Abstract

Pendahuluan: Konstruksi Gigi Tiruan Lengkap (GTL) perlu memperhatikan hubungan dalam arah vertikal (dimensi vertikal) dan horizontal (relasi sentrik). Posisi kondilus pada fossa glenoidalis merupakan salah satu parameter untuk menentukan relasi sentrik. Kurva Spee pada penyusunan gigi tiruan juga penting untuk diperhatikan, karena akan menentukan kontak gigi posterior. Metode Niswonger banyak digunakan karena metodenya sesuai dengan karakteristik fisik keturunan asli Indonesia. Parameter objektif melalui radiografi sefalometri diperlukan untuk mengevaluasi secara lebih akurat. Kesalahan pada proses pembuatan GTL dapat membuat adanya perubahan pada Dimensi Vertikal Oklusal (DVO), posisi kondilus terhadap fossa glenoidalis, dan kurva Spee dalam proses pembuatan GTL. Tujuan penelitian adalah untuk membandingkan DVO, posisi kondilus terhadap fossa glenoidalis, dan kurva Spee sebelum dan sesudah insersi GTL. Metode: Jenis penelitian ini adalah penelitian kuantitatif dengan metode observasional analitik dan rancangan penelitian studi cross sectional. Hasil: Hasil uji dependent t-test menunjukkan adanya perubahan dalam bentuk penurunan yang signifikan pada DVO (p<0,05), serta adanya perubahan rata-rata posisi kondilus terhadap fossa glenoidalis dan kurva Spee antara sebelum dan sesudah insersi gigi tiruan lengkap. Simpulan: Terdapat perubahan DVO, posisi kondilus terhadap fossa glenoidalis, serta kurva Spee sebelum dan sesudah insersi gigi tiruan lengkap.Kata kunci: Dimensi vertikal oklusal, gigi tiruan lengkap, kurva Spee, metode Niswonger, posisi kondilus, fossa glenoidalis.  ABSTRACTIntroduction: Complete denture fabrication needs to pay attention to the relationship in the vertical direction (vertical dimension) and horizontal (centric relation). The position of the condyle in the glenoid fossa is one of the parameters for determining centric relations. The curve of Spee in the preparation of dentures is also important to note because they will determine the contact of the posterior teeth. Niswonger method is widely used because the method is following the physical characteristics of native Indonesian descendants. Objective parameters through cephalometric radiography are needed to evaluate more accurately. Errors in the process of making complete denture can make changes to the Occlusal Vertical Dimension (OVD), the position of the condyle against the glenoidal fossa, and the Spee curve in the process of making complete denture. The study was aimed to compare OVD, condyle position on glenoid fossa and curve of Spee before and after insertion of complete denture. Methods: The type of this research was quantitative research with observational analytic method and cross-sectional study design. Results: The results of the dependent t-test showed a change in the form of a significant decrease in OVD (p < 0.05), as well as an average change in the position of the condyle against the glenoidal fossa and the curve of Spee between before and after the insertion of complete dentures. Conclusion: There was a change in OVD, condyle position on glenoid fossa and curve of Spee before and after insertion of complete denture.Keywords: Occlusal vertical dimension, complete denture, the curve of Spee, Niswonger method, condyle position, glenoid fossa.

Abstract

Gingival enlargement due to drug side effects is one of the complaints in the oral cavity in patients with certain conditions. The term gingival deformity which is clinically larger than normal is known as gingival enlargement. One of the etiologies of gingival enlargement is a systemic factor caused by the consumption of certain drugs. Drugs that can cause gingival enlargement are Calcium Channel Blocker (CCB) drugs. Amlodipine consumed by hypertensive patients can trigger enlargement of the gingiva. A 72-year-old woman came to the General Soedirman University Dental and Oral Hospital with complaints of the front gums of her upper jaw and a swollen lower jaw since six months ago. The results of the general examination showed that the patient was suffering from hypertension and was taking 5 mg of amlodipine since one year ago. Intraoral examination revealed gingival enlargement, a mild scale extending to the anterior regions of the maxilla and mandible. The Oral Hygiene Index Simplified (OHI-S) examination score was 2.9 (moderate). The patient's diagnosis was gingival enlargement et causa amlodipin. The management was given to patients is Dental Health Education (DHE) and instructions on how to maintain oral hygiene. The conclusion of this case report is that patients with hypertension who consume amlodipine can trigger gingival enlargement.

Abstract

Gingival enlargement is a kind of oral manifestation, that can caused by induction of anticonvulsant drugs. Phenytoin is one of anticonvulsant drug that can caused gingival enlargement. Gingival enlargement in this case developed until grade III. The patient was highly phenytoin dependent and can’t be reconciled. The aim of this paper is to find out the cause of phenytoin induced gingival enlargement and its treatment. A 20 years old woman came to RSGM UNSOED to check her swollen gums. The patient’s history was epilepsy and had been regularly taking phenytoin. The patient was taking 30 mg phenobarbital, 10 mg clobazam, 1 mg folic acid and 100 mg phenytoin 2 times a day with 2 capsules each time that had been consumted for 4 years. Phenytoin as anticonvulsant drug has side effects that can caused gingival enlargement, this is because phenytoin can increase extracellular connective tissue, collagen and fibrous.

Abstract

Diabetes Mellitus is a metabolic disorder characterized by an increase in blood glucose levels (hyperglycemia) due to disturbances in insulin secretion and insulin action. Diabetes mellitus (DM) is a disease characterized by high blood glucose levels, because the body cannot release or use insulin adequately. There are many factors that trigger or aggravate periodontitis, including plaque accumulation, calculus (tartar), and systemic factors such as diabetes mellitus. Objective: To explain that there is a close relationship between elevated blood glucose levels and periodontitis. Case Description: A 77 year old woman presented with a loose left upper tooth and pain during eating. The patient admitted to having a history of systemic disease, namely diabetes mellitus. Intra oral examination was found on 27th grade 3 luxation, 6 mm gingival recession, 4 mm probing depth, debris around the teeth, oral hygiene tends to be poor. The therapy provided is in the form of education on the effect of diabetes mellitus on teeth and the condition of the oral cavity in general, as well as the importance of maintaining health and food consumption so that blood sugar conditions are controlled. Conclusion: Periodontal disease can be affected by DM. Periodontitis is one of the manifestations of DM in the oral cavity. The severity of periodontitis can result from an increase in blood glucose levels.

Abstract

Gingivitis gravidarum is a pathological process that occurs during pregnancy on the supporting tissues of the teeth where there is gingival enlargement which causes discomfort due to increased estrogen and progesterone hormones. The etiology of gingival gravidarum originates from plaque in the oral cavity triggered by systemic factors such as hormones in pregnancy that cause inflammation and enlargement of the gingiva. The purpose of this case report is to find out about the management of gingivitis gravidarum that occurs in pregnant women. A 23-year-old female patient came with complaints of swelling of the anterior upper and lower gingival regions. These complaints have increased since entering the fourth week of pregnancy and spontaneous bleeding gums when brushing teeth. Intraoral examination revealed gingival swelling in the maxillary anterior region and mandibular interdental area extending to the gingival margin. Bleeding on probing showed positive results, while the O'leary score showed 28.28% and OHI-s showed 2.9 results. The treatment that will be carried out is USS scaling and patient education to always maintain oral hygiene. Gingivitis gravidarum often occurs in the area of the interdental gingival margin, which looks red and swollen. Periodontal treatment in pregnant women is more focused on controlling periodontal disease and eliminating the causative factors and treatments such as scaling can be carried out, while periodontal surgical treatment for pregnant women such as gingivectomy should be postponed until the delivery period is complete.

Abstract

Background: Composite resin is a material that is widely used for caries restoration, enamel abrasion, and aesthetic needs because of the good fit with teeth. Restorative materials that release fluoride ions may prolong the therapeutic and protective effects of the enamel. Nowadays, composite resins have been developed a new hybrid restorative material, named giomer. Giomer containing pre-reacted glass-ionomer (PRG) which can release and recharge fluoride ions. Composite resins have a disadvantage, a presence of shrinkage during polymerization, which can cause gaps between the tooth surface and composite resin. Preheating is carried out before irradiation with conventional oven. Preheating makes the composite resin stronger, reduces viscosity makes it easier to adapt and apply, and improves mechanical properties. This study aims to determine the effect of preheating on surface hardness.Method: The research sample used is 27 pieces cylinder-shaped with 5 mm diameter and 2 mm depth. The number of treatments was divided into 3 treatment groups. The treatment groups were divided based on the preheating treatment temperature of 37°C, 60°C, and control group. The surface hardness test was carried out using Vicker's microhardness tester.Result: The statistical One Way ANOVA test showed a significant result between the groups (p<0,05). LSD test (Least Significance Difference test) showed a significant result between the treatment temperature of 37°C, 60°C, and control group (p<0,05).Conclusion: The result of the giomer surface hardness test with preheating showed a higher result than the control group. The preheating group with a temperature of 60°C had the highest result.

Abstract

Background: Secondary caries occur due to imperfect plaque control. Prolong the protective and therapeutic effects can be done with restorative materials that release fluoride. Now composite resins have been developed a new restorative hybrid material with new matrix component, namely giomer. Giomer composition containing surface pre reacted glass ionomer (S-PRG) as a major source of fluoride production. Increasing the mechanical strength and minimize microleakage to prevent secondary caries can be done with preheating treatment. Purpose: This study is conducted to determine the effect of preheating temperature on the release of giomer’s fluoride. Methods: This study used 9 cylindrical samples in 10 mm diameter and 2 mm thickness each group, divided into 3 groups and 3 subgroups. Group 1: preheating at 37°C for 30 minutes. Group 2: preheating at 60°C for 30 minutes. Group 3: as a control group (without preheating treatment). Each group divided into 3 subgroups immersion, on day 1, day 7 and day 14 with artificial saliva. Fluoride release test was conducted by Spectrophotometer. IBM's SPSS Statistics used for the Data analysis. Results: The addition of preheating treatment decrease the amount of fluoride release. One-way ANOVA test showed a significant difference (P < 0.05). A significant difference between groups and sub groups showed in LSD test (P < 0.05). Conclusion: The group without preheating treatment has highest fluoride release and the 60°C preheating treatment group was the lowest. Addition of preheating treatment may increase the mechanical strength and minimize microleakage, but also descrease the amount of fluoride release.

Abstract

Composite resin is the material most often used as a restorative material. Acid etching is one of the procedures to help provide a place for penetration of bonding materials and composite resins. The acidic etching material that is often used is 37% phosphoric acid, but 37% phosphoric acid can cause inflammatory cell infiltration. Tartaric acid composed of weak acids that has potential to be an alternative acid etching. The aim of this study is to determine whether there are differences in microleakage of nanohybrid composites after application of tartaric acid etching between concentrations of 10%, 35%, and 40% in prevention of microleakage. This research is a laboratory experimental research with a posttest-only group design. Research samples are 24 maxillary first premolar were divided in 4 groups. Group I with 10% tartaric acid, group II with 35% tartaric acid, group III with 40% tartaric acid and group IV with 37% phosphoric acid etching. Based on the results of the LSD test it shows that There is microleakage of nanohybrid composites after etching application of tartaric acid concentrations of 10%, 35%, 40%, and 37% phosphate in class I cavities. Tartaric acid concentration of 40% can be used as an alternative to etching 37% phosphoric acid in preventing microleakage.

Abstract

Periodontitis kronis merupakan inflamasi jaringan periodontal yang disebabkan oleh biofilm bakteri plak dan ditandai dengan pembentukan poket periodontal, resesi gingiva, resorpsi tulang alveolar yang berakibat pada kegoyangan gigi. Perawatan utama periodontitis kronis adalah scaling root planing (SRP) untuk menghilangkan bakteri sebagai etiologi utama namun seringkali membutuhkan terapi adjuvant. Pengembangan terapi adjuvant dari bahan alami diharapkan dapat mengurangi efek samping, salah satunya daun piladang yang diketahui mengandung senyawa aktif seperti flavonoid, saponin, serta tanin. Penelitian ini bertujuan untuk mengetahui pengaruh ekstrak daun piladang terhadap kadar procalcitonin dan fibroblast growth factor-2 (FGF-2) saliva pada tikus model periodontitis kronis. Dua puluh lima tikus Wistar jantan 2-3 bulan, berat badan 150-200 gram dan dibagi menjadi 5 kelompok, yaitu kelompok periodontitis kronis dengan perlakuan ekstrak daun piladang dosis 150 mg/kg BB, 300 mg/kg BB, dan 600 mg/kg BB (P1, P2, P3), kelompok periodontitis kronis dengan perlakuan Na-CMC 1% (kontrol negatif/ KN), serta kontrol sehat (KS). Perlakuan selama tiga hari dilanjutkan pengambilan sampel saliva pada hari ke-empat. Kadar procalcitonin dan FGF-2 saliva diukur dengan uji ELISA. Analisis statistic menggunakan uji One-Way Anova dilanjutkan Post hoc LSD. Hasil menunjukkan penurunan kadar procalcitonin dan FGF-2 saliva pada kelompok perlakuan ekstrak daun piladang (P1, P2, P3) seiring peningkatan konsentrasi ekstrak, berbeda signifikan daripada kontrol negatif (p≤0,05), dan menyamai kondisi sehat (p>0,05). Hal ini mengindikasikan ekstrak daun piladang dapat mempercepat fase inflamasi dan proliferasi pada tikus model periodontitis kronis.

Abstract

A lichenoid reaction is commonly finding in the oral cavity. A lichenoid reaction may be caused by systemic drugs. Among the drugs are antihypertensive drugs. The aim of this study is to know the treatment of patient with oral lichenoid reaction. Male patient came into RSGM Unsoed complaining of white patches on the right gums near 3rd molar three months ago. The white patches were painless and approximately 0.5 in diameter. Anamnesis was presenting patients is conducted and has been shown that they take 6 years of antihypertensive drugs and are prescribed by their own physician in the city of Semarang. The patient was then instructed on how to keep the oral hygiene and be evaluated two weeks later. Patients then having a normal tension and were off drugs for the following 2 weeks and got a slightly reduced lesion when it was carried out by control. Treatment of a lichenoid reaction can be done with communication, information, and education about his condition to the patient first. Another conservative treatment of topical corticosteroids is recommended when symptomatic symptoms occur. Treatment of patients with an asymptomatic reaction can be done with communication, information, and education to the patient and drug substitution if perceived as needed. Another treatment that can be taken is by using the topical corticosteroid if the lesion is symptomatic.
