Abstract

Hyaluronic acid is known as hyaluronan or hyaluronate, which has the function of increasing the mediator of periodontal regeneration. Hyaluronate acid has many roles in the early stages of inflammation, such as providing a structural framework through the interaction of hyaluronate with fibrin clots that modulate host inflammation and infiltration of the extracellular matrix of cells at the wound site so that this material becomes a therapeutic material used in various fields, especially in the field of periodontics. All periodontal tissues have shown the presence of hyaluronate, which is specifically concentrated in non-mineral tissues such as gingiva and periodontal ligament, which affects the growth, development, and repair of tissues in periodontal disease. The role of hyaluronic acid in periodontal healing will be discussed in this article.

Abstract

Menopause describes the cessation of a woman’s menstrual period due to the ovaries’ shutdown of estrogen and progesterone production. Decreased levels of these hormones cause menopause symptoms and reduce the quality of life. Therefore, this study aims to determine how the quality of life of postmenopausal women and the practice of periodontics in Medan City are related to the condition of teeth. A descriptive method was used with a cross-sectional design to adapt the questionnaires. The number of samples was 60 people based on medical record data at the periodontics installation of the RSGM USU and dentists’ practice in Medan. Furthermore, a Likert scale was used to assess the quality of life of postmenopausal women with periodontitis. The results showed that the quality of life, which strongly influenced the teeth, was Stage IV Grade B. In terms of social relations, the most decisive influence on the condition of the teeth was appearance. The aspect of the physical ability which strongly influences the condition is chewing hard food. In conclusion, menopausal women with periodontitis influence their quality of life from aspects of physical ability, social relations, general health, and psychology. Abstract

Periodontal disease is an inflammatory disease of the teeth-supporting tissues which consist of the gingiva, alveolar bone, cementum, and periodontal ligament. Enlargement and bleeding gums preceding tooth loss are clinical symptoms of this disease. Moreover, early diagnosis is difficult for patients until severe destruction has occurred. The impacts include feelings of disturbance and discomfort that will affect the quality of life. Periodontal health is not only limited to relieving pain but also includes aesthetic purposes, preventing further damage, and maintaining general health. This is because dental and oral health is an integral part of general well-being, which allows individuals to achieve their most important goals, namely a better quality of life. This study aims to examine the relationship between quality of life and periodontal disease.

Abstract

Organic deposits such as food residue, pellicle, dental plaque, and calculus can easily stick to the surface of smokers' teeth due to the tar content that sticks to the teeth, causing the tooth surface to become rough. This study aims to describe periodontal conditions through plaque accumulation and the need for periodontal treatment in smokers. Cross-sectional study at the Dental and Oral Hospital, University of North Sumatra, with a sample of 60 smokers. Plaque accumulation examination uses the Loe and Silness index and periodontal status needs use the Community Periodontal Index of Treatment Needs (CPITN) index. The results of the periodontal status of smokers showed that 36.7% of smokers had a periodontal status score of 0 (healthy periodontium), 33.3% had a periodontal status score of 2 (supragingival calculus), 28.3% had a score of 1 (spontaneous bleeding) and 1.7 % had periodontal status with a score of 3 (pockets 4-5 mm). The results showed that 36.7% did not need treatment (TN 0), 35% of smokers needed better dental care at home/improved oral hygiene and tartar cleaning (TN II), and 28.3% of smokers only needed adequate dental care. better (TN I). The conclusion of this study is that the periodontal treatment needs of smokers at RSGM USU are based on the number of cigarettes and duration of smoking, which is most often found in periodontal status and treatment with scores of 0 and 2.

Abstract

Banyak pasien dewasa membutuhkan perawatan ortodonti karena alasan estetis. Selain itu, perawatan ortodonti untuk mengoreksi maloklusi dapat membantu pasien untuk melakukan prosedur kontrol plak yang adekuat. Namun, klinisi tidak dapat memulai perawatan ortodonti ketika jaringan periodontal belum stabil dan dalam kondisi inflamasi. Oleh karena itu, klinisi harus dapat mendiagnosa dan mempersiapkan kondisi periodontal agar dalam kondisi yang sehat dan stabil sebelum memulai perawatan ortodonti. Selain itu, diperlukan waktu untuk mencapai kondisi periodontal yang sehat dan stabil setelah terapi periodontal. Prosedur pembedahan dan regenerasi memerlukan waktu penyembuhan yang jauh lebih lama dibandingkan pendekatan yang lebih sederhana. Tujuan penulisan laporan kasus ini adalah untuk menunjukkan pendekatan terapi periodontal yang lebih sederhana agar perawatan ortodonti dapat dimulai lebih awal. Terapi periodontal yang sederhana seperti scaling, root planing, dan kuretase, efektif untuk meningkatkan kondisi periodontal kompromis menjadi lebih sehat dan stabil. Selain itu, terapi periodontal tetap harus dilanjutkan selama dan sesudah perawatan ortodonti, untuk mempertahankan kondisi periodontal yang bebas inflamasi.

Abstract

ABSTRACTIntroduction: Periodontitis treatment includes scaling and root planing, which can be supplemented with additional therapy, such as the administration of propolis. Propolis, a potential local natural resource, contains active metabolites that act as anti-inflammatory agents in periodontal healing. This study aims to analyze the effectiveness of propolis extract gel from Kelulut (stingless bees) as an anti-inflammatory agent in periodontal treatment. Methods: This laboratory-based experimental research employed a post-test-only control group design, divided into five groups: 50%; 60%; 70% propolis extract gel; Metronidazole Gel and Placebo Gel. The anti-inflammatory test was conducted using 50 male Wistar rats induced with periodontitis through silk ligature and P.gingivalis bacteria. The rats were sacrificed on the third and seventh days after treatment, and macrophage cells count were performed under a microscope at 400x magnification using Haematoxylin-Eosin staining. Results: The ANOVA test results showed no significant differences in the macrophage cells counts among the treatment groups on day 3 and day 7 in the periodontitis-induced rat model (p>0.05). The anti-inflammatory test results indicated a decrease in the mean macrophage cells count in each group by day 7. Conclusion: The use of Kelulut propolis extract gel from Kabanjahe was effective in reducing the number of macrophages in periodontitis-induced rats, with the 70% propolis extract gel demonstrating the best anti-inflammatory effectiveness in periodontal treatment. Potensi gel ekstrak propolis lebah kelulut kabanjahe sebagai agen anti inflamasi pada perawatan periodontal: eksperimental laboratoris Pendahuluan: Perawatan periodontitis meliputi scaling dan root planing serta dapat didukung dengan terapi tambahan berupa pemberian propolis. Propolis merupakan salah satu sumber daya alam lokal yang potensial, mengandung metabolit aktif yang berperan sebagai zat anti-inflamasi dalam penyembuhan periodontal. Penelitian ini bertujuan untuk menganalisis efektivitas gel ekstrak propolis dari lebah Kelulut (stingless bees) sebagai anti-inflamasi pada perawatan periodontal. Metode: Penelitian eksperimental laboratoris ini memiliki desain post-test-only control group design yang dibagi ke dalam lima kelompok yaitu kelompok gel ekstrak propolis 50%; 60%; 70%, gel Metronidazole dan gel Placebo. Pengujian anti-inflamasi menggunakan 50 ekor tikus wistar jantan yang diinduksi periodontitis dengan ligatur Silk 3/0 dan bakteri P.gingivalis. Tikus dikorbankan pada hari ketiga dan hari ketujuh setelah perlakuan, kemudian dilakukan penghitungan sel makrofag dengan mikroskop pada perbesaran 400x dengan pewarnaan Haematoxylin-Eosin. Hasil: Hasil uji ANOVA, tidak terdapat perbedaan bermakna pada setiap kelompok perlakuan terhadap jumlah makrofag. sel pada hari ke-3 dan hari ke-7 pada model tikus periodontitis (p<0.05). Hasil uji anti-inflamasi menunjukkan adanya penurunan rerata jumlah sel makrofag pada masing-masing kelompok pada hari ke-7. Simpulan: Terdapat efektivitas penggunaan gel ekstrak propolis lebah Kelulut Kabanjahe terhadap jumlah makrofag pada tikus yang diinduksi periodontitis dan gel ekstrak propolis 70% memiliki efektivitas terbaik sebagai anti-inflamasi pada perawatan periodontal.

Abstract

Background: Vegetarians are known to be more susceptible to experiencing bone mineral density decreases compared to nonvegetarians. The trabecular bone is sensitive to any changes that disrupt the balance of bone metabolism. On panoramic radiographs, bone density can be evaluated by analyzing the fractal dimensions (FD) of the mandibular trabecular bone. Purpose: This analytical cross-sectional study aimed to assess the mandibular trabecular bone FD values on panoramic radiographs of vegetarians and nonvegetarians in Medan City. Methods: This study used a purposive-sampling technique and obtained 30 digital panoramic radiographs of vegetarians and nonvegetarians in Medan City, aged 20 to 40 years, between January and February 2023, at the radiology installation of Dental and Oral Hospital Universitas Sumatera Utara and Laboratory Clinic Pramita Medan. Fractal analysis was performed using ImageJ 1.54c software with the box-counting method. The data distribution was assessed using the Shapiro–Wilk test. The data were analyzed using an independent samples T-test. Results: The results showed that the FD values of the mandibular trabecular bone were significantly different between the two groups (p = 0.000). Conclusion: The mean FD values on the panoramic radiographs were lower in the vegetarian mandibular trabecular bone than in the nonvegetarian mandibular trabecular bone. Considerations need to be made by dentists when performing treatments related to mandibular trabecular bone surgery in vegetarians.
