Abstract

PREPARASI DAN KARAKTERISASI POLIMORFISME OBAT ANTI MALARIA ARTESUNATE. Artesunate (AS) adalah derivat semisintetis dari Artemisinin, suatu obat anti malaria dari tumbuhan Artemisia annua yang terdiri atas struktur seskuiterpen endoperoksida. AS merupakan derivat artemisinin yang paling banyak digunakan sebagai obat antimalaria untuk menggantikan banyak obat malaria lain yang telah resisten terhadap malaria falsiparum. Khusus untuk AS, belum banyak studi yang dilakukan maupun literatur yang tersedia untuk mempelajari keadaan padatan (solid state) dan khususnya keberadaan polimorfisme dari AS. Polimorfisme dari suatu padatan memiliki fasa kristalin yang berbeda dalam susunan kisi kristal internalnya. Perbedaan kisi kristal (polimorfisme maupun pseudopolimorfisme) ini dalam bidang farmasi akan memberikan pengaruh terhadap ketersediaan hayati obat dan khasiat penggunaan obat secara klinis. Tujuan penelitian ini adalah untuk memperoleh dan mengamati perbedaan polimorfisme AS serta mengkarakterisasinya dengan analisis PXRD, IR dan DTA. Untuk lebih memperkuat hasil analisis, dilakukan juga cara mikroskopik baik dengan Hot Stage Microscopy (HSM) maupun dengan Scanning Electron Microscope (SEM). Hasil penelitian menunjukkan keberadaan dua bentuk polimorf yang berbeda dari obat antimalaria AS. Bentuk I merupakan bentuk komersial dari AS yang biasa digunakan sebagai bahan baku obat dan bentuk II diperoleh dari perlakuan bentuk I yang mengalami proses freeze drying.

Abstract

Abstrak: Angka penularan penyakit COVID-19 di dunia masih tinggi dan menjadi perhatian dunia termasuk Indonesia dengan angka penularan yang relatif masih terbilang tinggi hingga pada saat ini. Pemerintah menggelontorkan biaya yang besar untuk melaksanakan program vaksinasi kepada seluruh masyarakat Indonesia secara gratis yang bertujuan untuk menurunkan angka penularan penyakit COVID-19 di Indonesia. Tujuan pengabdian untuk mengedukasi masyarakat tentang vaksinasi COVID-19, meyakinkan masyarakat tentang pentingnya melakukan vaksinasi dan menyukseskan program vaksinasi yang dilakukan oleh Pemerintah. Metode pelaksanaan yang telah dilakukan adalah sosialisasi melalui presentasi dan penyebaran flayer informasi vaksin, serta penyuluhan informasi terkait vaksin dan aspek terapeutik vaksinasi kepada 300 peserta yang bekerja sama dengan tim vaksinator Puskesmas Citeureup di puskesmas Citeureup, Jawa Barat. Maka dapat disimpulkan bahwa dengan adanya sosialisasi dan edukasi vaksinasi ini, warga mendapatkan manfaat, kepercayaan diri, pengetahuan terhadap jenis vaksin yang diberikan, proses mengatasi efek kejadian setelah vaksin serta peningkatan kesadaran terhadap protokol kesehatan. Sehingga kegiatan ini dapat membantu pemerintah untuk menekan tingkat penularan COVID-19 di wilayah Kecamatan Citeureup, Jawa Barat.Abstract: The global transmission rate of COVID-19 is still high, causing alarm in countries throughout the world, particularly Indonesia, which has had a high transmission rate up until now. The government has invested a significant amount of money to conduct a free vaccination campaign for all Indonesians to minimize COVID-19 transmission rates. The service's goal is to educate the public about COVID-19 immunization, persuade them of the significance of vaccination, and help them complete the government's vaccination campaign. Socialization through the presentation and distribution of vaccine information flyers, as well as information dissemination related to vaccines and the therapeutic aspects of vaccination, has been carried out to 300 participants who work closely with the Citeureup Health Center's vaccinator team at the Citeureup Health Service Center, West Java. As a result, residents get advantages, confidence, understanding of the type of vaccine administered, the process of overcoming the impacts of events after the vaccine, and increased awareness of health protocols because of this vaccination socialization and education. So that this activity can help the government to reduce the level of transmission of COVID-19 in the Citeureup District, West Java.

Abstract

Indonesia is abundant in natural substances with immunomodulatory and tonic qualities that can help maintain immunity and keep the body healthy in the face of the COVID-19 pandemic. This paper Analysing herbal plants that are effective in terms of safety and actual efficacy in increasing the endurance and fitness of Indonesians in the face of the Covid 19 Pandemic becomes the first step in pioneering efforts to create national drug independence and sovereignty in the context of national health sector resilience. This study suggests that herbal plants such as Papaya (Carica papaya), Turmeric/Curcumin (Curcuma domestica), Meniran (Phyllanthus niruri), Brotowali (Tinospora crispa), and Ginger (Zingiber officinale Rosc.) which have immunomodulatory and tonic properties, as well as data on the potential for increasing endurance and body wellness by enhancing immune response, to raise awareness of the nation's ability and potential natural resources in supporting national drug independence, resilience, and sovereignty. Therefore, the usage of immunomodulator herbal medications is thought to have an activity to boost the immune system and protect the body from COVID-19 and preparedness for the next pandemic. However, these findings should be confirmed through additional scientific research or clinical trials.

Abstract

Appendicitis is often identified based on clinical characteristics. Appendectomy is the main therapy because antibiotics alone are not effective. However, appendectomy poses a risk of Surgical Site Infection (SSI), necessitating prophylactic antibiotic usage. This study aims to analyze the relationship between leukocyte type, clinical characteristics, and surgical factors with the type of appendicitis and to analyze the effectiveness of prophylactic antibiotic use. Antibiogram analysis used pus specimen cultures. A cross-sectional retrospective design covered 139 subjects treated from 2019-2021 at Gatot Soebroto Army Hospital. Data collection, through purposive sampling, followed set criteria. The chi-square test was used to determine relationships, followed by the logistic regression test. Complicated appendicitis appeared in 18% of males and 11.5% of females. The 15-40 age group (59%) had most cases and a high complicated appendicitis rate (51.2%). Three subjects experienced SSIs across age groups: <15, 15-40, and 41-65, with two of them being male. Neutrophil and lymphocyte counts related to appendicitis type. Dominant factors linked to appendicitis type were length of stay, operation duration, ASA (American Society of Anasthesiologists) score, age, and type of surgery. The most commonly used prophylactic antibiotics are cephalosporins. Prophylactic antibiotics were mostly effective in preventing SSI (97.8%). Gram-negative bacteria generally cause appendicitis, antibiotics that are resistant are generally from the penicillin group. Apendiktomi menjadi terapi utama karena antibiotik saja tidak efektif, namun apendiktomi berisiko IDO (Infeksi Daerah Operasi) sehingga diperlukan antibiotik profilaksis. Penelitian ini bertujuan untuk menganalisis hubungan antara tipe leukosit, karakteristik klinis, dan faktor pembedahan dengan tipe apendisitis serta menganalisis efektivitas penggunaan antibiotik profilaksis. Analisis antibiogram dilakukan berdasarkan kultur spesimen pus. Desain retrospektif cross-sectional digunakan dengan 139 subjek yang dirawat sejak 2019 hingga 2021 di RSPAD Gatot Soebroto. Pengumpulan data menggunakan metode purposive sampling sesuai kriteria yang dibuat oleh peneliti. Uji chi square digunakan untuk mengetahui adanya hubungan, kemudian dilanjutkan dengan uji regresi logistik. Apendisitis komplikata terjadi pada 18% pasien pria dan 11,5% pasien wanita. Usia terbanyak pada rentang 15-40 tahun (59%) dan usia tersebut memiliki persentase appendisitis komplikata tertinggi (51,2%). 3 subjek mengalami IDO berusia  <15 tahun, 15-40 tahun, dan 41-65 tahun, 2 di antaranya adalah pria. Terdapat hubungan antara neutrofil dan limfosit dengan jenis apendisitis. Urutan faktor yang paling berhubungan dengan tipe apendisitis adalah lama rawat, lama operasi, skor ASA (American Society of Anasthesiologist), usia, dan tindakan operasi. Antibiotik profilaksis yang paling sering digunakan adalah golongan sefalosporin. Antibiotik profilaksis sebagian besar efektif mencegah kejadian IDO (97,8%). Bakteri Gram-negatif secara umum menjadi penyebab apendisitis, antibiotik yang resisten umumnya adalah golongan penisilin. Kata Kunci:         Apendisitis, faktor risiko, antibiotik profilaksis, IDO

Abstract

Etoricoxib merupakan salah satu golongan anti inflamasi selektif COX-2 yang diklasifikasikan dalam BSC kelas II. Pada penelitian ini dilakukan usaha peningkatan kelarutan etoricoxib melalui metode dispersi padat dengan menggunakan Kollidon® VA 64. Percobaan ini diawali dengan uji pendahuluan untuk mencari optimasi perbandingan antara etoricoxib dengan Kollidon® VA 64. Setelah dilakukan uji didapatkan perbandingan ETO-KOL-3 (1:3) adalah yang optimal. Kemudian dilakukan pembuatan dispersi padat dengan menggunakan metode penguapan pelarut yang terbagi tiga yaitu pengeringan pelarut pada suhu ruang, pengeringan pelarut dengan laju pemanasan dan pengeringan pelarut dengan Spray Drying. Hasil dispersi padat kemudian dikarakterisasi dengan Powder X-Ray Diffractometry (PXRD), Differensial Scanning Calorimetry (DSC) dan Scanning Electron Microscope(SEM). Hasil PXRD dari ETO-KOL-3-SR, ETO-KOL-3-SE dan ETO-KOL-3-SD memiliki derajat kristalinitas sebesar 37,62, 36,72 dan 35,55. Hasil DSC ETO-KOL-3-SR, ETO-KOL-3-SE dan ETO-KOL-3-SD memiliki Tg sebesar 72,78, 80,87 dan 88,48°C. Hasil SEM ETO-KOL-3-SR dan ETO-KOL-3-SE memiliki bentuk lempeng irregular tetapi ETO-KOL-3-SD memiliki bentuk spherical. Hasil disolusi ETO-KOL-3-SR, ETO-KOL-3-SE dan ETO-KOL-3-SD pada media dapar fosfat pH 6,8 memiliki persen disolusi 53,39±0,34 %, 63,05±0,24% dan 72,60±0,19%. Pada penelitian ini dapat dibuktikan bahwa pembentukan dispersi padat etoricoxib dengan Kollidon® VA 64 dengan metode Spray Drying merupakan salah satu pendekatan yang potensial untuk meningkatkan laju disolusi etoricoxib.

Abstract

Background: Recent studies have highlighted the significance of inflammatory markers in diagnosing appendicitis; however, the specific roles of stab and segmented neutrophils in assessing disease severity remain poorly understood. Objective: This study aims to analyze the relationship between the types of appendicitis (complicated and uncomplicated) and neutrophil counts (stab and segmented), along with demographic variables such as age and gender, to evaluate their diagnostic significance. Methods: A retrospective, descriptive study was conducted on 48 appendicitis patients treated at Gatot Soebroto Army Central Hospital during 2020–2021. Data were analyzed using the Mann-Whitney U test, with a significance threshold set at p<0.05. Results: Neutrophil segment counts demonstrated a statistically significant difference between complicated and uncomplicated appendicitis cases (p=0.000), with higher levels observed in complicated cases. In contrast, neutrophil stab counts, age, and gender did not show statistically significant differences (p>0.05). Conclusion: Neutrophil segment counts serve as a reliable marker for assessing appendicitis severity, while neutrophil stab counts and demographic factors such as age and gender lack predictive value. Further research is warranted to investigate the temporal dynamics of neutrophil levels and their integration with other inflammatory markers.

Abstract

Releases from healthcare facilities account for a large portion of environmental pollution, with wastewater carrying a cocktail of pharmaceutical contaminants, especially antibiotics. These compounds accelerate the accumulation of antimicrobial resistance. Objective: The study synthesizes the scientific record on the concentration, fate, and effects of pharmaceutical compounds within hospital wastewater; it foregrounds the links to growing microbial resistance and delineates the threat to public health. The review adhered to the 2020 PRISMA framework. The search strategy applied a combination of keywords ("pharmaceutical residues" OR "antibiotic residues") AND ("hospital wastewater") AND ("antimicrobial resistance") AND ("environmental impact" OR "community"). The search was restricted to titles and abstracts of studies published 2015-2025. The article selection process consisted of title and abstract screening, full-text evaluation, and consensus-based discussion. Of the 405 articles initially identified, 21 met the eligibility criteria and were narratively synthesized. Hospital wastewater was found to contain a variety of antibiotics, particularly β-lactams, fluoroquinolones, tetracyclines, macrolides, and sulfonamides, at concentrations higher than those observed in domestic wastewater. Several ARGs including blaNDM, blaKPC, blaOXA, sul1, sul2, qnr, tet, and mcr were frequently detected, alongside resistant pathogenic isolates such as Escherichia coli, Klebsiella pneumoniae, and Pseudomonas aeruginosa. Conventional wastewater treatment processes were shown to be only partially effective, achieving removal efficiencies of 16-50% for pharmaceutical residues and ARGs. Consequently, resistant bacteria and ARGs were still detected in receiving water bodies several kilometers downstream from discharge points. Hospital wastewater serves as a major source of pharmaceutical contamination and plays a critical role in the amplification of microbial resistance in the environment. These findings highlight the urgent need for advanced wastewater treatment technologies, strengthened antibiotic stewardship programs, and the integration of environmental and epidemiological surveillance within the One Health framework to curb the spread of antimicrobial resistance.
