Abstract

Early Initiation of Breastfeeding (IMD) is the process of a baby suckling immediately after birth, in which the baby is allowed to find the mother's nipple on its own (not thrust into the nipple). Breastfeeding properly and correctly can increase the success of exclusive breastfeeding, the coverage of breastfeeding is to prevent infant mortality, impaired infant development, and prevent hypothermia. The IMD process creates closeness between mother and baby. This study aims to determine the relationship between knowledge and attitudes of midwives on the implementation of Early Breastfeeding Initiation (IMD). The research method used was analytic observational with a cross sectional approach, with a sample of 34 midwives who opened their own practice around Medan Tuntungan District. The results of the analysis showed that of the 34 respondents who had sufficient knowledge as many as 18 people (53%) and the majority carried out IMD as many as 12 people (66.7%). And of the 34 respondents the majority had a negative attitude as many as 27 people (79.5%), and the majority did not carry out IMD as many as 17 people (47%). There is a significant relationship between the knowledge and attitudes of midwives towards the implementation of the IMD, where the better the knowledge, the more motivated the midwife to implement the IMD. With a value of = 0.015. And a negative attitude will cause the midwife not to carry out the IMD with a value of = 0.021. It is hoped that the midwives at BPM, Medan Tuntungan Sub-district in 2021, will further improve the IMD implementation program in the future.

Abstract

Nutritional problems among pregnant women remain a major public health concern in Indonesia, particularly related to anemia, low birth weight (LBW), and stunting. One of the main contributing factors is the lack of knowledge among pregnant women about the importance of consuming a balanced diet during pregnancy. This community service activity aimed to increase pregnant women’s knowledge regarding balanced nutrition through a participatory, direct educational approach. The activity was conducted in May 2025 at the Independent Midwife Practice of Deby Cyntia Yun, Medan City, involving 30 pregnant women from various trimesters. The methods used included interactive lectures, group discussions, practical menu planning simulations, and evaluations through pre- and post-tests. The results showed an increase in knowledge scores from an average of 63% to 83%, indicating a 20% improvement. Participants responded very positively, with 93% reporting increased confidence in choosing healthy foods and 100% expressing interest in participating in similar future activities. The clinic partner also welcomed the program and suggested its regular implementation.  This activity demonstrates that a communicative, contextual, and practical educational approach to balanced nutrition is effective in enhancing pregnant women’s understanding. It is hoped that similar programs can be replicated in other healthcare facilities and contribute to stunting prevention and the improvement of maternal and child health quality in the community.

Abstract

In Indonesia, Acute Respiratory Infection (ARI) is one of the causes of 4 million deaths in children under five. ISPA is also often on the list of the 10 most common diseases at the Simalingkar Health Center. The mortality rate of ISPA in Indonesia is still very high and morbidity because ISPA in Indonesia per year ranges from 10%-20% of the population under five. Acute Respiratory Tract Infection (ISPA) is an inflammatory process caused by viruses, bacteria, atypical (microplasma), aspiration of foreign substances involving one or all parts of the respiratory tract, a condition in which disease germs successfully attack the body tools used to breathe, namely through the nose, hair, esophagus, throat, throat to the lungs and lasts no more than 14 days. The etiology of ISPA consists of more than 300 types of bacteria, viruses and riketsia. The bacteria that cause ISPA include the genus Streptococci, Staphylococci, Pneumococcus, Haemofillus, Bordetelia and Corinebacterium. The viruses that cause ISPA include Myxoviruses, Adnoviruses, Coronaviruses, Pikornaviruses, Mycoplasma, Herpesviruses and others. In addition to being caused by viruses and bacteria, ISPA is also caused by microplasma or aspiration of foreign substances involving one or all parts of the respiratory tract. Studies in various countries show that in developing countries Streptococus pneumonia and Haemofillus influenza are bacteria that are always found in two-thirds of the isolation results, namely 73.9% of lung aspirates and 69.1% of the isolation results from blood specimens. Meanwhile, in developed countries, adult pneumonia in children is generally caused by a virus. ISPA can be transmitted through saliva, sneezing, and breathing air that contains germs that are inhaled by healthy people into their respiratory tract. ISPA that continues to become pneumonia often occurs in children, especially if there is malnutrition and combined with unhygienic environmental conditions. Germs of ISPA disease are transmitted from the patient to others through the air of breathing or the spitting of the sufferer. In principle, the ISPA germs in the air are sucked in by the new host and enter the entire respiratory tract. From the respiratory tract, germs spread throughout the body if the infected person is vulnerable, then he will be affected by ISPA.
