Abstract

PT Apac Inti Corpora merupakan perusahaan industri tekstil yang berlokasi di Semarang, Jawa Tengah. PT Apac Inti Corpora memproduksi tiga macam produk yaitu yarn, greige fabrics, dan denim. Berdasarkan data yang didapatkan, dari bulan Januari sampai November 2016, terdapat 41 kasus kecelakaan kerja pada PT Apac Intri Corpora dengan  presentase kecelakaan ringan 19,51%, kecelakaan sedang 78,05%, dan kecelakaan besar 2,44%. Terdapat 4 kasus kecelakaan di divisi spinning unit 4 RY dengan klasifikasi kecelakaan sedang dan ringan. Dalam upaya meminimalisir kecelakaan kerja, dapat dilakukan analisis kecelakaan kerja dengan menggunakan metode Hazard and Operability Study (HAZOP) melalui perangkingan. Hasil penelitian menemukan 25 bahaya di divisi spinning unit 4 RY dan dapat dikategorikan menjadi 14 sumber bahaya. Dari hasil penilaian level risiko, terdapat 2 sumber hazard yang tergolong ekstrim, 3 sumber hazard yang tergolong risiko tinggi, 4 sumber hazard yang tergolong risiko sedang, dan 5 sumber hazard yang tergolong risiko rendah. Penelitian ini memberikan rekomendasi perbaikan berupa melakukan inspeksi kepada pekerja, memberikan tanda-tanda atau poster mengenai kegunaan dan kebutuhan akan APD, melakukan pengawasan (sistem punishment dan reward), larangan masuk ke area kerja produksi tanpa menggunakan APD dan melakukan pemeriksaan medis pada pekerja yang terpapar debu. Abstract [Hazard Potential Analysis With Hazard and Operability Study Method Through Risk Assessment Ranking Case Study: Spinning Division Unit 4 Ring Yarn PT Apac Inti Corpora] PT Apac Inti Corpora is a textile company located in Semarang, Central Java. PT Apac Inti Corpora produces three kinds of products namely yarn, greige fabrics, and denim. Based on the data from January to November 2016, there were 41 cases of work accidents at PT Apac Intri Corpora with a percentage of light accidents 19.51%, moderate accident 78.05%, and a major accident 2.44%. There were 4 accident in the 4 RY spinning unit with the classification of moderate and minor accidents. In an effort to minimize work accident, analysis can be done by using Hazard and Operability Study (HAZOP) method through ranking. The results found 25 hazards in the 4 RY spinning unit and can be categorized into 14 hazard sources. From the level of risk assessment, there are 2 sources of hazard classified as extreme, 3 sources of hazard classified as high risk, 4 sources of hazard classified as medium risk, and 5 sources of hazard classified as low risk. The recommendations are inspection, providing signs or posters about the use and need for PPE, monitoring (punishment and reward systems), prohibition to entry wrok areas without the use of PPE and performing medical examinations for workers.

Abstract

PT PLN (Persero) merupakan salah satu perusahaan BUMN (Bdan Usaha Milik Negara) yang mengelola kelistrikan di Indonesia mulai dari pembangkit, penyaluran sampai pendistribusian serta penjualan energi listrik. Pada PT PLN (Persero) Area Semarang jumlah pelanggan yang menggunakan listrik di bulan Desember 2016 tercatat sebanyak 1.426.003 pelanggan dengan energi listrik yang tersalurkan dari Gardu Induk PT PLN Area Semarang sebesar 456.759.038 kWh sedangkan energi yang terjual sebesar 428.528.298 kWh. Hal tersebut dikarenakan adanya susut energi. Susut energi disebebakan oleh dua faktor yaitu faktor teknis dan non teknis. Oleh karena itu, pada penelitian ini dilakukan identifikasi penyebab yang mempengaruhi susut energi listrik menggunakan metode Failure Mode and Effect Analysis (FMEA). Berdasarkan data bulan Januari sampai dengan Oktober 2016 dapat diketahui penyebab terbesar dari susut energi listrik yaitu pada Jaringan Tegangan Menengah (JTM). Terdapat beberapa kegagalan pada JTM, dimana ada satu jenis kegagalan yang memiliki nilai RPN terbesar yaitu sebesar 250 pada kegagalan kawat/konduktor rusak. Sehingga kegagalan tersebut perlu diberi perhatian lebih. ABSTRACT                 PT PLN (Persero) is a STATE-OWNED enterprise (BUMN) who manage electricity in Indonesia starting from the production, distribution and sales to the distribution of electrical energy. In PT PLN (Persero) Area Semarang the number of customers who use electricity in December 2016 recorded as many as 1,426,003 customers with electrical energy from Mains Substation PT PLN Area Semarang of 456,759,038 kWh of energy while sold amounted to 428,528,298 kWh. That is because, the existence of losses energy. Losses energy caused by two factors, there are the technical and non technical factors. Therefore, this research was conductedon the identification of the causes that influence losses electrical energy using the method of Failure Mode and Effect Analysis (FMEA). Based on data for January to October 2016 can be known to be the biggest cause of reduced electrical energy “Jaringan Tegangan Menengah” (JTM). There are some failures at JTM, where there is one type of failure that has a largest RPN value is 250 on failure of wire/conductor is damaged. So the failure needs to be given more attention.

Abstract

Keselamatan kerja merupakan faktor penting yang harus diperhatikan oleh manajemen PT. Masscom Graphy. Dari pengamatan di lapangan, ditemukan beberapa indikator lemahnya penerapan keselamatan kerja salah satunya ialah masih rendahnya kesadaran dalam penggunaan alat pelindung diri saat bekerja. Penggunaan Alat Pelindung Diri(APD) merupakan upaya dalam mencegah terjadinya cidera pada pekerja apabila terjadi kecelakaan kerja. Kecelakaan kerja sangat mungkin terjadi di setiap perusahaan karena faktor unsafe condition  dan unsafe behavior. Sehingga untuk mengurangi resiko kecelakaan kerja maka dilakukan penelitian dengan menggunakan metode checksheet untuk menganalisis keselamatan kerja yang ditinjau dari penggunaan Alat Pelindung Diri (APD) oleh pekerja. Maka pada studi kasus ini juga akan dilakukan usulan perbaikan dalam penerapan keselamatan kerja melalui rekomendasi Alat Pelindung Diri (APD) untuk meminimalisir unsafe behavior dan bagaimana lingkungan kerja yang baik untuk meminimalisir unsafe condition, sehingga diharapkan dapat memberi kenyamanan dan keamanan  pada pekerja serta juga meningkatkan produktivitas kinerja operator.ABSTRACTAn analysis of the application of occupational safety with the methods checksheet PT. Masscom Graphy. Occupational safety is an important factor that must be considered by management of PT .Masscom graphy. The result of the observation, found some indicators of the weakness of the application of occupational safety one of them is the lack of awareness in the use of a protective herself when work. Tool use protective self (APD) is an effort to prevent the occurrence of workers injury if suffers employment accident. Work accident very likely to happen in every company because factors unsafe condition and unsafe behavior. So as to reduce the risk of the accident and investigation by using the method checksheet to analyze safety work in terms of the use of a self protection (APD ) by workers. This case study will also be done repair proposal in the application of occupational safety equipment through recommendations self protection (APD) to minimize unsafe behavior and unsafe condition, so as to provide convenience and security on workers and also increase productivity performance operator.

Abstract

Inas Rosita, Penilaian Kualitas Dan Analisis Faktor Yang Memperngaruhi Kinerja Supplier Pada Melati Interior. Penelitian pada jurnal ini bertujuan untuk memberikan gambaran mengenai penilaian kinerja supplier pada Melati Interior. Penilaian kinerja supplier ini berguna untuk memberikan usulan perbaikan terhadap faktor-faktor yang belum dapat memuaskan Melati Interior. Melati Interior merupakan salah satu perusahaan yang bergerak pada bidang industri mebel, dimana perusahaan ini memproduksi barang-barang untuk interior dari rumah, rumah sakit, kantor, dan lain-lain. Contoh bahan baku dari Melati Interior adalah besi, kayu, beberapa aksesoris (kaca, amplas, engsel, dan lain-lain), pelapis dalam (melaminto), HPL, dan lain sebagainya. Hubungan antara Melati Interior dengan pihak supplier memang belum sempurna sehingga tidak jarang pihak Melati Interior menemukan beberapa masalah. Setelah dilakukan perhitungan service quality, didapatkan angka 2,815. Angka ini termasuk ke dalam golongan di bawah rata-rata dan harus dilakukan pengolahan data dengan SPSS agar dapat diketahui faktor apa yang harus diperbaiki agar service quality meningkat. Setelah dilakukan pengolahan data dengan metode Importance Performance Analysis, didapatkan dua faktor atau indikator yang memiliki kepentingan tinggi tetapi performance rendah.AbstractInas Rosita, Quality Assessment And Analysis Factors That Affect Supplier Performance in Melati Interior. This research was purposed to give an overview of Melati Interior’s performance assessment. This performance assessment will be useful to give some improvement on factors that didn’t satisfy Melati Interior. Melati Interior is a company that specializes on furniture industry, where this company produces goods for the interior of houses, hospitals, office, etc. Steel, wood, some accessories (glass, sandpaper, hinge, etc), melaminto, HPL, and others are example of the raw materials from Melati Interior. The relationship between Melati Interior with their supplier is not perfect and Melati Interior could have found any problem quite often. After calculating the service quality, the result was shown as 2,815. This number belongs to under the average class and the data needs to be processed with SPSS to find out the factor that needs to be addressed. Later, there are two factors or indicators that has a high importance with a low performance. The results are shown from the Importance Performance Analysis method.

Abstract

PT. Gudang Garam Tbk. Merupakan perusahaan dalam produksi rokok berbagai varian.Dalam melakukan produksi rokok yang ada di PT.Gudang Garam Tbk. menggunakan sistem make-to-stock.Namun untuk studi kasus yang digunakan dalam penelitian ini dilakukan pada area perbaikan mesin produksi rokok yang ada.Lantai produksi dalam area perbaikan mesin produksi di PT.Gudang Garam Tbk. menggunakan sistem make-to-order dimana perbaikan-perbaikan mesin ataupun order part mesin yang dilakukan dan yang diproduksi berdasarkan dari pesanan unit produksi PT.Gudang Garam Tbk.Adanya banyak order dan proses pengerjaan perbaikan mesin dan pengerjaan part-part yang dibutuhan untuk mesin produksi di unit produksi mengakibatkan para pekerja yang ada dan mesin-mesin dalam lantai produksi terus melakukan proses kerja secara terjadwal.Dalam rutinitas proses kerja yang ada di lantai produksi di unit perbaikan mesin produksi (Workshop) seringkali ditemukan banyak paparan kebisingan yang timbul dari proses kerja mesin-mesin produksi ataupun dari interaksi manusia dengan pekerjaannya.Penelitian ini bertujuan untuk mengidentifikasi dan melakukan analisis tingkat kebisingan yang ada di area produksi unit perbaikan mesin (Workshop) PT.Gudang Garam Tbk. menggunakan peta kendali  dan R.Dalam melakukan pengambilan sampel data untuk analisis dan perhitungan tingkat kebisingan di area produksi tersebut dilakukan dengan melakukan clustering zona jenis-jenis mesin yang terbagi menjadi 3(tiga) zona sampel kebisingan area permesinan yaitu zona mesin-mesin CNC (Computer Numerical Control),zona mesin-mesin turning dan milling dan zona pengerjaan produk dengan kerja bangku.Selain membagi 3(tiga) zona pengambilan data,juga dilakukan pembagian waktu dalam mengambil sampel data tingkat kebisingan yang terbagi menjadi 3(tiga) kali pengambilan dalam 1(satu) hari kerja antara lain waktu pagi pada pukul 10.00 WIB,waktu siang pada pukul 14.00 WIB dan waktu sore pada pukul 16.00 WIB.Pengambilan sampel dilakukan dengan sound level meter dari aplikasi berbasis android.Setelah sampel terkumpul maka akan dilakukan proses analisis dan perhitungan apakah rata-rata tingkat kebisingan yang ada di dalam lantai produksi unit perbaikan mesin masih berada di dalam nilai ambang batas <NAB 80 atau telah melebihi nilai ambang batas >NAB 80. ABSTRACTNoise level analysis of production area in unit repair of production machine (Workshop) for SKM cigarette production machine (Sigaret Kretek Machine) PT.Gudang Garam Tbk. Using the  and R control chart. PT. Gudang Garam Tbk. Is a company in the production of various cigarette variants. In doing cigarette production in PT.Gudang Garam Tbk. using make-to-stock system. But for the case study used in this research is done on the existing cigarette production machine repair area. The production floor in the area of production machine repair in PT.Gudang Garam Tbk. using a make-to-order system where machine or part order improvements are made and produced based on the order of production unit of PT.Gudang Garam Tbk.There are many orders and process of repairing machines and workmanship of parts that are needed for the production machine in the production unit resulted in the existing workers and machines in the production floor continues to perform the work process on schedule. In routine work processes that exist in the production floor in the production machine repair unit (Workshop) is often found a lot of noise exposure arising from the process of working the machine the production engine or from human interaction with the work. This research aims to identify and conduct noise level analysis in the production unit of PT.Gudang Garam Tbk. using X and R control charts. In sampling the data for analysis and noise level calculation in the production area is done by clustering the machine type zones which are divided into 3 (three) zone noise sampling machining areas ie the CNC machining zone (Computer Numerical Control), zone of turning and milling machines and work area of the product with the work of the bench. In addition to dividing 3 (three) data retrieval zones, also conducted the time division in sampling the noise level data is divided into 3 (three) in 1 (one) working day, among others, morning time at 10:00 pm, noon time at 14.00 pm and evening time at 16.00 pm. Sampling is done with sound level meter of android-based applications. After the sample is collected it will be done the analysis process and the calculation of whether the average noise level in the production floor of the machine repair unit is still in the threshold value <NAB 80 or has exceeded the threshold value> NAB 80.

Abstract

Analisis Penyebab Defect Pada Raw Material Kertas Rol Dengan Menggunakan Metode Seven Tools & Solusi Perbaikan Dengan Metode Failure Mode And Effect Analysis (FMEA) Kualitas menjadi salah satu pertimbangan konsumen dalam memilih produk. Oleh sebab itu, setiap perusahaan pastinya memiliki bagian quality control untuk memastikan produk yang dihasilkan telah sesuai standar kualitas perusahaan. Bagian quality control PT Masscom Graphy sendiri berusaha menjaga produk koran yang dihasilkan memiliki kualitas baik. Pada penelitian kali ini ditemukan masalah yang dapat menimbulkan dampak besar pada bagian quality assessment perihal banyaknya defect pada raw material kertas rol. Penelitian ini berusaha mengamati, menganalisis, dan memberikan saran untuk mengurangi masalah defect pada raw maerial kertas rol yang merupakan bahan baku pembuatan koran tersebut. Dengan metode seven tools dan FMEA, didapatkan hasil jenis defect tertinggi raw material kertas rol adalah sisi tertekuk, lembab, serta klontong terlalu besar. Dari ketiga defect tersebut, faktor operator mesin menjadi kunci utama penyebab kecacatan (defect) berdasarkan dengan nilai Risk Priority Number (RPN) yang sudah dihitung dengan mendapatkan bobot nilai RPN sebesar 1365. Usulan perbaikan untuk mengatasi masalah tesebut adalah dengan memberikan teguran bagi para operator mesin. Abstract Analisis Penyebab Defect Pada Raw Material Kertas Rol Dengan Menggunakan Metode Seven Tools & Solusi Perbaikan Dengan Metode Failure Mode And Effect Analysis (FMEA) Quality becomes one of consumer consideration in choosing the product. Therefore, each company must have a quality control section to ensure that the products are in accordance with company quality standards. The quality control part of PT Masscom Graphy itself tries to keep the product produced good quality. In this study, a problem was found that could have a major impact on the quality assessment of the number of defects in raw paper roll material. This research tries to observe, analyze, and give suggestions to reduce defect problem on the raw material of roll paper which is the raw material of the newspaper. With the method of seven tools and FMEA, obtained the highest defect type of raw material of roll paper is side bent, moist, and klontong too big. Of the three defects, the factor of the machine operator is the key to the cause of the defect based on the Risk Priority Number (RPN) value which has been calculated by obtaining RPN value of 1365. The proposed improvement to overcome the problem is by giving a reprimand to the machine operator.

Abstract

Salah satu cara untuk bertahan ditengah persaingan, industri farmasi harus menerapkan pengelolaan sumber daya manusia yang baik. Pengelolaan sumber daya manusia yang baik diharapkan dapat menghasilkan tenaga kerja yang efektif dan efisien sehingga akan mendorong tercapainya visi misi dan tujuan perusahaan. PT Phapros Tbk (Industri Pengolahan Farmasi) merupakan perusahaan nasional yang memproduksi & memasarkan produk farmasi. PT Phapros membutuhkan perencanaan dan pengeloaan sumber daya manusia yang baik sehingga sumber daya manusia yang dihasilkan dapat bekerja dengan profesional agar kualitas produk yang dihasilkan dapat bersaing dengan produk kompetitor. Pada jurnal ini akan membahas mengenai beban kerja pada unit betalactam departemen produksi PT Phapros Tbk sebagai dasar penentuan jumlah karyawan pada rekruitmen mendatang. Perhitungan beban kerja dengan metode FTE (Full Time Equivalent) didapatkan dari observasi berdasarkan uraian tugas di setiap jabatan unit betalaktam. Masih ada ketidaksesuaian antara jumlah pekerja yang ideal dengan jumlah pekerja aktual sehingga perlu dilakukan peninjauan ulang dalam kebijakan rekruitmen di masa depan. Abstract [Workload Analysis Using Full Time Equivalent (FTE) For Determine Labor Requirements on Production Department Betalactam Unit PT Phapros, TBK]  one way to survive in the midst of competition, pharmaceutical industry must implement good human resource management. Good human resource management is expected to produce an effective and efficient workforce that will encourage the achievement of the vision and mission goals of the company. PT Phapros Tbk (Pharmaceutical Processing Industry) is a national company that produces & markets pharmaceutical products. PT Phapros needs good human resource planning and management so that the human resources can work as professionals and quality of the product can compete with the competitor's product. This paper will discuss the workload on the unit betalactam production department of PT Phapros Tbk as the basis for determining the number of employees in the next recruitment. Workload calculation by FTE method (Full Time Equivalent) is obtained from observation based on job description in every position of betalactam unit. The result is a discrepancy between the ideal number of workers and the actual number of workers so that a review of future recruitment policies is needed.
