Abstract

Abstract: Giant-cell  Tumour (GCT ) is a benign Tumour, locally aggressive neoplasm which is composed of sheets of neoplastic oviod mononuclear cells uniformly distributed large, osteoclast like giant cell. Incidence GCT of proximal fibula is rare. Peroneal nerve function and stability of knee joint must be considered at the proximal fibula GCT. In 2017 we found three cases of GCT of the proximal fibula in the outpatient clinic Saiful Anwar General Hospital. All three of these patients had a confirmed GCT of proximal fibula  which planned for Tumour resection, peroneal nerve presevation and LCL ligament reconstruction. This is study of three patients with  GCT of proximal fibula was confirm with Clinical Pathological Confrence (CPC). All of patient had presented of lump,  slowly growing pain in lateral side of knee, limping when they walk and numbness in dorsal side of foot. One patient cannot dorsoflexion of ankle. We evaluated a peroneal nerve function and stability of knee after operation and one year after resection of tumour. All patient had occurred peroneal nerve lesion with no instability of knee post operatively. Two patient had complaint with numbness in dorsal side of foot and weakness ankle dorsoflexion and after one year both of patient had improved ankle dorsoflexion but still numbness in dorsal side of foot. One patient  still complaining numbness in dorsal side of foot and weakness ankle dorsoflexion post operation and one year after operation. There is no LCL ligament instability  in all of patient post operation and one year after operation. The peroneal nerve function and LCL ligament stability must consider when facing benign tumours in proximal fibula such as giant cell tumour proximal fibula. These resections result in an unavoidable loss of knee stability because of resecting the lateral collateral ligament (LCL) insertion site on the fibular head. Based on the literature, the incidence rate of postoperative peroneal nerve palsy ranges from 3% to 57%. Giant cell tumour in proximal fibula is rare and require wide excision with intraarticular resection of the proximal tibiofibular join. Abstract

Abstract: Primary tumours of musculoskeletal tissues are relatively uncommomn, they represent only 1% of all malignancies in all ages groups, but the incidence is arising each year. Many medical research and experiments were developed to found appropriate methods of treatment for tumor patients. Mortality rates of malignant musculoskeletal tumor patients still higher than benign cases. The aim of this study was to describe the incidence, age and sex distribution, location and histology of benign and malignant musculoskeletal tumors diagnosed and/or treated at a tertiary referral Orthopaedic Department serving the East Java. This was a retrospective analysis of prospectively collected data of medical records of all patients diagnosed and/or treated for any type of musculoskeletal tumor at Orthopaedic and Traumatology Department of Saiful Anwar Hospital, in the period from January 2011 to December 2017. This study resulted there were 531 cases of musculoskeletal tumor, within the characteristic: the peak of incidence occurs in the patient older than 60; the highest case was Osteosarcoma (92 cases); and breast metastases (40 cases) distributed highest mortality rates for metastastic bone tumor patients. Result of this study was people at age 60 and older have higher risk for Muskulokeletal Tumor, and Osteosarcoma was the most frequent case.  Keywords: Musculoskeletal tumor, data based system, incidence, East Java

Abstract

Abstract: Achilles tendinopathy produce morbidity, long-lasting disability in athletes and non-athletes and remain a challenge for clinician. Tendinopathy may lead to reduced tensile strength and a predisposition to rupture.The aim of our study was to evaluate the effects of activated and non-activated PRP on the healing process of ruptured tendinopathic achilles tendon in rat. Tendinopathy achilles condition induced by injecting collagenase bacterial type-1 locally. Right achilles tendon in 48 rats ruptured by transecting it transversely and repaired it by using a Kessler technique. Further, the wound immobilized with PRP and injecting activated PRP (5% CaCl2 added) in  one group, non-activated PRP on the other group and saline on control group. Tendons from each group were collected at the 1st and 2nd week postoperatively also assessed for biomechanical test. Tendons were also evaluated histologically by using hematoxylin-eosin to know adhesion degree based on Tang criteria. The significant differences was found between intervention group and control (p<0.05) at the 1st week but there was not any significant differences at the 2nd week in tensile strength test (p>0.05). Adhesion degree of the intervention group also reduce better rather than the control at the 1st and 2nd week (p<0.05). PRP have a positive effect on healing tendons by improving healing time, mechanical strength and decreasing adhesion degree. Keywords: achilles tendinopathy, PRP, healing time, tensile strength, adhesion

Abstract

Abstract: Musculoskeletal tumors are relatively rare, with 0.2%-0.5% of all malignant tumors in all ages. The geographic distribution of musculoskeletal tumors varies significantly around the world. This study aims to describe the musculoskeletal tumor profile in Saiful Anwar General Hospital. All data were obtained from the department of orthopaedic and traumatology database, Saiful Anwar general hospital. Thus, patients who were diagnosed with either bone tumor or soft tissue tumor between January 2011 to December 2018 were selected for the present study. The clinical-pathological conference (CPC) was carried out to ensure the validity of all the registered data. A total of 577 patients with tumors from January 2011 to December 2018 was obtained. Out of all the cases, 439 (76%) cases are bone tumors, and the remaining 138 (24%) cases are soft tissue tumors. The most frequent locations of the musculoskeletal tumors are the long bones of the thigh and lower leg. This study shows an increasing trend in musculoskeletal tumors incidence, despite decreasing numbers in several years. This result goes along with a rising trend in malignancy cases with a higher amount compared to the benign one. Moreover, MBD and osteosarcoma were found to be the most common tumor and primary bone tumors, respectively. However, the fact that there is some data loss in the study limited the study for a more accurate result.   Keywords: Musculoskeletal tumor; Bone tumor; Osteosarcoma; Malang; East Java

Abstract

Tulang merupakan predileksi ketiga tersering dari kejadian metastasis. Pasien dengan tumor metastasis pada umumnya mengalami nyeri hebat yang berpengaruh pada menurunnya mobilitas pasien, fraktur patologis, dan komplikasi metabolik seperti hiperkalsemia. Manajemen penatalaksanaan pasien metastasis pada tulang memerlukan metode diagnostik yang tepat dengan menerapkan prinsip triple diagnostic. Penelitian ini bertujuan untuk mengetahui profil klinikopatologi dan metode diagnostik tumor metastasis pada tulang di RSUD dr. Saiful Anwar Malang periode 2016-2020. Metode penelitian yang digunakan adalah deskriptif cross sectional, dengan mengumpulkan data dari arsip CPC di Laboratorium Patologi Anatomi dan data Tumor  Registry Departemen Ortopedi dari pasien dengan tumor metastasis pada tulang di RSUD dr. Saiful Anwar Malang. Didapatkan 104 sampel dalam penelitian ini, dengan hasil rentang usia terbanyak adalah 50-59 tahun (dekade ke-6) (43,2%), dengan mean 53,7 tahun dan median 53,5 tahun. Jenis kelamin terbanyak adalah perempuan (62,5%) dengan rasio perempuan dibanding laki-laki 5:3. Lokasi tumor terbanyak pada tulang apendikular (55,8%), dengan asal tumor primer terbanyak adalah unknown origin (26,9%), disusul dengan karsinoma payudara (20,1%), karsinoma tiroid (19,2%), dan karsinoma paru (13,4%). Metode diagnostik radiologi yang paling banyak digunakan adalah X-ray (95,1%) dan bone survey (67,3%), sedangkan untuk metode diagnostik patologi anatomi terbanyak menggunakan FNAB (83,6%). Diagnosis patologi anatomi untuk jenis tumor metastasis pada tulang yang terbanyak adalah adenocarcinoma (74%). Hasil penelitian ini diharapkan dapat memberikan peran dalam penentuan diagnosis dan tatalaksana yang efektif dan tepat untuk meningkatkan kualitas hidup pasien dengan tumor metastasis pada tulang.

Abstract

Osteoporosis is a global health issue. Fragility fractures, which occur due to minor trauma, are one of the serious consequences of osteoporosis. Aims: This study investigates the correlation between demographic characteristics and the level of knowledge about osteoporosis in patients with fragility fractures. This research used a descriptive design with a cross-sectional approach. The data was gathered through validated and reliable questionnaires. Respondents were patients with fragility fractures treated at Dr. Saiful Anwar General Hospital, Malang. Data analysis was performed using statistical tests to evaluate the correlation between age, gender, and knowledge about osteoporosis. The analysis revealed significant gender differences in osteoporosis knowledge (p-value = 0.037) but no substantial age-related disparities (p-value = 0.194). There were no significant differences in nutritional status based on gender (p-value = 0.881) or age (p-value = 0.521). Similarly, medication adherence showed no significant variance by gender (p-value = 0.883) or age (p-value = 0.589). Lastly, the understanding of osteoporosis did not significantly differ by gender (p-value = 0.467) or age (p-value = 0.782). In conclusion, the study reveals a significant gender disparity in osteoporosis knowledge, highlighting the need for tailored educational interventions and physician initiatives to ensure equitable awareness across diverse demographic groups.

Abstract

Introduction: Management of trauma still poses a challenge in Indonesia without significant improvement in mortality rates in the last 12 years, especially in multiple trauma involving at least 2 organ systems. This study aims to predict the prognosis of multiple trauma patients treated at Saiful Anwar Hospital (RSSA) Malang based on the anatomical location of the injury. Methods: Retrospective cohort study of 506 multiple trauma patients in RSSA during 2021-2022. The analyses used a chi square test and logistic regression test to obtain a regression equation. Results: Multiple trauma cases in RSSA were dominated by males in productive age (15-54 years). Head injury was the most influential variable in predicting the prognosis of multiple trauma patients in hospital, followed by thoracic injury, spinal injury, and abdominal injury. The logistic regression model was quite capable of explaining the data (p=0.931; Hosmer-Lameshow test), while the area under the curve (AUC-ROC) for prognosis in hospital care from this equation was 0.706 with a 95% confidence interval of 0.653-0.759 (good category). Conclusion: Head injuries, thoracic injuries, abdominal injuries, and spinal injuries are predictors of mortality of trauma cases in hospital care in RSSA, Malang, Indonesia.

Abstract

Introduction: Organophosphate is one of the most commonly used insecticides to control vector-borne illnesses. Both organophosphate and biodiesel (commonly used as solvents for outdoor space spraying methods) intoxications have cardiovascular manifestations. Case: A 58-year-old male with a sudden onset of shortness of breath after spraying insecticides 1 hour before admission. On examination, there were no SLUDGEM (salivation, lacrimation, urination, defecation, gastrointestinal distress, emesis, miosis) fndings. Bibasilar rales were present on auscultation, and a 12-lead ECG showed ST elevation in leads V1-V6. There was no preceding chest pain. The patient received oxygen, antiplatelet loading, and insulin therapy for hyperglycemia and was prepared for primary PCI. Diagnostic coronary angiography revealed a mid-LAD total occlusion, and a drug-eluting stent was implanted with fnal TIMI 3 fow. Discussion: Dyspnea has various causes, including organophosphate (OP) poisoning, which may result in cholinergic syndrome, respiratory dysfunction, and life-threatening cardiovascular complications. Early recognition through rapid diagnostic evaluation and timely interventions such as atropine, oximes, and reperfusion therapy in STEMI are essential to improve survival and outcomes. Conclusion: Early recognition and prompt revascularization are critical in STEMI to reduce infarct size and improve outcomes. Organophosphate exposure may be associated with cardiovascular efects, and clinicians should consider acute coronary occlusion in patients with respiratory complaints after insecticide exposure, even in the absence of classical cholinergic signs.

Abstract

Introduction: Cryoablation with liquid nitrogen has shown promise in treating bone sarcomas, but its effect on fibroblast morphology and viability in ligament tissues remains unclear. This study aims to evaluate the impact of liquid nitrogen on fibroblasts in the patellar tendon ligament of male Wistar rats. Methods: This experimental study utilized 28 male Wistar rats (6 months old, 300-400 grams) that were randomly divided into four groups. The control group was not exposed to liquid nitrogen, while the experimental groups were exposed to liquid nitrogen for 1, 5, and 10 minutes. After a 7-day acclimatization, the rats were exposed to liquid nitrogen and euthanized. The patellar tendon ligaments were collected for histological analysis, stained with Hematoxylin-Eosin, and examined under a microscope. Statistical analysis was performed using One-Way ANOVA, followed by Tukey post hoc tests to determine the significant differences between the exposure times. Results: Significant changes were observed in fibroblast morphology and count with increasing exposure times. One-way ANOVA analysis revealed significant differences (p < 0.001) in fibroblast count and morphological alterations across exposure groups. Tukey post hoc tests revealed that 10-minute exposure resulted in substantial reductions in fibroblast count and viability, accompanied by progressive morphological changes as exposure time increased. Discussion: Liquid nitrogen exposure significantly impacts fibroblast cells, with longer exposure times causing more severe cellular damage. The exposure duration should be carefully controlled in cryotherapy to optimize therapeutic benefits. Conclusion: Exposure to liquid nitrogen adversely affects fibroblast cells in the patellar tendon ligament, with prolonged exposure resulting in decreased cesll viability and morphological changes.
