Abstract

BackgroundEndothelial dysfunction and associated increased blood viscosity (BV) play an important role in the formation of atherosclerosis during inflammatory processes. C-reactive protein (CRP) is a biomarker of inflammation with an active role in endothelial dysfunction and development of atherosclerosis that is marked by degree of coronary stenosis (CS). The purpose of this study was to determine the relationship of BV and CRP with varying degrees of CS among coronary heart disease (CHD) patients.MethodsThis cross-sectional study involved 24 subjects taken consecutively among patients with CHD who underwent angiography. Blood viscosity levels were determined using an Ostwald viscometer, with reference limits of 1.5 to 1.72 (cP). C-reactive protein was determined by turbidimetric immunoassay, with a normal reference value of <3 mg/L. Degree of CS was examined with angiography, where 0% = no stenosis; <50% = nonsignificant stenosis; > 50% = significant stenosis. Data was analyzed using non-parametric Spearman correlation test.ResultsThere were 17 male and 7 female subjects, with mean age of 55.96 ± 7.29 years. The angiography results were: non-significant stenosis in 2 subjects (8.3%), significant stenosis in 22 subjects (91.7%) and none with normal stenosis. Statistical analysis of the relationship of BV with degree of CS found a moderate significant positive relationship (r=0.549; p=0.005). A poor significant positive relationship was found between CRP and degree of CS (r=0.481; p=0.017).ConclusionBlood viscosity increased the degree of CS in CHD patients. Therapy should target both BV-associated risk and angiographically evident stenosis.

Abstract

Patients with end-stage renal disease (ESRD) who undergo hemodialysis have chronic inflammation caused by several factors,including biocompatibility of dialyzer membran. Inflammation will decrease the fetuin A level in serum. The elevation of the markerinflammation (CRP), hyperphosphatemia and decrease of fetuin A level are the risk faktors for vascular calcification. The aim of thisstudy was to know the relationship between serum phosphate, CRP and fetuin A levels in ESRD patients who underwent hemodialysis. Theresearch was carried out by observational cross sectional analytical approach with consecutive sampling on 31 ESRD patients undergoingchronic hemodialysis in the Hemodialysis Unit of the Dr.Kariadi Hospital, by examining levels of CRP by ELISA, serum phosphate byPhosphomolibdate methods and fetuin A by ELISA. The relationships between phosphate serum and CRP, CRP and fetuin A, phosphateserum and fetuin A were analyzed using Spearman correlation test. In this study it was found that 100% of subjects had CRP levels >0.3mg/dL (reference range 0.1-0.3 mg/dL); 61.3% of subjects had serum phosphate >5.1 mg/dL (reference range 2.5-5.1 mg/dL) and allsubjects (100%) had fetuin A levels <0.5 ng/mL (reference range 0.5-1 ng/mL). There was a moderate positive significant correlationbetween serum phosphate and CRP (p=0.024 and r=0.404), there was a strong negative significant correlation between CRP and fetuinA (p=0.000 and r=-0.628). No significant correlation between the phosphate serum and fetuin A (p=0.365 and r=-0.168) was found.Based on this study, it was found that the higher serum phosphate level resulted in a higher CRP and lower fetuin A.

Abstract

Background: Morinda citrifolia L leaf has been proven contains active polyphenol, antioxidant and flavonoid, these active compound have potential effect as reduced stress oxidative, developed renal function regeneration through protein extracellular matrix and growth factor. Early detection of renal function was used microalbuminuria (MAU) and VEGF expression of renal. This study aimed to observe the effect of Morinda citrifolia L leaf on decreased microalbuminuria and expression VEGF-IHC of diabetes nephropathy of Sprague Dawley rat Induced by Streptozotocin. Method: This study was an aminal experimental with a randomized post test only control group design. Thirty Sprague Dawley rat were divided into five group; a positive control group induced by streptozotocin; and four group induced by streptozotocin, then administered with extract of Morinda citrifolia L leaf for two weeks. Extract doses for each groups are 50; 100; 200; 400 mg/kgBW. The measure of blood glucose, microalbuminuria and expression VEGF of urine and renal tissue was observed and evaluated by non parametric Kruskal Wallis continued with Mann Whitney, the level of significance used was a p value of < 0,05. Result: There were a significant difference on measured of blood glucose, microalbuminuria and expression VEGF between four groups administered with Morinda citrifolia L leaf extract compared with positive control. It proven that Morinda citrifolia L leaf extract had potential effect in repairing the measure of microalbuminuria and expression VEGF renal, althoughit could not reach a normal condition yet. Mayer?s hematoxillin staining and VEGF ?IHC of renal showed better result parallel with dose increasing level. Conclusion: Extract of Morinda citrifolia L leaf has potential effect in repairing the renal function by microalbuminuria and expression VEGF caused by streptozotocin.Keywords: Morinda citrifolia L, diabetes nephropathy, microalbuminuria, expression VEGF-IHC. ABSTRAK   Latar belakang: Mengkudu (Morinda citrifolia L) telah diketahui secara ilmiah memiliki beberapa bahan aktif polifenol, antioksidan dan flavonoid yang berperan menurunkan stress oksidatif, kemampuan meregenerasi fungsi ginjal melalui pengukuran berbagai ekstraselular matriks protein dan faktor pertumbuhan. Perbaikan fungsi ginjal diukur berdasarkan mikroalbuminuria (MAU) dan ekspresi VEGF ginjal.Penelitian ini bertujuan untuk melihat pengaruh pemberian Morinda citrifolia L terhadap mikroalbuminuria dan ekspresi VEGF ginjal  pada diabetes nefropati tikus Sprague Dawley yang diinduksi streptozotocin. Metode: Penelitian menggunakan desain randomized post test only control group. Tiga puluh ekor tikus jantan Sprague Dawley, dibagi dalam lima kelompok: kontrol positif yang diinduksi streptozotocin; dan empat kelompok perlakuan yang diinduksi streptozotocin dan diberi ekstrak Morinda Citrifolia L dosis bertingkat 50; 100; 200; 400mg/kg BB selama dua minggu.  Kadar gula darah, mikroalbuminuria dan ekspresi VEGF pada tikus urin dan jaringan ginjal dinilai berdasarkan uji non parametrik Kruskal Wallis dilanjutkan uji Mann Whitney dengan ? <0,05. Hasil: Pemeriksaan kadar gula darah, mikroalbuminuria dan ekspresi VEGF ginjal pada keempat kelompok yang diberi ekstrak Morinda citrifolia L, semuanya menunjukkan perbedaan bermakna dibandingkan kontrol positif, membuktikan bahwa pemberian ekstrak secara bermakna memperbaiki kadar albumin dalam urin dan ekspresi VEGF ginjal, namun perbaikan ini belum mampu mencapai keadaan normal. Perbaikan yang ditunjukkan oleh gambaran imunohistokimia ekspresi VEGF sejalan dengan peningkatan dosis ekstrak yang diberikan. Simpulan: Ekstrak Morinda citrifolia L, memiliki efek perbaikan terhadap fungsi ginjal yang dideteksi melalui mikroalbumnuria dan ekspresi VEGFakibat induksi streptozotocin. Abstract

Latar belakang: Diabetes melitus (DM) tipe 2 dapat menyebabkan komplikasi salah satunya nefropati. Kontrol glikemik yang dinilai dengan HbA1c dan dislipidemia yang dinilai dengan rasio trigliserida/high density lipoprotein (TG/HDL) diduga berhubungan dengan komplikasi nefropati. Cystatin C merupakan petanda yang dapat menilai kerusakan fungsi ginjal dini. Hubungan kadar HbA1c dan rasio TG/HDL dengan Cystatin C pada pasien DM tipe 2 belum diketahui dengan jelas. Tujuan: Mengetahui hubungan antara kadar HbA1c dan rasio TG/HDL dengan Cystatin C pada pasien diabetes melitus tipe 2. Metode: Penelitian observasional analitik dengan pendekatan belah lintang dilakukan pada bulan April - Juni 2019 melibatkan 34 pasien DM tipe 2 di Puskesmas Karang Ayu yang memenuhi kriteria inklusi dan ekslusi. Pemeriksaan kadar HbA1c menggunakan metode HPLC, rasio TG/HDL dihitung dengan perbandingan TG dengan HDL yang diperiksa menggunakan alat kimia klinik otomatis, kadar Cystatin C menggunakan metode ELISA. Uji statistik menggunakan korelasi Spearman. p < 0,05 dianggap signifikan. Hasil: Rerata±SD kadar HbA1c adalah 8,21±1,65. Median (minimum-maksimum) rasio TG/HDL dan Cystatin C berturut-turut adalah 3,65(1,3–9,7), 0,72(0,46 – 1,22) mg/L. Korelasi HbA1c dengan Cystatin C dan rasio TG/HDL dengan Cystatin C berturut-turut adalah (r = 0,505; p = 0,002) dan (r = 0,471; p = 0,005). Simpulan: Terdapat hubungan positif sedang bermakna antara kadar HbA1c dengan Cystatin C dan rasio TG/HDL dengan Cystatin C pada pasien diabetes melitus tipe 2. The correlation of HbA1c and TG/HDL ratio with serum cystatin C in type 2 diabetes patients Background: Type 2 diabetes mellitus (DM) can cause chronic complications such as nephropathy. Glycemic control assessed with HbA1c and dyslipidemia assessed by the tryglyceride/high density lipoprotein (TG / HDL) ratio is thought to be associated with complications of nephropathy. Cystatin C is a marker that can assess early kidney function damage. Relationship between HbA1c levels and TG / HDL ratio with Cystatin C in type 2 DM patients is not clearly known. Objective: to investigate the correlation of HbA1c and TG/HDL ratio with Cystatin C in acute type 2 diabetes mellitus patients. Methods: Analytic observational study with cross sectional approach was conducted in April-June 2019 involving 34 type 2 diabetes mellitus patients at the Karang Ayu Puskesmas who were screened according to the inclusion and exclusion criteria. The level of HbA1c were measured by the HPLC method, TG/HDL ratio was measured by automatic chemistry analyzer and Cystatin C was measured by ELISA method. Statistical analysis used Spearman Correlation Test. p <0.05 was considered significant. Results: The mean ± SD HbA1c level was 8.21 ± 1.65. The median (minimum-maximum) ratio of TG / HDL and Cystatin C were 3.65 (1.3–9.7), 0.72 (0.46 - 1.22) mg / L, respectively. Correlation of HbA1c with Cystatin C and the ratio of TG / HDL with Cystatin C respectively (r = 0.505; p = 0.002) and (r = 0.471; p = 0.005). Conclusions: There are significant moderate positive correlation between HbA1c with Cystatin C and TG/HDL ratio with Cystatin C in type 2 diabetes mellitus patients. Abstract

Latar belakang: Kondisi Chronic kidney disease (CKD) dapat terjadi proses inflamasi dan aterosklerosis akan melibatkan marker inflamasi, disfungsi endotel Soluble vascular cell adhesion molecule-1 (sVCAM-1), asam urat (AU) dan peran elektrolit magnesium berhubungan dengan patofisiologi CKD .sVCAM-1 merupakan biomolekuler marker inflamasi dan disfungsi endotel, AU sebagai marker inflamasi dan magnesium berperan dalam metabolisme elektrolit ginjal. Tujuan: Membuktikan hubungan antara sVCAM-1 dengan asam urat (AU) dan magnesium (Mg) pada pasien CKD. Metoda Penelitian: Penelitian belah lintang dilakukan pada 33 penderita CKD belum pernah hemodialisis, sampel diambil selama bulan Maret - Juni 2019. Nilai Laju Filtrasi Glomerulus (LFG) dihitung menggunakan rumus Cockcroft-Gault. Kadars VCAM-1 diperiksa menggunakan metode Enzyme Link Immunosorbent Assay. Kadar asam urat diperiksa dengan metode fotometrik enzimatik dan magnesium diperiksa dengan kolorimetrik Xylidil blue menggunakan alat Advia. Uji hubungan menggunakan uji Pearson untuk data normal dan Spearman’s untuk data tidak normal. Hasil: Nilai LFG pada penelitan ini 46,48 ± 11,44 ml/min/1,73m2. Median kadarVCAM-1 adalah 715 (564 – 991) ng/dL. Median kadar asam urat adalah 9,2 (7,8 – 15,2) mg/dL dan median untuk kadar magnesium 1,03 (0,79 – 1,5) mmol/dL. Hubungan sVCAM-1 dengan kadar asam urat serum didapatkan korelasi dengan r = 0,488 dan p = 0,004.Terdapat hubungan sVCAM-1 dengan kadar magnesium serum dengan r = -0,442; p = 0,010. Simpulan: Terdapat hubungan positif sedang antara sVCAM-1 dengan asam urat serum, terdapat hubungan negatif sedang antara sVCAM-1dengan magnesium serum. Kata kunci :CKD,LFG, sVCAM-1, asam urat, magnesium Background: Chronic kidney disease (CKD ) is a condition that occurs because of the inflammatory process and atherosclerosis thus involve the marker of inflammation, endhothelial dysfuntion Soluble vascular cell adhesion molecule-1(sVCAM-1), uric acid (UA) and magnesium, associated with the pathophisiology of CKD. VCAM-1 is a marker of inflammation and endhothelial dysfuntion, UA as a marker of inflammation and Mg plays a role in renal electrolytes metabolism. Objective: To prove the relationship between the sVCAM-1 and uric acid (UA) and magnesium (Mg) levels in CKD patients Methode: A cross sectional study conducted on 33 patients with chronic kidney disease who had never hemodialysis during March-June 2019. Value of Glomerular Filtration Rate (GFR) is calculated using the Cockcroft-Gault formula which necessary creatinine levels. Value of GFR is calculated using the Cockcroft-Gault formula.sVCAM-1 levels were examined using the Enzyme Link Immunosorbent Assay method. Uric acid were axamined enzymatic photometric and magnesium levels were examined Xylidil blue colorimetric methods by Advia. Pearson correlation test using the test for normal data and Spearman's when data is not normal. Results: The value of this study GFR at 46.48 ± 11.44 ml/min/1,73m2. Median levels of sVCAM-1 was 715 (564 to 991) ug / dL. Median for uric acid is 9.2 (7.8 to 15.02) mg/dL and magnesium 1,03 (0,79 – 1,5) mmol/dL There is a significant correlation between VCAM-1 with serum uric acid levels showed a correlation with(r = 0.488; p = 0.004). There is a correlation between sVCAM-1 and serum magnesium(r = -0.442; p = 0.010). Conclusion: There is a moderate positive correlation between sVCAM-1 with uric acid serum, there is a moderate negatif correlation between sVCAM-1 and serum magnesium. Abstract

Pendahuluan : Chronic Kidney Disease (CKD) merupakan gangguan fungsi ginjal yang irreversible, yaitu kemampuan tubuh gagal untuk mempertahankan metabolisme dan keseimbangan cairan dan elektrolit dan juga mineral. Sebagian besar proses metabolisme memerlukan dan dipengaruhi oleh elektrolit. Konsentrasi elektrolit mineral yang tidak normal dapat menyebabkan banyak gangguan . Saat fungsi ginjal semakin memburuk menjadi Stadium CKD 4 dan 5, ekskresinya cenderung berkurang dan tidak dapat dikompensasi. Tujuan penelitian ini adalah melihat pengaruh CKD pada mineral yaitu magnesium dan calcium. Metode : Penelitian ini adalah penelitian observasional retrospektif dengan pendekatan belah lintang (cross sectional) dengan melihat rekam medik pada senter tunggal di RSUP Dr. Kariadi Semarang selama periode 1 Januari 2017 – 31 Desember 2018 untuk kemudian dilihat hubungan pengaruh kadar kreatinin serum dengan magnesium dan calcium pada pasien CKD dengan terapi hemodialisis. Hasil dan pembahasan: Terdapat hubungan signifikan positif lemah antara kreatinin serum dengan magnesium (p=0,032, r= 0,327). Tidak terdapat hubungan antara kreatinin serum dengan kadar kalsium. Simpulan : Dari hasil penelitian dapat disimpulkan bahwa secara statistik terdapat hubungan yang bermakna antara kadar kreatinin serum dengan magnesium pada pasien Chronic Kidney Disease (CKD) dengan terapi hemodialisis. ABSTRACT Introduction: Chronic Kidney Disease (CKD) is an irreversible kidney function disorder, which is the body's ability to fail to maintain metabolism and balance fluid and electrolytes and minerals. Most metabolic processes require and are influenced by electrolytes. Abnormal mineral electrolyte concentrations can cause many disturbances. When kidney function getting worse to stages CKD 4 and 5, the excretion tends to decrease and cannot be compensated. The purpose of this study is to look at the effect of CKD on minerals namely magnesium and calcium. Method: This study was a retrospective observational study with a cross sectional approach by looking at medical records at RSUP Dr. Kariadi Semarang during the period of 1 January 2017 - 31 December 2018 to see the correlation of influence of serum creatinine levels with magnesium and calcium in CKD patients with hemodialysis therapy. Results and discussion: There was a significantly weak positive correlation between serum creatinine and magnesium (p = 0.032, r = 0.327). There is no correlation between serum creatinine and calcium levels. Conclusion: The results of the study it can be concluded that there is a statistically significant correlation between serum creatinine levels with magnesium in Chronic Kidney Disease (CKD) patients with hemodialysis therapy. Abstract

The purpose of this study was to examine the relationship or influence of self-efficacy, social entrepreneurship, priority experience, and financial security on social entrepreneurship intentions. The research sample consisted of 100 MM STIE BPD Jateng students. The data were analyzed using a structural equation model to test the relationship between these variables, using PLS version 4. The results showed that social entrepreneurship intentions were significantly influenced by social entrepreneurship self-efficacy and priority experience with a p value <0.5. In addition, financial security also plays an important role in supporting social entrepreneurship intentions. Individuals with high levels of self-efficacy and priority practical experience tend to have strong intentions to enter social entrepreneurship. Financial security provides additional support by reducing the risks and uncertainties faced by prospective social entrepreneurs. This study provides important implications for the development of training programs and policies that support social entrepreneurship intentions. Increasing self-efficacy and providing opportunities for practical experience can encourage more individuals to engage in social entrepreneurship intentions. In addition, creating an environment that supports financial security will strengthen the success of social entrepreneurship intentions. Further research with strong theoretical support is recommended to explore and examine the dynamics of social entrepreneurship intentions and other factors that influence them

Abstract

Osteopontin (OPN) is an important protein in cancer development and progressivity. Lymphocyte-to-Monocyte Ratio (LMR) as a systemic immunity and inflammatory indicator could be an ideal predictor biomarker because of its method’s simplicity and availability. Elevated CA-125 and OPN as well as decreased LMR were reported as signs of ovarian malignancy. Limited studies about OPN and LMR as diagnostic biomarkers, as well as various specificity and sensitivity of CA-125 intrigued the researcher to prove OPN, LMR, and CA-125 as diagnostic biomarkers for ovarian carcinoma. This study aimed to measure the diagnostic value of OPN, LMR, and CA-125 levels against histopathology results for ovarian carcinoma diagnosis. Eighty patients involved with suspected ovarian carcinoma who were referred to Dr. Kariadi Hospital, Semarang. Osteopontin and CA-125 levels were measured using ELISA, and LMR was calculated from absolute lymphocyte and monocyte counts using an automated hematology analyzer. The receiver operating characteristics curve was used to determine the cut-off and 2x2 table. The cut-off values for OPN, LMR, and CA-125 were 124 ng/mL, 3.7 and 45.4 U/mL, respectively. The sensitivity for OPN, LMR, and CA-125 was 67.24%, 62.07% and 60.34%. Specificity for OPN, LMR, and CA-125 were 68.18%, 54.55% and 59.09%. Osteopontin is the best parameter for determining the diagnosis of ovarian carcinoma but it is still not sufficient because OPN cut-off was still within the normal reference value.

Abstract

Maximum loads due to physical exercise cause a decrease in antibody production and a decrease in leukocyte function in general. Many studies have been conducted on the effects of physical exercise, including Zumba, on CRP levels and peripheral blood parameters, but the research results have yielded different conclusions. This study aimed to find out the effect of regular and irregular Zumba exercises on CRP concentration and peripheral blood examination parameters. This experimental study used a pre-test and post-test design. There were 30 females aged 18-40 years who were divided into 2 training groups. Group I Zumba training 5x/week and group II Zumba training 1x/week. Both groups had to practice 60 minutes/session for 6 weeks. After 6 weeks of intervention, there was no significant correlation of CRP concentration (p=0,100). There were significant differences in Hb concentration (p=0.035), leukocyte count (p=0.001), erythrocyte count (p=0.001), and platelet count (p=0.018) in both groups. There was no significant effect of RZ and IZ exercise on CRP concentration and leukocyte differential count. But there were significant differences in changes in Hb concentration, leukocyte count, erythrocyte count, and platelet count, after regular and irregular Zumba exercise.
