Abstract

Abstrak: Evaluasi Nilai CTDIvol Dan DLP Pada Pemeriksaan CT Scan Stonografi Periode Januari Sampai Maret 2023 Di RSUD Sanjiwani Gianyar. CT Stonografi adalah pemeriksaan untuk mendiagnosis urolithiasis, pemeriksaan ini dapat memberikan gambaran diagnostik yang cepat dan akurat. CT Stonografi menjadi goal standard di RSUD Sanjiwani Gianyar. Namun, CT Scan memiliki radiasi yang tinggi sehingga perlu dilakukan optimasi dosis dengan prinsip As Low As Reasonably Achievable (ALARA). Langkah awal dalam optimasi dosis adalah menetapkan Diagnostic Reference Level (DRL), tujuan: Mengetahui nilai CTDIvol dan DLP pada pemeriksaan CT Scan Stonografi di RSUD Sanjiwani Gianyar dan membandingkannya dengan Indonesian Diagnostic Reference Level (IDRL) tahun 2021, metode: Kuantitatif deskriptif dengan pendekatan survey untuk mengevaluasi nilai CTDIvol dan DLP pada pemeriksaan CT Scan Stonografi dewasa di RSUD Sanjiwani Gianyar periode Januari sampai Maret 2023. Menggunakan data retrospektif, nilai median (50 persentil) dari CTDIvol dan DLP dihitung dengan aplikasi Microsoft Excel dan rumus dari Bapeten, hasil: Nilai Median (50 persentil) pada pemeriksaan CT Scan Stonografi yaitu sebesar 6,79 mGy untuk CTDIvol dan 513 mGy*cm untuk DLP, kesimpulan: Nilai DRL pada pemeriksaan CT Scan Stonografi di RSUD Sanjiwani Gianyar berada dibawah IDRL tahun 2021, yang menunjukan bahwa pemeriksaan CT Scan Stonografi di RSUD Sanjiwani Gianyar tergolong aman.

Abstract

Klungkung Regional General Hospital is one of the health services owned by the Klungkung district government in the form of a hospital, managed by the Klungkung district government and recorded as a type B hospital. At the Klungkung Regional General Hospital, there are several things that are not in accordance with the radiation safety management system determined by BAPETEN. Some of these things that are not suitable such as the lack of compliance of radiation workers with the standard operating procedures (SOPs) of radiation work in conventional rooms which are often found things outside the provisions, the use of equipment such as aprons and there are doors in the radiological examination room that have leaks, because environmental exposure measurements have been made in the area and leaks were found, and also monitoring the health of radiation workers who have not been optimal. This type of research uses descriptive qualitative research methods with an observational approach from the routine activities of radiation workers at the Radiology Installation of the Klungkung Regional General Hospital. The Radiology Installation of the Klungkung Regional General Hospital has met the BAPETEN Perka standard No.4 of 2020 but there are still some that have not met the standards for the Implementation of the Radiation Safety Management System in the Conventional Room at the Radiology Installation of the Klungkung Regional General Hospital, including the conventional room inspection door to the operator room experiencing a leak, it has been reported to the relevant party in this case for follow-up so that the detected radiation leak can Dealt with immediately.

Abstract

Background: The number of Lumbosacral MRI examinations at the Radiology Department of the Regional General Hospital of West Nusa Tenggara Province averages 80 patients per month, with the most common case being Hernia Nucleus Pulposus (HNP). The normal duration for each examination is 17-20 minutes. Compressed Sense (CS) was always activated during the author's observation of these examinations, reducing the total scan time for Lumbosacral MRI examinations to 8 minutes. In some MRI cases, there are challenges with non-cooperative patients, such as traumatic cases and being unable to supine position, which causes moving artifacts and makes image results blur, activating the CS technique can solve this problem making examination times faster. The research aims to analyze and describe the technique, the role of the technique, as well as the advantages and disadvantages of the CS technique in Lumbosacral MRI examinations for HNP cases.Methods: This research is qualitative descriptive research with a case study and literature review approach. use 3 patients, 3 radiographers, dan 3 radiologists, do in March to April 2024. Data was collected by observation, deep interviews, and questionnaires for radiologists about image quality.Result: Three patients with clinical HNP underwent Lumbosacral MRI examinations without any special preparation. The Lumbosacral MRI protocol includes Survey, Coronal T2W TSE, Sagittal T1W TSE, Sagittal T2W TSE, Sagittal T2W TSE SPIR, Axial T1W TSE, Axial T2W TSE, and MYELO Radial. The protocols use the CS technique with a reduction score of 1.4, except MYEOLO Radial, which can reduce scan time to 7-10 minutes without decreasing image quality. The sequences used serve to provide anatomical information and pathology in the lumbosacral area.Conclusions: Lumbosacral MRI examinations using the Compressed Sense technique can shorten the examination time without significantly reducing the quality of the images produced, it proved by the questionnaire result. This can increase patient comfort during the examination. The use of reduction values in this CS technique must be considered as it can increase the value of Peripheral Nerve Stimulation (PNS).

Abstract

Background: In the process of work to gain the maximum results, a radiologist needs a viewing box tool to read radiographs. Therefore, the authors want to develop a viewing box tool, which in general the work if this tool resembles the factory manufactured tool. The viewing tool box made can adjust the intensity of the light produced.Objective: to create a viewing box tool by using a potentiometer system.Methods: This study used applied research method by creating and using the design of viewing box tool by using a potentiometer system and testing the viewing box tool created by using a Lux meter and 15 respondents consisting of five radiologists and 10 radiographers who should fulfill the questionnaire form.Results: The mean of viewing box illumination reached 220 lux. The results of the questionnaire showed that 100% radiologist gave an A (excellent) and expressed that the viewing box tool created could be used properly and 90% radiographers provided an A (excellent) and expressed that the viewing box tool created could be used properly, while 10% radiographer gave a value of B (moderate).Conclusion: viewing box tool created could be used properly and obtained optimal results as a tool in reading radiographs. Potentiometer system contained in the viewing box was very helpful in reading radiographs because it allowed to adjust the light intensity according to user needs.Keywords       :  Viewing box, Potentiometer Bibliography   : 1980-2011

Abstract

Kanker payudara berada di urutan pertama penderita kanker terbanyak di Indonesia serta merupakan salah satu penyebab utama kematian akibat kanker. Pengobatan kanker payudara meliputi pembedahan, kemoterapi, dan radioterapi. Pembedahan adalah terapi yang paling awal dikenal, sementara radioterapi menggunakan sinar pengion sebagai salah satu jenis terapi utama selain pembedahan dan kemoterapi. Radioterapi sering menggunakan bolus untuk meningkatkan dosis permukaan dan mengarahkan penetrasi elektron pada jaringan kanker di permukaan kulit. Penelitian ini bertujuan untuk menganalisis perbedaan penggunaan bolus dan tanpa bolus dalam radioterapi pada pasien kanker payudara pascamastektomi menggunakan teknik Intensity Modulated Radiation Therapy (IMRT) di Instalasi Onkologi Radiasi Rumah Sakit Indriati. Penelitian ini merupakan studi kuantitatif dengan pendekatan eksperimen yang membandingkan penggunaan bolus dan tanpa bolus dalam terapi radiasi pascamastektomi pada 20 pasien kanker payudara, dengan 10 pasien menggunakan bolus dan 10 pasien tidak menggunakan bolus. Hasil penelitian menunjukkan adanya perbedaan signifikan antara kelompok yang menggunakan bolus dan yang tidak dalam hal nilai Organ at Risk (OAR) dan Planning Target Volume (PTV), berdasarkan uji statistik paired t-test (p-value < 0.05). Kesimpulan dari penelitian ini adalah bahwa penggunaan bolus memberikan perubahan signifikan dalam efektivitas radioterapi pada pasien kanker payudara, dengan meningkatkan distribusi dosis yang lebih tepat pada area target dan mengurangi paparan pada organ di sekitarnya.

Abstract

Pemeriksaan CT abdomen non kontras merupakan salah satu metode diagnostik yang penting dalam mendeteksi kelainan fungsi dan struktur anatomi di area perut tanpa menggunakan bahan kontras. Salah satu parameter teknis yang memiliki peran penting dalam kualitas hasil scanning serta keamanan pasien adalah rotation time pada CT scan. Rotation time yang berbeda dapat mempengaruhi dosis radiasi yang diterima oleh pasien dan juga durasi scanning, sehingga pemilihan parameter ini harus dilakukan dengan hati-hati. Penelitian ini bertujuan untuk mengevaluasi pengaruh variasi rotation time terhadap CT Dose Index (CTDI) dan Dose Length Product (DLP) pada pemeriksaan CT abdomen non kontras yang dilakukan di Laboratorium CT Scan ATRO Bali. Desain penelitian ini adalah eksperimental dengan pendekatan kuantitatif, menggunakan water phantom sebagai subjek penelitian. Data dikumpulkan melalui pengukuran CTDI dan DLP pada variasi rotation time, yaitu 1 second dan 1.5 second. Analisis data dilakukan menggunakan uji nonparametric correlations untuk mengevaluasi hubungan antara rotation time dengan CTDI dan DLP. Hasil penelitian menunjukkan bahwa terdapat pengaruh signifikan dari variasi rotation time terhadap nilai CTDI dan DLP, di mana rotation time yang lebih lama (1.5 second) menghasilkan nilai CTDI dan DLP yang lebih tinggi dibandingkan dengan rotation time yang lebih pendek (1 second). Hasil ini menekankan pentingnya pemilihan rotation time yang optimal dalam pemeriksaan CT abdomen non kontras untuk mengurangi dosis radiasi yang diterima pasien, sambil tetap mempertahankan kualitas gambar yang memadai.
