Abstract

The prevalence of Diabetes Mellitus (DM) globally is elevated continually. Poor blood glucose control to diabetes patients caused both macrovascular complications (ex.coronary heart disease/CHD) and  microvascular complications (ex.nephropathy). Blood glucose controlled is need to be done in every 3 months regularly using HbA1C as a diabetic parameter, regarding to the high risk mortality of the complications. Low density lipoprotein cholesterol (LDL-C) is dyslipidemia  characteristics and LDL-C is a lipoprotein proatherogenic. Albuminuria test is also needed as an early marker of micro and macrovascular disorders, which reflects the general process of endothelial damages (vascular dysfunction).The aim of this study is to prove any relationship between HbA1C levels with LDL-C and albuminuria levels in diabetic patients with history of coronary heart complications.This study is performed an analytical observational study with cross sectional approach. Thirty diabetic patients with any history of CHD complications were enrolled, then laboratory analysis of HbA1C by ion exchange HPLC, LDL-C levels were using colorimetric enzymatic method and albuminuria test with  the photometric method. Differences between variables were analyzed using by Spearman correlation-testHbA1C levels in 10 patients (33,3 %) are <7% and in 19 patients are ≥ 7% (66,7 % ). LDL-C (<100mg/dl) in 13 patients (43,3%), LDL-C (≥100mg/dl) in 17 patients (56,7%), Normoalbuminuria (< 20 mg/L ) in 11 patients (36.7%), albuminuria (≥20 mg/L) in 19 patients (63,3%).There is a moderate positive relationship between HbA1C and LDL-C levels (r=0.385, p=0.014) and a positive strong relationship between HbA1C and albuminuria levels (r=0.52, p=0.004).Conclusions: The higher HbA1C levels, the higher LDL-C and albuminuria levels would be.

Abstract

Background: Diabetes mellitus is associated with increased blood glucose and triglyceride levels, which can lead to an oxidative stress. Zinc (Zn) is a micronutrient that has antioxidant properties and involved in lipid and glucose metabolism. This study aimed to evaluate the effect of zinc on the levels of fasting blood glucose (FBG), triglycerides (TG), and malondialdehyde (MDA) in male diabetic Rattus norvegicus Wistar rats.Methods: Diabetes was induced by intraperitoneal (i.p.) injection of 40 mg/kg BW streptozotocin (STZ) and confirmed by FBG level higher than 200 mg/dL after 2 weeks. The rats were randomly divided into three groups: control group (STZ), treatment I (STZ + zinc 5 mg/kg BW), and treatment II (STZ + zinc 10 mg/kg BW). Zinc was administered by oral gavage for 30 days. At the end of the experiment, levels of FBG, TG, and MDA were measured. Data were analyzed using paired t-test or Wilcoxon test as appropriate.Results: Supplementation of 5 mg/kg zinc significantly decreased the levels of FBG (pre-intervention: 328.95±70.90 mg/dL, post-intervention: 144.35±34.27 mg/dL, p<0.05), TG (pre-intervention: 252.48±26.30 mg/dL, post-intervention: 147.90±12.18 mg/dL, p<0.05), and MDA (pre-intervention: 12.11±6.46 nm/mL, post-intervention: 4.75±2.34 nm/mL, p<0.05). Moreover, supplementation of 10 mg/kg zinc decreased the levels of TG (pre-intervention: 275.62±56.25nm/mL, post-intervention: 165.58±22.63 nm/mL, p<0.05) and MDA (pre-intervention: 13.08±6.60 nm/mL, post-intervention: 5.08±2.40 nm/mL, p<0.05).Conclusion: Supplementation of zinc significantly reduced the levels of FBG, TG, and MDA in diabetic rats.

Abstract

Diabetes mellitus is a metabolic disease characterized by hyperglycemia. Giving Zn could reduce the reactivity of ROS due to hyperglycemia by increasing SOD status. Zinc ability as an antioxidant also can improve blood glucose level in diabetes mellitus patients. To prove the effect of Zn with the doses of 3 mg/kg BW and 5 mg/kg BW for 30 days onblood glucose and SOD levels in Wistar rats induced by STZ. Twenty-eight rats were divided into 4 groups includes K1 as a negative control group, K2 as a positive control group that is induced by STZ 40 mg/kg BW, K3 as? group that is induced by STZ 40 mg/kg BW and Zn were given at a dose of 3 mg/kg BW for 30 days orally and K4 as aa group that is induced by STZ 40 mg/kg BW and Zn were given at a dose of 5 mg/kg BW for 30 days orally. Examination of blood glucose levels used GODPAP methods and SOD levels checked with a colorimeter method. Data were analyzed using Paired T-Testsand Wilcoxon tests. There was a significant decrease in blood glucose levels after administration of 3 mg/kg BW Zn (p = 0,003) and 5 mg/kg BW (p = 0.018) for 30 days in Wistar rats induced by STZ. There was no significant increase in SOD levels after administration of 3 mg/kg BW Zn (p = 0,120) but a significant increase in SOD levels after administration of 5 mg/kg BW Zn (p = 0,038) for 30 days in Wistar rats induced by STZ. Administration of 5 mg/kg BW Zn for 30 days showed a better effect on blood glucose and SOD levels in Wistar rats induced by STZ when compared with administration of 3 mg/kg BW Zn.

Abstract

Background: Iodine and iron were micronutrient that highly related with formation of thyroid hormone. Iodine and iron deficiency during pregnancy, will affect fetal development. Weight and born baby’s head circumference were early indicator of infant development, because it describe the result of interaction of several factors for fetus during pregnancy. Objectives: To determine correlation between iodine and iron status of pregnant women in third trimester with born baby’s weight and head circumference. Methods: Cross sectional study with 49 pregnant women on third trimester and their baby born. Measurement of iron status used sTfR level by ELISA and iodine status used EIU. Meanwhile for the born baby, measurement was taken place on the baby’s weight and head circumference. Results: The average of iron status is 22,15 nmol/L, meanwhile average of iodine status is 168,63 μg/L with urine median value 165 μg/L. Statistical analysis with multiple correlation test showed no significant relationship between iron and iodine status of third trimester pregnant women to birth weight (r = 0,286; p = 0,141) as well as the babies head circumference (r = 0,195; p = 0,412).Conclusion: Iron and iodine status of third trimester pregnant women have low correlation to the born babies weight or head circumference in endemic area.

Abstract

Background: Obese is risk factor for abnormal glucose homeostasis and causes oxidative stress that can decrease insulin sensitivity resulting in hyperglycemia and hyperinsulinemia. Black soy bean contains anthocyanins and isoflavones, it is expected to reduce oxidative stress. Objective: Determine the effect of various dosage of black soybean extract (Glycine soja.) on blood glucose, insulin levels, and HOMA - IR in obese Sprague Dawley (SD) rats.Methods: True experimental study with randomized controlled pre test-post test design in male SD rats. Twenty four samples divided into 4 groups: control (K), 500 (P1), 750 (P2), and 1000 mg/kgBW (P3). Rats were induced to become obese with High Fat Sucrose Diet (HFSD) for 4 weeks then black soybean extracts were given for 2 weeks..Results: There was no difference in blood glucose before and after treatment (p> 0.05). There were differences on insulin levels in 500 (p = 0.012) and 750mg/kgBW (p = 0.020) doseges. There were differences on HOMA-IR in 500 (p = 0.043) and 750mg/kgBW (p = 0.028) dosages. There was difference in insulin levels’s difference (p = 0.009) between groups.Conclusion: There are differences on insulin levels and HOMA – IR in 500 and 750 mg/kgBW dosages groups. However, there is no difference on blood glucose before and after black soybean extract treatment. The most decreased levels of blood glucose, insulin and HOMA- IR were in 750mg/kgBW dosage.

Abstract

Background : Hyperglycemia on diabetes mellitus can cause increasing of free radicals production. Free radicals caused lipid peroxidation reaction by forming malondialdehyde (MDA). β-carotene has antioxidant activity may inhibit the formation of ROS.Objective : To prove the effect of multilevel doses β-carotene 1 mg/kg BW, 20 mg/kg BW and 20 mg/kg BW on alternate days within 30 days orally supplementation on blood glucose level and MDA level on Sprague Dawley male rats induced by streptozotocin (STZ). Methods : Thirty rats were randomly divided into 5 groups: X1=Negative control/diabetic, X2 (STZ 40 mg/kg BW + BC 1 mg/kg BW), X3 (STZ 40 mg/kg BW + BC 10mg/kg BW), X4 (STZ 40 mg/kg BW + BC 20 mg/kg BW), X5 (technic control/non diabetic). β-Carotene supplementation was given by nasogastric tube on alternate days within thirty days. Blood glucose level was measured by GOD-PAP and MDA level by ELISA with TBARS methods. Data was analized using paired t-test, wilcoxon, one way anova and post hoc bonferroni. Results : there was a significant difference of blood glucose level (p = 0,0001) and MDA level (p = 0,0001) after suplementation β-carotene on alternate days within 30 days orally. β-carotene 10 mg/kg BW was the most effective and efficient dose to lowering blood glucose, while 20 mg/kg BW to lowering MDA level. Conclusion : The multilevel doses β-carotene (1 mg/kg BW, 10 mg/kg BW and 20 mg/kg BW) on alternate days within 30 days orally supplementation can decrease blood glucose and MDA level. β-carotene 10 mg/kg BW is the most effecetive and efficient to decrease blood glucose and β-carotene 20 mg/kg BW to decrease MDA level.

Abstract

Background : diabetes mellitus (DM) is associated with increased total cholesterol and triglyceride, also well characterized by increased malondialdehyde production. β-carotene has antioxidant activity, glycemic and lipid control. Objective : to analyze the effect of β-carotene on total cholesterol, triglyceride and MDA on diabetic Rattus norvegicus sprague dawley. Methods : thirty rats were randomly divided into 5 groups : 1 (STZ), 2 (STZ+β-carotene 1 mg/kg BW), 3 (STZ+β-carotene 10 mg/kg BW), 4 (STZ+β-carotene 20 mg/kg BW), 5 (normal). Streptozotocin induced intraperitoneal 40 mg/kg BW. β-carotene was given by nasogastric tube on alternate days within thirty days. Blood glucose level was measured by GOD-PAP, total cholesterol by CHOD-PAP, triglyceride by GPO and MDA by ELISA with TBARS methods. Hypothesis test used one way anova then followed by post hoc bonferroni to analyze the efficient dose effect. Results : there was a significant difference of total cholesterol (p=0.002) after β-carotene 10 mg/kg BW supplementation on alternate days within 30 days orally. β-carotene 10 mg/kg BW was the most efficient dose to lowering total cholesterol. There were significant differences of triglyceride (p=0.0001) and MDA (p=0.0001) after β-carotene 1, mg/kg BW, 10 mg/kg BW and 20 mg/kg BW supplementation on alternate days within 30 days orally. β-carotene 10 mg/kg BW was the most efficient dose to lowering triglyceride, while 20 mg/kg BW to lowering MDA. Conclusion : β-carotene 10 mg/kg BW is the most efficient dose to lowering total cholesterol and triglyceride. β-carotene 20 mg/kg BW is the most efficient dose to lowering MDA.

Abstract

Latar belakang: Diabetes melitus merupakan penyakit inflamasi kronis yang ditandai dengan peningkatan kadar glukosa plasma. Kadar glukosa plasma yang tinggi dalam jangka waktu lama (hiperglikemia kronik), menstimulasi peningkatan kadar C-reactive protein (CRP). Penderita DMT2 dengan obesitas kemungkinan memiliki kadar CRP yang lebih tinggi karena penumpukan sel adiposa juga dapat memicu reaksi inflamasi di dalam tubuh. Mengetahui hubungan antara obesitas, DMT2, dan kadar CRP sangat diperlukan untuk tujuan klinis serta mencegah terjadinya komplikasi penyakit.Tujuan: Tujuan dari penelitian ini adalah untuk menganalisis hubungan antara kadar HbA1c dan CRP pada penderita diabetes melitus tipe 2 dengan obesitas dan tanpa obesitasMetode: Penelitian observasional analitik dengan pendekatan belah lintang dilakukan di Rumah Sakit Nasional Diponegoro dan beberapa klinik pratama di Semarang pada Mei-Juli 2020. Subjek penelitian adalah 30 penderita diabetes melitus tipe 2 dengan obesitas dan 30 penderita diabetes melitus tipe 2 tanpa obesitas usia 35-60 tahun Pengukuran kadar HbA1c dan CRP dilakukan menggunakan alat ichroma metode fluorescence immunoassay (FIA). Analisis statistik untuk menguji normalitas data menggunakan Shapiro wilk test, Independent t test dilakukan untuk mengetahui perbedaan rerata karakteristik antara kelompok obesitas dan non-obesitas, dan Spearman Rank digunakan untuk melihat korelasi antara HbA1c dengan kadar CRP.Hasil:  Tidak ada korelasi antara kadar HbA1c dengan CRP pada penderita diabetes melitus tipe 2 dengan obesitas p = 0.420, r = 0,153 dan tanpa obesitas yaitu p= 0,182, r = 0,250.Simpulan: Kadar CRP yang terjadi pada pasien diabetes mellitus tipe 2 dimana inflamasi berjalan kronis tidak dipengaruhi oleh tingginya HbA1c baik pada kondisi obesitas maupun tidak obesitas.

Abstract

Pendahuluan: Gagal jantung kongestif adalah ketidakmampuan jantung sebagai pompa untuk mempertahankan curah jantung (cardiac output) dalam memenuhi kebutuhan darah untuk metabolisme tubuh. Akibat penurunan cardiac output,perfusi darah keorgan hati kurang optimal, sehingga terjadi hipoksia yang bila berlangsung kronis dapat mengakibatkan atrofi sel hepatosit.Keadaan ini mengakibatkan peningkatan kadar enzim hepar (SGOT, SGPT),dan dapat menimbulkan kelainan hemodinamik, gangguan koagulasi, gangguan fungsi hati dan gangguan fungsi sintesis albumin. Klasifikasi NYHA membagi gagal jantung kongestif berdasarkan derajat keterbatasan fungsional jantung. Belum pernah dilakukan penelitian tentang derajat keterbatasan fungsional dengan tes fungsi hati pada penyakit gagal jantung kongestif.Metode: Desain penelitian ini adalah belah lintang. Sampel adalah serum 30 penderita penyakit jantung kongestif berbagai derajat sesuai kriteria NYHA pada laki-laki dan perempuan usia 20-55 tahun yang dirawat di RSUP Dr. Kariadi bulan Januari 2016-Maret 2016. Pemeriksaan kadar  SGOT,SGPT,albumin,bilirubin total,bilirubin direk dan bilirubin indirek yang diteliti menggunakan autoanalyzer. Hasil pemeriksaan dianalisis mengunakan uji stastistik, mengunakan uji korelasi Spearman’s rho dengan batas kemaknaan p<0,05Hasil:Terdapat hubungan positif sangat kuat antara derajat fungsional dengan bilirubin total (r=0,950, p<0,000), bilirubin direk (r=0,927, P<0,000), bilirubin indirek (r=0,946, p<0,000), SGOT (r=0,966, p<0,000), SGPT (r=0,964, p<0,000),   dan terdapat hubungan negatif sangat kuat antara derajat fungsional dengan albumin (r=-0,949, p<0,000).Simpulan:Semakin tinggi derajat keterbatasan fungsional akan semakin tinggi bilirubin total, bilirubin direk, bilirubin indirek, SGOT dan SGPT.Sebaliknya albumin,semakin tinggi derajat keterbatasan fungsional akan semakin rendah albumin.   Kata kunci :Congestive Heart Failure, Liver fuction test

Abstract

Latar belakang Proses inflamasi memegang peranan penting dalam patogenesis diabetes mellitus (DM) tipe 2 dan mendahului timbulnya komplikasi kardiovaskular. Parameter neutrophyl lymphocyte ratio (NLR), platelet lymphocyte ratio (PLR) dan monocyte lymphocyte ratio (MLR) merupakan biomarker potensial, merefleksikan inflamasi namun belum digunakan secara rutin. Penelitian ini ingin mengetahui perbedaan NLR, PLR dan MLR pada pasien DM tipe 2 dengan dan tanpa komplikasi kardiovaskular. Metode Penelitian deskriptif analitik dengan pendekatan belah lintang (cross sectional) pada 30 pasien DM dan 30 pasien DM dengan komplikasi jantung pada bulan Juli - Oktobertahun 2016 di RSUP Dr. Kariadi Semarang. Subyek penelitian berusia 30-80 tahun dan sudah menderita penyakit lebih dari 5 tahun. Pasien dengan infeksi/inflamasi lain, kelainan sistem hematologi, riwayat kanker/sedang menjalani kemoterapi/radiasi dan penderita yang meminum selain obat diabetes dan obat jantung dikeluarkan dari penelitian. Analisis data dengan uji t tidak berpasangan. Signifikan jika p < 0,05. Hasil Nilai NLR dan MLR pada pasien DM tipe 2 tanpa komplikasi jantung lebih rendah daripada pasien DM dengan komplikasi jantung (2,58 (0,98 – 14,72) VS 2,98 ( 1,42 – 7,17) p=1,000) dan (0,27 (0,12 – 0,87) VS 0,31 ( 0,14 – 1,60), p=0,183). Nilai PLR pasien DM tipe 2 tanpa komplikasi jantung lebih tinggi daripada pasien DM dengan komplikasi jantung (125,35 (34,48 – 540,93) VS 121,40 (58,90 – 354,69), p=0,723). Simpulan Tidak terdapat perbedaan bermakna NLR, PLR dan MLR pada pasien DM tipe 2 dengan dan tanpa komplikasi kardiovaskular.
