Abstract

Tuberculous Meningitis (TBM) is a severely disabling condition that often leads to death. It requires the right intensive treatment to minimize disability, speed up recovery, and reduce the risk of death. Rehabilitation is one of the key components in managing TBM. Rehabilitation is expected to speed up recovery and prevent disabilities. The purpose of this case series was to highlight the effectiveness of a comprehensive rehabilitation program in severe TBM patients at Intensive Care Unit (ICU) settings. We present two cases of severe TBM that showed significant changes after the rehabilitation program started. Both patients were at GCS 111. Rehabilitation program was a therapist-based multimodal sensory stimulation in a comprehensive ICU setting with approximately 2 weeks duration. After 5 days of rehabilitation, the first patient’s consciousness began to improve. The second patient’s consciousness began to improve after 1 week of rehabilitation. After approximately 2 weeks, both of them were already at GCS 456 and then transferred to the High Care Unit (HCU). There were only minor disabilities as they were transferred. The rehabilitation program was adapted and continued at HCU. After 1 month, both of them got discharged to home. These cases highlight the importance of rehabilitation programs to be included in the management of TBM patients.

Abstract

Improper initial management of emergency conditions during football matches causes long-term morbidity, affects the athlete’s future performance, and may be life-threatening. Football coaches and referees may come from non-medical backgrounds, but they should understand how to provide primary care in emergency situations during matches. This program aims to improve the primary emergency care understanding of the coaches and referees in East Java. The program was conducted in September 2022 – January 2023. All referees, assistant referees, and coaches of the Football Association of the Indonesia (FAI) region East Java (128 participants) were included. The primary emergency care training material consisted of video, PowerPoint media, and hands-on mannequins. Participants were given pre-and post-test to assess their level of knowledge, with a passing grade 75. Comparative tests (Wilcoxon signed rank, McNemar, and Chi-square test) were used to analyze the data. The median of the pre-and post-test scores were 62.5 and 87.5. There was a significant difference between pre-and post-test scores (p < 0.001). There was no significant difference between coaches and referee groups in pre-test scores (p = 0.743). This program improved the primary emergency care understanding during matches of coaches and referees.

Abstract

Background: Intracerebral hemorrhage is associated with a high mortality and disability rate, 40% of which happened in non-traumatic ICH patients. Putamen hemorrhage is the most common cause of ICH and strongly associated with hypertension. Until now, bilateral putamen hemorrhagic stroke is still a rare case. Case: Male, 54 years old, admitted to the ER for being unconscious 1 hour earlier. The patient had a history of hypertension but did not receive any treatment. The patient also had a history of schizophrenia and was taking Clozapine, Trifluoperazine, Sertraline, and Dimenhydrinate. Patient was presented with GCS 112, blood pressure 176/104 mmHg, SpO2 98%. Other physical examinations were within normal limits. Head CT scan showed acute ICH in the right putamen (vol ±7.8cc) and left putamen (vol ±20.7cc). Conservative management was given, but there was no neurological improvement. On day 14, complications of pneumonia occurred and the patient later died due to cardiopulmonary arrest. Discussion: Spontaneous bilateral putamen hemorrhagic stroke is a rare type of ICH and loss of consciousness is the main manifestation of the condition. Hypertension is the most common etiology of bleeding, but the exact mechanism is still unknown. Mortality in patients with spontaneous bilateral putamen bleeding is very high and to date, conservative therapy is the main treatment for patients with these conditions. Conclusion: Spontaneous bilateral putamen hemorrhagic stroke is a rare case with a high mortality rate and so far, the main treatment for this case is conservative management. Abstract

Masyarakat lanjut usia erat kaitannya dengan banyak komorbiditas salah satunya sindrom metabolik. Gereja Katolik turut berperan serta mengembangkan jemaat lansia melalui komisi lansia yang bertujuan untuk mewujudkan karakter kristiani dan peningkatan kualitas hidup umat lansia agar sehat secara fisik, mental dan rohani baik sebagai individu, anggota dalam keluarga maupun komunitas. Tujuan kegiatan pengabdian ini untuk meningkatkan taraf hidup kesehatan masyarakat, membantu meringankan beban saudara yang membutuhkan, serta membantu pemerintah dalam mewujudkan program pelayanan kesehatan, khususnya kedokteran pencegahan dari penyakit-penyakit terkait sindrom metabolik melalui kegiatan promosi kesehatan dan skrining pencegahan sindrom metabolik. Kegiatan yang dilakukan mencakup pengukuran antropometri tinggi dan berat badan, lingkar pinggang-panggul, kadar lemak, tekanan darah, kadar gula darah dan kolesterol menggunakan alat medis yang sesuai. Kegiatan edukasi pencegahan untuk setiap masing-masing individu masyarakat turut dilakukan melalui konsultasi dengan dokter setelah melalui pemeriksaan kesehatan. Terdapat 233 masyarakat yang dilayani dengan hasil didapatkan usia 44,64 tahun dimana proporsi perempuan 65,6%, obesitas 41,63%, hipertensi 48,5%, hiperglikemia 2,15%, dan dislipidemia 75,54%. Tingkat pengetahuan masyarakat jemaat terhadap sindrom metabolik sebesar 86,78 poin dengan skor domain gejala sindrom metabolik paling tinggi skornya (92,7 poin). Masyarakat perlu meningkatkan kesehatan diri dengan menghindari risiko sindrom metabolik dan mencegah komplikasinya.
