Abstract

This paper attempts to analyze maternal mortality in Nias Island, North Sumatra, where MMR is relatively higher than in other areas in Indonesia. This paper tries to examine the basic right highlighted in ICPD 1994 PoA. In addition, Corrêa and Petchesky propose that the fulfillment of women’s reproductive health rights must meet four principal elements, those are, bodily integrity, personhood, equality, and diversity. In line with the perspective suggested by Correa and Petchesky, this paper demonstrates the “omission, neglect, or discrimination” of women’s right for reproductive health. Social actors who play important roles in women’s reproductive health assessed in this paper are husbands, mother’s-in-law, TBAs, midwifes, and the government. This research concludes that maternal mortality in Nias and in Indonesia is a persistent problem since the social actors who are supposed to be responsible to prevent maternal mortality fail to do their job well. Instead, they tend to intentionally negate women’s right of reproductive health.

Abstract

Latar belakang: Otitis media supuratif kronik (OMSK) dengan kolesteatoma merupakan penyakitpada telinga tengah dengan salah satu komplikasi berupa destruksi tulang, yang dapat menyebabkankematian. Receptor Activator of Nuclear Factor Kβ-Ligand (RANKL) merupakan salah satu sitokinyang berperan dalam proses destruksi tulang.Tujuan: Mengetahui hubungan ekspresi RANKL denganderajat destruksi tulang akibat kolesteatoma pada penderita OMSK tipe bahaya.Metode: Penelitian inibersifat analitik dengan desain cross sectional. Sebanyak 40 sampel kolesteatoma dilakukan pemeriksaanimunohistokimia untuk dinilai ekspresi RANKL pada kolesteatoma.Hasil: Terdapat hubungan yangsignifikan antara ekspresi RANKL dengan derajat destruksi tulang dan komplikasi yang terjadi (p<0,05).Tidak terdapat hubungan antara ekspresi RANKL dengan kelompok umur dan jenis kelamin (p>0,05).Kesimpulan: Terdapat hubungan yang signifikan antara derajat destruksi tulang dengan ekspresi RANKLmelalui pemeriksaan imunohistokimia.Kata kunci: RANKL, destruksi tulang, kolesteatoma, OMSK ABSTRACTBackground: Chronic suppurative otitis media (CSOM) with cholesteatoma is a middle ear disease,leading to bone structure destruction which can cause fatal case. Receptor Activator of Nuclear FactorKβ-Ligand (RANKL) is known as one of the cytokine that has important role in the process of bonedestruction.Purpose: To identify an association between RANKL expression with bone destruction leveldue to cholesteatoma in malignant CSOM patients. Method: This was an analytical study with crosssectional design. About 40 cholesteatoma sample was examinated by using immunohistochemistry tomeasure its RANKL expression. Result: There was a significant relationship (p<0.05) between RANKLexpression with bone destruction level and complication. There was no significant relationship betweenRANKL expression with age and sex (p>0.05). Abstract

Background: Untreated or undertreated, pulmonary tuberculosis could cause severe complications until death. After treatment, residual lesions might occur. The presence of residual lesions is varied, including: fibrosis, cavity, bronchiectasis and calcification. Transforming growth factor-β (TGF-β) is a cytokine associated with lung inflammation, which plays a role in lung fibrosis. However, only a few studies have assessed the serum level of TGF-β in post-treatment tuberculosis patients. The main objective of this study was to determine the relationship between TGF-β levels and pulmonary fibrosis in patients with pulmonary tuberculosis. Methods: Among 51 patients that had undergone anti-tuberculosis treatment were observed, consisting of 31 men, 20 women. Of all patients, there were 26 people with a smoking history, including 25 men and 1 woman. All patients had been recovered, confirmed by clinical state, laboratory and radiology examination. The ELISA test was performed to measure TGF-β level, while chest X-ray was used to evaluate for the occurence of pulmonary fibrosis. Results: The mean level of TGF-β in patients with lesion (+) was 7628.02 pg/mL (SD: ±4928.38) while the mean level of TGF-β in patients with lesion (-) was 2315.11 pg/mL (SD: ±505.83). Abstract

Pasien PPOK yang mengalami atrofi dan disfungsi otot akan mengurangi pengambilan oksigen maksimal (VO2 maks) dalam jaringan. Berat badan dan penipisan massa bebas lemak (fat free mass) adalah masalah umum pada pasien PPOK dan terkait dengan kelemahan otot dan intoleransi latihan, kedua hal ini akan menyebabkan penurunan aktivitas dan kualitas hidup. Olahraga (latihan fisik) pada pasien PPOK memberikan efek yang baik terhadap toleransi latihan dan kualitas hidup. Tujuan dari penelitian ini adalah untuk menilai peran olahraga terhadap VO2 maks dan fat free mass pada pasien PPOK. Desain penelitian ini adalah eksperimental, dilakukan pada 20 pasien PPOK. Pasien melakukan latihan tubuh bagian atas dan bawah dengan latihan otot pernapasan dan sepeda statis, dua kali seminggu selama 4 minggu, mulai dari 5-20 menit/sesi. Pengukuran VO2 maks dihitung sebelum dan sesudah intervensi menggunakan Nury’s Formula® yang dirancang khusus untuk orang Indonesia (r = 0,68; p < 0,005), dengan mengkonversi jarak berjalan 6 menit. Fat free mass diukur dengan BIA (Bio Impendances analysis) pre dan pasca intervensi dan indeks massa bebas lemak dihitung sebagai FFM kg/m2. Analisis statistik dilakukan oleh T Test. Terdapat peningkatan VO2 maks diperoleh dengan tes berjalan 6 menit, dari rerata 9.24 menjadi 9.44 tetapi tidak signifikan secara statistik (p = 0,066). Pada FFM ada peningkatan tetapi tidak signifikan secara statistik pada indeks massa bebas lemak dari rerata 18.37 menjadi 18.44 (p < 0,689) setelah intervensi. Kejadian eksaserbasi tidak ditemukan ketika intervensi dilakukan. Abstract

The pillars of comprehensive pulmonary rehabilitation program for chronic obstructive pulmonary disease (COPD) patients include physical exercise and good nutrition. The aim of this study was to evaluate the efficacy of pulmonary rehabilitation, which included physical exercise and chicken egg white supplementation, on the quality of life (QoL) and functional capacity among patients with stable COPD. The COPD patients were enrolled prospectively in this quasi-experimental study and completed a 12-week smartphone-guided home-based physical exercise program that comprised strength and resistance training three times per week for 30 minutes each session. Participants were divided into two groups: the control group who underwent physical exercise only, and the intervention group who had physical exercise and chicken egg white supplementation as a protein source. Patient characteristics including sex, age, nutritional status, comorbidities, smoking status, and obstruction severity, were evaluated. The COPD assessment test (CAT) score and six-minute walk test (6MWT) were used as the parameters to evaluate QoL and functional capacity, respectively. Of the total 50 patients included in the study, 12 were excluded due to follow-up and adherence problems. Our data indicated there were significant CAT score reduction and 6MWT improvement in both control and intervention groups after 12 weeks compared to baseline data. However, reduction of mean CAT score was higher in intervention compared to control group (-13.47±6.49 vs -5.42±5.07, p<0.001). In addition, the improvement of 6MWT was also higher in intervention group compared to control group (145.47±69.2 vs 32.42±17.3 meters, p<0.001). In conclusion, chicken egg white supplement to male patients with stable COPD who exercise with resistance and strength training could improve the QoL and functional capacity.

Abstract

Exclusive breastfeeding for the first six months of a baby's life has been proven to provide significant benefits, both in terms of health and developmental outcomes. However, in many countries, including Indonesia, the coverage of exclusive breastfeeding remains very low, contributing to high rates of malnutrition, including stunting. This scoping review was conducted to identify and analyze the social factors influencing the practice of exclusive breastfeeding, with the aim of informing more effective intervention strategies. The study employed a scoping review methodology, involving a comprehensive literature search across three major databases: PubMed, Scopus, and Google Scholar, covering publications from 2014 to 2024. Of the 401 articles identified, 7 were selected for analysis following a rigorous selection process. The review findings indicate that support from husbands, extended family, and the broader social environment are key factors in the success of exclusive breastfeeding. Identified barriers include a lack of family support, the influence of local cultural beliefs, misinformation about breastfeeding, and misconceptions regarding infant needs. These factors significantly affect mothers' decisions to practice exclusive breastfeeding. Furthermore, the role of healthcare professionals in providing accurate education to mothers and families is crucial in improving exclusive breastfeeding rates. In conclusion, to enhance the success of exclusive breastfeeding, a comprehensive approach involving the entire family and considering local cultural contexts is required. Policy support, intensive educational campaigns, and improved access to lactation counseling services should be prioritized to achieve higher exclusive breastfeeding rates in Indonesia.

Abstract

Background:During the Coronavirus Disease 2019 (COVID-19) pandemic, nurses and doctors served as the frontline healthcare workers. Consequently, mental health issues among healthcare workers can develop into significant health problems and impose a substantial social burden, particularly on nurses and doctors working in hospital settings. Objective:To analyze the prevalence and factors influencing anxiety levels among healthcare workers, specifically nurses and doctors, at Adam Malik General Hospital Medan during the COVID-19 pandemic. Methods:This study employed a cross-sectional design with bivariate analysis. Data were collected using the DASS-42 questionnaire, which was distributed to 114 participants comprising nurses and doctors working in both the COVID-19 emergency isolation and ward isolation units. Results:The findings revealed that 29 participants (25.4%) exhibited symptoms of anxiety, with the majority experiencing mild anxiety (13.2%). The nursing profession demonstrated the highest prevalence of anxiety disorders, with 33.3% of nurses reporting symptoms. Being a nurse was significantly associated with higher anxiety levels compared to being a doctor, and working in the emergency isolation room was significantly associated with higher anxiety levels compared to working in the isolation ward or both units (p<0.05). Conclusion:Factors such as gender, marital status, parental status, post-COVID-19 status, family history of COVID-19, vaccination history, and comorbidities did not significantly affect anxiety levels among healthcare workers during the COVID-19 pandemic at Adam Malik General Hospital Medan. However, anxiety was prevalent among healthcare workers, with nurses and those working in the emergency isolation room being particularly affected. Abstract

This study aims to analyze the clean and healthy living behaviour (PHBS) among students post-Covid-19 pandemic. The study employs a qualitative method with in-depth interviews to explore behavioural changes in maintaining hygiene and health after Covid-19 is no longer considered a pandemic. The results indicate that awareness and practice of PHBS among students have declined following the end of the pandemic. Factors contributing to this decline include habits, personal reasons, and social environment. The study suggests the need for more intensive health campaigns to promote PHBS among the younger generation to prevent the spread of other infectious diseases in the future.
