Abstract

Background. Cancer cachexia is common in head neck cancer caused by increasing proinflammatory cytokines, has effect on hipermetabolism, increased nutritional needs, anorexia, decreased muscle mass and body weight. Omega-3 fatty acids play a role in reducing inflammation, improving muscle mass and handgrip.Objective. This cross sectional study conducted in Department of Radiotherapy Dr. CiptoMangunkusumo Hospital, aimed to investigate correlation between omega-3 fatty acids plasma with muscle mass and hand grip-muscle strength in head neck cancer subjects undergoing radiotherapy.Method. This study was conducted from June to August 2016. The subjects were head neck cancer patients in stage I‒IV (18–65 years old) and had received >25 times radiation, and  obtained by consecutive sampling method. Total omega-3 fatty acids intake was obtained by semiquantitative Food Frequency Questionnaire. Anthropometric measurements used ShorrBoard and Smic® ZT-120, muscle mass used Omron HBF375®, and handgrip used Jamar® dinamometer. Omega-3 fatty acids plasma were examined by gas chromatography flame ionized detector. Correlation omega-3 fatty acids plasma with muscle mass were analyzed by Pearson, and correlation with handgrip by Spearman.Results. There were 52 subjects completed all examinations, received radiotherapy ≥25 times combination chemotherapy, 57% male, 50 years old. Most sites at nasopharynx, mostly stage IV, 25% subjects had normal body mass indeks, 75% were low. Most subjects had nutritional problems caused by inadequate intake of energy, protein, fat, and omega-3 fatty acids. Majority (75%) had small muscle mass (28.4±4.7%), mostly (75%) normal handgrip, median 37.1(25.7‒68.5) kg, and all subjects had very low omega-3 fatty acids plasma (2.5±0.8%). There was strong correlation between omega-3 fatty acids plasma with muscle mass (r =0.6, p <0.05) and handgrip (r =0.8, p <0.001) who received radiotherapy  (>60–70 Gy), no correlation less than that doses.Conclusion. There was correlation between omega-3 fatty acids plasma with muscle mass and handgrip, at radiotherapy doses >60–70 Gy.

Abstract

Introduction: SRS is a non-invasive modality in management of vestibular schwannoma. There is limited study comparing dosimetric parameters between three techniques SRS in vestibular schwannoma cases, thus IMRT Step and Shoot (IMRT-SS), VMAT, and Helical Tomotherapy (HT). Aim: To compare the dosimetry parameters on SRS among IMRT-SS, VMAT, and HT techniques on Vestibular ScwhanomaMethod: Treatment planning with IMRT-SS, VMAT, and HT on eleven CT plan data for schwannoma vestibular cases. The marginal dose is 12 Gy with single fraction. Results: Mean tumor size was 8.23 cm + 5.08 cm3. No significant difference were found in the mean CI, GI, V100%, and V50% among three techniques. There was no significant difference in maximal dose to brainstem, ipsilateral cochlea, chiasma opticum, ipsilateral and contralateral optic nerve between the three techniques. There was significant difference of maximum dose on contralateral cochlea between IMRT-SS and VMAT techniques. The longest beam-on time was obtained with HT technique (1209,18 +390,20 second), followed by IMRT-SS technique (665,05 + 73,40 second), and the shortest was with VMAT technique (362,87 + 24,55 second). There was significant difference in mean MU and beam on time between three techniques. Conclusion: VMAT technique could be an option for SRS for vestibular schwannoma cases to provide conformity and gradient index as well as IMRT-SS and HT techniques, with better sparing to contralateral cochlea compared with IMRT-SS technique, and provides shorter beam-on time rather than IMRT-SS and HT.

Abstract

Plasma cell neoplasm is a clonal plasma cell malignancy which is the result of B cell differentiation with special characteristics the presence of immunoglobulin secretion. The clinical presentation varies from monoclonal gammopathy to plasma cell leukemia. Only five to six percent of plasma cell neoplasms manifest as plasmacytoma.Solitary extramedullary plasmacytoma (EMP) in the vulva region is a rare case. The current study presents a recurrence case of EMP in left labia minora-vulva in 61-years-old patients who have undergone surgery with a complete response. Radiotherapy was given with a total dose of External Beam Radiotherapy (EBRT) 50.4 Gy and continued with brachytherapy cylinder intravagina five Gy in three fractions. Six months after complete radiation, there was no recurrence, metastasis or progression at follow-up.

Abstract

Background: The availability of enteral nutrition is one of nutritional intervention that can be given to prevent a decrease in nutritional intake and nutritional status in HNC patient. Research objective: This study aims to see the correlation between the availability of enteral nutrition with nutritional fulfillment and nutritional status. Methodology: A cross sectional study was conducted on adult subjects with HNC after radiation therapy at the RSCM radiotherapy outpatient clinic. Nutritional fulfillment was assessed by semi-quantitative FFQ while nutritional status was measured by calculating body mass index (BMI). Research results: Forty-one subjects mean age of 51 years participated in the study. The mean BMI of subjects with enteral nutrition was lower than those on oral nutrition, 18,2±2,6 kg/m2 compared to 21,2±3,5 kg/m2 respectively. The mean total energy intake of subjects with enteral nutrition route was higher, which was 1498,1±430,6 Kcal/day compared to 1291,4±393,3 Kcal/day. There was a moderate negative correlation between the availability of enteral nutrition and nutritional status (r=-0,346, p=0,027), meanwhile there was a weak positive correlation with nutritional fulfillment (r=0,216, p=0,174). However, in this study we found that the proportion of subjects with enteral nutrition who experienced a decrease in BMI was less than the proportion of subjects on the oral route, which was 22,2% compared to 43,8% respectively. Conclusion: There is a moderate negative correlation between the availability of enteral nutrition with nutritional status and a weak positive correlation with nutritional fulfillment which is still influenced by confounding factors.

Abstract

Background: Globally, hepatocellular carcinoma (HCC) is the most prevalent kind of primary liver cancer. Treatment options for HCC include radiotherapy, immunotherapy, surgery, targeted therapy, transarterial chemoembolization, and hepatic arterial infusion chemotherapy. Limited efficacy was observed since 70-80% of cases are diagnosed late and unresectable. Growing evidence reported that SBRT is a viable option for locoregional treatments like radiofrequency ablation (RFA) or transarterial chemoembolization (TACE) for patients who did not respond to those treatments. Methods: A thorough search of electronic databases including PubMed, Cochrane Library, and Embase was conducted for studies on Stereotactic Body Radiation Therapy for Hepatocellular Carcinoma published in English over the past 10 years. The review process followed Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines to ensure a systematic approach. Inclusion criteria encompassed studies focusing on SBRT use in HCC treatment, with extracted data synthesized to provide a comprehensive overview of current evidence. Any discrepancies during the review were resolved through consultation with a third reviewer when necessary. Results: In comparison to conventional radiation therapy, Stereotactic Body Radiation Therapy (SBRT) offers precise administration of the high dose of radiotherapy, given in fewer fractions, and may be used in conjunction with other therapy modalities. Global guidelines, including in Indonesia, have proposed the implementation of SBRT. Local control was reportedly achieved in around 90–95% of HCC patients. Multimodal therapies, combined with TACE, showed superior results regarding the local control and overall survival. SBRT may eventually become the definitive treatment for early-stage HCC patients and has a critical role as a transitional therapy for patients awaiting liver transplantation. Improved outcomes and quality of life were also observed in patients with portal vein thrombosis (PVT) and extrahepatic metastases who underwent SBRT. Conclusions: SBRT results in promising local control, raises overall survival, and improves the quality of life in HCC patients with various stages.
