Abstract

High recurrence rate and the necessity for repeated surgical interventions contribute to the chronicity and treatment-resistant nature of chronic rhinosinusitis with nasal polyps (CRSwNP). Thymoquinone, known for its protective effects on epithelial integrity, has not been previously explored in CRSwNP. The aim of this study was to investigate the therapeutic potential of thymoquinone to restore epithelial integrity by assessing p63 transcription factor and claudin protein expressions, as well as periostin mRNA expression in an animal model. An in vivo study using post-test-only control group design was conducted in which male Wistar rats were randomly assigned to three groups, each consisting of 10 animals: healthy group, CRSwNP group, and thymoquinone-treated group for three weeks. Immunohistochemistry was used to analyze the p63 and claudin protein expressions, while periostin mRNA expression was quantified using quantitative reverse transcription polymerase chain reaction (qRT-PCR). This study found that thymoquinone significantly reduced p63 transcription factor expression compared to the untreated CRSwNP group (p=0.009). Claudin protein expression was significantly higher in thymoquinone-treated group compared to CRSwNP group (p=0.007), indicating improved epithelial barrier function. Periostin mRNA expression showed no significant difference between healthy and thymoquinone-treated groups (p=0.564), but a significant decrease was observed in CRSwNP group compared to thymoquinone-treated group (p=0.000) and between the healthy and CRSwNP groups (p=0.002), suggesting attenuation of tissue remodeling and inflammation. In conclusion, thymoquinone could enhance sinonasal epithelial barrier integrity in CRSwNP by downregulating p63 transcription factor, upregulating claudin protein expression, and reducing periostin mRNA expression. These findings emphasize the potential of thymoquinone as a therapeutic agent to mitigate inflammation and tissue remodeling in CRSwNP, warranting further investigation as a novel treatment option.

Abstract

This research is motivated by the high disaster risk in South OKU Regency and the significant capacity gaps in human resources for disaster management, both within official institutions (BPBD) and among unorganized community volunteers. This gap hinders the effectiveness of disaster mitigation and education. The focus of this research is to analyze the process and success of strengthening volunteer capacity through an innovative change project called the Disaster Knowledge Training Post (PASWACANA P3K). Using a qualitative approach with a descriptive case study design, data were collected through in-depth interviews, document analysis, and participant observation of the project's leadership and implementation processes. Key findings indicate that this initiative was successful thanks to strategic leadership that secured internal legitimacy and orchestrated a multi-helix collaboration model, culminating in the formation of the South OKU Siaga Volunteer Forum (ForOKUSS). This training program effectively improved the volunteers' skills, which were then directly applied in community education activities. It is concluded that the First Aid (P3K) model is an innovative solution that successfully bridges the capacity gap by empowering local potential, transforming volunteers from mere responsive forces into proactive mitigation agents, and offering a proven blueprint for building community resilience. ABSTRAKPenelitian ini dilatarbelakangi oleh tingginya risiko bencana di Kabupaten OKU Selatan serta adanya kesenjangan kapasitas yang signifikan pada sumber daya manusia penanggulangan bencana, baik di lembaga resmi (BPBD) maupun di kalangan relawan komunitas yang belum terorganisir. Kesenjangan ini menghambat efektivitas mitigasi dan edukasi kebencanaan. Fokus penelitian ini adalah menganalisis proses dan keberhasilan penguatan kapasitas relawan melalui sebuah proyek perubahan inovatif bernama Posko Pelatihan Pengetahuan Kebencanaan (PASWACANA P3K). Menggunakan pendekatan kualitatif dengan desain studi kasus deskriptif, data dikumpulkan melalui wawancara mendalam, analisis dokumen, dan observasi partisipatif terhadap proses kepemimpinan dan implementasi proyek. Temuan utama menunjukkan bahwa inisiatif ini berhasil berkat kepemimpinan strategis yang mampu mengamankan legitimasi internal dan mengorkestrasi kolaborasi model multi-helix, yang puncaknya adalah pembentukan Forum Relawan OKU Selatan Siaga (ForOKUSS). Program pelatihan ini secara efektif meningkatkan keterampilan relawan yang kemudian langsung diaplikasikan dalam kegiatan edukasi masyarakat. Disimpulkan bahwa model P3K merupakan solusi inovatif yang berhasil menjembatani kesenjangan kapasitas dengan memberdayakan potensi lokal, mengubah relawan dari sekadar tenaga responsif menjadi agen mitigasi proaktif, dan menawarkan cetak biru yang terbukti efektif untuk membangun ketangguhan komunitas.

Abstract

Diseases of ear, nose, and throat (ENT) are common diseases in the world. The diseases are essential due to morbidities, which affect the physiological functions in the head and neck region. Cerumen impaction is the most common finding disease of ENT in children. The community service was done to find the distribution of ear, nose, and throat examination results of students in Letjen Jamin Ginting Junior High School, Berastagi, especially cerumen impaction which then were treated with cerumen removal. The community service also aimed to increase student's knowledge about ENT disease. The ENT examination was done to 188 students. The students with cerumen impaction were treated with cerumen removal. The students also educated about ENT disease to improve knowledge about ENT disease. Among 188 students, the number of male students was more than female with 98 students (52.1%), where 76 students (40.4%) were 14 years old. 150 students (79.8%) did not have any complaint about their ear, nose, and throat. Ear fullness was a common chief complaint from the students with 15 students (8.0%). Ear disease was a frequent disease with 44 students (47.3%) and cerumen impaction as the most common findings with 43 students (22.9%). In ENT disease education, the evaluation was done with the highest result was get by 8 students with 80% corrected answers. Ear disease had high prevalence with cerumen impaction as the most common findings of ENT examination.
