Abstract

Latar belakang: Otitis media supuratif kronik (OMSK) dengan kolesteatoma merupakan penyakitpada telinga tengah dengan salah satu komplikasi berupa destruksi tulang, yang dapat menyebabkankematian. Receptor Activator of Nuclear Factor Kβ-Ligand (RANKL) merupakan salah satu sitokinyang berperan dalam proses destruksi tulang.Tujuan: Mengetahui hubungan ekspresi RANKL denganderajat destruksi tulang akibat kolesteatoma pada penderita OMSK tipe bahaya.Metode: Penelitian inibersifat analitik dengan desain cross sectional. Sebanyak 40 sampel kolesteatoma dilakukan pemeriksaanimunohistokimia untuk dinilai ekspresi RANKL pada kolesteatoma.Hasil: Terdapat hubungan yangsignifikan antara ekspresi RANKL dengan derajat destruksi tulang dan komplikasi yang terjadi (p<0,05).Tidak terdapat hubungan antara ekspresi RANKL dengan kelompok umur dan jenis kelamin (p>0,05).Kesimpulan: Terdapat hubungan yang signifikan antara derajat destruksi tulang dengan ekspresi RANKLmelalui pemeriksaan imunohistokimia.Kata kunci: RANKL, destruksi tulang, kolesteatoma, OMSK ABSTRACTBackground: Chronic suppurative otitis media (CSOM) with cholesteatoma is a middle ear disease,leading to bone structure destruction which can cause fatal case. Receptor Activator of Nuclear FactorKβ-Ligand (RANKL) is known as one of the cytokine that has important role in the process of bonedestruction.Purpose: To identify an association between RANKL expression with bone destruction leveldue to cholesteatoma in malignant CSOM patients. Method: This was an analytical study with crosssectional design. About 40 cholesteatoma sample was examinated by using immunohistochemistry tomeasure its RANKL expression. Result: There was a significant relationship (p<0.05) between RANKLexpression with bone destruction level and complication. There was no significant relationship betweenRANKL expression with age and sex (p>0.05). Conclusion: There was a significant relationship betweenbone destruction level with the RANKL expression by immunohistochemistry examination.Keywords: RANKL, bone destruction, cholesteatoma, CSOM

Abstract

Pseudomonas aeruginosa and Staphylococcus aureus are common causative bacteria in Chronic Suppurative Otitis Media (CSOM). Honey, as a natural substance, has shown antibacterial activity. To evaluate the antibacterial effect of honey at different concentrations on P. aeruginosa (clinical isolates and ATCC 27852) and S. aureus (ATCC 29213) compared to ofloxacin. An experimental in vitro study using post-test only design was conducted. Bacterial isolates from CSOM patients and ATCC strains were exposed to honey at concentrations of 60%, 70%, 80%, 90%, and 100%. Zone of inhibition was measured using the agar diffusion method. Honey exhibited significant antibacterial activity against all tested bacteria. The highest inhibition zones were observed at 100% concentration. Ofloxacin showed greater inhibition than honey in general, but honey still demonstrated moderate to strong activity. Honey has potential as a natural antibacterial agent against P. aeruginosa and S. aureus, and may serve as an alternative or adjunct treatment for CSOM.

Abstract

Otitis media supuratif kronis merupakan penyakit telinga umum di negaranegara berkembang. Komplikasi otitis media supuratif kronis tipe bahaya mempunyai tanda dan gejala klinis yang khas.Tujuan penelitian ini adalah mengetahui profil penderita otitis media supuratif kronis (OMSK) tipe bahaya di RSUP H. Adam Malik. Penelitian deskriptif terhadap 119 penderita dari tahun 2006-2010. Sekitar 28,57% penderita dijumpai pada tahun 2010, sekitar 31,93% terjadi pada usia 11-20 tahun, sekitar 53,78% laki-laki, dan sekitar 38,66% pada telinga kanan. Sebanyak 68,91% terjadi akibat riwayat otitis media berulang dan 61,34% dengan keluhan utama telinga berair. Gejala dan tanda klinis yang sering terjadi adalah telinga berair (76,47%) dan perforasi membran timpani (74,79%), baik perforasi atik (0,84%), marginal (1,68%), subtotal (23,53%), dan total (48,74%). Gangguan pendengaran terbanyak adalah tuli konduktif (58,82%). Pada foto proyeksi Schuller, 62,18% dijumpai gambaran mastoiditis kronis dengan kolesteatoma. Dari hasil kultur dijumpai 21,01% Pseudomonas aeruginosa. 86,55% terjadi komplikasi mastoiditis.Profil penderita OMSK tipe bahaya di RSUP H. Adam Malik Medan sesuai dengan profil penderita OMSK tipe bahaya pada umumnya. Chronic suppurative otitis media (CSOM) is a common ear disease in developing countries. The complications of CSOM have a unique set of clinical signs and symptoms. This study aimed to identify the profile of dangerous type CSOM patients at H. Adam Malik General Hospital Medan in 2006-2010. A descriptive study of 119 patients in 2006-2010. From 119 patients, 28.57% were found in 2010, 31.93% were at age between 11-20 years old, 53.78% men and 38.66% were at right ear. 68.91% due to a history of recurrent otitis media and 61.34% with a main complaint of draining ears. The most clinical symptoms and signs were aqueous ears (76.47%) and tympanic membrane perforations (74.79%), as attic perforation (0.84%), marginal (1.68%), subtotal (23.53%), and total (48.74%). The most hearing impairments were conductive deafness (58.82%). In Schuller projections, 62.18% were found the imaging of chronic mastoiditis with cholesteatoma. From the culture results, 21.01% were Pseudomonas aeruginosa. 86.55% were mastoiditis complications. The profile of dangerous type CSOM patients at H. Adam Malik General Hospital Medan is similar with the other profile of dangerous type CSOM commonly.
