Abstract

Ekstrak temulawak (Curcuma xanthorrhiza) yang sukar larut dalam air sudah banyak dikaji dan potensial untuk pengobatan berbagai penyakit. Pembuatan Self-Nano Emulsifying System (SNES) merupakan salah satu metode yang mampu meningkatkan kelarutan dan ketersediaan hayati suatu zat aktif obat dengan mencampurkannya ke dalam pembawa yang sesuai. Penelitian bertujuan untuk memformulasi, karakterisasi dan menguji stabilitas SNES ekstrak temulawak. Pembuatan SNES dimulai dari skrining dan optimasi pembawa ekstrak temulawak yang terdiri beberapa minyak, surfaktan dan kosurfaktan. SNES ekstrak temulawak dikarakterisasi meliputi pengukuran transmittan, ukuran partikel, indeks polidispersi (IP), potensial zeta, penentuan stabilitas termodinamika, uji ketahanan dan uji stabilitas dipercepat. Formula optimal SNES ekstrak temulawak adalah kombinasi Labrasol (20%), Tween 20 (60%), dan propilenglikol (20%), dengan drug loading ekstrak temulawak adalah 23%. Nilai parameter karakterisasi yang didapatkan adalah transmittan 100,2 ± 0,0%, ukuran partikel 13,0±1,4 nm dengan IP 0,3 ± 0,1, dan potensial zeta -42,4 ± 0,6 mV. Uji stabilitas termodinamika menunjukkan tidak terjadi pemisahan fase. Uji ketahanan menunjukkan bahwa ukuran partikel stabil selama proses pengenceran. Selain itu, SNES ekstrak temulawak stabil selama uji stabilitas dipercepat selama 3 bulan. Disimpulkan bahwa, SNES ekstrak temulawak menghasilkan sediaan yang stabil dengan drug loading yang tinggi.

Abstract

Background: Meniran (Phyllanthus niruri L.) is a traditional medicinal plant widely utilized in various countries due to its pharmacological potential. Objective: This study aims to explore the biological activity by summarizing scientific findings regarding the pharmacological effects and toxicological profile of meniran, as well as highlighting the understanding related to safe therapeutic doses. Methods: This review was conducted using a systematic narrative approach, utilizing literature from journal articles, scientific reports, reference books, and publication data from databases such as PubMed, Scopus, and Google Scholar. Inclusion criteria comprised peer-reviewed studies published in the last 10 years, while exclusion criteria eliminated non-scientific articles and those not relevant to pharmacology or toxicology. Results: A total of 200 articles were identified, resulting in 34 articles selected for review after applying inclusion and exclusion criteria. From the four journals/studies reviewed, it was found that bioactive compounds in meniran, such as flavonoids and lignans, play a crucial role in therapeutic effects including antioxidant activity, hepatoprotection, antimicrobial effects, and anti-inflammatory properties. Additionally, two journals/studies indicated that long-term use of meniran may have toxic effects. Conclusion: Meniran exhibits pharmacological activities such as antioxidant effects, hepatoprotection, antimicrobial effects, and anti-inflammatory effects; however, it may have toxic effects on the kidneys and liver when used in high doses. Further research is necessary to ensure the safety and effectiveness of its use in medical therapy.

Abstract

The therapeutic efficacy of natural extracts such as propolis, sea cucumber or gamat (Stichopus hermanii), and pegagan (Centella asiatica) is supported by their anti-inflammatory, antioxidant, and regenerative properties. However, their pharmaceutical application is limited due to poor solubility and low bioavailability. This study aimed to develop and characterize a Self-Nanoemulsifying Drug Delivery System (SNEDDS) using Labrasol to enhance the delivery and stability of a combination of these three extracts, that exhibit synergistic effects as tissue regeneration, antimicrobial, anti-inflammatory, and antioxidant properties. SNEDDS formulations were prepared by a high energy method with Labrasol, Tween 80, and propylene glycol. The formulations were evaluated for droplet size, polydispersity index (PDI), zeta potential, and thermodynamic stability test. The optimized SNEDDS exhibited nano-sized droplets (151.13 nm ± 17.26) with low PDI (0.39 ± 0.01) and zeta potential (-50.30 ± 1.04) within the stable range, indicating a uniform, physically stable nanoemulsion, and passed the thermodynamic stability test. In conclusion, the optimized SNEDDS formulation demonstrated nano-sized droplets with low PDI and stable zeta potential, indicating good physical stability.

Abstract

Diabetes Nefropati (DN) merupakan penyebab utama terjadinya end‐stage kidney disease. Studi molekuler menunjukkan ncRNA berperan penting dalam mengatur peradangan ginjal, apoptosis, autofagi, proliferasi sel, dan fibrosis ginjal di DN. Pada penelitian ini dilakukan tinjauan artikel mengenai ncRNA sebagai prospek baru dalam pengembangan diagnosis, prognosis, dan pengobatan untuk DN. Metode penelitian dilaksanakan dengan pendekatan studi literatur yang diperoleh dari penelusuran pada database elektronik pubmed dan google scholar dengan kata kunci “ncRNA and diabetes nephropathy” yang mencakup terkait etiologi, patofisiologi, biomarker. ncRNA yang meliputi miRNA, lncRNA, circRNA berperan dalam progresifitas penyakit DN. Studi mengenai ncRNA sebagai terapi masih dalam tahap pengembangan. Miravirsen dan RG-101 adalah beberapa obat miRNA yang telah diuji klinis, tetapi belum ditemukan obat miRNA yang spesifik untuk DN. Salah satu inovasi terapi yang dikembangkan adalah nanopartikel-miRNA-146a-5p (nano-miR-146a-5p) yang menunjukkan aktivitas penekanan ekspresi inflamasi sitokin yang memiliki efek perlindungan yang terjadi melalui regulasi respon inflamasi dan apoptosis. Pengembangan terapi ncRNA pada DN membutuhkan penelitian dan kajian lebih lanjut, sehingga mampu menjadi pilihan pengobatan DN di masa medatang.
