Abstract

Examinations in radiology installations have increased annually, resulting in a higher risk of radiation hazards to staff, particularly in interventional, conventional X-ray, and CT-Scan environments. Furthermore, not all hospitals are equipped with radiation survey meter monitoring devices. Therefore, an innovative radiation exposure monitoring system based on the Internet of Things (IoT) was developed. This study aims to: (1) develop an IoT-based radiation exposure monitoring system, (2) evaluate the device’s performance, and (3) assess the effectiveness of the system. The research method used was research and development (R&D), which included preliminary information gathering, product planning and development, expert validation, device performance testing, effectiveness testing, and final outcomes. This system was developed for real-time radiation dose monitoring.  The product development involved a Geiger-Muller M4011, an ESP32 microcontroller, and a Node-Red web application. Performance testing of the IoT-based radiation exposure monitoring device and a standard survey meter in CT-Scan and conventional X-ray rooms used the T-Test and Mann-Whitney tests. The results showed p-values > 0.05, indicating no significant difference between the two devices. Effectiveness testing through pre-test and post-test questionnaires demonstrated improved usability—namely flexibility, operability, learnability, and understandability—with an N-Gain value of 78.95%, indicating effectiveness. In conclusion, the IoT-based radiation exposure monitoring system proved effective and feasible for real-time radiation dose monitoring in radiology installations, offering advantages in IoT-based data integration, ease of access, and structured data recording. In addition to performance testing, the system was evaluated in terms of its integration with hospital infrastructure. It was designed to provide real-time radiation dose monitoring, automatically updating the dose data and sending alerts when safe exposure levels are exceeded. The system also enables remote monitoring, making it easier for radiology departments to oversee radiation safety without the need for direct physical checks.

Abstract

Background: Multislice CT Scan (MSCT) brain examination in pediatric cases should pay attention to the lowest possible dose but still provide optimal anatomical information. Pediatric has a higher risk of developing cancer compared to adults who receive the same dose. The purpose of this study is to obtain the optimal value of the use of the Adaptive Statistical Iterative Reconstruction (ASIR)-V technique with variations in tube current strength and Iterative Reconstruction value which is implemented in pediatric patients under the age of 5 (five) years. Methods: This research was experimental with brain image samples of pediatric patients at the Radiology Installation of RSUD dr. Moewardi Surakarta. The variations of the tube currents selected were 80, 100, and 165 mA with a combination of AR20, AR40, AR60, and AR80 variations. Anatomical information processed descriptively sourced from the assessment of 3 (three) radiologists who were experienced in assessing MSCT images. Meanwhile, the effect of tube current variation on radiation dose was tested using linear regression. Results: The results showed differences in anatomical information between the combinations of 165 mA-AR60 and 165 mA-AR80 "very clear", 165 mA-AR20 "clear", 80 mA-AR20 "fairly clear" and 100 mA-AR80 "less clear". There was an effect of strong variation of tube current on radiation dose with a regression coefficient value of 0.690. The estimated effective doses of the three tube current values were still below the national Dose Reference Level (DRL). Conclusions: It was concluded that there were differences in anatomical information on the MSCT brain pediatric examination with variations in tube current and ASiR-V, there was an effect of variations in mA on radiation dose and mA values and the optimal value of the combination of techniques studied is 80 mA and 20% Iterative Reconstruction.

Abstract

Background: MRCP examination using natural manganese contrast media cannot provide optimal images of gastroduodenal suppression covering the biliary system when compared with standard contrast media. Various studies on manganese contrast media have been developed, but not much has been linked to the use of natural manganese nanoparticles as an alternative contrast media. This study aims to prove that manganese nanoparticles extracted from oolong tea as an alternative oral negative contrast medium for MRCP examinations are thought to improve image quality and do not cause neurotoxic effects in healthy volunteers. Methods: The research design carried out was experimental research. The research included making extracts and making oolong tea nanoparticles, and each expired or unexpired ingredient was observed using organoleptic tests in terms of taste, color, texture, and aroma, at 30 days, 90 days, and 120 days. Results: This research shows that Mn nanoparticle oral negative contrast media for oolong tea experiences significant changes in taste, color, texture, and aroma with storage time. Oolong tea stored for 60 days has the best characteristics with a fresh taste, bright yellow color, and smooth texture. After 90 days, the taste starts to become sour and bitter, with a grainy texture. 120 days of storage produced tea with a very sour and bitter taste, as well as a rough texture that the panelists didn't like. Organoleptic tests showed that panelists preferred 60-day (T1) oolong tea based on taste, color, texture, and aroma. Storage at a temperature of 4-25°C and paper sack packaging is more effective in maintaining tea quality. Conclusions: Storage of oolong tea with Mn nanoparticles shows that tea stored for 60 days has the best quality. Tea stored for 90 days begins to degrade, and after 120 days it is disliked because of its sour and bitter taste. Temperatures of 4-25°C and paper sack packaging are effective in maintaining tea quality.

Abstract

Sedation of propofol, chloral hydrate, midazolam, and ketamine is commonly used in MRI examinations. There are still risky side effects from the previous 4 sedations, so melatonin tablets are a safer option. Melatonin synthesized in tablets can still cause quite adverse side effects. Pistachios processed into milk can be used as an alternative to natural melatonin oral sedation. Research Objective: To determine the potential of pistachio milk as an alternative to oral melatonin sedation to support MRI examinations. Research Method: This research uses the R&D (Research and Development) method with 4 stages, namely finding potentials and problems, data collection, product design, and design validation. The product in the form of 200 mL pistachio milk with concentration variants of 35 g, 45 g, 55 g, 65 g was tested in a laboratory to determine its potential from the test results with a reference to melatonin tablets of 10 mg. Research Results: Laboratory tests showed the presence of melatonin and other accompanying ingredients such as phenolics, carotene, and polyphenols. The melatonin content of the 35 g, 45 g, 55 g, 65 g variants is 5.5 mg, 6.1 mg, 8.7 mg, 10.5 mg, respectively. Conclusion: Pistachio milk with a 65 g variant has a melatonin content of 10.5 mg which is close to the reference for sedation of melatonin tablets of 10 mg, so it has the potential to be an alternative to oral sedation of melatonin to support MRI examinations.

Abstract

Background: MR Spectroscopy is a radiological examination using MRS supporting software in the MRI modality that can show the correlation of metabolic or biochemical anatomical and physiological information contained in the body, both in normal and abnormal conditions[1]. MRS is done after a routine brain without contrast or contrast. Proton MR spectroscopy of brain tissue shows the spectral of several metabolites[2]. Some of the main metabolites in brain tissue detected by MRS are NAA, Cholin, Creatinien, and Lipid, Lactate and Myo Inositol. MRS results are in the form of a graph showing the ratio of NAA levels per cholin to the patient's body metabolites and a graph on normal metabolites forming Hunter's angle[3].Methods: This method is a qualitative research with a descriptive approach using comprehensive literatures studies. Results: MRI and MR Spectroscopy examinations in cases of brain disorders will strengthen the diagnosis by looking at changes in the pattern of metabolites in the brain. Some characteristic spectrum patterns can be used as markers of certain diseases, so they can replace biopsy. MR Spectroscopy examination showed that online game addicts had lower levels of NAA concentrations than normal people in the right frontal cortex and lower levels of choline (Cho) in the medial temporal cortex.[4].Conclusion: MR Spectroscopy does not play a role in replacing MRI imaging, but as an addition to non-invasive metabolic information that has a high degree of accuracy for diagnosing abnormalities in brain regions by evaluating spectrum patterns or metabolite ratios.
