Abstract

Seiring dengan berjalannya waktu, jumlah kunjungan pasien ke UGD Rumah Sakit Bali Royal semakin meningkat. Tujuan penelitian ini adalah untuk mengetahui beban kerja perawat dan kebutuhan perawat di UGD Rumah Sakit Bali Royal. Penelitian ini merupakan penelitian deskriptif kuantitatif. Pengamatan dilakukan oleh peneliti untuk mengetahui waktu yang dihabiskan oleh perawat dalam melayani pasien dengan menggunakan metode Time Motion Study. Hasil penelitian menunjukkan beban kerja untuk pasien gawat darurat sebesar 63,4 menit per hari (1,1 jam per hari), pasien gawat tidak darurat 1.585,5 menit per hari (26,4 jam per hari), dan pasien darurat tidak gawat 2.187,7 menit per hari (36,5 jam per hari) dan kebutuhan tenaga perawat di UGD Rumah Sakit Bali Royal sebesar 17 orang. Diperlukan penambahan tenaga perawat pelaksana sebanyak 3 orang. Meanwhile, patients that admitted to Emergency Unit of Bali Royal Hospital is getting more. The objective of this study was to determine the workload of nurses and nurses needs in the Emergency Unit of Bali Royal Hospital. This study was a descriptive quantitative study. Observations were made by the researcher to determine the time spent by nurses in serving patients by using the Time Motion Study. The results shown the workload for critically emergency patients was 63.4 minutes per day (1.1 hours per day), critically non emergency patients was 1,585.5 minutes per day (26.4 hours per day), and 2,187 minutes per day (36.5 hours per day) for non critical-but emergency patients and the need for nurses in the Emergency Unit of Bali Royal Hospital was 17 people. Nurses needed additional power as much as 3 people.

Abstract

Berdasarkan data statistik, jumlah peserta JKN sudah mencapai 96% dibandingkan dengan jumlah penduduk Indonesia di tahun 2024. Sekitar 64% rumah sakit swasta di Indonesia sudah bekerja sama dengan BPJS Kesehatan di tahun 2024. Dari banyak penelitian yang telah dilakukan ditemukan masih banyak pelayanan farmasi yang masih belum memenuhi standar pelayanan minimal yang sudah ditetapkan. Tujuan dari penelitian ini adalah untuk mengetahui faktor yang memengaruhi waktu tunggu pelayanan resep rawat jalan pasien BPJS di instalasi farmasi rumah sakit swasta. Penelitian ini menggunakan studi tinjauan literatur. Penelusuran jurnal diperoleh melalui google scholar dan researchgate mulai tahun 2021 sampai tahun 2024. Terdapat 7 artikel yang memenuhi kriteria inklusi dan eksklusi yang telah ditentukan. Berdasarkan hasil literature review yang telah dilakukan didapatkan bahwa 3 artikel sudah memenuhi standar pelayanan minimal rumah sakit dan 4 artikel belum memenuhi standar pelayanan minimal rumah sakit. Faktor-faktor yang memengaruhi belum terpenuhinya standar pelayanan minimal rumah sakit di antaranya adalah peresepan masih dilakukan secara manual, pengaturan jadwal poliklinik spesialis secara bersamaan sehingga berdampak jumlah resep yang masuk ke Instalasi Farmasi, banyaknya jumlah resep pasien BPJS Prolanis dan belum terpenuhinya jumlah SDM sesuai dengan kebutuhan pelayanan di Instalasi Farmasi. Kurangnya sumber daya manusia (SDM) yang kompeten di bidang farmasi juga menjadi kendala. Kondisi ini tidak hanya berdampak pada kepuasan pasien, tetapi juga dapat meningkatkan risiko terjadinya kesalahan pengobatan.

Abstract

Bauran komunikasi pemasaran rumah sakit menjadi satu hal yang vital pada rumah sakit di Indonesia terutama di era pandemi Covid19 yang terjadi sejak tahun 2020. RS Hermina Karawang merupakan salah satu dari empat rumah sakit yang ditunjuk Pemerintah Kabupaten Karawang untuk menjadi rumah sakit rujukan Covid19 di Kabupaten Karawang, Jawa Barat. Tujuan penelitian ini untuk mengetahui implementasi elemen bauran promosi/komunikasi pemasaran yang digunakan oleh RS Hermina Karawang di era Covid19. Penelitian ini metode kualitatif dengan menggunakan observasi dan wawancara terstruktur. Didapatkan hasil penelitian bahwa RS Hermina Karawang telah menerapkan seluruh bauran komunikasi promosi pemasaran walaupun ada beberapa yang belum efektif, yaitu periklanan, promosi penjualan, hubungan masyarakat dan publisitas, pemasaran langsung, pemasaran interaktif, pemasaran dari mulut ke mulut dan penjualan personal. Selama pandemi berlangsung hubungan masyarakat belum maksimal dilakukan sehingga evaluasi secara spesifik perlu dilakukan untuk mencegah masalah/kendala terjadi berulang.

Abstract

Immunization is one of the most cost-effective public health interventions, averting an estimated 2 to 3 million deaths every year. Before the coronavirus pandemic, more than nearly 14 million children didn't receive any vaccines, and 19 million children below the age did not receive the recommended vaccines in 2019. COVID 19 is leaving many of the world's most marginalized children without access to immunization services. Some are from Indonesia, India, and Pakistan, including ten countries with the highest number of unimmunized and partially immunized children. The study aims to find out the differences in the policy of routine immunization programs during the COVID-19 Pandemic in Indonesia, India, and Pakistan in terms of policy actors, policy context, policy process, and policy content. The method used is a literature review from sources such as books, journals, and relevant government regulations and policies related to routine immunization programs during the COVID-19 Pandemic. Literature study shows that the three countries have some similarities in the policy actors, policy context, and policy process. The only difference is the policy content. The policy in the form of technical guidance by the Indonesian government is more detailed, comprehensive, and structured than the other two countries.

Abstract

According to the Minister of Health Regulation no. 33/2015 (Permenkes 33) on Guidelines for Composing the Human Resources in the Health Field (HRH) Planning, planning for HRH must be done from the bottom up. In 2016, Depok City has implemented the regulation, but the results were not applicable. This qualitative study analyzes the process of creating the planning document at Depok City in 2017 through in-depth interviews and documentary research. We discovered that the 2017 HRH planning document in Depok City did not completely follow all the steps stated in Permenkes 33. Ineffective communication caused lack of commitment and support from the stakeholders, which in turn, caused the lack of support from the bureaucratic structure, such as standard operational procedures, regulation, and allocation of resources, whether it was manpower, funding, or facilities. Therefore, we recommend that the socialization or advocation process to the stakeholders to be increased.

Abstract

Indonesia has entered an aging population as showed by the number of elderly population which has reached more than 7% of the population. Basically, the increase in the elderly population will also increase the number of illnesses, because the more a person ages, the physical condition will decrease and be more susceptible to disease. This research is a quantitative study that aims to determine the determinants of outpatient health service use for Indonesian elderly in 2017. This research used secondary data from the 2017 National Socio-Economic Survey (Susenas) results with the unit of analysis is people aged ≥ 60 years who had health complaints. The analytical method used is logit regression analysis. The results showed that gender, education, employment status, residential area, health insurance ownership, economic status, and disruption of activity due to illness had a p-value of 0,000 < 0,05 which meansthat it was significantly associated with the utilization of outpatient health services. The most dominant variable is the disruption of activity due to illness. The government can issue policies related to efforts to register each elderly person into health insurance through optimizing a healthy Indonesian program with a family approach. The government is expected to be more assertive in monitoring and evaluating elderly health services by considering factors that support the elderly in utilizing health services to achieve healthy aging and active aging.

Abstract

Data and information on the health profile of Indonesia in 2016 showed only 29.5% of infants receive exclusive breastfeeding until 6 months, the low level of exclusive breastfeeding made the government issue a regulation on exclusive breastfeeding in Government Regulation Number 33 of 2012. The study aimed to analyze the policy implementation of exclusive breastfeeding at Cicalengka Community Health Center. The study used qualitative analysis with in-depth interview method, focus group discussion and literature study. The result of this study shows that breastfeeding policy implementation in health centers is not optimal, as seen from the low coverage of exclusive breastfeeding. Socialization of the policy has not been done as a whole, the time and task division is unclear and has no special budget and the standard operating procedures is not really used in carrying out the policy. Communication is the most influential factor in the implementation of the policy. There is no support and commitment from all employees in the implementation of exclusive breastfeeding policy. The suggestion is to consistently socialize to employees and the public, carry out supervision in an effort to secure the policy, run the Standard Operating Procedure (SOP), allocate budget activities in 2018, create a memorandum of understanding with other agencies, and Self-assessment and program evaluation absolutely must do continuously.

Abstract

The continuity of the participation in Prolanis is one of the primary health care services indicators of commitment, so finding out the level of adherence and the factors influencing it is very important. The aim of the study is to investigate the adherence level of Prolanis participanst and its determinants in type 2 diabetes patients at five BPJS primary health care in Bekasi. This is a cross sectional study, using questionnare to 220 participants with type 2 diabetes by simple random sampling method proportionally. Adherence level of the participants is 3.59 out of 6 times. Length of illness, perceived benefits, perceived barriers and the implementation of the program guidelines are correlated with the adherence level and the dominant factor is perceived barriers. Factors in individual and provider can be used as evaluation tools for the primary health care in facilitating the participants’ needs so that it would increase the level of adherence.

Abstract

Rekam Medis Elektronik (RME) telah menjadi bagian penting dalam layanan kesehatan modern, namun fragmentasi data dan kurangnya interoperabilitas antar sistem masih menjadi tantangan utama. Fragmentasi ini dapat menghambat efisiensi pelayanan dan meningkatkan risiko kesalahan dalam perawatan pasien. Penelitian ini bertujuan untuk menilai dampak penerapan interoperabilitas RME terhadap peningkatan layanan rumah sakit, terutama dalam hal efisiensi dan keselamatan pasien. Penelitian ini menggunakan desain tinjauan sistematis berdasarkan pedoman PRISMA. Pencarian literatur dilakukan di database ProQuest, PubMed, dan SpringerLink dengan kata kunci "Rekam Medis Elektronik", "Interoperabilitas", dan "Pertukaran Informasi Kesehatan". Jurnal yang diterbitkan dalam lima tahun terakhir dan berkaitan dengan layanan rumah sakit dipilih sebagai sampel. Data dianalisis secara deskriptif dengan metode naratif.  Dari 370 artikel yang diidentifikasi, tujuh artikel memenuhi kriteria inklusi dan dianalisis lebih lanjut. Hasil penelitian menunjukkan bahwa penerapan interoperabilitas RME dapat meningkatkan efisiensi dalam transfer pasien, mengurangi kesalahan transkripsi data, serta memperluas akses informasi pasien secara real-time di berbagai fasilitas kesehatan. Selain itu, interoperabilitas RME berkontribusi pada peningkatan koordinasi perawatan antar penyedia layanan. Namun, beberapa penelitian juga mencatat adanya potensi risiko terhadap keamanan pasien, terutama jika sistem RME tidak diterapkan dan dikelola dengan benar. Penerapan interoperabilitas RME memberikan dampak positif pada peningkatan kualitas layanan rumah sakit, terutama dalam hal efisiensi dan keamanan pasien. Meskipun demikian, tantangan terkait dengan pelatihan tenaga kesehatan dan keamanan data masih perlu diatasi untuk memaksimalkan manfaat RME.

Abstract

Latar belakang: Remunerasi dokter adalah pemberian apresiasi berupa imbalan atas kontribusi dan kinerja dokter di rumah sakit. Remunerasi dokter di Indonesia sebagian besar menggunakan skema retrospektif yaitu fee for service. Terdapat kesenjangan antara fee for service dengan tarif paket Indonesia Case Based Group (INA-CBG) pada program Jaminan Kesehatan Nasional (JKN) sehingga menimbulkan ketidakpuasan dokter. Belum ada acuan standar pemberian remunerasi dokter dalam pelayanan pasien JKN. Tujuan: mengetahui skema remunerasi dan imbalan jasa yang diberikan bagi dokter di rumah sakit dalam pelaksanaan asuransi kesehatan dan dampaknya terhadap pelayanan. Metode: Penelitian ini melakukan tinjauan terhadap literatur terkait skema remunerasi dengan melakukan penelusuran pada database PubMed, Scopus dan Sciencedirect dan EBSCO dengan menggunakan kata kunci physician, remuneration, salary, wage, incentive dan health insurance. Metode PRISMA digunakan untuk mendapatkan jurnal berbahasa inggris dan full open access sesuai kriteria inklusi penelitian di rumah sakit dan pembiayaan melalui asuransi kesehatan pada 10 (sepuluh) tahun terakhir. Hasil: Penelusuran menghasilkan 1.387 artikel sesuai kriteria pencarian dengan 11 artikel memenuhi kriteria inklusi dan dilakukan sintesis data. Skema yang digunakan adalah fee for service, activity based, value based payment, dan pay for performance. Skema pay for performance terbanyak digunakan dan dapat meningkatkan produktivitas dokter tanpa meningkatkan biaya pelayanan kesehatan. Evaluasi kebijakan skema remunerasi dokter di rumah sakit dapat dilakukan seiring kebijakan pembiayaan kesehatan untuk meningkatkan kualitas dan efisiensi pelayanan Kesimpulan: Terdapat berbagai skema remunerasi dokter di rumah sakit dalam memberikan pelayanan pada jaminan kesehatan. Pemilihan skema yang digunakan dapat dari hasil evaluasi untuk meningkatkan efektifitas dan efisiensi pelayanan asuransi kesehatan.
