationship between NOX-4 expression and ISUP grading was assessed using Fisher's exact test.   Results Of the 30 ccRCC samples, high grade ccRCC was found in 21 cases (70%) and strong expression of NOX-4 in cytoplasm and nucleus in 20 cases (66.7%). Strong expression of NOX-4 in high grade ccRCC was found in 17 cases (81.0%). A significant relationship was observed between NOX-4 expression and ISUP grading on ccRCC with a p-value of 0.030 (p<0.05) using Fisher's exact test.   Conclusion There is a significant relationship between NOX-4 expression and ISUP grading in ccRCC. Therefore, NOX-4 expression can be used for the prognosis of ccRCC and therapy development.

Abstract

Background Colorectal cancer ranks third highest of all malignancies worldwide and ranks fourth out of the 10 most common cancers encountered in Indonesia. More than 90% of colorectal cancers are adenocarcinoma type. The mechanism of autophagy has a dual role in tumorigenesis, namely as a tumor suppressor and as a tumor promoter. Microtubule-associated protein 1 light chain 3B (LC3B) is one of the widely used autophagy markers. The role of autophagy requires further research so that LC3B can be considered as one of the predictors of prognosis, especially in colorectal adenocarcinoma. The aim of this research is to determine the correlation of Light Chain 3B immunohistochemical expression with histopathological grading of colorectal adenocarcinoma. Methods Cross-sectional analytic study of 41 samples diagnosed with colorectal carcinoma at H. Adam Malik General Hospital Medan. Staining was performed using a polyclonal antibody rabbit Light Chain 3B immunohistochemistry (GeneTex) which would appear positive in the cytoplasm of tumor cells at a dilution of 1: 500. LC3B expression was assessed semiquantitatively with low and high expression categories. Data analysis was tested with the Spearman correlation test. Results LC3B immunohistochemical expression has a very strong correlation with the histopathological grading of colorectal adenocarcinoma and in a positive direction where the higher the colorectal adenocarcinoma grade, the higher the LC3B immunohistochemical expression, and vice versa (p=0,0001; r=0,763). Conclusion There was a significant relationship between LC3B immunohistochemical expression and histopathological grading of colorectal adenocarcinoma.

Abstract

LC3-B is a specific autophagy marker that plays a role in ccRCC. Research regarding the relationship between clinicopathological characteristics of ccRCC and LC3-B expression is still very limited and controversial. Objective: To assess the relationship between clinicopathological characteristics of ccRCC and LC3-B immunohistochemical expression. Materials and methods: This study used 22 slide samples/paraffin blocks of ccRCC cases that had been diagnosed histopathologically at the Anatomic Pathology Unit of H. Adam Malik General Hospital, Medan from January 2017 to April 2023. All clinicopathological data was taken from medical records/pathology archives. LC3-B expression was assessed in the cytoplasm of tumor cells with assessment semiquantitatively based on the multiplication of intensity and percentage of expressed tumor cells. Statistical analysis was carried out using Fischer's Exact and Kruskal Wallis tests. Results: Among the 22 ccRCC samples, weak expression of LC3-B was found in the age group ≤60 years, male gender, tumor size >10 cm, positive LVI, negative PNI, grade IV, severe stromal TILs, mild intratumoral TILs and low budding in intratumoral budding tumors. Fischer's Exact and Kruskal Wallis statistical tests showed that there was no significant relationship between gender, tumor size, LVI, PNI, ISUP grading, TILs and peritumoral tumor budding with LC3-B expression. Conclusion: There was no significant relationship between clinicopathological characteristics of ccRCC with LC3-B expression (The Null Hypothesis is accepted (H0 ρ=0)).
