Abstract

This research aimed to design of outpatient disease index information system using V-Model Method and implementation with visual programming. This research use qualitative research methods with descriptive approach, with data collection techniques based on observation, interview, and literature review. In developing of this information system of outpatient disease index built using waterfall method and Data Flow Diagram (DFD) aids.Problems found in this research are : (1) Data processing was done in hand writing, so the officers took a long time. (2) There was no special system of disease index computerized. (3) Making report of disease index activities manually.The suggestion from writer are : (1) It needs using Standar Operating Prosedur (SOP) of disease index the writer made a draft of Standar Operating Prosedur (SOP) so that indexing activities can run continuously. (2) Adding human resources to manage of outpatient disease index. (3) It’s needs to evaluated the system and training about information system for user.

Abstract

Primary clinics play an essential role as first-level healthcare facilities that provide both medical and administrative services to the community. In efforts to improve the quality of healthcare services, the management of medical records serves as one of the main components that must be carried out in an orderly, accurate, and systematic manner. Medical records function not only as patient documentation but also as legal documents, communication tools among healthcare professionals, and data sources for service quality evaluation. This Community Service (PKM) activity was conducted at Klinik Prima Husada, Bandung City, with the aim of strengthening staff understanding of proper procedures for organizing and completing medical records. The methods included socialization, education, and mentoring sessions involving healthcare and administrative personnel. Based on the implementation results, the medical record system has been functioning but still faces challenges, particularly in diagnosis coding and form completeness. Through this activity, it is expected that the staff’s knowledge and skills in managing medical records will improve, thereby supporting work efficiency, administrative orderliness, and the overall quality of healthcare services within the clinic.

Abstract

Pengelolaan data rekam medis di fasilitas kesehatan tingkat pertama sering menghadapi permasalahan duplikasi data yang dapat menyebabkan inefisiensi, ketidakakuratan laporan, serta risiko dalam pelayanan pasien. Kegiatan Pengabdian kepada Masyarakat ini bertujuan untuk menerapkan sistem data cleansing guna mencegah dan menghilangkan duplikasi rekam medis di Puskesmas Gemuruh. Metode yang digunakan meliputi pelatihan, implementasi sistem, dan evaluasi terhadap 20 orang tenaga kesehatan. Proses data cleansing dilakukan melalui tahapan identifikasi data, validasi, standarisasi, dan penghapusan data duplikat. Hasil kegiatan menunjukkan adanya penurunan signifikan terhadap data duplikat serta peningkatan akurasi data. Peserta juga mengalami peningkatan pemahaman dan keterampilan dalam pengelolaan data. Program ini berkontribusi dalam meningkatkan kualitas layanan kesehatan melalui tata kelola data yang lebih baik.
