Abstract

Background: Uterine leiomyomata, often known as fibroids, are frequently accompanied by symptoms that are common to a variety of different pelvic disorders. When considering treatment choices, it is critical to correctly identify myomata and rule out other disorders, including malignancies. In this case report, the author emphasizes the difficulty in establishing a preoperative diagnosis and the actions taken during the operation.Case report: Female, 46 years old, has three children who live with two children. She was complaining of left abdominal pain and penetrating to the waist. There were no complaints of defecation or urination. A mass in the lower abdomen, solid springy, and half of the symphysis center tend to the right with limited mobility. The preoperative diagnosis is pedunculated uterine myoma. It was decided to do a Supravaginal Hysterectomy and Bisalphingo-oforectomy. The pelvic cavity still looks bulging, and exploration was carried out to find uterine myomas of the intrasacrouterina ligament. Then, it was decided to do a Myomectomy.Conclusion:Pedunculated uterine myoma intrasacrouterina ligament performed Supravaginal Hysterectomy and Myomectomy

Abstract

Background: Ileus occurs when there is a non-mechanical decrease or stoppage of the flow of intestinal contents. The cause of ileus has yet to be clearly defined. Various factors influence the risk for an ileus, each affecting a small part of the complex neuroimmune system. The patient will present with bloating and abdominal distension. Plain abdominal films and computed tomography (CT) scans of the abdomen are usually the first diagnostic imaging obtained. The most important principle for treating ileus is treating the underlying cause. Treating the infection, electrolyte abnormalities, and decreasing opiate use can all potentially decrease the durability of an ileus. Anesthesia procedures must be performed to treat patients preoperatively, during surgery, and postoperatively. Enhanced recovery protocols, regional anesthesia, opioid-sparing analgesics, and laparoscopy have all improved the number of postoperative ileus cases.Case Presentation: The following is a 67-year-old male patient who presents with abdominal pain with a scale of 4 out of 10, diagnosed with ileus, which was planned for laparotomy surgery by general anesthesia with supine position.Conclusion:We conclude that anesthetic procedures are critical in ileus patients in performing laparotomy operations, both in selecting anesthetic techniques, anesthetic drugs, and supporting scores (Aldrete score). Before induction, anesthesiologists must carefully assess preoperative and monitor postoperative

Abstract

Background: Meningiomas, common primary brain neoplasms, become more frequent with age. They can affect osseous structures, manifesting as hyperostosis and significantly diminishing the well-being of patients. Although meningiomas' precise etiology and pathogenesis remain ambiguous, hormonal influences and other factors have been implicated in their development. This study seeks to explain surgical management and cranioplasty reconstruction to reduce recurrence and improve the well-being of patients.Objective: This case series encompasses a retrospective analysis of medical records from six patients diagnosed with meningiomas with hyperostosis, treated between December 2022 and January 2024, at Dr. Soegiri General Hospital, Lamongan. The analysis focuses on surgical interventions and cranioplasty reconstruction.Cases Presentation: Six patients had chronic cephalgia and cranial masses persisting for 3-6 years. Computed tomography imaging revealed hyperostotic changes contiguous with the neoplastic lesions, prompting craniotomy. Subsequent acrylic cranioplasty was undertaken to ensure encephalic protection and to achieve cosmetic restoration.Conclusion: For patients presenting with meningiomas and hyperostosis, symptomatic management through surgical excision and resection of the hyperostotic bone is recommended. Early implementation of cranioplasty, preferably acrylic materials, is recommended to shield cerebral structures and restore cranial morphology, given its economic feasibility and reduced infection risk compared to autologous bone grafts

Abstract

Background: The research explores the advancements and implications of e-prescribing systems within modern healthcare.Objective: The primary objective is to assess the growing body of literature, examining global trends, effectiveness, and challenges associated with e-prescribing implementation.Methods: Using a comprehensive literature review methodology, data were sourced from the Scopus database, revealing many publications from 1980 to 2024.Results: A notable increase in scholarly interest was observed, particularly from 2015 to 2024, highlighting e-prescribing as a mature research subject. The results indicate a high concentration of research activity in advanced healthcare systems, contrasting with the limited contribution from developing contexts such as Indonesia.Conclusion: The study identifies that while e-prescribing improves medication safety and efficiency, gaps in understanding its long-term impacts on patient outcomes persist. Additionally, disparities in technology adoption among demographic groups emerge as significant barriers. This research contributes to existing knowledge by underscoring the need for localized studies and international collaboration to enhance e-prescribing practices. Future research directions should focus on evaluating long-term outcomes, improving digital literacy, and assessing training impacts on healthcare professionals.

Abstract

Pemerintah melalui Badan Penyelenggara Jaminan Produk Halal (BPJPH) mewajibkan sertifikasi halal baik untuk produk maupun jasa. Puskesmas dengan pelayanan rawat inap pelayanan obstetri neonatus emergensi dasar (PONED) di puskesmas Kedundung terdapat suplaier gizi. Suplaier Gizi Puskesmas Kedundung saat ini belum tersertifikasi halal. Kondisi tersebut yang dimanfaatkan oleh Tim Pengabdi untuk memfasilitasi para suplaier gizi dalam meningkatkan kesadaran akan pentingnya kaidah halal pada produk-produknya melalui sosialisasi dan pendampingan sertifikasi halal. Sertifikasi halal produk unit gizi dilakukan dengan sosialisasi halal, pelatihan system jaminan produk halal (SJPH), komitmen halal, dan pendampingan sertifikasi halal. Sertifikasi halal produk gizi unit PONED Puskesmas Kedundung berfungsi sebagai salah satu sarana untuk meningkatkan nilai tambah pelayanan yang dihasilkan, serta memberikan ketenangan bagi pasien, terutama bagi pasien Muslim. Selain itu, para suplaier gizi ditantang untuk menerapkan Sistem Jaminan Halal secara berkesinambungan sehingga mutu dan kualitas pelayanan bahkan integritas suatu unit pelayanan menjadi lebih baik di mata pasien.

Abstract

Latar Belakang : Gestational Diabetes Mellitus (GDM) didefinisikan sebagai suatu derajat intoleransi glukosa yang timbul atau pertama kali dikenali pada masa kehamilan yang merupakan penyebab klinis yang paling umum dari kelahiran prematur, gangguan hipertensi pada kehamilan, kelainan cairan ketuban, gawat janin, hambatan pertumbuhan janin, makrosomia, dan Intrauterine Fetal Death (IUFD). Sedangkan superimposed Preeklampsia didefinisikan sebagai memburuknya tekanan darah pada usia kehamilan lebih dari 20 minggu dengan timbulnya proteinuria atau meningkatnya proteinuria dan keterlibatan organ lain sesuai dengan preeklampsia. Tujuan: Laporan kasus ini bertujuan untuk menjelaskan tentang kejadian Gestasional Diabetes dan Superimposed Preeklampsia dengan kejadian Intrauterine Fetal Death (IUFD) dalam kehamilan. Presentasi Kasus : Perempuan usia 35 tahun GIIP10000 UK 28 minggu T/H/IU ke IGD RS Ibu dan Anak Fatimah Lamongan mengeluhkan kenceng kenceng dan keluar darah banyak pervaginam bergumpal warna merah kehitaman serta tidak merasakan gerakan bayi dalam kandungan sejak 2 hari belakangan. Kesimpulan : Gestational Diabetes Mellitus (GDM) adalah kasus yang banyak ditemukan di kalangan masyarakat. Diagnosa ditentukan berdasarkan anamnesis, gejala klinis, dan pemeriksaan penunjang. GDM juga meningkatkan resiko terjadinya resiko superimposed preeklampsia pada ibu dengan hipertensi kronik. Risiko kematian fetal pada wanita hamil dengan GDM dan Superimposed Preeklampsia lebih tinggi daripada risiko pada wanita dengan kehamilan normal.
