Abstract

The completion of disease treatment at the health center was important for payment of Capitation Based on Fulfillment of Service Commitment (PCFSC/KBPKP) and the Non-Specialist Case Outpatient Referral (NSOR/RRNS) was one of the key indicators. This study aimed to assess public health center readiness in the completion of disease treatment for those stipulated in Ministerial Decree number 514 of the year 2015. This study used Health Facility Research (Rifaskes) data conducted in 2019 with a total of 9831 health centers as observations. Analysis was conducted by comparing the availability of equipment, laboratories, and medicines with existing regulations. Health center readiness in completion of disease treatment differs according to geographical aspects, accreditation status, and financial management. Health centers located in Java, accredited, and holding Regional Public Service Agency (BLUD) financial management had better readiness for disease treatment. Health center readiness for disease treatment still experiencing problems in terms of the availability and adequacy of equipment, supporting examinations, and medicines. Therefore, it is recommended to synchronize and strengthen regulations for determining the type of disease that must be completely treated at the health center; review the type of disease that can be treated; and complete the minimum requirements of equipment, laboratories, and medicines.

Abstract

Background:Anemia is a prevalent hematological complication among individuals with Human Immunodeficiency Virus (HIV), contributing significantly to increased morbidity and mortality. This study investigates the intricate relationships between antiretroviral therapy (ART), opportunistic infections (OIs), iron metabolism, and anemia in HIV-positive patients in Indonesia. Objective:To elucidate the multifactorial associations among ART regimens, opportunistic infections, iron biochemical parameters, and anemia development in individuals living with HIV. Method:A cross-sectional epidemiological study was conducted at Sulianti Saroso Infectious Disease Hospital, Indonesia, from July to November 2023. The study involved 300 adult HIV-positive patients undergoing ART, with exclusion criteria encompassing pregnant or postpartum individuals, those with recent blood transfusions, and patients with chronic kidney diseases. Data collection included anemia status, ART regimen details, history of opportunistic infections, and iron metabolic profiles (serum iron, total iron-binding capacity [TIBC], and ferritin levels). These were obtained through systematic medical record reviews and laboratory diagnostics. Descriptive statistics, bivariate, and multivariate analyses were employed to explore predictive factors for anemia. Results:The study identified an anemia prevalence of 24.3%, with significantly higher rates among female patients (p = 0.004) and older age groups (p = 0.031). Multivariate regression analysis highlighted critical predictors of anemia: second-line ART (adjusted odds ratio [AOR]: 2.536, p = 0.028), serum iron concentrations (AOR: 0.984, p < 0.001), ferritin levels (AOR: 1.002, p = 0.023), and absolute lymphocyte count (AOR: 0.516, p = 0.003). Conclusion:The findings emphasize the necessity of longitudinal monitoring of hematologic and biochemical parameters to enhance anemia management in HIV-infected populations. The complex interplay between ART regimens, immune status, and iron metabolism underscores the importance of tailored, patient-centered clinical interventions.  Keywords:anemia; HIV; antiretroviral therapy; opportunistic infections; iron metabolism

Abstract

Drug-resistant tuberculosis (DR-TB) presents a significant challenge to global TB control efforts, with medication non-adherence identified as a major contributing factor. This study aimed to investigate the influence of medication non-adherence on the occurrence of DR-TB in Serang City, Indonesia. Utilizing an observational analytic study with a case-control design, the research included 76 TB patients, comprising 38 DR-TB cases and 38 drug-sensitive TB controls. Data were collected from secondary sources and validated questionnaires and analyzed using univariate and bivariate methods, including Chi-square tests and Odds Ratios (OR). The findings revealed a significant relationship between medication non-adherence and DR-TB, with an OR of 11.56, indicating that non-adherent patients are 11.5 times more likely to develop DR-TB compared to adherent patients. However, demographic factors such as age, occupation, and education did not significantly influence the incidence of DR-TB. The study concludes that medication non-adherence is a critical risk factor for the development of DR-TB, underscoring the need for targeted interventions to improve treatment adherence, particularly through educational campaigns and community-based support. Health authorities are encouraged to implement intensive educational programs focused on the risks of non-adherence and ensure that TB treatment is accessible and supported by community health workers. Policymakers should allocate sufficient resources to these initiatives to effectively reduce the incidence of DR-TB.

Abstract

Promotive, preventive, curative, and rehabilitative efforts that are comprehensive, integrated, and sustainable are employed to enhance the health state of the global population. Within this context, however, the quality of primary health care depends on job satisfaction, which leads to the happiness of human resources in the health sector. Therefore, this study aimed to analyze and formulate a job satisfaction model among primary health care midwives in Indonesia. This study was an advanced secondary data analysis of a cross-sectional study conducted in 2017 by the National Institute of Health Research and Development, Ministry of Health of the Republic of Indonesia. A total of 87,341 midwives from all 9,669 primary health cares in Indonesia participated in this study. Data were collected by distributing the Minnesota Satisfaction Questionnaire, elaborating on the satisfaction level and relevant contributing factors. The prefilled Likert scale questionnaire was analyzed using logistic regression. The findings suggested a model indicating that motivation, work area (region), history of salary delay, and training received were important for their job satisfaction, whereas the motivation aspect contributed the most. Therefore, the local and central governments must consider these factors in the human resource policymaking process.
