Abstract

Sifilis dapat menyerang seluruh bagian mata, menjadikannya “great masquerade” dan harus dipertimbangkan pada pasien dengan gangguan sistem imun. Anamnesis, pemeriksaan fisik, pemeriksaan oftalmologi, dan pemeriksaan serologis dapat membedakan sifilis okular dari etiologi lain. Tujuan dari laporan kasus ini adalah untuk melaporkan diagnosis dan penatalaksanaan sifilis mata yang disertai dengan Herpes Zoster Ophthalmic. Pada laporan kasus ini, seorang pasien laki-laki berusia 27 tahun mengeluh erupsi vesikel yang disertai nyeri pada dahi dan kelopak mata kiri sejak dua hari yang lalu, serta pandangan kabur pada kedua mata sejak tujuh bulan yang lalu. Pemeriksaan fisik didapatkan ruam makulopapular pada kedua telapak tangan. Ditemukan sel vitreus dan kabut serta diskus optik bilateral yang kabur. Hasil laboratorium menunjukkan reaktivitas pada VDRL/RPR dan TPHA. Pasien didiagnosis menderita Uveitis Intermediate karena Sifilis, Herpes Zoster Ophthalmic Sinistra, dan Optik Neuritis Atipikal Bilateral. Pasien diberikan Benzatin Penisilin intramuskular, Valasiklovir oral, kortikosteroid sistemik, steroid topikal, dan dirujuk ke dokter spesialis kulit dan kelamin. Herpes Zoster Ophthalmic pada usia muda dapat menjadi tanda kondisi immunocompromised. Anamnesis, pemeriksaan fisik, dan pemeriksaan laboratorium penting dalam menentukan penyebab uveitis. Pasien dengan Okular sifilis akan mendapat manfaat dari terapi antibiotik dini. Kesalahan diagnosis akan menyebabkan kerusakan yang lebih berat dan mempengaruhi tajam penglihatan.

Abstract

Abstrak: Kornea melindungi permukaan okular terhadap paparan eksternal dan patogen. Struktur kornea memiliki regulasi sehingga kejernihan dan fungsinya tetap terjaga. Jumlah sel endotel yang memadai menyebabkan fungsi optimal dari epitel dan stroma, berfungsi bersama dalam homeostasis dan kejernihan. Tujuan laporan kasus ini adalah untuk menyajikan kasus pasien dengan distrofi kornea Fuchs dan membahas terkait beberapa etiologi edema kornea. Laporan kasus: Pasien laki-laki 79 tahun dirujuk dengan keluhan utama buram dan nyeri pada mata kiri. Pasien sebelumnya telah diobati dengan terapi anti-virus karena sebelumnya diagnosis dengan keratitis Herpes Simpleks Virus. Pemeriksaan oftalmologi menunjukkan adanya guttae pada lapisan endotel kornea mata kanan serta edema pada mata kiri. Pasien menjalani pemeriksaan dengan specular microscope dan Optical Coherence Tomography segmen anterior untuk melihat detil anatomi kornea. Pasien didiagnosis dengan Fuchs’ endothelial corneal dystrophy; pasien mendapat terapi obat tetes mata hypertonic agent dan bandage contact lens. Pada waktu kontrol satu minggu berikutnya terjadi peningkatan tajam penglihatan. Diskusi: Kornea yang edema dapat disebabkan oleh beberapa sebab. Gambaran klinis masing-masing etiologi dapat tumpang tindih. Kesimpulan: Fuchs’ Endothelial Corneal Dystrophy ditandai dengan adanya kehilangan sel endotel yang progresif, pada dekade ke 5 kehidupan. Gambaran khas berupa guttata pada endotel dan penebalan membran Descemet’s, sehingga terjadi edema, nyeri dan penurunan tajam penglihatan. Abstract

Antifungal intervention fails in approximately half of fungal keratitis patients, demonstrating its limitations. Voriconazole use for fungal keratitis has raised new interest because of its broad spectrum and good ocular penetration. However, its effectiveness has not been systematically evaluated. Here we try to clarify the benefits of voriconazole in fungal keratitis cases. Randomized controlled trials (RCTs) comparing vorico­nazole to placebo or other antifungal medications for fungal keratitis were searched in several databases, in­cluding PubMed, Scopus, Cochrane Library, ClinicalTrials, and WHO-ICTRP. The primary outcome that analyzed was best spectacle-corrected vision acuity (BSCVA). The secondary outcomes were treatment success, corneal perforation or need for therapeutic penetrating keratoplasty (TPK). From 621 records, nine studies were se­lected for analysis. The results were as follows: As an initial therapy, topical natamycin outperformed vori­conazole in BSCVA (mean difference = 0.14; 95% CI 0.02 to 0.26; P =.03). Voriconazole also has a greater risk of corneal perforation or TPK than natamycin (RR=1.69; 95% CI 1.11 to 2.58; P=.02). As an adjuvant, there is no significant difference found in BSCVA, treatment success, event of corneal perforation, or need for TPK between voriconazole and the other antifungal agents (itraconazole, ketoconazole, amphotericin B, natamy­cin, and placebo). This study shows that voriconazole is less superior than natamycin in treating early infec­tions of fungal keratitis. More RCTs with larger samples are needed to evaluate voriconazole's adjuvant efficacy.

Abstract

Distiroid Eksoftalmus atau penyakit mata tiroid (Thyroid Eye Disease; TED) merupakan komplikasi penyakit Graves pada mata. Pasien dapat menunjukkan berbagai gejala, termasuk terganggunya fungsi penglihatan yang ireversibel dan keterbatasan aktivitas harian jika tidak diobati. Kami menyajikan seri kasus yang terdiri dari dua kasus TED. Kasus pertama adalah anak berusia 14 tahun dengan penyakit Graves yang rutin kontrol di unit endokrinologi, dengan keluhan eksoftalmus dan lagoftalmus bilateral, serta keterbatasan gerakan mata pada mata kiri. Penglihatan mata kiri menurun akibat infeksi. Kasus kedua adalah seorang pria berusia 32 tahun dengan TED dengan temuan keratitis eksposur, keratopati, dan prolaps iris. Mata kanan tidak memiliki persepsi cahaya dan mata kiri terancam prolaps isi bola mata. Pendekatan multidisiplin dan penilaian klinis yang komprehensif, termasuk pemeriksaan mata, pada pasien gangguan hormon tiroid wajib dilakukan untuk mencegah komplikasi yang tidak dapat dipulihkan.

Abstract

Health communication has a strong relationship with human efforts to maintain their health, both at the individual, group, organizational, and government levels. Government communication can be seen during an interview with the Deputy Minister of Health of the Republic of Indonesia, Professor Dante Saksono Harbuwono, on the topic "The Indonesian Government's Steps to Prevent Citizens from Going for Treatment Abroad." Interview on December 6, 2023, with CNBC Indonesia media. In the interview session, there was a lot of medical terminology used. This research aims to discover the origins of the medical terms used in this interview, their meanings, and how they are absorbed into Indonesian. This type of research is descriptive qualitative. Each word will receive an explanation of how it was absorbed into Indonesian, namely through adoption, adaptation, translation, and creation. A total of 24 medical terms were absorbed into Indonesian. The most origin language is from English and the biggest absorption process is through adaptation. Given that English is the most used foreign medical terminology, future medical students need to focus on improving their English language proficiency to help academics, communication, and scientific pursuits such as research.

Abstract

Introduction: Bacterial keratitis, accounting for 90% of microbial keratitis cases globally, poses a serious threat to vision and is a leading cause of corneal blindness globally. This study provides novel insight and integrated analysis of bacterial keratitis at the National Eye Center, providing epidemiological insight in a tertiary eye center, which has not been previously reported in published literature to guide effective clinical decision making and public health strategies. Methods: This descriptive retrospective study was done at the National Eye Center. Data on demographics, presenting signs and symptoms, bacteriological examination results, treatment administered, and complications observed were collected from medical records between January to December 2023. Result: Among 33 patients, 51.5% were female with a mean age of 41.6 years. The majority (81.8%) had predisposing factors, with corneal trauma (48.5%) being the most common. Microbiological analysis by corneal scraping revealed gram-positive cocci in 72.7% of cases. Most patients (78.8%) were managed with topical antibiotics alone. Surgical intervention was required in 21.2% of cases due to complications. Conclusion: The findings highlight corneal trauma as the leading risk factor, predominantly gram-positive cocci involvement, and the necessity of timely intervention to prevent complications. Further large-scale studies are needed to refine management strategies.

Abstract

Ocular tuberculosis (TB) is one of the causes of uveitis, but there is often a skepticism and lack of acceptance regarding this diagnosis. We present a case of a-62-year-old female patient who complained of blurred vision, pain, and redness in her right eye. These symptoms had persisted for the past 16 months. The latest visual acuity in the right eye was 0.0167, and in the left eye was 0.63. Upon examination, keratic precipitates were found in the right eye, along with flare and cells +1/+1, pupil seclusion, a cloudy lens, and opacity in the posterior segment. The left eye was within normal limits. The patient was diagnosed with panuveitis of the right eye due to ocular TB and a complicated cataract. She had been referred by the local general hospital with complaints of blurred vision and was initially diagnosed with anterior uveitis due to ocular TB based on clinical presentation and positive Interferon-Gamma Release Assay (IGRA) test results. The patient was then referred back to the Internal Medicine Department of the local general hospital and the community health centre to start the anti-tuberculosis therapy (ATT). However, the patient refused treatment and, as a result, did not receive anti-tuberculosis drugs. Panuveitis TB, without adequate treatment, can lead to a poor prognosis.
