Abstract

ABSTRAK Di Wilayah Puskesmas Trucuk Kasus Kematian Ibu Tahun 2020 ada 1  dari Lahir hidup 510 atau Angka Kematian Ibu (AKI) sebesar 196/100.000 KH dari target 102/100.000 Puskesmas Trucuk yang mempunyai wilayah desa kelurahan yang melaksanakan melaksanakan Program Perencanaan Persalinan dan Pencegahan Komplikais (P4K) Tahun 2020 sebesar 8,33% dari Target 100%. Mengacu kepada butir analisa permasalahan yang teridentifikasi maka ditemukan dua masalah utama yang dihadapi mitra yaitu kasus kematian Ibu di wilayah Puskesmas Trucuk tiap ahun ada dan cendrung AKI meningkat tajam dan belum maksimalnya pelaksanaan P4K. Strategi penurunan kematian ibu diperlukan adanya pemberdayaan masyarakat melalui Pemantapan P4K yaitu dilakukan upaya peningkatan pengetahuan dan sikap keluarga dan masyarakat, kader kesehatan mengenai pentingnya memahami Pelaksanaan P4K. Berdasarkan analisis situasi dan permasalahan mitra tersebut diatas, kami melaksanakan kegiatan pengabdian kepada masyarakat berupa Pemberdayaan Masyarakat Dalam Upaya Penurunan Angka Kematian Ibu melalui Pelatihan Kader Kesehatan Kesehatan Ibu dan Anak tentang Pemantapan Program Perencanaan Persalinan dan Pencegahan Komplikasi (P4K) di Wilayah Puskesmas Trucuk Kabupaten Bojonegoro. Pelatihan ini, sasaran Kader Kesehatan di Desa Sumberejo dan Desa Pagerwesi Wilayah Puskesmas Trucuk dilaksanakan selama 3 (tiga) hari bertempat di Balai Desa Sumberejo dengan protokol kesehatan yang ketat, diikuti 40 peserta dengan menggunakan metode ceramah, dan pemberian materi tentang Pemantapan P4K meliputi Persiapan Fisik, Psikis, penolong dan tempat bersalin, pendamping persalinan, dana, transportasi,  calon donor darah, perlengkapan ibu dan bayi.  Hasil kegiatan adalah terbentuknya 40 Kader KIA dengan hasil rata-rata pengetahuan peserta pelatihan sebesar 96 dan 100% peserta pelatihan mempunyai nilai post test sangat baik. Untuk nilai rata-rata keterampilan peserta dalam penyuluhan sangat baik (83,63) dan 95% nilai KIE sangat baik, terbentuk komitmen bersama dari Kepala Puskesmas Trucuk, Kepala Desa Sumberrejo, Kepala Desa Pagerwesi , Bidan Desa Sumberejo, Bidan Desa Pagerwesi, Bidan Koordinator Puskesmas Trucuk dan seluruh peserta pelatihan Pemantapan P4K (100%). Hasil Survey Kepuasan peserta didapatkan 93,80% menyatakan sangat puas. Evaluasi pelaksanaan menilai Kader KIA yang sudah  dilatih membina 1 ibu hamil di lingkungan desa sekitarnya sejumlah 40 Ibu hamil dengan penyuluhan tentang P4K dengan mengunakan leaflet, Pengisian Sticker P4K (Perencanaan Persalinan dan Pencegahan Komplikasi) dan menempel Sticker P4K di Pintu Depan Rumah Ibu Hamil. Luaran Pengabdian masyarkat adalah tersusunnya Modul tentang Pemantapan P4K, publikasi dan HKI. Kader Kesehatan disarankan memaksimalkan pelaksanaan kelompok Keja P4K sampai tercapai100%, yang harapannya bisa menurunkan AKI. Kata Kunci: Pemberdayaan Masyarakat,P4K  ABSTRACT In the Trucuk Community Health Center area, cases of maternal death in 2020 were 1 in 510 live births or the Maternal Mortality Rate (MMR) was 196/100,000 KH out of the target of 102/100,000 Trucuk Community Health Center which has a sub-district village area that implements the Childbirth Planning and Complication Prevention Program (P4K ) In 2020 it was 8.33% of the 100% target. Referring to the identified problem analysis points, it was found that there were two main problems faced by partners, namely cases of maternal death in the Trucuk Community Health Center area every year and the tendency for MMR to increase sharply and the implementation of P4K not yet optimal. The strategy to reduce maternal mortality requires community empowerment through strengthening P4K, namely efforts to increase the knowledge and attitudes of families and communities, health cadres regarding the importance of understanding the implementation of P4K. Based on the analysis of the situation and problems of the partners mentioned above, we carried out community service activities in the form of Community Empowerment in Efforts to Reduce Maternal Mortality Rates through Training of Maternal and Child Health Cadres on Strengthening the Childbirth Planning and Complication Prevention (P4K) Program in the Trucuk Community Health Center Area, Bojonegoro Regency. This training, targeting Health Cadres in Sumberejo Village and Pagerwesi Village in the Trucuk Community Health Center area, was carried out for 3 (three) days at the Sumberejo Village Hall with strict health protocols, attended by 40 participants using the lecture method, and providing material on P4K Strengthening including Physical Preparation , Psychics, helpers and birthing places, birth companions, funds, transportation, prospective blood donors, mother and baby equipment. The result of the activity was the formation of 40 KIA cadres with the average knowledge of the training participants being 96 and 100% of the training participants having very good post test scores. For the average value of participants' skills in counseling was very good (83.63) and 95% of the KIE scores were very good, a joint commitment was formed from the Head of the Trucuk Community Health Center, the Head of Sumberrejo Village, the Head of Pagerwesi Village, the Sumberejo Village Midwife, the Pagerwesi Village Midwife, the Coordinating Midwife Trucuk Community Health Center and all P4K Strengthening training participants (100%). The results of the participant satisfaction survey showed that 93.80% were very satisfied. The implementation evaluation assessed that KIA cadres who had been trained to support 1 pregnant mother in the surrounding village area amounted to 40 pregnant women with counseling about P4K by using leaflets, filling out P4K (Delivery Planning and Complication Prevention) Stickers and sticking P4K Stickers on the Front Door of Pregnant Women's Houses. The output of community service is the preparation of a module on strengthening P4K, publications and IPR. Health cadres are advised to maximize the implementation of the P4K Work group until it reaches 100%, which is expected to reduce MMR. Keywords: Community Empowerment, P4K

Abstract

Background: Hypertension in pregnancy affects 10% of pregnant women worldwide and includes preeclampsia, eclampsia, gestational hypertension, and chronic hypertension. Preeclampsia is the most common, with a global incidence of 2–8% of pregnancies. In Indonesia, preeclampsia accounts for 128,273 cases annually, or approximately 5.3% of pregnancies. Several risk factors contribute to preeclampsia, including a history of preeclampsia, diabetes, chronic hypertension, kidney disorders, a family history of severe preeclampsia, obesity, maternal age, nulliparity, and multiple pregnancies. Medical records from Dr. R. Sosodoro Djatikoesoemo Bojonegoro Regency Hospital show that preeclampsia cases constituted 17.57% (195 of 1,110 cases) in 2020 and 8.26% (139 of 1,696 cases) in 2021. This study aims to analyze the risk factors for preeclampsia at RSUD Dr. R. Sosodoro Djatikoesoemo Bojonegoro Regency. Methods: This observational analytic study employed a cross-sectional design. The sample consisted of 105 pregnant women in their second and third trimesters, selected using simple random sampling. Data were analyzed using the Multiple Logistic Regression Test to assess risk factors. Results: The multivariate analysis yielded a significance value of 0.243, indicating that maternal age, history of preeclampsia, and pregnancy spacing influence preeclampsia incidence. The Model Summary table showed Nagelkerke's R Square value of 0.037. Conclusion: Maternal age, pregnancy spacing, and a history of preeclampsia are significant risk factors for preeclampsia.

Abstract

Pesticides are one of the toxic and dangerous chemicals (B3) and are persistent organic pollutants. If you use unsafe pesticides, it can affect a person's health. WHO estimates that around one million suffer pesticide poisoning every year. 11,000 would have fatal consequences. The mechanism of action of pesticides in the human body is to influence the work of enzymes and hormones and can damage tissue by inducing the production of serotonin and histamine. Pesticides are cytotoxic, carcinogenic and mutagenic for health and can be toxic to the reproductive system. Toxins in the reproductive system that are metabolized will enter the ovaries and cause the cells in the ovaries to become damaged. Pesticides can also cause decreased fertility, hormonal imbalance, damage to the germinal epithelium, genetic changes in men, exposure during the preconception period resulting in an increased risk of spontaneous abortion, slow fetal growth and premature birth. The aim is to increase the knowledge and skills of Village Health Cadres regarding the dangers of pesticide exposure to reproductive health The methods used are lectures, demonstrations, pre and post tests. Pre and post with 20 questions about the dangers of pesticide exposure for health. The participants were 35 health cadres from Mojodeso Village, Kapas District, Bojonegoro Regency; the results of the most 18 (51.4%) training participants' knowledge during the post-test were in the very good category. All participants (100%) were able to provide counseling to 75 families in the good category
