Abstract

Pendahuluan: Gingivitis adalah manifestasi oral yang paling tinggi prevalensinya pada ibu hamil. Perubahan hormon dan vaskular yang menyertai kehamilan diketahui memperparah respons inflamasi terhadap iritan lokal. Seringkali ibu hamil mengabaikan dalam menjaga kesehatan mulutnya yang mengakibatkan adanya retensi plak pada gigi dan tepi gingiva, hal ini dapat menyebabkan peradangan pada gingiva. Berdasarkan data dari hasil Riskesdas, prevalensi gingivitis pada wanita di Indonesia adalah 74%. Penelitian ini bertujuan menganalisis status kesehatan gigi dan gusi serta faktor-faktor risiko kehamilan pada ibu hamil. Metode: Jenis penelitian analitik dengan menggunakan desain crossectional. Populasi penelitian adalah ibu hamil di Puskesmas Kecamatan Kelapa Gading, Jakarta Utara yang datang pada bulan November 2016. Ibu hamil diperiksa plak skor dan indeks gingivitis. Jumlah sampel adalah 90 ibu hamil dan sebagian besar (81,1%) berada di trimester 3. Hasil penelitian dianalisis dengan uji ANOVA dan Chi-square. Hasil: Sebanyak 29% ibu hamil memiliki skor plak yang baik (0,1-0,9). Prevalensi gingivitis terdapat pada semua ibu hamil (100%) yang terdiri dari 16,7% gingivitis ringan, 26,7% gingivitis sedang dan 56,7% gingivitis berat. Peningkatan tajam pada gingivitis ditemukan dari trimester 1 ke trimester II dan stabil pada trimester III. Terdapat hubungan yang signifikan antara tingkat keparahan gingivitis dan umur kehamilan, waktu dan frekuensi menyikat gigi, indeks plak, pendidikan, dan pengetahuan (p <0,05). Simpulan: Terdapat hubungan antara gingivitis dengan beberapa faktor risiko kehamilan yaitu umur kehamilan, frekuensi sikat gigi, waktu sikat gigi dan indeks plak.Kata kunci: Kehamilan, gingivitis, plak, hormon. ABSTRACTIntroduction: Gingivitis is the most prevalent oral manifestations associated with pregnancy. The hormonal and vascular changes that accompany pregnancy are known to exaggerate the inflammatory response to the local irritants. Often pregnant women ignore to maintain their oral health which will cause plaque retention on the teeth and gingival edges. This can cause inflammation of the gingiva. Based on the Indonesia Basic Health Research, the prevalence of gingivitis among Indonesian women was 74%. This study was aimed to analyse the oral health status and risk factors for gingivitis in pregnant women. Methods: Analytic method using cross-sectional design was conducted in November 2016. Pregnant women was examined for their plaque scores and gingivitis index using the WHO periodontal probe. The number of samples was 90 pregnant women, and most of them (81.1%) were in the trimester III gestational period. The study was then analysed by using ANOVA and chi-square test. Results: Only 29% of pregnant women had a good plaque score (0.1-0.9). Gingivitis was prevalent in all of the pregnant women, which consisted of 16.7% mild gingivitis, 26.7% of moderate gingivitis, and 56.7% of severe gingivitis.  A high increase in gingivitis was found in the period of trimester I to trimester II, and tend to be stabilised in the trimester III gestational period. In further analysis, we found significant relationships between the severity of gingival inflammation and gestational age, time and frequency of tooth brushing, plaque index, education, and knowledge (p < 0.05). Conclusion: There is a relationship between gingivitis with several pregnancy risk factors, namely gestational age, toothbrush frequency, toothbrush time, and plaque index.Keywords: Pregnancy, gingivitis, plaque, hormone.

Abstract

Introduction: Malocclusion, or dental occlusion disorder, is one of the main factors disrupting the function of the oral cavity, especially the function of mastication. Malocclusion can be treated using orthodontic appliances. Orthodontic appliances are sometimes used not only for the benefit of dental treatment but as mere fashion due to a lack of knowledge, causing negative effects. Incompetent operators and poor maintenance procedures in orthodontic treatment will have a detrimental impact. Aims: The objective was to determine the knowledge level of junior high school students about the side effects of using fixed orthodontic appliances and the treatment of fixed orthodontic appliances by general dentists, orthodontists, and dental specialists. Methods: The type of research used is quasi-experimental, with one group and two measurements (pretest and post-test). Participants in this study were junior high school students in Central Jakarta. The sampling technique used total sampling. Data collection techniques using a questionnaire and analysis of data using the Wilcoxon Test P<0.05 are considered significant. The total number of students included in the study was 344. Result: There was a significant difference in student knowledge between the pretest and post-test after receiving an explanation among junior high school students (p<0.05). Conclusion: A fixed orthodontic treatment case was determined to be treated by an orthodontic specialist, and the general dentist has a limited range of cases treated with removable orthodontic appliances. Dental artisans can legally only treat removable dental prostheses. This information was conveyed to junior high school students; hence, they have been cautioned about choosing the right operator.

Abstract

ABSTRAK Pendahuluan: Klasifikasi periodontal telah berubah, dan sistem baru perlu diperkenalkan dan diterapkan dalam praktik klinis. Selama ini pengajaran klasifikasi periodontal diajarkan melalui pengajaran langsung dan atau workshop. Modifikasi pengajaran dibuat melalui aplikasi seluler. Aplikasi seluler yang dipilih adalah berbasis android karena dapat digunakan secara global dan dapat digunakan untuk menentukan keputusan yang ditentukan dalam diagnosis. Tujuan dari penelitian ini adalah membuat dan menguji sistem aplikasi pakar menggunakan diagnosis penyakit periodontal klasifikasi tahun 2017 berbasis android. Metode: jenis penelitian pre-eksperimental. Aplikasi yang dikembangkan berbasis android dengan System Development Life Cycle dengan metode inkremental. Empat langkah yang digunakan termasuk persyaratan, desain, dan pengembangan, pengujian dan implementasi. Desain dan pengembangan diterapkan menggunakan flowchart, use case diagram, dan activity diagram. Pengujian sistem dilakukan dengan black box testing. Analisis data menggunakan system usability scale (SUS) untuk mengetahui efisiensi aplikasi dan kepuasan pengguna. Skor SUS mempunyai tiga kategori yaitu (D) buruk (< 68), (C) dapat diterima (68-73,9); (B) baik (74-80,2) dan (A) sempurna (≥80,3). Hasil: Uji aplikasi sistem pakar diagnosis penyakit periodontal berbasis android dengan system usability scale pada 35 subjek menunjukan skor 75,14 atau nilai B dengan kategori baik. Simpulan: Aplikasi ini dapat digunakan untuk mendiagnosis penyakit periodontal menurut klasifikasi penyakit periodontal baru dan menjadi alternatif metode pengajaran untuk diagnosis penyakit periodontal.Kata kuncisistem pakar, diagnosis, periodontitis, aplikasi seluler, klasifikasi.Developing and testing mobile expert system to assist diagnosis in 2017 periodontal disease classification: nonrandom pre-experimental study ABSTRACTIntroduction: The periodontal classification has changed according to the latest consensus, necessitating the introduction and integrating of new systems into clinical practice. The updated periodontal classification is presently taught through methods such as direct instruction and hands-on training. A mobile app has been employed to enhance the teaching process. The Android operating system was selected due to its global popularity and its potential to aid in diagnosing periodontal diseases. The objective of this research is to develop a mobile application for the purpose of diagnosing periodontal conditions. Methods: The application was created for the Android platform using the System Development Life Cycle, employing an incremental approach. The process comprised four key phases: requirements gathering, design, development, testing and implementation. Design and development were realized through flowcharts, case diagrams and activity diagrams. The system underwent evaluation through black box testing. Data analysis was conducted using the System Usability Scale (SUS) to assess the application's efficiency and user satisfaction. The SUS scores were categorized as follows: <68 for poor (D), 68-73.9 for acceptable (C), 74-80.2 for good (B), and ≥80.3 for excellent (A). Results: The usability test of the Android-based mobile application for periodontal diagnosis was conducted on 35 subjects, resulting in a score of 75.14. This score falls within the Grade B category, indicating a good level of usability. In conclusion, this application holds the potential for diagnosing periodontal diseases based on the updated periodontal disease classification. Abstract

Pendahuluan: Pengetahuan memiliki hubungan yang erat terhadap perilaku seseorang terhadap suatu penyakit dan juga upaya pencegahannya. Seseorang yang memiliki kondisi gigi crowding memiliki tingkat risiko yang tinggi terhadap terjadinya penyakit periodontal. Tujuan: Untuk mengetahui hubungan antara tingkat pengetahuan kesehatan gigi dan mulut terhadap kondisi status kesehatan jaringan periodontal pada kondisi gigi crowding siswa MTsN 9 Jakarta. Metode: Desain penelitian adalah potong lintang. Sampel dipilih dengan menggunakan metode purposive sample. Jumlah sampel adalah 96 subjek. Pengambilan data dilakukan melalui pengisian kuesioner tingkat pengetahuan kesehatan jaringan peridontal, mengukur indeks oral hygiene dan prosentase BoP. Subjek kemudian difoto keadaan gigi rahang bawahnya dan dikategorikan tingkat crowding giginya menggunakan sebuah aplikasi. Data disajikan secara deskriptif dan analitik. Hasil dan Pembahasan: Prevalensi gigi crowding yang ditemukan sebesar 52%. Tingkat pengetahuan siswa-siswi mengenai kesehatan jaringan periodontal sebesar 55,2% memiliki kategori baik. Terdapat korelasi negatif bermakna antara tingkat pengetahuan dengan status OHI dan BoP (p<0,005). Gigi crowding tidak mempengaruhi skor OHI maupun BoP. Simpulan: Tingkat pengetahuan yang baik tentang kesehatan jaringan periodontal dapat diimplementasikan pada dirinya sehingga mereka memiliki kesehatan jaringan periodontal yang baik. Gigi crowding tidak mempengaruhi kesehatan jaringan periodontal bila dengan penjagaan kesehatan gigi dan mulut yang baik.

Abstract

Abstract Introduction: Research has investigated understanding and behaviors related to periodontal health maintenance and its health consequences. Failure to maintain periodontal health increases the likelihood of periodontal disease. Additionally, rising levels of obesity pose a risk for periodontal disease. Islam mandates its adherents to pursue knowledge in religious matters, which encompasses various aspects of life, including knowledge about maintaining health. Moreover, good behavior should be rooted in sound knowledge. Adequate knowledge and behaviors regarding oral health care promote cleanliness and healthiness of periodontal tissues. Objective: To determine the differences in knowledge and health behavior regarding periodontal health among students based on nutritional status. Material and Methods: This study used a cross-sectional method with a sample of 104 YARSI University students as respondents. The instrument used was a questionnaire containing 25 questions. The data analysis used was the Chi- Square categorical correlational analytical test. Results: The results of the Chi-Square test between knowledge of periodontal health care and nutritional status showed no significant relationship p = 0.378 (p>0.05). Meanwhile, between periodontal health behavior based on dental visits and nutritional status, there were significant results p = 0.035 (p<0.05). Discussion: Dental visit behavior based on BMI shows the same frequency between normal and obese respondents, this is not in line with previous research by Nihtila et al. (2016) which showed that people who are overweight tend not to visit the dentist in the last two years compared to people who are not overweight. Coclusion: 1) There is no correlation between awareness of periodontal health care and nutritional status, (2) Dental visit behavior correlates with nutritional status.

Abstract

Abstract Introduction: Fixed orthodontic appliance is one of the tools used for malocclusion treatment. The parts of the tool stick to the teeth which makes it difficult to clean the teeth, so it will facilitate plaque accumulation and can cause gingivitis. Objective: This study is to determine how the influence of the long use of orthodontic devices on the occurrence of gingivitis in individuals aged 15-17 years and its views in an Islamic perspective. Material and Methods: Type of observational study with cross-sectional study design. The study subjects were individuals aged 15-17 years who wore fixed orthodontic tools taken by purposive sampling method. The study data were taken by questionnaire, measurement of oral hygiene level with dental and oral hygiene questionnaire, and gingival health with Gingival Index (Loe and Silness) and CPITN Index. Results: The prevalence of gingivitis based on the gingival index was obtained as much as 97.2%. The prevalence of subjects who have periodontal disease based on the CPITN index is 88.9%. Discussion: Type of observational study with cross-sectional study design. The study subjects were individuals aged 15-17 years who wore fixed orthodontic tools taken by purposive sampling method. The study data were taken by questionnaire, measurement of oral hygiene level with dental and oral hygiene questionnaire, and gingival health with Gingival Index (Loe and Silness) and CPITN Index. Discussion: There is no significant difference between the duration of use of orthodontic devices and the Gingival Index (p = 0.051). The relationship was not significant between the duration of use of orthodontic devices and the CPITN Index (p = 0.128) and there was also no significant relationship between the duration of use of orthodontic devices and dental and oral hygiene questionnaires (p = 0.853). Conclusion: There is no influence between the duration of use of orthodontic devices and gingivitis.

Abstract

Abstract Introduction: Treatment of dental malocclusion with fixed orthodontic appliances can lead to plaque collection which can cause periodontal disease. Fixed orthodontic appliances are widely used among adolescents aged 15-17 years in Jakarta high schools because the prevalence of malocclusion in Indonesia is quite high at 80%. Objective: To determine the relationship between the use of fixed orthodontic appliances and periodontal tissue health between adolescents who wear and adolescents who do not wear fixed orthodontic appliances aged 15-17 years in Jakarta High School. Material and Methods: This type of research is comparative analytic with cross sectional research design. Data were collected by clinical examination using the CPITN Index and Gingival Index and filling out an oral hygiene questionnaire. Results: The results of the study on 103 subjects were divided into 50 people who wore fixed orthodontic devices and 53 people who did not wear fixed orthodontic devices. The prevalence of gingivitis based on the Gingival Index of all subjects was 96.11%. The CPITN Index examination resulted in 92.23% having periodontal tissue disease. Chi-Square test of the relationship between subjects who wear and do not wear fixed orthodontic devices with the CPITN Index has a significant relationship P-value = 0.002. Discussion: There is a significant difference between the respondent groups with the CPITN index in accordance with the research of Moosa (2015) and between gender and attitude in accordance with the theory of Lewin (1970) and Green (1991). Conclusion: Subjects who wear and do not wear fixed orthodontic devices have unhealthy periodontal tissue, so subjects must be given counseling and periodontal care.

Abstract

Introduction: Aesthetic factors greatly affect a person's self-confidence. One of the periodontal tissue conditions that causes many aesthetic complaints is gingival enlargement. Gingivectomy is a treatment that can be done in cases of gingival enlargement. This case report describes the management of gingivectomy using the scalpel method in the case of gingival enlargement of teeth 11 and 21. Case History: A 47-year-old female patient came with complaints of swollen gums in the upper front between the incisors. The patient wants the gums to be treated. Previously, the gums had been operated on in 2014 in Brunei but the gums returned to swelling 8 months later. The management in this case was gingivectomy with the scalpel method, namely sterilization of the operating area and infiltration anesthesia. Making bleeding points using a pocket marker. The incision is made with blades no. 11, 12, 15. Remove granulation tissue, calculus and soft cementum by scaling and root planning, then control bleeding and irrigation. Perform gingivoplasty and irrigation with 10% Nacl and Povidone Iodine. Manipulation and application of periodontal dressing, then the patient was given medication and postoperative instructions. Discussion: In this case the choice of treatment using a scalpel because epithelial regeneration can take place in a short time when compared to laser and electrosurgery. Conclusion: The use of a scalpel can be an alternative to the management of gingivectomy with a relatively low cost and a better incision, creating the least amount of tissue damage and faster healing.

Abstract

Introduction: The gag reflex is one of the complicating factors that is quite influential on the success of dental treatment, one of which is in the treatment of dentists in patients with partially erupted third molars. Case Report: A 22-year-old female patient came with a complaint of wanting to clean tartar. The patient also complained that the lower right back tooth was often stuck with food and was difficult to clean because it was covered by the gums. The patient had no systemic disease, had no drug and food allergies, and did not smoke. This patient during examination was found to have a high gag reflex, so special measures were needed to overcome this problem. Discussion: Patients with low and moderate gag reflex can be handled easily during treatment, but patients with high gag reflex require special handling. Handling of patients who have a high gag reflex can use several methods including breath control and distraction during treatment. Conclusion: In dental treatment, excessive gag reflex can interfere with the patient and the dentist's work process which causes the patient to worry every time they visit the dentist. The method of managing the gag reflex can be done so that a comfortable and productive treatment can be obtained.
