Abstract

Background: Coronavirus disease 2019 (Covid-19) caused by SARS-Cov-2 had been spreading worldwide including Indonesia, but In Indonesia, it was difficult to diagnose confirmation Covid-19 by Polymerase Change Reaction (PCR). and in clinical practice to treat covid-19,  Indonesia Health Ministry use term probable covid-19 for patients under observation Covid-19, but study for neurological manifestation on  probable Covid-19 is very lacking.Objective: Knowing the neurological manifestation of patient probable Covid-19.Methods: Observational study from Covid-19 medical record and neuro-Covid-19 medical record of all probable Covid-19 hospitalized in the isolation ward of Saiful Anwar Hospital (RSSA) Malang since March-May, 2020 (3 months). Probable Covid-19 criteria were given by Covid-19 task force team based on Health Minister Indonesia criteria. This criterion was based on clinical manifestation (severe upper respiratory symptoms, or mild-severe pneumonia), radiologic examination (lung infiltrate and or chest CT scan), and laboratory examination. Neurological manifestation is based on clinical symptom appeared while patient hospitalized, supported with radiologic and laboratory data which was supervised by neurologist or neurologist resident. All data was inputted to neuro-Covid-19 e-registry on the smartphone application, sent periodically to the data center in Neurology Department RSSA Malang.Results: A hundred and fourteen probable Covid-19 was registered with mean age 34,5 year old and mostly at the young age of 20-50 years (38%). The neurological manifestation was observed in 4 patients (3,4% from all possible Covid-19 patients) with thrombotic stroke as the most common manifestation (50%), followed by myelum tumor (25%) and hypoxia encephalopathy (25%). Stroke patients in probable Covid-19 had old age (72 and 82 years) accompanied by several comorbid such as hypertension, DM and CVA history and Covid symptoms of cough, shortness of breath and fever. Hemiparesis was found improved with aspirin and neurotropic therapy. Patient with myelum tumor had a covid symptom of shortness of breath, cough, and fever with UMN tetraparesis. Previous MRI examination results suspected meningioma. The patient died while hospitalized. Patients with hypoxia encephalopathy admitted with decreased consciousness due to shortness of breath with acidosis respiratory symptoms (pH 7,22 ). This condition improved after oxygenation without focal neurologic deficit.Conclusion: Neurological manifestation in probable Covid-19 was 3,4% of all patients. The most common diagnosis was a thrombotic stroke, followed by myelum tumor, hypoxia encephalopathy, and mortality rate is 25%.

Abstract

The 2023 Quality Committee Report of Hospital X revealed lower levels of completeness and timeliness in the surgical ward, especially in the initial assessment, initial medical plan, and patient progress notes. Regarding this, the present study aims to analyze problematic Electronic Medical Records (EMR) components and the factors causing incompleteness and delay in data entry in surgeons and non-surgeons. This cross-sectional study was conducted by using 362 inpatient EMRs to analyze four key components, namely initial assessment, initial medical plan, Integrated Patient Progress Note (IPPN), and medical resume. Besides, 293 doctors completed questionnaires assessing their understanding of EMR-related regulations and their data entry timing for the four components. Completeness and timeliness analysis was performed using Chi-Square, while understanding of regulations and entry time was analyzed using Mann-Whitney. The findings indicated that non-surgeons showed higher completeness and timeliness in the initial assessment, medical resume, and CPPT (p<0.05), while surgeons excelled in the initial diagnostic plan in the initial medical plan (p<0.05). There was no difference in understanding of regulations, but non-surgeons took longer to enter data on assessment and initial medical plan (p<0.05). Finally, contributing factors include EMR technology, physician workflow, and physician interaction time with the EMR.

Abstract

Background: TB spondylitis is a disease that can cause neurological disorders, ranging from 1-2% of the total cases of tuberculosis where the most frequent infection is the spine followed by other large joints such as hips, knees and shoulders. The increase in TB spondylitis associated with MDR TB in 2015 showed 52% or about 32,000 incidences globally.Collaboration of individual alloy MDR TB therapy with surgery in TB spondylitis can reduce patient mortality and morbidity.Case: We report three cases of patients in 2017-2018 in Dr. Saiful Anwar with complaints of back pain and lumps in the back, from the results of anamnesa, physical examination and supporting examination of patients diagnosed with TB spondylitis, patients were given medical management, operative and rehabilitation.Conclusion: This case is interesting because from the results of Molecular Rapid Tests whose samples were obtained from a biopsy of the transpendicular debridement surgery tissue obtained the results of Resistant Rifampicin, all three patients received individualized OAT treatment, the development of the conditions of the three patients improved and quality of life improved.

Abstract

Introduction: Cryoablation with liquid nitrogen has shown promise in treating bone sarcomas, but its effect on fibroblast morphology and viability in ligament tissues remains unclear. This study aims to evaluate the impact of liquid nitrogen on fibroblasts in the patellar tendon ligament of male Wistar rats. Methods: This experimental study utilized 28 male Wistar rats (6 months old, 300-400 grams) that were randomly divided into four groups. The control group was not exposed to liquid nitrogen, while the experimental groups were exposed to liquid nitrogen for 1, 5, and 10 minutes. After a 7-day acclimatization, the rats were exposed to liquid nitrogen and euthanized. The patellar tendon ligaments were collected for histological analysis, stained with Hematoxylin-Eosin, and examined under a microscope. Statistical analysis was performed using One-Way ANOVA, followed by Tukey post hoc tests to determine the significant differences between the exposure times. Results: Significant changes were observed in fibroblast morphology and count with increasing exposure times. One-way ANOVA analysis revealed significant differences (p < 0.001) in fibroblast count and morphological alterations across exposure groups. Tukey post hoc tests revealed that 10-minute exposure resulted in substantial reductions in fibroblast count and viability, accompanied by progressive morphological changes as exposure time increased. Discussion: Liquid nitrogen exposure significantly impacts fibroblast cells, with longer exposure times causing more severe cellular damage. The exposure duration should be carefully controlled in cryotherapy to optimize therapeutic benefits. Conclusion: Exposure to liquid nitrogen adversely affects fibroblast cells in the patellar tendon ligament, with prolonged exposure resulting in decreased cesll viability and morphological changes.
