Abstract

Changes in lifestyle that influence food consumption and physical activity pattern tend to increase the prevalence of childhood obesity in Indonesia. The purpose of this study is to evaluate the relationship of dietary and physical activity pattern with the development of primary obesity in children. This study was conducted to 122 school age children in Bandung municipality from May to July 2000. Obesity status was determined by body mass index (BMI) and weight for height (W-H) classifications. Interview was done to assess the dietary pattern i.e habitual dietary pattern (HD) and 24-hour dietary record (DR) and daily physical activity pattern, i.e. physical activity score (PAS) and energy expenditure (EE). According to BMI classification, there are 81 obese and 41 non-obese children. While by W-H classification, the number of obese and non-obese children is 47 and 75. The study shows that obese children consume more calorie than non obese, and significant difference found in calorie intake based on DR between non obese and obese according to BMI (p=0.026) and W-H (p=0.001). Macro nutrient intake based on HD and RD does not differ significantly between obese and non obese children according to BMI and W-H (p>0.05). Based on HD and BMI degree of obesity, fat is the most in-fluencing macronutrient (F=3.392;p=0.006) but according to W-H, calorie has more effects (F=2.557;p=0.042). While by 24-hour DR in the BMI as well as W-H classifications carbohydrate intake was the most related macronutrient. No significant difference was found in micro nutrient intake except for calcium intake by DR according to W-H classification (p=0.043). Abstract

Tindakan suction endotrachealpada pasien cedera kepala berat dapat menyebabkan terjadinya oxygendesaturationarteri yang berakibat pada peningkatan intracranial pressure, pembengkakan otak dan hipoksemiasistemikbahkan dapat menyebabkan kematian. Penelitian ini bertujuan untuk mengevaluasi pengaruh suplai oksigen melalui catheter mouthsaat suction terhadap saturasi oksigen pada pasien cedera kepala yang terpasang ventilator yang dirawat di ruang perawatan intensif. Penelitian ini menggunakan desain penelitian desain kuasi eksperimen pendekatan pre dan post test dengan kelompok kontrol dan perlakuan. Jumlah sampel pada penelitian ini adalah 40 responden yang dibagi menjadi kelompok perlakuan sebanyak 20 responden yang dipasang catheter mouthdan kelompok kontrol sebanyak 20 responden tanpa menggunakan catheter mouth. Teknik sampel menggunakan accidental sampling. Penurunan saturasi oksigen nilai thitung (-16,538) < ttabel (-2,024) dan nilai p-value(0.000) < 0.05. Saturasi oksigen saat suctionpada kelompok kontrol dengan kelompok perlakuan adalah memang berbeda secara nyata. Hasil penelitian ini dapat dijadikan evidence basedbagi perawat di ruang intensif untuk meningkatkan pelayanan perawatan dalam melakukan tindakan suctionuntuk mengurangi resiko terjadinya penurunan saturasi oksigen pada pasien cedera kepala berat yang terpasang ventilator. Kata kunci : catheter mouth, suction,saturasi oksigen AbstractThe intervention of endotracheal suction to patients with the severe head injury may have effected to oxygen desaturation in arteries. The oxygen desaturation process could be impacted to increase the intracranial pressure, brain’s swelling, hypoxemia systemic, and deaths. This study aimed to evaluate the influence of the oxygen supply via oral catheterization in the suction process to the oxygen saturation level in the patient with the severe head injury who using ventilator. This research has been conducted in the intensive care unit. This study was applied the quasi experiment design using pre and post-test. Samples were chosen using accidental sampling. There were two groups of samples, the first group was the intervention group with 20 respondents using oral catheterization, and the second group was the control group with 20 respondents. The result showed that the oxygen saturation was reduced with t value (-16.538) < t table (-2.024), and p value (0.000) < 0.05. The oxygen saturation was significantly different between two groups. This study could be an evidence that nurses should enhance their services to reduce the risk of decreasing the oxygen saturation in the severe head injury patient who using ventilator.Key words:Catheter Mouth, oxygen saturation, suction

Abstract

Masa remaja adalah masa transisi dengan kecenderungan munculnya perilaku yang menyimpang, salah satunyaadalah seks pra-nikah. Perilaku remaja dipengaruhi oleh lingkungan salah satunya adalah pola asuh orang tua. Penelitianini bertujuan untuk menganalisis pola asuh orang tua,jenis kelamin dan asal sekolah yang berhubungan dengan persepsiremaja tentang perilaku seks pra-nikah.Metode dalam penelitian ini adalah studi survey (cross sectional) dengan tehnik pengambilan sampel multi stagerandom sampling. Subjek penelitiannya adalah siswa-siswi pelajar SMA kelas XI dengan sampel berjumlah 200 orang.Data dianalisis dengan chi kuadrat serta regesi logistik ganda.Hasil penelitian ini adalah pola asuh orang tua mempunyai nilai P= 0,001. Jenis kelamin p= 0,001 dan asalsekolah p=0,008 sehingga ketiga variabel tersebut mempunyai hubungan dengan persepsi remaja tentang perilaku seksualpra-nikah. Hasil analisis multivaribel menunjukkan Asal sekolah swasta berisiko untuk terjadinya persepsi buruk sebesar3,118 kali bila dibandingkan dengan asal sekolah negeri. Untuk jenis kelamin perempuan dengan rasio prevalens dibawah 1,ini menunjukkan semakin rendah risiko untuk memiliki persepsi buruk. Selanjutnya, untuk pola asuh makin kearah permisifsemakin tinggi risiko untuk memiliki persepsi seks pra-nikah yang buruk. Dengan demikian diperlukan orang tua yangmemberikan pola asuh yang bertanggung jawab yaitu pola asuh demokratis dengan memperhatikan karakteristik anak.Simpulan dari penelitian ini adalah pola asuh orang tua, jenis kelamin dan asal sekolah mempunyai hubungandengan persepsi remaja tentang perilaku seksual pra-nikah.

Abstract

Background: The immune response of thalassemia patients is different from usual; therefore, thalassemia patients are susceptible to infection. A study at Thalassemia Center in Cipto Mangunkusumo Hospital showed that all thalassemia patients experience zinc deficiency. The decreased cellular immune response is associated with zinc deficiency, whereas splenectomy exacerbates the condition. This study aims to evaluate the improvement of cellular immune response in splenectomized thalassemia major patients after zinc supplementation. Method: Randomized double-blinded controlled trial was conducted on splenectomized thalassemia major patients in 12 weeks period. The inclusion criteria were aged > 12 years and had negative HIV test results. The subject receiving corticosteroids were excluded. Fifty-six subjects were randomly divided into two groups, the zinc group and the placebo group. Between 2 groups, data on zinc serum, T lymphocyte count, CD4+ T lymphocyte count, CD8+ T lymphocyte count, and CD4+/CD8+ ratio were evaluated at the beginning and the end of the study and were analyzed with unpaired t-test, Mann Whitney test, and Wilcoxon Signed Rank test. Result: After zinc supplementation, only 18 of 28 subjects in the zinc group recovered normal zinc serum levels. There were no significant changes after zinc supplementation in all parameters of cellular immune response (p > 0.05) between the two groups. This might be due to the subjects' adherence, which was lower in the zinc group (75.82%) than in the placebo group (83.19%). Conclusion: The effect of zinc supplementation on cellular immune response in splenectomized thalassemia major patients had not been proven yet.
