Abstract

Introduction: Carpal Tunnel Syndrome (CTS) is a compressive neuropathy, caused by mechanical distortion produced by a compressive force of the median nerve at the level of the wrist. Primary sign is pain in the wrist, tingling sensation, pain or numbness in thumb, index finger, middle finger, and radial side of the ring finger, also there is a reduction of the grip strength and function of the affected hand. This sign tend to be worse at night and clumsiness during the activities that requiring wrist flexion. This syndrome is well-known and frequent accounts for 90% of all entrapment neuropathies. Incidence rates up to 276:100.000 per year. More common in females than in males, its occurrence is commonly bilaterally with a peak age range of 40 to 60 years. This is the productive age, which is often reported that the Carpal Tunnel Syndrome are work-related musculoskeletal disorders caused by strain and repeated movements. Disscussion: Dentist are high risks to go through musculoskeletal disorders covering wrist joint. The condition may happen because of the dentist position while handling patients is not in ergonomic position. These include repetitive prolonged hand activities, forceful static posture of wrist, vibration, and localized mechanical stress. To reduce the symptoms of musculoskeletal disorders since the beginning, has developed an integrated concept of teamwork in a modern dental practice. This concept is known as the four-handed dentistry which consists of dentists and assistants with their respective skills. Conclusion: The four-handed treatment techniques has been developed that is now largely acceptable. The concept of four-handed dentistry is expected to prevent the movement that makes the muscles tense, especially in the area around the wrist. And may ultimately reduce the incidence of Carpal Tunnel Syndrome

Abstract

Pendahuluan: Neutrofil berperan penting dalam mengatur peradangan selama penyembuhan luka. Camellia sinensis (teh hijau) terbukti mengandung Epigallocatechin Gallate (EGCG) yang memiliki aktivitas anti inflamasi. Kitosan dengan kemampuannya menjaga stabilitas EGCG, berpotensi untuk meningkatkan efektivitas EGCG dalam mengurangi inflamasi. Penelitian ini bertujuan untuk menganalisis perbedaan efek pemberian gel ekstrak Camellia sinensis yang dikombinasikan dengan kitosan terhadap jumlah neutrofil sebagai indikator utama fase inflamasi pada tikus Wistar pasca ekstraksi gigi. Metode: Gigi insisivus mandibula kiri pada tikus diekstraksi setelah dilakukan anestesi. Soket pada kelompok perlakuan diberi gel kombinasi ekstrak Camellia sinensis dan kitosan, sedangkan kelompok kontrol tidak diberikan intervensi. Pemeriksaan histopatologi soket dilakukan untuk menilai keberadaan neutrofil pada hari pertama, ketiga, kelima, dan ketujuh pasca perlakuan. Data yang diperoleh dianalisis menggunakan uji Kruskal-Wallis dan Mann-Whitney berdasarkan karakteristik distribusi. Hasil: Jumlah neutrofil pada kelompok perlakuan mengalami penurunan yang signifikan disetiap hari pengamatan dibandingkan kelompok kontrol yang menunjukan proses penyembuhan yang lebih baik dengan tidak adanya inflamasi persisten (p=0.000, p<0.05). Simpulan: Kombinasi ekstrak Camellia sinensis dan kitosan berefek terhadap penurunan jumlah neutrofil pasca ekstraksi gigi tikus Wistar.The effect of gel Camellia sinensis extract in combination with chitosan on neutrophils count of Wistar rats after tooth extraction: A Laboratory Experimental StudyIntroduction: Neutrophils play a key role in regulating inflammation during wound healing. Camellia sinensis has been proven to contain Epigallocatechin Gallate (EGCG), which exhibits anti-inflammatory activity. Chitosan with its ability to maintain EGCG stability, offers the potential to enhance the effectiveness of EGCG in reducing inflammation. This research aims to analyze the effect of extract gel Camellia sinensis combined with chitosan on the number of neutrophils as the main indicator of the inflammatory phase in Wistar rats after tooth extraction. Methods: The left mandibular incisor was extracted after the rats were anesthetized. The socket in the treatment group was treated with gel combining Camellia sinensis extract and chitosan, while the control group did receive any intervention. A histopathological examination of the socket was conducted to assess the presence of neutrophil on days one, three, five, and seven after treatment. The obtained data were analyzed using Kruskal-Wallis and Mann-Whitney tests based on the distribution characteristics. Results: The number of neutrophils in the treatment group decreased significantly on each day of observation compared to the control group, indicating a more advanced healing process with the absence of persistent inflammation (p=0.000, p<0.05). Conclusion: The combination of Camellia sinensis extract and chitosan had an effect on reducing the number of neutrophils in Wistar rats after tooth extraction.

Abstract

Prosedur ekstraksi gigi adalah tindakan rutin yang dilakukan oleh dokter gigi. Meski demikian, sering kali ditemukan komplikasi seperti rasa nyeri setelah ekstraksi. Salah satu cara untuk meminimalkan komplikasi pasca pencabutan gigi adalah dengan melakukan penjahitan, yang dapat mempercepat proses penyembuhan luka. Teknik penjahitan yang umumnya digunakan adalah simple interrupted. Namun, terdapat juga teknik penjahitan lain yang sering kali memberikan hasil penutupan luka yang lebih optimal yaitu figure-of-eight. Tujuan penelitian ini adalah untuk menentukan bagaimana perbandingan efektivitas dan efisiensi dalam proses penyembuhan luka setelah pencabutan gigi antara teknik penjahitan figure-of-eight dan simple interrupted. Penelitian ini dilakukan dengan metode studi kepustakaan, di mana berbagai jurnal online yang relevan dikaji untuk mengumpulkan data pendukung. Kajian literatur ini melibatkan penelaahan berbagai sumber ilmiah yang menjelaskan teknik penjahitan, hasil klinis, dan pengalaman pasien terkait penyembuhan luka pasca ekstraksi gigi. Terdapat perbedaan yang signifikan dalam pengukuran lebar soket, namun tidak ditemukan perbedaan yang signifikan dalam skor rasa nyeri. Penjahitan dengan teknik figure-of-eight menghasilkan penutupan luka yang lebih baik dibandingkan dengan teknik simple interrupted.

Abstract

Background: Predominant advanced glycation end products (AGEs) in diabetes mellitus (DM) patients lead to increased reactive oxygen species, causing oxidative stress and cell apoptosis, which hinders wound healing. Curcuma xanthorrhiza contains active compounds such as curcumin, flavonoids, and saponins, which can increase fibroblast cell production, the primary indicator of wound healing, and serve as AGE inhibitors. Purpose: This study aimed to determine the effect of 5% C. xanthorrhiza extract gel on the number of fibroblasts in diabetic Wistar rats during the healing of tooth extraction over a specific duration. Methods: The left mandibular incisor was extracted after using alloxan to induce diabetes in the rats. The socket was then treated with different compounds, including 5% C. xanthorrhiza extract gel, placebo CMC-Na, or Gengigel. A histopathological examination of the socket was conducted to assess the presence of fibroblasts on days one, three, and five after treatment. Results: The number of fibroblasts in the sockets treated with 5% C. xanthorrhiza extract gel was lower than in the Gengigel group but higher than in the placebo group (p < 0.05). Conclusion: 5% C. xanthorrhiza extract gel can increase the number of fibroblasts, thereby potentially accelerating wound healing in DM.
