Abstract

Objective: To study the comparison of encouragement, stimuli, orgasm, pain and satisfaction of female sexual function at 3 months postpartum between spontaneous delivery and cesarean section in Dr. Cipto Mangunkusumo Hospital in Jakarta. Method: This was an observational research, sexual function was measured at three months post-delivery with Female Sexual Function Index (FSFI) questionnaire. The study design used was cross sectional with consecutive sampling. Analysis for comparative nonpaired categorical variables was done using Chi square or Fisher analysis. Analysis for confounding variables was carried out using multivariate logistic regression. Result: From 150 respondents, 43.3% had sexual dysfunction, with 52% of the spontaneous labor group and 34% of the cesarean section group. Bivariate analysis showed that occurrence of sexual dysfunction at three months post-spontaneous labor was 1.5 times higher (95% CI 1.02-3.19) compared with cesarean section. Sexual encouragement shows a two-fold difference (95% CI 1.17-3.40) compared to cesarean section. However, orgasm disturbance was 8 times higher (95% CI 1.90-3.58) in the spontaneous labor group, with confounding variable of perineal rupture. Disturbance of sexual stimuli, satisfaction, and pain were not significantly different between spontaneous labor and cesarean section. Multivariate analysis found that spontaneous labor was statistically significant for sexual dysfunction at three months post-delivery in patients with sexual encouragement (RR=2.716, p=0.008) and orgasm accession dysfunction (RR= 6.952, p=0.031). However, the more than 30 years old of age variable was statistically significant in sexual dysfunction variable with RR= 2.60 and p=0.021. Conclusion: Spontaneous labor is statistically significant for sexual dysfunction at three months post-delivery, especially for sexual encouragement and orgasm accession. Meanwhile, the variables with the age of 30 years old or older of age were influential on sexual dysfunction, especially to the sexual stimuli variable. Abstract

Objective: To know the prevalence of postpartum anxiety and depression in patients who received intrapartum oxytocin. Method: Across-sectional observational study was conducted in Dr. Cipto Mangunkusumo hospital. Observation used Edinburgh Postnatal Depression scale and Beck Anxiety Inventory before delivery day, on the first and fourteenth day of postpartum. Result: Of 112 patients, we found the prevalence of mild and moderate anxiety were 94.6% and 5.4%. There were no anxiety women before delivery and on the first day of postpartum. The prevalence of mild and moderate anxiety on the fourteenth day of postpartum were 83% and 16.9%. There was no severe anxiety found at those time. On the other hand, the prevalence of postpartum depression on the first and fourteenth day of postpartum were 31.3% and 32.1%. There was no association between exogenous oxytocin and postpartum anxiety also depression. In multivariate analysis, we found that women with low self image were more prone to postpartum anxiety (OR 0.16, 95% CI 0.06-0.46). Meanwhile, postpartum depression was associated significantly to self image (OR 0.17, 95% CI 0.07-0.83), low income (OR 10.35, 95% CI 1.72-62.45) and pregnancy plan (OR 0.17, 95% CI 0.06-0.53) Conclusion: The patients who received intrapartum oxytocin are more prevalent to have mild anxiety. The prevalence of depression before delivery day, on the first and fourteenth day of postpartum are similar. In statistic, there is no relationship between intrapartum oxytocin administration and postpartum anxiety or depression. Abstract

BACKGROUND A notable rise in sexually transmitted infection (STI) cases among youth correlates with a lack of reproductive health awareness, affecting nearly 80% of Indonesian adolescents. Additionally, sociocultural taboo acts as a challenge to reproductive health implementation in Indonesia. Therefore, this study aimed to develop an online reproductive health module focused on STIs for Indonesian adolescents. METHODS This research was a mixed methods approach, integrating quantitative study using univariate analysis (survey and questionnaire) and qualitative study using the Delphi method (open discussion and focus group discussion). The study was conducted between March 2 and July 28, 2022. We began by conceptualizing a literature-based module, an online survey, and virtual discussions involving students, teachers, and parents. Then, we consulted with clinical psychologists and dermatovenereologists. Additionally, students completed an assessment to establish a baseline of their knowledge, attitudes, and behaviors (KAB) regarding reproductive health and STIs. RESULTS Of 327 high school students across Jakarta, analysis of the 129 completed KAB surveys showed that only 56.6% and 65.1% attained acceptable scores (>65) for knowledge and behavior, respectively, with 24.8% being at risk for STIs. Key recommendations included incorporating multimedia elements, ensuring clarity and simplicity in language, adopting non-judgmental tone, providing comprehensive explanations of STIs and their long-term impact, and supporting learning with face-to-face sessions and ongoing access to concise, cost-effective educational resources. CONCLUSIONS Adolescents need a comprehensive, interactive, instructive, accessible, non-discriminatory reproductive module that strengthens the self-concept that upholds Indonesian values and norms.

Abstract

Objectives: To systematically evaluate the current evidence on the effectiveness of the telemedicine approach on maternal practices in rural communities. The author hopes that this study offers a breakthrough to draw a solid conclusion regarding the feasibility of implementing telemedicine to improve maternal health in rural areas of Indonesia. Methods: A systematic literature search was performed using international databases, such as Cochrane, PubMed, EBSCOhost, Science Direct, Scopus, and Google Scholar, published from 2012 to August 31, 2022. Risk of bias assessment was conducted using the Cochrane Risk of Bias tools 2.0 for randomized controlled trials and converted to the AHRQ standards. Results: This study included five randomized controlled trials from different countries, namely India, Ethiopia, and Kenya. Telehealth interventions, delivered using mobile applications, phone calls, or short text messaging, improved the number of ANC visits, safe childbirth in hospitals or by skilled staff, exclusive breastfeeding, and immunization compliance, with a significant difference compared to the control group that received no intervention (p<0.05). Nevertheless, considering the already widely used mobile phone technologies in rural settings, it is now clear that the telehealth approach should be scaled up and implemented in clinical settings. Conclusion: Telehealth-based intervention is a promising approach to promote better maternal health practices among pregnant mothers in rural and low-resources settings. This approach has been proved to successfully made a significant difference in terms of antenatal care visits and safe childbirth practice. Keywords: Maternal health practices, pregnant women, rural areas, systematic review, telemedicine.

Abstract

Objective: This systematic review and meta-analysis aimed to evaluate the effectiveness of laser therapy in improving urinary and sexual function in women with SUI.Methods: The literature was selected using the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) protocol. We did literature searches in several databases, including Science Direct, Pubmed, Google Scholar, Cochrane, ProQuest, and Scopus, from inception until January 2025. Quality assessment was performed using the Cochrane Risk of Bias Tool. Mean differences (MD) with 95% confidence intervals (CI) were calculated using a random-effects model.Result: There were ten journals with 598 participants that met the criteria. The results of the pooled analysis showed a statistically significant improvement in post-laser therapy pad weight (MD = -0.49; 95% CI: -0.97 to -0.01; p = 0.0435). Despite the non-significant differences and heterogeneity found in the meta-analysis of sexual function outcome (MD = -0.49; 95% CI: -0.97 to -0.01; p = 0.0435), the individual studies stated that the sexual function was improved in women with SUI post-laser therapy.Discussion: Laser therapy, particularly CO₂ and Er:YAG modalities, appears effective in enhancing sexual function in women with SUI. Variability in protocols and outcome measures among studies may contribute towards heterogeneity. Most trials had small sample sizes and short follow-ups.Conclusion: Laser therapy shows promise for improving urinary and sexual function in women with SUI, warranting further robust research. Penilaian kualitas dilakukan menggunakan Cochrane Risk of Bias Tool.
